Targeting CXCR4 abrogates resistance to trastuzumab by blocking cell cycle progression and synergizes with docetaxel in breast cancer treatment.

BACKGROUND: Although trastuzumab and other HER2-targeted therapies have significantly improved survival in patients with HER2 overexpressed or amplified (HER2+) breast cancer, a significant proportion of patients do not respond or eventually develop clinical resistance. Strategies to reverse trastuzumab resistance remain a high clinical priority. We were the first to report the role of CXCR4 in trastuzumab resistance. The present study aims to explore the therapeutic potential of targeting CXCR4 and better understand the associated mechanisms. METHODS: Immunofluorescent staining, confocal mic

Development of dansyl based copper(ii) complex to detect hydrogen sulfide in hypoxia.

Hydrogen sulfide (H2S) has been reported as a gaseous signaling molecule in cells. H2S modulation is dependent on the partial pressure of oxygen in cells, which means hypoxia can induce H2S production under various pathophysiological conditions. Hypoxia is a common condition in solid tumors and can lead to malignant tumors that may become aggressive and result in worse prognosis. We designed and synthesized probe Cu-CD for H2S detection under hypoxia conditions. It is selective and sensitive toward various biological thiols, reactive nitrogen species (RNS), and reactive oxygen species (ROS). T

Defining the in vivo characteristics of acute myeloid leukemia cells behavior by intravital imaging.

The majority of acute myeloid leukemia (AML) patients have a poor response to conventional chemotherapy. The survival of chemoresistant cells is thought to depend on leukemia-bone marrow (BM) microenvironment interactions, which are not well understood. The CXCL12/CXCR4 axis has been proposed to support AML growth but was not studied at the single AML cell level. We recently showed that T-cell acute lymphoblastic leukemia (T-ALL) cells are highly motile in the BM; however, the characteristics of AML cell migration within the BM remain undefined. Here, we characterize the in vivo migratory beha

Targeting the biophysical properties of the myeloma initiating cell niches: a pharmaceutical synergism analysis using multi-scale agent-based modeling.

Multiple myeloma, the second most common hematological cancer, is currently incurable due to refractory disease relapse and development of multiple drug resistance. We and others recently established the biophysical model that myeloma initiating (stem) cells (MICs) trigger the stiffening of their niches via SDF-1/CXCR4 paracrine; The stiffened niches then promote the colonogenesis of MICs and protect them from drug treatment. In this work we examined in silico the pharmaceutical potential of targeting MIC niche stiffness to facilitate cytotoxic chemotherapies. We first established a multi-scal

Steady-state mobilization with on-demand plerixafor after CD38 antibody-based induction in multiple myeloma patients.

BACKGROUND: High-dose chemotherapy followed by autologous stem cell transplantation (ASCT) remains the standard of care for fit patients with newly diagnosed multiple myeloma (MM). The increasing use of CD38 antibody-based quadruplet induction regimens such as daratumumab-VTd (Dara-VTd) has raised concerns regarding impaired stem cell mobilization. STUDY DESIGN AND METHODS: We conducted a retrospective single-center analysis comparing steady-state stem cell mobilization after Dara-VTd versus bortezomib-cyclophosphamide-dexamethasone (VCd) induction. [Role of CXCR4/STAT3 in mesenchymal stromal cell-mediated drug resistance of acute leukemia cells].

OBJECTIVE: To explore the role of CXCR4/STAT3 in mesenchymal stromal cell (MSC)-mediated drug resistance of AML cells. METHODS: AML cell lines U937 and KG1a and primary AML cells were co-cultured with MSC from bone marrow of healthy donors. The AML cell lines cultured alone were used as control. Apoptosis induced by mitoxantrone was measured by flow cytometry. Expression of CXCR4 and STAT3 protein were detected by Western blot. After incubated with STAT3 inhibitor Cucurbitacin I or CXCR4 antagonist AMD3100, the apoptosis of AML cells induced by mitoxantrone was evaluated. RESULTS: Apoptosis

Proper desensitization of CXCR4 is required for lymphocyte development and peripheral compartmentalization in mice.

Desensitization controls G protein-dependent signaling of chemokine receptors. We investigate the physiologic implication of this process for CXCR4 in a mouse model harboring a heterozygous mutation of the Cxcr4 gene, which engenders a desensitization-resistant receptor. Such anomaly is linked to the warts, hypogammaglobulinemia, infections, myelokathexis (WHIM) syndrome, a human rare combined immunodeficiency. Cxcr4(+/mutant(1013)) mice display leukocytes with enhanced responses to Cxcl12 and exhibit leukopenia as reported in patients. Treatment with CXCL12/CXCR4 antagonists transiently rever

Mobilization strategies in hard-to-mobilize patients with lymphoid malignancies.

Depending on definitions and patient characteristics, 5-40% of patients with lymphoid malignancies are hard-to-mobilize. Several premobilization factors such as previous chemotherapy (CT), disease, marrow infiltration and platelet count at the time of mobilization influence the efficacy of CD34(+) cell mobilization. In general, however, prediction of hard-to-mobilize patient is difficult. Postmobilization factors especially low blood CD34(+) counts are important in clinical practice as we have now ways to enhance mobilization at this point. Plerixafor combined with granulocyte-colony stimulati

Safe Peripheral Stem Cell Collection in Pediatric Sibling Donors: A Single-Centre Experience From South India.

BACKGROUND: Peripheral blood stem cell (PBSC) collection from healthy pediatric sibling donors is increasingly preferred for allogeneic hematopoietic stem cell transplantation (HSCT) because of higher yields and faster engraftment. However, challenges such as small circulating blood volumes, difficult venous access, and the need for sedation pose safety concerns, particularly in low-weight donors. Data from developing countries remain scarce. STUDY DESIGN AND METHODS: We retrospectively analyzed 35 healthy pediatric sibling donors who underwent PBSC mobilization and collection at a single cen

Pathological accumulation of aged neutrophils contributes to intestinal inflammation in inflammatory bowel disease.

Neutrophils play a dual role in inflammatory bowel diseases (IBD), contributing to both host defense and tissue damage. Among them, CXCR4+CD62Llo aged neutrophils have been implicated in chronic inflammation, though their precise involvement in the pathogenesis of IBD remains unclear. This study aimed to explore the functional characteristics and therapeutic potential of aged neutrophils in IBD. Samples of peripheral blood and colonic biopsies from 61 patients with IBD (47 with active disease, 14 with inactive disease) and 36 healthy controls were analyzed to quantify aged neutrophils. Plerixafor Plus G-CSF Versus Etoposide Combined With Cyclophosphamide and G-CSF for Autologous Stem Cell Mobilization in Multiple Myeloma in the Era of Novel Agents: A Single Center Observational Propensity Score-Matched Cohort Study.

Autologous stem cell transplantation (ASCT) is the standard of care for eligible patients with multiple myeloma (MM). The optimal stem cell mobilization strategy for these patients remains a matter of debate in the era of novel agents. To compare the efficacy and outcomes of peripheral blood stem cells mobilization with plerixafor (PXF) and granulocyte-colony stimulating factor (G-CSF) against etoposide combined with cyclophosphamide (EC) plus G-CSF in MM patients. We included 119 patients with MM who underwent stem cell mobilization in our center, 56 patients received PXF + G-CSF and 63 recei

Comparison of two autologous hematopoietic stem cell mobilization strategies in patients with multiple myeloma: CE plus G-CSF versus G-CSF only: A single-center retrospective analysis.

BACKGROUND: Despite recent advances in the treatment of multiple myeloma, high-dose chemotherapy followed by autologous hematopoietic stem cell transplantation (ASCT) remains an essential therapeutic keystone. As for the stem cell mobilization procedure, different regimens have been established, usually consisting of a cycle of chemotherapy followed by application of granulocyte-colony stimulating factor (G-CSF), although febrile neutropenia is a common complication. Following national guidelines, our institution decided to primarily use G-CSF only mobilization during the COVID-19 pandemic to

Cxcr4 and Sdf-1 are critically involved in the formation of facial and non-somitic neck muscles.

The present study shows that the CXCR4/SDF-1 axis regulates the migration of second branchial arch-derived muscles as well as non-somitic neck muscles. Cxcr4 is expressed by skeletal muscle progenitor cells in the second branchial arch (BA2). Muscles derived from the second branchial arch, but not from the first, fail to form in Cxcr4 mutants at embryonic days E13.5 and E14.5. Cxcr4 is also required for the development of non-somitic neck muscles. In Cxcr4 mutants, non-somitic neck muscle development is severely perturbed. In vivo experiments in chicken by means of loss-of-function approach ba

Poor Mobilization in T-Cell-Deficient Nude Mice Is Explained by Defective Activation of Granulocytes and Monocytes.

It has been reported that both SCID mice and SCID patients poorly mobilize hematopoietic stem/progenitor cells (HSPCs) in response to granulocyte colony-stimulating factor (G-CSF). This defect has been proposed to result from a lack of naturally occurring IgM immunoglobulins to trigger activation of the complement cascade (ComC) and release of C5 cleavage fragments crucial in the mobilization process. However, SCID individuals also have T-cell deficiency, and T cells have been shown to modulate trafficking of HSPCs. To learn more about the role of T lymphocytes, we performed mobilization studi

Plerixafor may treat intractable post-herpetic neuralgia.

Varicella-zoster virus (VZV) causes varicella (chicken pox) and establishes latency in ganglia. A reactivation of latent VZV leads to herpes zoster (shingles). Herpes zoster often causes herpetic pain that can last for months or years after the rash has healed. Prolonged herpetic pain is defined as post-herpetic neuralgia (PHN). There is an unmet need to explore novel therapeutic approaches for intractable PHN. Postmortem studies have shown that VZV induces neuro-inflammation and damage to the ganglia and spinal cord. These pathological changes may be critical factors resulting in PHN. Accumul

CXCR4 antagonism increases T cell trafficking in the central nervous system and improves survival from West Nile virus encephalitis.

The migration of lymphocytes into the CNS during viral encephalitis is hindered by the blood-brain barrier (BBB) such that most infiltrating cells remain localized to perivascular spaces. This sequestration of leukocytes away from the parenchyma is believed to protect the CNS from immunopathologic injury. Infections of the CNS with highly cytopathic neurotropic viruses, such as West Nile virus (WNV), however, require the parenchymal penetration of T lymphocytes for virus clearance and survival, suggesting that perivascular localization might hinder antiviral immune responses during WNV encepha

Plerixafor and G-CSF Versus Placebo and G-CSF in Moderately Mobilizing Patients With Multiple Myeloma to Optimize Hematopoietic Stem Cell Collection Results (OPTIMIZE).

Efficient collection of hematopoietic progenitor cells (HPCs) is essential for autologous stem cell transplantation (ASCT) in multiple myeloma (MM). Although plerixafor is routinely used in poor mobilizers, its role in patients with intermediate mobilization capacity remains unclear. In this randomized, double-blind, placebo-controlled phase IV trial, MM patients with peripheral blood (PB) CD34+ counts of 15-40 cells/μL after steady-state G-CSF mobilization were randomized to receive plerixafor or placebo. The primary endpoint was successful collection of ≥ 6.0 × 106 CD34+ cells/kg, sufficient

Pharmacological Targeting of CXCR4 Attenuates Sepsis-Induced Intestinal Injury by Suppressing NLRP3/GSDMD-Mediated Pyroptosis.

Sepsis-induced intestinal barrier dysfunction is a critical driver of multiple organ failure and associated mortality. Although pyroptosis contributes to sepsis pathogenesis, the upstream receptors that trigger this process and constitute viable therapeutic targets remain elusive. Here, we identify C-X-C chemokine receptor type 4 (CXCR4) as a key upstream regulator of intestinal epithelial pyroptosis during septic injury. In a murine cecal ligation and puncture (CLP) model, sepsis robustly upregulated CXCR4 expression, which subsequently induced NLRP3 upregulation, caspase-1 activation, and ge
