This paper describes the treatment of a severe case of acute mountain sickness with a portable hyperbaric chamber. A 37-year old climber was treated for acute high altitude pulmonary oedema, which developed on the North Col of Mount Everest, at an altitude of 7,060 m. The treatment in the portable Gamow bag hyperbaric chamber lasted two hours, with a bag pressure of 103 mm Hg (0.136 kg/cm2 or 2 psig) using ambient air, without the addition of oxygen. With this pressure increase, the hyperbaric chamber lowered the patient's effective ambient altitude from 6,050 to 4,400 m. The treatment was suc

Authors examined 43 caisson-miners with symptoms of the upper extremities because of suspicion of hand-arm vibration syndrome. Also vibration measurements were performed on the pneumatic hammer used by the workers. The acceleration of the vibration exceeded 2.5-3.5 times the maximum allowable level according to the ISO 5349. Symptoms and signs of hand-arm vibration syndrome were found in 39 cases (90.7%). The vascular, peripheral neurological and locomotor system of the upper extremities were affected in similar frequency: 54.8; 51.6 and 51.2%, respectively. The most common angiological altera

INTRODUCTION: The aim of this review was to synthesise evidence on ear, nose, and throat (ENT) and related head and neck complications in scuba divers, with emphasis on mechanisms of injury, dive phase-specific risk patterns, and implications for clinical management and fitness-to-dive decision-making.
METHODS: A structured literature search of PubMed, Embase (Ovid), and the Cochrane Library identified systematic reviews, clinical trials, cohort studies, and case series reporting clinically relevant ENT outcomes in scuba divers. Animal studies, freediving studies, and papers focused solely on

We reviewed data on 43 patients with gastroschisis treated at The Hospital for Sick Children, London, between 1959 and 1975. In the past five years, the overall mortality has been reduced from 76% to 33%. The introduction of the Silastic silo has been the most important single factor in this improvement. Mortality may be further reduced by achieving all-layer closure following thorough bowel decompression and stretching of the abdominal wall.

Patent foramen ovale (PFO) is present in 15%-30% of the general population and has been associated with various pathologic states, including cryptogenic stroke, platypnea-orthodeoxia syndrome, decompression sickness and migraine with auras. Transesophageal echocardiography (TEE) has a major role in the diagnostic evaluation of PFO, as well as in the post-procedural assessment after transcatheter closure. The goals of this article were to synthesize the echocardiographic transesophageal techniques required for accurate PFO diagnosis and careful anatomic assessment of its anatomic variants, to f

Whilst underwater, the body is submitted to significant variations of the surrounding pressure according to the depth. These conditions modify the hemodynamic and the ventilatory mechanics considerably. Some repercussions, like pulmonary barotrauma, are related to simple physical phenomena. Others, like decompression sickness, are due to more com- plex processes. Breath-hold diving disrupts haematosis and can be complicated by alveolar haemorrhage and loss of consciousness. Acute pulmonary oedema during scuba-diving, breath-hold diving and swimming has been reported more recently. In case of p

The cause of right-to-left atrial shunting despite normal intracardiac pressures and normal or near-normal pulmonary function through a patent foramen ovale has still not been completely clarified. It is probably responsible for several linked diseases, such as paradoxical embolism, platypnea-orthodeoxia syndrome, migraine with aura, transient global amnesia, and decompression sickness in sport divers. Despite modern diagnostic methods, the underlying anatomophysiologic and pathogenic mechanisms of right-to-left atrial shunting without abnormal intracardiac pressures remain a matter of debate

Eccentric upper-body exercise performed 24 h prior to high-altitude decompression has previously been shown to aggravate venous gas emboli (VGE) load. Yet, it is unclear whether increasing the muscle mass recruited (i.e., upper vs. whole-body) during eccentric exercise would exacerbate the decompression strain. Accordingly, this study aimed to investigate whether the total muscle mass recruited during eccentric exercise influences the decompression strain. Eleven male participants were exposed to a simulated altitude of 24,000 ft for 90 min on three separate occasions. Twenty-four hours before

Closed-circuit rebreathers have opened new frontiers in technical diving, but rebreathers are not invulnerable and therefore a 'bailout' gas supply is required. For extreme dives, open-circuit bailout is logistically impossible. This has led teams to adopt dual rebreather configurations, where a second rebreather serves as bailout. This article presents operational experiences from the Wetmules and COBRA Divers, who independently developed dual rebreather practices for extended range diving. The teams evolved contrasting approaches through extensive field testing. The Wetmules initially adopte

INTRODUCTION: Venous gas emboli (VGE) detected via ultrasound can be used as a surrogate marker for decompression stress. While Doppler ultrasound is the historical gold standard, two-dimensional (2D) ultrasonography offers advantages for on-site monitoring, including a wider field of view and reduced dependence on noise-free environments. This systematic review evaluates 2D ultrasonography protocols used in decompression research since the 2015 International Meeting on Ultrasound for Diving Research, identifying methodological similarities, differences, and adherence to consensus recommendati

INTRODUCTION: Diving injuries are influenced by a multitude of factors. Literature analysing the full chain of events in diving accidents influencing the occurrence of diving injuries is limited. A previously published 'chain of events analysis' (CEA) framework consists of five steps that may sequentially lead to a diving fatality. This study applied four of these steps to predominately non-lethal diving injuries and aims to determine the causes of diving injuries sustained by divers treated by the Diving Medical Centre of the Royal Netherlands Navy.
METHODS: This retrospective cohort study w

Rothera is a British Antarctic Survey research station located on Adelaide Island adjacent, to the Antarctic Peninsula. Diving is vital to support a long-standing marine science programme but poses challenges due to the extreme and remote environment in which it is undertaken. We summarise the diving undertaken and describe the medical measures in place to mitigate the risk to divers. These include pre-deployment training in the management of emergency presentations and assessing fitness to dive, an on-site hyperbaric chamber and communication links to contact experts in the United Kingdom for

BACKGROUND: Scuba diving is an increasingly popular activity that involves the use of specialized equipment and compressed air to breathe underwater. Scuba divers are subject to the physiological consequences of being immersed in a high-pressure environment, including, but not limited to, increased work of breathing and kinetic energy expenditure, decreased fluid absorption, and alteration of metabolism. Individual response to these environmental stressors may result in a differential risk of decompression sickness, a condition thought to result from excess nitrogen bubbles forming in a diver'

INTRODUCTION: Venous gas emboli (VGE) are widely used as a surrogate endpoint instead of decompression sickness (DCS) in studies of decompression procedures. Peak post-dive VGE grades vary widely following repeated identical dives but little is known about how much of the variability in VGE grades is proportioned between-diver and within-diver.
METHODS: A retrospective analysis of 834 man-dives on six dive profiles with post-dive VGE measurements was conducted under controlled laboratory conditions. Among these data, 151 divers did repeated dives on the same profile on two to nine occasions s

Doppler ultrasound (DU) measurements are used to detect and evaluate venous gas emboli (VGE) formed after decompression. Automated methodologies for assessing VGE presence using signal processing have been developed on varying real-world datasets of limited size and without ground truth values preventing objective evaluation. We develop and report a method to generate synthetic post-dive data using DU signals collected in both precordium and subclavian vein with varying degrees of bubbling matching field-standard grading metrics. This method is adaptable, modifiable, and reproducible, allowing

Pulmonary barotrauma is a kind of disease caused by the injury of lung tissue or blood vessel when the gas pressure of lung is too high or too lower than the external pressure of the body, which causes the air to enter the blood vessel and adjacent tissue. It could be happened in the escape of the divers with the light diving equipment or the sailors from submarine. Generally, the decompression chamber was used to treating the disease, and the minimum air pressure of 0.5 MPa recompression therapeutic schedule was used to selecting. In November 2019, a patient with pulmonary barotrauma combined

The development of osteonecrosis after exposure to altered air pressures is consistent with cellular injury brought about by active oxygen species. The syndrome is considered to arise as a result of an unusual combination of circumstances in which hyperoxia itself, together with the additive responses of the endocrine system to hyperoxia, hypothermia and exertion, each appear to play a part; the net result is thought to increase the mitochondrial generation of superoxide. It is suggested that effective prophylaxis may be possible primarily by establishing a nutritional status that is adequate
