Expanding access to prehospital blood products: a narrative review of clinical evidence, systems barriers, and statewide strategies.

Traumatic hemorrhage remains a leading cause of preventable death in the USA, and early administration of blood products improves outcomes in hemorrhagic shock. However, access to prehospital transfusion on ground ambulances remains limited and uneven. This narrative review synthesizes current clinical evidence, implementation barriers, and emerging models for prehospital blood use in the USA, with a focus on the potential role of statewide coordination within trauma systems. A targeted review of peer-reviewed literature, consensus guidelines, and system implementation reports was conducted, e

Lifetime abortion incidence when abortion care is covered by Medicaid: Maryland versus five comparison states.

OBJECTIVE: To estimate the association of Medicaid coverage of abortion care with cumulative lifetime abortion incidence among women insured by Medicaid. DATA SOURCES AND STUDY SETTING: We use 2016-2019 (Pre-Dobbs) data from the Survey of Women studies that represent women aged 18-44 living in six U.S. states. One state, Maryland, has a Medicaid program that has long covered the cost of abortion care. The other five states, Alabama, Delaware, Iowa, Ohio, and South Carolina, have Medicaid programs that do not cover the cost of abortion care. Our sample includes 8972 women residing in the study

Impact of medialization laryngoplasty on dynamic nanomechanical vocal fold structure properties.

OBJECTIVES/HYPOTHESIS: Although the primary goal of medialization laryngoplasty is to improve glottic closure, implant placement is also likely to alter the biomechanical properties of the vocal fold (VF). We sought to employ novel, nanoscale technology to quantify these properties following medialization based on the hypothesis that different medialization materials will likely yield differential biomechanical effects. STUDY DESIGN: Ex vivo. METHODS: Nine pig larynges were divided into three groups: control, Silastic (Dow Corning, Midland, Michigan, U.S.A.) block medialization, or Gore-Tex

Incidence and management of cerebrospinal fluid fistulas in 336 multilevel laminectomies with noninstrumented fusions.

BACKGROUND: The incidence (e.g., 3-27%) and the types of cerebrospinal fluid (CSF) fistulas occurring during multilevel lumbar laminectomy with noninstrumented spinal fusions varies. METHODS: From 2000 to 2015, we retrospectively evaluated the incidence/etiologies of CSF fistulas occurring for 336 patients undergoing average 4.7 laminectomies with average 1.4 level noninstrumented fusions over a 15 year period. The varied etiologies of CSF leaks included; ossification of the yellow ligament (OYL) extending through the dura, postoperative surgical scar, iatrogenic traumatic leak, epidural ster

Trends in Cerebrovascular Disease-Related Mortality and Disparities in the United States: A 25-Year National Cross-Sectional Analysis.

Cerebrovascular disease remains a leading cause of death in the United States, yet recent mortality trends and inequities across population subgroups remain incompletely characterized. Annual age-adjusted mortality rates (AAMRs; per 100,000 population) for cerebrovascular disease (ICD-10: I60-I69) were extracted from the CDC WONDER Multiple Cause of Death database for 1999-2023, standardized to the 2000 U.S. population. Temporal patterns were summarized using segmented log-linear Joinpoint models and reported as annual percent change (APC) and average annual percent change (AAPC) with 95% conf

The Value of Social Determinants of Health Needs Assessments in Primary Care.

Social determinants of health (SDOH) such as income, housing, food security, education, transportation, and healthcare access, exert a far greater influence on health outcomes compared with traditional clinical care alone. This policy commentary argues for the routine integration of SDOH needs assessments into primary care to enable more comprehensive, equitable, and effective patient care. By screening for social drivers, providers can identify non-medical obstacles to treatment adherence, connect patients with community resources, and develop realistic care plans tailored to individual circu

Same name, different drugs: variability of fentanyl ('dope') products sold under the same label.

BACKGROUND: Unregulated opioid markets are increasingly characterized by complex and variable drug compositions. In North-eastern and Mid-Atlantic US street opioid markets, fentanyl-containing powders are commonly sold in stamped glassine bags. These "stamps" function as branding and are often used by people who use drugs, harm reduction programs and public health agencies as informal indicators of product composition or risk. This study quantified the extent to which stamp identity is associated with the presence and concentration of fentanyl and co-occurring compounds. METHODS: We analyzed

Penalizing underage alcohol use is associated with lower mortality for young drivers: Use/lose laws and their association with motor vehicle collision mortality.

BACKGROUND: Motor vehicle collisions (MVC) continue to be a leading cause of mortality for youth in the United States. Since 2010, seven states have revoked mandatory laws that suspended licenses for underage alcohol use, also known as use/lose laws. This study analyzed whether each state's policy change was associated with increased youth MVC mortality. METHODS: State mortality data for youth ages 15 years to 20 years in MVCs involving a young driver (i.e., ages 15-20 years) were obtained from the Fatality Analysis Reporting System. Population data was retrieved from the Centers for Disease

Racial disparities in triple negative breast cancer: toward a causal architecture approach.

BACKGROUND: Triple negative breast cancer (TNBC) is an aggressive subtype of invasive breast cancer that disproportionately affects Black women and contributes to racial disparities in breast cancer mortality. Prior research has suggested that neighborhood effects may contribute to this disparity beyond individual risk factors. METHODS: The sample included a cohort of 3316 breast cancer cases diagnosed between 2012 and 2020 in New Castle County, Delaware, a geographic region of the US with elevated rates of TNBC. Multilevel methods and geospatial mapping evaluated whether the race, income, an

A graphene-based hydrogel monolith with tailored surface chemistry for PFAS passive sampling.

Aquatic contamination by per- and polyfluorinated alkyl substances (PFAS) has attracted global attention due to their environmental and health concerns. Current health advisories and surface water regulatory limits require PFAS detection in the parts per trillion (ppt) range. One way to achieve those low detection limits is to use a reliable passive sampling-based monitoring tool for PFAS, as exists for numerous nonpolar persistent organic pollutants. Here we introduce a new graphene-based hydrogel monolith and describe its synthesis, chemical functionalization, property characterization, and

Gender of offspring and maternal ovarian cancer risk.

OBJECTIVE: A single live birth compared to nulliparity significantly reduces the risk for ovarian cancer, but exactly how pregnancy reduces ovarian cancer risk is unknown. We sought to determine whether offspring gender, which differentially alters maternal hormonal milieu, may be associated with maternal ovarian cancer risk. METHODS: Parous women (n = 511) with incident ovarian cancer were compared to parous community controls (n = 1136) participating in a population-based case-control study of ovarian cancer (Delaware Valley, 1994-1998). In subgroup specific models for women with one, two,

Utilization of Integrated and Colocated Behavioral Health Models in Pediatric Primary Care.

Integrating behavioral health services within pediatric primary care may help address barriers to these services for youth, especially the underserved. Models of primary care behavioral health include coordinated, colocated, integrated, and collaborative care. This study began exploring the comparative utility of these models by investigating differences in the demographics and diagnoses of patients seen for a behavioral health warm handoff (integrated model) and a scheduled behavioral health visit (colocated model) across 3 pediatric primary care sites. The 3 sites differed in their rates of
