Clinical course and personality in reactive, compared with nonreactive, delusional disorder.

OBJECTIVE: Reactive delusional disorder (DD) (with a precipitating factor) has been postulated to differ clinically from nonreactive DD and to show a better prognosis. Our study tests this hypothesis in a sample of patients with persistent DD (International Classification of Diseases, 10th Revision) or DD (Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition) followed during a period of more than 10 years. METHOD: As part of a long-term study on DD, 19 patients with DD and a stressful life event preceding the onset of the disorder were compared with 24 DD patients without suc

The Severity of Personality Pathology: A Risk Factor for Concurrent Substance Use Disorders in Alcohol Use Disorder.

Objective: Co-occurrence of substance use disorders (CO-substance use disorders) among individuals with alcohol use disorder (AUD) is largely recognized as a critical clinical issue. However, the specific clinical variables involved are still unclear. The recent findings are controversial in pointing out the unique contribution of both impulsivity and emotion dysregulation on CO-substance use disorders. Furthermore, the co-variation between AUD and other substance use disorders includes different aspects of maladaptive personality functioning (i.e., overall severity and specific features). The

An investigation of doubt in obsessive-compulsive disorder.

BACKGROUND: Clinicians have long considered doubt to be a fundamental characteristic of obsessive-compulsive disorder (OCD). However, the clinical relevance of doubt in OCD has not been addressed. METHODS: Participants included 1182 adults with OCD who had participated in family and genetic studies of OCD. We used a clinical measure of the severity of doubt, categorized as none, mild, moderate, severe, or extreme. We evaluated the relationship between doubt and OCD clinical features, Axis I disorders, personality and personality disorder dimensions, impairment, and treatment response. RESULT

[Affective dependency].

Affective dependency is characterized by emotional distress (insecure attachment) and dependency to another person with a low self-esteem and reassurance need. The paper proposes a reflection on the definition of emotional dependency and the confusion caused by various denominations. Overprotective and authoritarian parenting, cultural and socio-environmental factors may contribute to the development of dependent personality. Psychological epigenetic factors, such as early socio-emotional trauma could <imprint>> on neuronal circuits in prefronto-limbic regions that are essential for emotional

Impulsivity and aggression as predictors of suicide attempts in alcoholics.

INTRODUCTION: The aim of this study was to assess the potential role of impulsive and aggressive behavior in the pathogenesis of suicide attempts in alcoholics. Impulsive and aggressive behavior as well as a psychiatric comorbidity with depressive conditions and personality disorders have been reported to be significant risk factors for suicide attempts in alcoholics. We hypothesized that alcoholics with a history of violent suicide attempts show an increased level of impulsive and aggressive behavior. Furthermore, the potential influence of concurrent personality disorders and depressive cond

[Passive-aggressive personality and dependent personality-- current status of the question].

The author makes a critical review of the literature concerning dependent and passive-aggressive personalities. After putting them in a historic perspective, he presents the various etiologic hypotheses. Special attention is given to the clinical and therapeutic aspects. The author concludes that despite the supposed high incidence of these two personality disorders, the reliability of the diagnoses and the natural history of the disorders remain to be explored in a prospective and systematic manner, before definite conclusions can be drawn regarding therapy.

From dysfunction to adaptation: an interactionist model of dependency.

Contrary to clinical lore, a dependent personality style is associated with active as well as passive behavior and may be adaptive in certain contexts (e.g., in fostering compliance with medical and psychotherapeutic treatment regimens). The cognitive/interactionist model conceptualizes dependency-related responding in terms of four components: (a) motivational (a marked need for guidance, support, and approval from others); (b) cognitive (a perception of oneself as powerless and ineffectual); (c) affective (a tendency to become anxious when required to function autonomously); and (d) behavior

Moderators and predictors of outcome in metacognitive and cognitive behavioural therapy for co-morbid anxiety disorders.

This study aimed to investigate whether two theoretically derived moderators of treatment, degree of worry and avoidance at pretreatment, moderated anxiety from pretreatment to post-treatment in a randomized controlled trial comparing metacognitive therapy and cognitive behavioural therapy. Personality problems, degree of co-morbidity, and demographic characteristics (work status and education) were also investigated. Seventy-four patients with a primary diagnosis of post-traumatic stress disorder, social phobia, or panic disorder with and without agoraphobia were analysed using multilevel mod

[Psychodynamic Oriented Multimodal Therapy for Inpatients with Borderline Personality Disorder: Effectiveness, Response, and Dropout].

Psychodynamically oriented multimodal therapy approaches are efficacious for outpatients with borderline personality disorder (BPD). Until now, no study has reported the effectiveness, response, and dropout in a psychodynamic oriented multimodal therapy program for inpatients (PDOMT). In this study, we compared the results of 269 individuals seeking a 3-month inpatient treatment with previous studies for BPD inpatients by examining 269 individuals. The Borderline Personality Index (CUT-20-R) was used as the main outcome measure. Therapy outcome was defined in effect size (ES), response, and re

[Philippe V's biographic misfortunes, first Spanish Bourbon].

The enlightened magic showed off by Philippe V during the first stage of his reign, was shattered because of two basic biographic misfortunes: first, to contract second marriage with Isabel Farnesio, a biting female personality that turned him into a sexual and emotional dependent person; second, the bipolar disorder associated with other mental disorders, above all obsessive personality and Cotard's syndrome. The bipolar disorder was characterized according to the prevalence of melancholic episodes. In this way, the opposite pole appeared only in form of hipomanic rises as reaction to a warli

Schemas, affect consciousness, and cluster C personality pathology: a prospective one-year follow-up study of patients in a schema-focused short-term treatment program.

In this prospective study the aim was to investigate the relationship between affect consciousness and Cluster C personality pathology (DSM-IV, Axis-II). Forty-four patients with panic disorder and/or agoraphobia and Cluster C personality traits were treated in a schema-focused program comprising a first panic/agoraphobia-focused phase and a second personality-focused phase, being finally assessed at a one-year follow-up. According to the treatment strategy, affect consciousness was expected to change during the second phase, independent of change in agoraphobic avoidance being focused in the

Dependency and suicidality in addictive disorders.

Excessive interpersonal dependency has been described in depression and addictive disorders. Moreover, excessive dependency and suicidality are linked in psychiatric subjects, but their relationships have not been studied in specific addictions. Separate samples of female anorectic patients (n=150), female bulimic patients (n=95), male (n=150) or female (n=68) alcoholics, male (n=94) or female (n=54) drug abusers and non-psychiatric control subjects (n=683) were included in the study. On the basis of a structured interview, suicidal ideations, number of previous suicide attempts and diagnoses

Personality traits and complex problem solving: Personality disorders and their effects on complex problem-solving ability.

Complex problem solving (CPS) can be interpreted as the number of psychological mechanisms that allow us to reach our targets in difficult situations, that can be classified as complex, dynamic, non-transparent, interconnected, and multilayered, and also polytelic. The previous results demonstrated associations between the personality dimensions neuroticism, conscientiousness, and extraversion and problem-solving performance. However, there are no studies dealing with personality disorders in connection with CPS skills. Therefore, the current study examines a clinical sample consisting of peop

Personality and psychiatric disorders in women affected by polycystic ovary syndrome.

BACKGROUND: Polycystic ovary syndrome (PCOS) is the most prevalent endocrine disorder among fertile women. Studies show reduced quality of life, anxiety, depression, body dissatisfaction, eating disorder, and sexual dysfunction, but the etiology of these disturbs remains still debated. The aim of our study is to verify whether this hyperandrogenic syndrome characterizes a strong psycho(patho)logical personality. METHOD: Sixty PCOS subjects (mean age 25.8 ± 4.7 years) were evaluated by anthropometric, metabolic, hormonal, clinical, and psychological parameters. After the certainty of the diagn

Hypnotizability and Disordered Personality Styles in Cluster A Personality Disorders.

AIM: Interpersonal sensitivity and mistrust are the main characteristics of cluster A personality disorders (CAPD) which might be due to the high accessibility to negative suggestions from environments. Yet the exact associations between hypnotic suggestibility and their personality disorder functioning styles remain unclear. METHODS: We invited 36 patients with CAPD and 115 healthy volunteers to undergo the Stanford Hypnotic Susceptibility Scale: Form C (SHSS:C) and Parker Personality Measure (PERM). RESULTS: Compared to controls; patients scored higher on PERM paranoid; schizoid; schizotyp

Antisocial personality by proxy: the Norton-Sims syndrome.

An antisocial personality disorder by proxy is defined by a proposed set of diagnostic criteria and a general description of proxy and perpetrator characteristics. Subtypes, dynamics, and features of this proposed disorder are described, and five-factor personality model (Costa & Widiger, 1994) loadings for the proxy and perpetrator are hypothesized. The relationship to abuse trauma and the five-factor personality model are discussed along with implications and suggestions for future research.

DSM-5 Personality Disorders and Traits in Patients With Severe Health Anxiety.

Severe health anxiety (SHA)/hypochondriasis (HY) is often associated with personality pathology; however, studies report inconsistent results. In general populations, 12% have a personality disorder (PD). We assessed physician-referred psychiatric outpatients with SHA enrolled for a treatment study (n = 84) with the Structured Clinical Interview for Diagnostic and Statistical Manual of Mental Disorders, 4th Edition (DSM-IV) axis II (SCID-II), Personality Inventory for DSM-5 (PID-5), Whiteley Index 7, and Short Health Anxiety Inventory, and the healthy controls (n = 84) with PID-5 only.
