Extraction of cardiac implantable electrical devices (CIED) is becoming an increasing necessity. Infection is among the most frequent and serious complications associated with the surge of CIED implants. We describe the extraction of an infected cardiac resynchronization therapy-defibrillator (CRT-D) in a patient with a tunneled implantable cardioverter defibrillator (ICD) lead implanted from the right jugular vein.

Left ventricular (LV) perforation caused by an implantable cardioverter-defibrillator (ICD) lead is a rare but potentially fatal complication. We describe the case of a 34-year-old man with hypertrophic cardiomyopathy who underwent single-chamber ICD implantation for primary prevention of sudden cardiac death using an active-fixation transvenous lead positioned in the right ventricular septum. Four days after implantation, he developed acute chest pain, dyspnoea, hypotension, with clinical signs of evolving cardiac tamponade. Transthoracic echocardiography demonstrated a moderate circumferenti

INTRODUCTION: Whole blood is the established matrix for biological monitoring of inorganic lead; however blood sampling is an invasive procedure. Saliva offers a potential non-invasive alternative. This study determines lead in whole blood and saliva. A novel method for saliva sampling and preparation is presented.
METHODS: Paired blood and saliva samples were obtained from 105 occupationally exposed UK workers. Saliva was collected using a StatSure sampling device, and a nitric acid digestion step was incorporated. The utility of the device for this application was evaluated. Whole blood was

INTRODUCTION: Durable left ventricular assist devices (LVADs) are susceptible to infection. The presence of a transvenous cardiac implantable electronic device (CIED) may further elevate this risk, making it necessary to remove the CIED system. This report presents the procedural characteristics and outcomes of CIED system extraction in patients with LVADs.
METHODS: This is a single-center retrospective study that included patients with durable LVADs who underwent transvenous CIED extraction at our institution from 2017 to 2023.
RESULTS: Twenty patients with LVAD (HeartMate II = 14; HeartMat

AIMS: During transvenous lead extraction (TLE) longer dwelling time often requires the use of powered sheaths. This study aimed to compare outcomes with the laser and powered mechanical tools.
METHODS AND RESULTS: Single-centre data from consecutive patients undergoing TLE between 2012 and 2021 were retrospectively analysed. Efficacy and safety of the primary extraction tool were compared. Procedures requiring crossover between powered sheaths were also analysed. Moreover, we examined the efficacy of each level of the stepwise approach. Out of 166 patients, 142 (age 65.4 ± 13.7 years) underwe

The illegal adulteration of sulfur dioxide in natural healthcare products may lead to serious health problems, which raise an urgent demand of straightforward approach for detecting sulfur dioxide. In this paper, surface-enhanced Raman scattering (SERS) sensor with sample preparation apparatus for headspace adsorption of SO2 has been developed, which was successfully applied to detect illegal adulteration of sulfur dioxide in traditional Chinese medicine (TCM). Functional membrane substrate of Si@Ag@PEI composite was synthesized to enhance the adsorption and Raman signal of SO2. A 3D-printed h

The increasing use of cardiac implantable electronic devices has led to a rise in transvenous lead extractions (TLEs). The most common indications for TLE are infections and lead malfunction. Over time, leads develop fibrotic encapsulation that makes extraction difficult and increases the risk of myocardial injury. Given the thin myocardial wall, particularly in the right atrial appendage and right ventricular apex, the use of advanced powered tools near the lead tip is contraindicated, posing additional challenges. Electrosurgical energy (EE) has been explored as an adjunct to manual extracti

The number of infections related to cardiac implantable electronic devices (CIEDs) has increased as the number of devices implanted around the world has grown exponentially in recent years. CIED complications can sometimes be difficult to diagnose and manage, as in the case of lead-related infective endocarditis. We present the case of a 48-year-old male diagnosed with Staphylococcus aureus device-related infective endocarditis, 12 years after the implant of a single chamber pacemaker. A recent history of the patient includes two urinary catheterizations due to obstructive uropathy in the cont

Implantable medical devices, such as pacemakers, have significantly improved the quality of life for patients with cardiac conditions, allowing them to maintain active lifestyles. Nonetheless, these devices can present unique challenges when interacting with the wearer's physical activities, potentially leading to unforeseen complications. Here, we present a case of an 81-year-old male golfer, with a history of atrial fibrillation, congestive heart failure, and sick sinus syndrome, who experienced atrial lead noise from his pacemaker, exclusively triggered by his golf swing. This incident, whi

PURPOSE: The prepectoral approach is the procedure of choice for pacemaker or defibrillator (device) implantation. Epicardial or transiliac approaches are reserved for patients in whom the pectoral approach is not feasible. We studied the viability of the axillary subpectoral approach for implanting devices in patients in whom the standard prepectoral approach was not feasible.
METHODS: Devices and leads were extracted from 16 patients with infected devices in the prepectoral position. The contralateral site was unsuitable for reimplantation because of infection or inadequate venous access. O

Isolation and purification of nucleic acid is an essential step in molecular assays for several application areas including healthcare, food safety and security, environmental monitoring, forensic science, etc. Nucleic acid extraction is a critical bottleneck towards field deployable nucleic acid-based assays, limiting them to laboratory setups and bench-top configurations. In addition, this lack of portability leads to longer timelines for sample processing and time-to-results, and higher testing costs, limiting access to this highly sensitivity assay tool in many instances. Several efforts h
