Long-acting cabotegravir and rilpivirine in people with HIV and obesity: Real-world outcomes from the RELATIVITY cohort.

INTRODUCTION: Long-acting injectable cabotegravir and rilpivirine (LAI CAB + RPV) is a well-established regimen for people with HIV (PWH) that offers high efficacy and tolerability. However, data are limited for obese individuals with a body mass index (BMI) ≥ 30 kg/m2, which may represent a potential risk factor for virologic failure (VF). METHODS: We conducted a multicentre, ambispective study (RELATIVITY cohort, Spain) of virologically suppressed PWH with BMI ≥ 30 kg/m2 who switched to LAI CAB + RPV. This study characterized this population and evaluated the factors associated with virolog

Real-World Patterns of Discontinuation and Virologic Failure with Long-Acting Cabotegravir-Rilpivirine.

Long-acting injectable cabotegravir-rilpivirine (LA CAB/RPV) is an antiretroviral option for virologically suppressed people with HIV, but real-world discontinuation and failure patterns remain incompletely described. We retrospectively reviewed 201 adults who received LA CAB/RPV at a large academic infectious diseases clinic (January 2022-April 2025). Median time on therapy was 380 days (IQR 206-729). At the end of follow-up, 160 (80%) remained on LA CAB/RPV and 30 (15%) discontinued, most commonly due to insurance barriers (n = 8, 27%); adherence issues and patient preference each accounted

Role of long-acting injectables in perinatally acquired HIV, with persistent viraemia and non-nucleoside reverse transcriptase inhibitor resistance.

This case describes successful management of a man with perinatally acquired HIV complicated by incomplete adherence, multidrug-resistant HIV and opportunistic infections-cryptococcal meningitis and mycobacterium avium intracellulare. Initial treatment included antifungal and antimycobacterials with oral antiretroviral therapy (ART), but persistent high HIV viral load (>100 000 copies/mL) prompted initiation of long-acting injectable ART (LAI-ART) with cabotegravir and rilpivirine combined with adjunct oral nucleoside reverse transcriptase inhibitor therapy (emtricitabine/tenofovir). This comb

Patterns and Correlates of Initiation of Long-Acting Injectable Cabotegravir for Preexposure Prophylaxis: A Case-Control Analysis.

We conducted a case-control analysis to evaluate the determinants for initiating long-acting injectable cabotegravir (LACAB) as preexposure prophylaxis (PrEP) in a real-world setting with limited drug access. Individuals selected for LACAB experienced more side effects from oral PrEP, riskier sexual behavior, longer prior PrEP use, and a significantly higher incidence of STIs during follow-up. Despite this, LACAB was well tolerated, and discontinuation rates were low. Our findings support prioritizing LACAB for individuals most exposed to HIV and with low adherence to oral regimens, especially

Virological Failure After Switch to Long-Acting Cabotegravir and Rilpivirine Injectable Therapy: An In-depth Analysis.

BACKGROUND: Long-acting (LA) injectable therapy with cabotegravir (CAB) and rilpivirine (RPV) is currently used as maintenance treatment for human immunodeficiency virus type 1, and has a low risk for virological failure (VF). Although the risk is low, the circumstances and impact of VF in the real-world setting merit further evaluation. METHODS: We performed an in-depth clinical, virological, and pharmacokinetic analysis on the reasons behind and the impact of VF during LA CAB/RPV therapy in 5 cases from the Netherlands. Genotypic resistance testing was performed after the occurrence of VF,

Physiologically-based pharmacokinetic modelling of long-acting injectable cabotegravir and rilpivirine in pregnancy.

AIMS: Long-acting cabotegravir and rilpivirine have been approved to manage HIV in adults, but data regarding safe use in pregnancy are limited. Physiologically-based pharmacokinetic (PBPK) modelling was used to simulate the approved dosing regimens in pregnancy and explore if Ctrough was maintained above cabotegravir and rilpivirine target concentrations (664 and 50 ng/mL, respectively). METHODS: An adult PBPK model was validated using clinical data of cabotegravir and rilpivirine in nonpregnant adults. This was modified by incorporating pregnancy-induced metabolic and physiological changes.

Two-drug regimens for the treatment of HIV in Africa.

Two-drug regimens for the treatment of HIV are increasingly available. The oral regimen of dolutegravir plus lamivudine is recommended as a preferred option in multiple national guidelines but is not currently included in WHO HIV treatment guidelines nor widely used in Africa. Long-acting injectable cabotegravir and rilpivirine is being rolled out in the USA, Europe, and Australia but its use in sub-Saharan Africa is currently restricted to clinical trials. Given the increasing life expectancy, rising prevalence of non-communicable diseases, and resulting polypharmacy among people living with

Efficacy and safety of long-acting cabotegravir compared with daily oral tenofovir disoproxil fumarate plus emtricitabine to prevent HIV infection in cisgender men and transgender women who have sex with men 1 year after study unblinding: a secondary analysis of the phase 2b and 3 HPTN 083 randomised controlled trial.

BACKGROUND: Injectable cabotegravir was superior to daily oral tenofovir disoproxil fumarate plus emtricitabine for HIV prevention in two clinical trials. Both trials had the primary aim of establishing the HIV prevention efficacy of long-acting injectable cabotegravir pre-exposure prophylaxis (PrEP) compared with tenofovir disoproxil fumarate plus emtricitabine daily oral PrEP. Long-acting PrEP was associated with diagnostic delays and integrase strand-transfer inhibitor (INSTI) resistance. This report presents findings from the first unblinded year of the HIV Prevention Trials Network (HPTN)
