[Penetrating atheromatous ulcer of the thoracic aorta. Apropos of a case].

The authors report a case of penetrating atheromatous ulceration of the descending thoracic aorta complicated by a haemothorax. Atheromatous ulceration of the aorta is usually observed in elderly hypertensives. It is caused by rupture of the internal elastic layer under an atheromatous plaque extending into the media and resulting in a parietal haematoma of the aorta. Clinically, the differential diagnosis is that of a dissection of the aorta or of fissuration of an atheromatous aneurysm. Nowadays, the investigations of choice are a chest CT scan with injection of contrast and MRI. Transoesoph

Hybrid endograft for one-step treatment of multisegment disease of the thoracic aorta.

PURPOSE: At present, a two-step surgical approach is necessary to treat patients with coexistent pathologic conditions involving the proximal and descending thoracic aorta. A hybrid endograft is described here that enables such treatment during a single operation. MATERIALS AND METHODS: The Chavan-Haverich endograft consists of a Dacron vascular prosthesis with stainless-steel stents affixed at its distal end. After approval by the institutional review board, the endograft was prospectively implanted in 22 patients with multisegment thoracic aortic disease (13 men, nine women; median age, 64

Surgical strategies in managing organ malperfusion as a complication of aortic dissection.

Between December 1978 and March 1994, 48 of 312 patients who underwent surgery for aortic dissection were diagnosed with major vascular complications. There were 18 patients with type A dissection and 30 patients with type B. In 23 patients with acute dissection, the site of vascular obstruction was the abdominal aorta in 12 patients, brachiocephalic artery in 7, iliac artery in 4, left common carotid artery in 3 and thoracic aorta in 2. In 26 patients with chronic dissection, the site of vascular obstruction was the abdominal aorta in 13 patients, brachiocephalic artery in 10, renal artery in

Clinics in diagnostic imaging (182). Acute descending aortic dissection with aortic root retrograde extension.

A 77-year-old man presented with acute-onset severe chest pain radiating to the back and elevated blood pressure. Multiphasic computed tomography of the aorta revealed an intimal tear in the descending thoracic aorta which extended both retrograde to the aortic root and antegrade to the infra-renal abdominal aorta. The initial impression, that the images showed a Stanford type B aortic dissection, was because the portion of the false lumen that extended beyond the aortic arch remained unopacified even on delayed phases, making it challenging to assess the extent of the dissection flap. [Results of surgical treatment of various vessel diseases. 1. Great vessel diseases (author's transl)].

Three hundred and three cases of great vessel diseases have been experienced during the last 18 years at the Second Department of Surgery, Hokkaido University Hospital. Those cases consisted of 36 congenital anomalies of the aorta, 41 aortitis syndromes, 145 aneurysms of the thoracic and abdominal aorta, 20 obstructions of the aorta, 10 arterio-venous fistulas, and 51 obstructions of the great vein. The results of surgical treatment of those cases were reviewed. Although the results have progressively improved, special problembs to be solved are remained in treatment of complexed aortic coarct

Ten Year Clinical and Aortic Remodelling Outcomes following Endovascular Repair of Chronic Type B Aortic Dissection.

OBJECTIVE: Thoracic endovascular aortic repair (TEVAR) has increasingly replaced open surgery for chronic type B aortic dissection (cTBAD) with aneurysmal degeneration. However, TEVAR may promote less aortic remodelling in the chronic phase owing to persistent false lumen patency distally, and few long term outcome data exist in this setting. This study details ten year clinical and aortic remodelling outcomes following TEVAR for cTBAD. METHODS: This was a single centre, retrospective cohort study of patients undergoing TEVAR for cTBAD with aneurysmal degeneration of the descending thoracic a

[Recent advancement of surgical treatment of aneurysms of the thoracic aorta].

The recent advancement of surgical treatment for aneurysms of the thoracic aorta with special reference to the operative technique and adjunctive methods of distal aortic perfusion during aortic cross-clamping were reviewed. Between 1960 and July, 1991, 415 patients underwent operation for aneurysms of the thoracic aorta in our institution. The overall early mortality rates were 7.7% for the nonruptured aneurysms and 30.6% for the ruptured aneurysms during the last 10 years with recent establishment of mechanical adjuncts and refinement of operative technique. Composite graft replacement with

Acute Stanford type A aortic dissection associated with aortic coarctation repaired by Tirone E. David & debranching techniques with combined axillar and femoral perfusion: a case report.

We present the case of a 38-year-old male with a diagnosis of Stanford A aortic dissection and associated coarctation of the thoracic aorta. Acute dissection associated with coarctation of the aorta is a rare problem and difficult to manage surgically. Establishing a cardiopulmonary bypass (CPB) with adequate flows is the main objective of the procedure; optimal cannulation ensures the protection of cerebral and visceral organs. We successfully performed aortic valve re-implantation surgery (T. David Surgery), replacement of the ascending aorta and aortic arch, as well as debranching of the su

Acute intraoperative aortic dissection following axillary artery cannulation.

We describe a 75-year-old woman who underwent right axillary artery cannulation in preparation for reconstruction of the aortic arch and the proximal descending aorta for athesosclerotic aortic aneurysm via a 'clamshell' incision. As soon as cardiopulmonary bypass was established, the ascending aorta and the aortic arch was dissected. The innominate artery was dissected including one-third of its circumferance anteriorly. Arterial perfusion was stopped immediately and the left femoral artery was cannulated to resume CPB. We proceeded with replacement of the ascending aorta, the aortic arch and

Intraoperative retrograde type I aortic dissection in a patient with chronic type IIIb dissecting aneurysm.

Iatrogenic acute aortic dissection of the ascending aorta during cardiac surgery is a rare but potentially fatal complication. We describe the emergency repair of iatrogenic acute aortic dissection of the ascending aorta during distal arch replacement in a patient with a chronic type IIIb dissecting aneurysm. We scheduled distal arch and descending aortic aneurysm repair through a left anterolateral thoracotomy with a femoro-femoral bypass. While trimming the proximal suture line, retrograde aortic dissection occurred from the cross-clamped site to the aortic root. Transesophageal echocardiogr

Long-Term Longitudinal Computational Study of a Marfan Syndrome Patient After Hybrid Repair of Aortic Arch Dissection With Parallel Stent-Grafts.

Hybrid repair is a valuable alternative treatment for aortic arch disease in Marfan syndrome patients after proximal aorta replacement. This study aimed to investigate the long-term durability of this technique with the use of parallel stent-grafts and evaluate strategies to prevent abdominal aortic dilation. One Marfan syndrome patient who underwent hybrid aortic repair with parallel stent-grafts for arch dissection after the Bentall procedure was admitted. Five patient-specific three-dimensional models were reconstructed based on preoperative and follow-up computed tomography angiography sca

Management of a chronic Stanford type B dissection in a patient with a right-sided aortic arch.

We report the case of a 44-year-old male with a right-sided aortic arch who presented with dissection extending from his distal arch to his left common iliac artery. He was initially managed conservatively, but later became symptomatic with back pain; enlargement of the descending thoracic aorta was demonstrated on computed tomography scanning. He underwent staged repair of the aortic arch and descending thoracic aorta with the elephant trunk procedure - two operations one year apart, each time making a good postoperative recovery.

Type B Aortic Dissection Treated With a Branched Aortic Arch Stent Graft and the STABILISE Technique.

We report the case of a 57-year-old woman diagnosed with an asymptomatic chronic type B aortic dissection. The maximum aortic diameter was 70 mm in the proximal descending thoracic aorta. The entry tear was located at the aortic isthmus, and the proximal neck included all of the supra-aortic trunks. The targeted proximal neck was ≥ 25 mm. The dissection extended to the infrarenal aorta. The patient was treated with a custom branched aortic graft with two branches, one for the innominate trunk and one for the left common carotid artery, combined with the stent-assisted balloon-induced intimal d

Lung cancer mimicking aortic dissecting aneurysm in a patient with situs inversus totalis.

Lung cancer and situs inversus totalis are two completely irrelevant conditions. The likelihood of both conditions occurring simultaneously in one person is very rare. We report here a case of a 50-year-old man who presented with intermittent chest pain. Enhanced computed tomography of the chest showed situs inversus totalis and a round mediastinal mass embracing the thoracic aorta. The primary diagnosis was suggested as pseudo aortic dissecting aneurysm. However, a tumor in the right lower lung was discovered during surgery, which enclosed and invaded the thoracic aorta. Finally, the patient

Twenty-Year Results of Open Surgical Suprarenal Aortic Fenestration for Acute Complicated Type B Aortic Dissection.

BACKGROUND: Open surgical suprarenal aortic fenestration (OSSAF) is a technique to treat complicated type B aortic dissection (cTBAD) by resecting the intimal membrane at the level of the visceral arteries. This invasive procedure is largely abandoned since the advent of thoracic endovascular aortic repair (TEVAR), which has become the gold standard for treating cTBAD. Identifying patterns in the late history of patients who underwent OSSAF might help better understand the evolution of type B aortic dissection (TBAD). Therefore, our aim was to investigate the long-term outcome and remodeling o

Fate of Residual Aorta After Surgery for Type A Aortic Dissection.

INTRODUCTION: Surgical treatment of type A aortic dissection is essentially palliative. Many patients who undergo the procedure still have a dissection flap in the residual aorta, with a persistent patent or partially thrombosed false lumen leaving them susceptible to the dilatation of distal aorta and aneurysm formation. METHODS: Patients who had undergone surgery for type A aortic dissection from January 2015 till December 2022 were recruited into the study. Two follow-up computed tomography scans were performed at least six months apart, the first one at least one month after the surgery.

Application of an extracorporeal prefenestrated stent graft in endovascular repair of ascending aorta and aortic arch lesions.

OBJECTIVES: Aortic dissection involving the ascending aorta and aortic arch is a serious condition. Treatment using traditional surgical methods has certain disadvantages. This study investigated the effectiveness of thoracic endovascular repair of aortic dissection using an external prefenestrated stent. METHODS: We present a series of aortic dissection cases involving the ascending aorta and aortic arch treated with an external prefenestrated stent. RESULTS: Postoperative follow-up of the patients showed that all stents were released at the proper position and that branch vessels were not

Morphological risk of acute type A aortic dissection in the mildly to moderately dilated aorta.

OBJECTIVES: This study aimed to analyse and determine the role of aortic length and curvature in the pathogenesis of acute type A aortic dissection (ATAAD) with ascending aortic diameters (AADs) <5 cm. METHODS: We reviewed the clinical and imaging data of patients with ATAAD (n = 201) and ascending aortic dilation (n = 83). Thoracic aortic bending index (TABI) was used to quantify aortic curvature and analyse its role in ATAAD below the diameter risk threshold. RESULTS: The AAD was <5.0 and <4.0 cm in 78% and 37% of patients with ATAAD, respectively. The median ascending aortic length (AAL)

Three Cases of Stent-Assisted Balloon-Induced Intimal Disruption and Relamination in Aortic Dissection Repair Effectively Treating Chronic Type B Aortic Dissection with False Lumen Aneurysm.

Successful thoracic endovascular aortic repair for chronic type B aortic dissection with an enlarged false lumen depends on complete exclusion of the false lumen. Stent-assisted balloon-induced intimal disruption and relamination in aortic dissection repair (STABILISE) creates a single lumen in the dissected thoracic and abdominal aorta by disrupting the intima. We report our experience in the treatment of 3 cases of chronic dissection using the STABILISE procedure at our hospital from December 2019 to May 2022. The STABILISE technique appears to be an effective procedure; however, further eva

Aortic arch surgery in real-life practice: is the frozen elephant trunk always a better option than conventional aortic arch replacement?

OBJECTIVE: Management of aortic arch disease (AAD) is complex due to its involvement in multiple aortic segments and the variety of patient presentations. We compared the clinical outcomes of conventional aortic arch repair (CAAR) and the Frozen Elephant Trunk (FET) technique across the main clinical indications: acute type A aortic dissection (ATAAD), chronic dissection and atheromatous aneurysm. MATERIALS AND METHODS: We conducted a retrospective, single-centre cohort study of 102 consecutive patients who underwent aortic arch repair between January 2012 and June 2023. Indication-specific s
