Increasing incidence of HIV- associated tuberculosis in Romanian injecting drug users.

BACKGROUND: A high prevalence of tuberculosis (TB) among HIV-positive injecting drug users (IDUs) may fuel the TB epidemic in the general population of Romania. We determined the frequency and characteristics of TB in HIV-infected IDUs referred to a national centre. METHODS: Prospective observational cohort study of all newly-diagnosed HIV-positive IDUs admitted to Victor Babes Hospital, Bucharest, between January 2009 and December 2014. Socio-demographics, clinical characteristics and outcomes of HIV/TB co-infected IDUs were compared to HIV-positive IDUs without TB. RESULTS: 170/598 (28.5%)

Post-ischemic Myoclonic Status Following Cardiac Arrest in Young Drug Users.

BACKGROUND: Myoclonic status may be observed following cardiac arrest and has previously been identified as a poor prognostic indicator in regard to return of neurologic function. We describe a unique situation in post-cardiac arrest patients with myoclonic status and hypothesize possible predictors of a good neurologic outcome. METHODS: Case series. RESULTS: We illustrate two cases of cardiac arrest due to a respiratory cause in young patients with evidence of illicit drug use at the time of hospital admission that suffered post-ischemic myoclonic status. These patients subsequently recover

Hepatitis C and HIV in injecting drug users in Armenia, Colombia.

A constant and progressive increase in the availability of heroin in Colombia in recent decades and the intravenous use  of this drug have established the need to prevent a possible epidemic of HIV and hepatitis C. This research determined the sero-prevalence of hepatitis C and HIV according to sociodemographic characteristics and risk behaviors in people who inject drugs in Armenia, Colombia. This is a cross-sectional study on 265 users captured through respondent-driven sampling after informed consent. Sero-prevalence of hepatitis C was 22.3 % [95% CI 12.3 % -23.5 %]; for HIV infection, it w

Subcutaneous anthrax in three intravenous drug users: a new clinical diagnosis.

Anthrax is extremely rare in the western world but is endemic to areas of south and central Asia. In early 2010 an outbreak was identified in heroin-injecting intravenous drug users in the United Kingdom and Europe. Afghanistan is currently the principal source of heroin which reaches the United Kingdom. When anthrax occurs, cutaneous disease accounts for over 95% of cases. At least 47 cases with 13 deaths have been confirmed so far. We present three cases presenting during this time with marked swelling, one resulting in compartment syndrome but all with an absence of the expected cutaneous a

High rates of midazolam injection among drug users in Bangkok, Thailand.

BACKGROUND: Reports from Thailand suggest that a growing number of people who inject drugs (IDU) are now injecting midazolam, a legal benzodiazepine with potent amnestic and ventilatory depressant effects. We therefore sought to examine midazolam injection among a community-recruited sample of Thai IDU. METHODS: We examined the prevalence and correlates of midazolam injection among 252 IDU participating in the Mitsampan Community Research Project, Bangkok, using multivariate logistic regression. We also examined the use of midazolam in combination with other drugs. RESULTS: 252 IDU participa

The emergence of meticillin-resistant Staphylococcus aureus among injecting drug users.

OBJECTIVE: To quantify an observed increase in meticillin-resistant Staphylococcus aureus (MRSA) infections in injecting drug users (IDUs) and, using genetic analysis and clinical presentation, contrast findings with those from previous studies in Brighton to further understanding of the clinical epidemiology and underlying molecular biology associated with the increase. METHODS: From 2006 to 2008, isolates of S. aureus were collected, together with clinical details from infected IDUs. Isolates were characterised using antibiotic susceptibilities, multilocus-sequence typing (MLST) and spa typ

Acceptability of hepatitis C virus testing methods among injecting drug users.

INTRODUCTION AND AIMS: The acceptability of testing methods and procedures has implications for uptake of blood-borne virus screening in sentinel samples of injecting drug users (IDUs) likely to participate in surveillance. The aim of the current study was to determine the acceptability of three methods of hepatitis C virus (HCV) testing among injecting drug users (IDUs): oral fluid, capillary blood and venous blood sampling. DESIGN AND METHODS: A cross-sectional survey of IDUs was conducted in inner-city Sydney in 2005 for a laboratory validation study of HCV antibody testing. Participants w

Neural mechanisms underlying drug-related cue distraction in active cocaine users.

Human drug dependence routinely features a difficulty with disengaging attention from drug-related stimuli, a symptom previously shown to be predictive of relapse during treatment. We examined the neural mechanisms underlying this attentional bias in cocaine users, varying working memory (WM) load to reflect the demands imposed by ruminative craving thoughts. Sixteen active users of cocaine were administered a WM task that manipulated the requirement for selective attention by varying the background contents, cocaine-related or neutral, upon which a recall probe item was shown. Behavioural and

Heart, liver and spleen pathology in chronic alcohol and drug users.

Alcohol- and drug-related deaths remain a major problem in the UK. Although the pathological findings of cardio-, hepato- and splenomegaly are frequently and empirically associated with chronic alcohol and drug use, there is limited published evidence available. This study hypothesises that organomegaly is associated with chronic substance use, and may represent a prognostic indicator. The weights of hearts, livers and spleens from 280 chronic alcoholics (CA) and 33 chronic drug users (CD) were compared to those of 291 controls. Using a forensic pathology database, CA and CD subjects were iden

Neurocognitive aspects of medication adherence in HIV-positive injecting drug users.

Cognitive deficits are associated with nonadherence to HIV medications. HIV-positive injecting drug users (IDUs) are at particular risk for nonadherence and cognitive barriers to adherence specific to this population should therefore be identified. The present study assessed the relation of three domains of cognitive functioning, executive functions, memory, and psychomotor speed, to self-reported antiretroviral adherence in a sample of HIV-positive IDUs. Depression, use of alcohol, heroin, cocaine/crack, or marijuana in the last week were also included in the models. Logistic regression analy

Homelessness among problem drug users: prevalence, risk factors and trigger events.

The present paper uses data from a prospective cohort study of 877 problem drug users entering treatment in Scotland to extend knowledge of homelessness and drug misuse in three important respects: First, the prevalence of homelessness among problem drug users is investigated; secondly, key risk factors for homelessness among problem drug users are identified; and thirdly, trigger events associated with movements into or out of homelessness by problem drug users over time are explored. Data were collected during two waves of interviewing which were conducted 8 months apart. Thirty-six per cent
