Morphology-informed deep learning for risk assessment of filamentous bulking in a full-scale industrial wastewater treatment plant.

Filamentous bulking (FB) is a recurring cause of solids-separation failure in activated sludge systems. Standard diagnostics such as sludge volume index (SVI) and routine microscopy are largely reactive, confirming deterioration only after settling performance has already degraded. Surrogate, rule-based risk assessment models offer interpretable early warnings, but they infer microbial states indirectly and may become unreliable under variable industrial operation. Here, we propose a deep-learning, morphology-informed framework that directly scores FB risk from sludge microscopy images. Two pr

Associations between Organ Dysfunction Scoring Systems and Dysregulated Host Responses in Adults with Severe Infection in Uganda: A Prospective Cohort Study.

Scoring systems for organ dysfunction (e.g., the quick Sepsis-related Organ Failure Assessment [qSOFA], Modified Early Warning Score [MEWS], and Universal Vital Assessment [UVA]) are proposed as clinical criteria for sepsis. The content validity of these scoring systems is poorly understood in sub-Saharan Africa, where the global sepsis burden is concentrated. In a prospective cohort of 288 adults hospitalized with suspected sepsis in Uganda, we show that qSOFA, MEWS, and UVA scores were significantly associated with soluble mediators of innate and adaptive immune activation, endothelial dysfu

Validation of 'Quick Sequential Organ Failure Assessment Score' as a Screening Tool for Early Identification of Sepsis Patients in the Emergency Department.

Sepsis and septic shock are major healthcare problems, affecting millions of people around the world each year. The speed and appropriateness of therapy administered in the initial hours of treatment are likely to influence the outcome. We conducted a study to validate the clinical assessment score named 'quick sequential organ failure assessment' (qSOFA) score for use in the early identification of sepsis patients in the emergency department. Our primary objective was to see the sensitivity and specificity of the qSOFA-score for diagnosing sepsis in the emergency department and our secondary

Dynamic monitoring of circulating tumor cells and PD-L1 combined positive score as prognostic biomarkers for adjuvant immunochemotherapy in stage III gastric cancer: A prospective observational study.

BACKGROUND: Patients with stage III gastric cancer (GC) face a high risk of recurrence after radical resection, and the significance of circulating tumor cells (CTCs) and Programmed death-ligand 1 (PD-L1) combined positive score (CPS) in adjuvant therapy remains unclear. METHODS: In this prospective observational cohort study, we analyzed 76 stage III GC patients post-D2 resection who received either chemotherapy alone (n = 46) or immunochemotherapy (n = 30) based on clinical decision-making. CTCs levels at baseline and after 6 treatment cycles were assessed using a folate receptor-based qPCR

Predicting dengue transmission rates by comparing different machine learning models with vector indices and meteorological data.

Machine learning algorithms (ML) are receiving a lot of attention in the development of predictive models for monitoring dengue transmission rates. Previous work has focused only on specific weather variables and algorithms, and there is still a need for a model that uses more variables and algorithms that have higher performance. In this study, we use vector indices and meteorological data as predictors to develop the ML models. We trained and validated seven ML algorithms, including an ensemble ML method, and compared their performance using the receiver operating characteristic (ROC) with t

Predicting pulmonary hemorrhage in very low birth weight infants.

BACKGROUND: Pulmonary hemorrhage (PH) in very low birth weight (VLBW) infants is a catastrophic event with significant mortality. Predicting PH clinically is challenging, with no tools for early detection. Our objective was to identify candidate physiologic biomarkers using heart rate (HR) and pulse oximetry (SpO2) predictive of PH events in VLBW infants. METHODS: We conducted a retrospective case-control study of VLBW infants from two level IV NICUs (2016-2023). We matched infants with PH to controls using gestational age, birth weight, and postnatal age. Taking HR and SpO2 data from 72 hour

Distinct CTC Specific RNA Profile Enables NSCLC Early Detection and Dynamic Monitoring of Advanced NSCLC.

Circulating tumor cells (CTCs) hold significant potential as biomarkers for the diagnosis and management of non-small cell lung cancer (NSCLC). However, their clinical utility is limited by the heterogeneity of CTC subtypes and the need for robust, quantitative assays. In this study, a quantitative CTC RNA assay incorporating multi-antibody-based CTC isolation and specific mRNA quantification by RT-ddPCR is developed. Two distinct models are established: NSCLC CTC ScoreD for detecting early NSCLC (stages I-II), and NSCLC CTC ScoreM for monitoring advanced NSCLC (stages III-IV), based on distin

Establishment of a novel risk score for in-hospital mortality in adult sepsis patients.

BACKGROUND: Existing scoring systems have limitations in predicting the in-hospital mortality of adult sepsis patients. We aimed to develop and validate a novel risk score for predicting the in-hospital mortality of adult sepsis patients. METHODS: The clinical data of 1,335 adult sepsis inpatients were retrospectively analyzed. Enrolled patients were randomly divided into a modeling group and a validation group at a 3:2 ratio. The modeling group (n=801) was used to develop the risk score by univariate and multivariate logistic regression analyses. The score's performance was validated in the

Brain-injury and Alzheimer's disease biomarkers are elevated in patients with suspected infection and physiological derangement: importance for context-specific interpretation of Alzheimer's biomarkers.

Following acute infective illness, patients frequently exhibit neurological symptoms, with persistent neurological symptoms commonly observed following severe infection. However, the association between systemic infection and the concentration of blood-based biomarkers of brain injury and the relationship between these and markers of the host response to infection are poorly characterized in the literature. Further, the association between acute illness and the Alzheimer's disease-associated biomarker phosphorylated-tau-217 (p-tau-217) is unknown. In acute samples from 26 patients attending th

How Much Do Healthcare Practitioners Know About Sarcoma?

Sarcoma awareness among healthcare practitioners in our institution was limited, prompting an evaluation of their ability to recognize early presentations and initiate appropriate work-up for soft tissue and bone sarcomas. A structured survey assessed familiarity with key clinical features, recommended investigations, and perceived contributors to diagnostic delay. Overall awareness was modest, but higher among healthcare practitioners with prior sarcoma exposure, oncology-focused training, longer experience, or heavier patient loads; female practitioners and oncology nurses also scored higher

[Disseminated cryptococcosis resulting in fungal septic shock: a case report].

Fungal sepsis and septic shock are syndromes involving acute organ dysfunction that result from a dysregulated host response to invasive fungal infections. These syndromes are characterized by diagnostic challenges, rapid progression, complex management, high medical costs, and high mortality, particularly among immunocompromised patients. This article mainly presents a case of fungal septic shock caused by disseminated Cryptococcosis, which resulted in death within a short period. It emphasizes the importance of the principle of "early warning and rapid intervention" in clinical practice for
