Non optimal temperatures and chronic cardiovascular diseases.

Extreme temperatures are emerging as major climate-sensitive drivers of chronic cardiovascular disease (CVD). Global warming has increased the frequency, duration, and intensity of heatwaves, while substantial cold-related risk persists, particularly in poorly insulated and socioeconomically vulnerable settings. Together, non-optimal temperatures account for millions of deaths annually, with a large and growing share attributable to CVD. In this narrative review, we synthesize epidemiological, mechanistic, and systems-level evidence linking heat and cold exposure to chronic cardiovascular outc

Mood in the moment: a study protocol for embedding ecological momentary assessments into established longitudinal cohorts to examine depression in real time.

INTRODUCTION: Depression is a major global health challenge, with onset commonly occurring in youth. There is an urgent need to better understand the epidemiology of depression to facilitate better interventions and preventions that are person-specific and time-specific. Long-term depression trajectories and ecological momentary assessment (EMA) can help address this need. Embedding EMA designs within established longitudinal cohorts offers a uniquely powerful approach to examine depression in both long- and short-term settings and to identify distal and proximal modifiable risk factors for de

Linking Cardiovascular Disease and Metabolic Dysfunction-Associated Steatotic Liver Disease (MASLD): The Role of Cardiometabolic Drugs in MASLD Treatment.

The link between cardiovascular disease (CVD) and metabolic dysfunction-associated steatotic liver disease (MASLD) is well-established at both the epidemiological and pathophysiological levels. Among the common pathophysiological mechanisms involved in the development and progression of both diseases, oxidative stress and inflammation, insulin resistance, lipid metabolism deterioration, hepatokines, and gut dysbiosis along with genetic factors have been recognized to play a pivotal role. Pharmacologic interventions with drugs targeting common modifiable cardiometabolic risk factors, such as T2

The Epidemiology of Meniscus Injury.

Meniscus surgery is one of the most commonly performed orthopedic procedures worldwide. Modifiable risk factors for meniscus injury include body mass index, participation in athletics and occupation. Nonmodifiable risk factors include age, sex, lower extremity alignment, discoid meniscus, ligamentous laxity, and biconcave tibial plateau. Conditions commonly associated with meniscal injury are osteoarthritis, anterior cruciate ligament injury, and tibial plateau fractures. Tear type and location vary by patient age and functional status. Surgical management of meniscus injury is typically cost-

The impact of new epidemiological cutoff values on Candida glabrata resistance rates and concordance between testing methods.

Interpretive criteria for Candida susceptibility testing were recently revised with the establishment of species-specific epidemiological cutoff values (ECV). To assess the effect of modified cutoff values on Candida glabrata resistance rates and agreement between testing methods, we tested the susceptibility of 598 clinical isolates to fluconazole, itraconazole, voriconazole, posaconazole, caspofungin, and amphotericin B using CLSI M27-A3 and E-test methods. The caspofungin MICs clustered above the ECV and below the CLSI cutoff (MIC50, 0.5 μg/mL). Applying the ECV reduced the proportion of it

Defining research priorities for bacterial sexually transmitted infections in Canada.

OBJECTIVES: We aimed to define priority areas for research on chlamydia, gonorrhea, and syphilis in Canada through the use of a consensus method. METHODS: A modified Delphi method was conducted, consisting of two online surveys and an in-person meeting. Participants included people working in bacterial sexually transmitted infections (STIs) across Canada, including clinicians, policy-makers, public health practitioners, and researchers. RESULTS: Consensus was achieved regarding the most important priority topics in bacterial STI research in Canada, within five general priority areas: the epi

Environmental pollutants as emerging risk factors in osteoarthritis: Mechanistic and epidemiological evidence.

Osteoarthritis (OA) is a multifactorial degenerative joint disease with increasing evidence implicating environmental pollutants as underrecognized contributors to its pathogenesis. This review EditedbyDr.CaterinaFaggiostematically synthesizes in vitro, animal, computational, and human epidemiological studies to elucidate the mechanistic and population-level effects of chemical exposures on OA. Airborne pollutants, including PM2.5, PM10, NO2, O3, and trihalomethanes, are associated with increased OA incidence, promoting cartilage degradation through oxidative stress, inflammatory cytokine over

Combination of toxicological and epidemiological approaches for estimating the health impact of atmospheric pollutants. A proof of concept for NO2.

BACKGROUND: Regular monitoring of the air pollutant nitrogen dioxide (NO2), an indicator for traffic-related emissions, is a priority in urban environments. The health impacts associated with NO2 exposure are the result of a combination of factors, including concentration, duration of exposure, and interactions with other pollutants. WHO has established air quality guidelines based on epidemiological studies. OBJECTIVE: This study develops a new concept "Health Impact Pathways (HIPs)" using adversity as a probabilistic indicator of health effects. For this purpose, it integrates available tox

Chronic Low-Dose Exposure to Xenoestrogen Ambient Air Pollutants and Breast Cancer Risk: XENAIR Protocol for a Case-Control Study Nested Within the French E3N Cohort.

BACKGROUND: Breast cancer is the most frequent cancer in women in industrialized countries. Lifestyle and environmental factors, particularly endocrine-disrupting pollutants, have been suggested to play a role in breast cancer risk. Current epidemiological studies, although not fully consistent, suggest a positive association of breast cancer risk with exposure to several International Agency for Research on Cancer Group 1 air-pollutant carcinogens, such as particulate matter, polychlorinated biphenyls (PCB), dioxins, Benzo[a]pyrene (BaP), and cadmium. However, epidemiological studies remain s

Human Papillomavirus-Related Oropharyngeal Carcinoma: Update on Etiology, Management and the Role of Oral Health Care Providers.

BACKGROUND: The role of oncogenic subtypes of the human papillomavirus (HPV) in the pathogenesis of cervical dysplasia, cervical cancer, anal intraepithelial neoplasia, penile intraepithelial neoplasia, vaginal intraepithelial neoplasia and oropharyngeal carcinomas (OPC) is well documented. The classification and management of HPV-positive OPC (HPV+ OPC) has been modified because of differences in the molecular, biological and clinical behaviour of this disease compared with HPV-negative OPC (HPV- OPC) and conventional oral squamous cell carcinoma (OSCC). DESCRIPTION: HPV+ OPC is associated w

[Therapeutic Options in Keratoconus].

A recent epidemiology study revealed that prevalence of keratoconus was much higher than previously assumed. Therefore, it is no longer deemed an "orphan disease", as it has a relevant socioeconomic impact on the healthcare system. One of the most important risk factors for developing keratoconus is chronic eye rubbing which, apart from other known risk factors such as atopy or Down's syndrome, is the only modifiable factor. Informing the patient and offering behavior modifying therapies seems to be essential. Further therapeutic options regarding improvement of visual function include the wea

Huntington disease.

Huntington disease is devastating to patients and their families - with autosomal dominant inheritance, onset typically in the prime of adult life, progressive course, and a combination of motor, cognitive and behavioural features. The disease is caused by an expanded CAG trinucleotide repeat (of variable length) in HTT, the gene that encodes the protein huntingtin. In mutation carriers, huntingtin is produced with abnormally long polyglutamine sequences that confer toxic gains of function and predispose the protein to fragmentation, resulting in neuronal dysfunction and death. In this Primer,

Invited commentary: how exposure to air pollution may shape dementia risk, and what epidemiology can say about it.

The number of Americans with Alzheimer's disease (AD) dementia is expected to triple by 2050. No effective treatments exist, and prevention research has focused on behaviors and medical conditions, which have been difficult to modify at the population level. Cardiovascular disease epidemiology can inform the search for AD risk factors; exposure to fine particulate matter (PM) air pollution increases cardiovascular risk, pollutant regulations appear to reduce cardiovascular deaths, and vascular pathology influences dementia risk. In this issue of the Journal, Ailshire and Crimmins (Am J Epidemi

[Rheumatoid arthritis].

Rheumatoid arthritis is an autoimmune disease manifested by a persistent inflammation of synovial joints, bone destruction, loss of cartilage and increased risk of cardiovascular and other intercurrent diseases. The treatment of rheumatoid arthritis should be based on the strategy of treat to target therapy which is achieving remission, in some cases low activity of the disease. Essential to the treat to target therapy are more frequent checkups and optimization of treatment based on disease activity. Methotrexate continues to be the essential medicine from the group of disease modifying antir

Update in the genetics of thalassemia: What clinicians need to know.

Thalassemia is a significant health problem worldwide. Prenatal diagnosis is the only effective way to prevent the birth of a fetus with severe thalassemias, which include hemoglobin Bart's hydrops fetalis and thalassemia major. However, accurate prenatal diagnosis depends on the comprehensive consideration of the molecular basis of thalassemias. To make a correct decision, the obstetrician should have a certain understanding of the genetics of thalassemias. Here we present a brief introduction of some fundamental genetic knowledge of thalassemias, including the production of hemoglobin, struc

Cholesterol-modifying strategies for Alzheimer's disease.

The link between cholesterol and Alzheimer's disease (AD) is indicated by genetic, epidemiological, experimental and clinical evidence, although the relationship is far from simple or straightforward. Compared with extracerebral cholesterol, little is known about brain cholesterol homeostasis. In addition, current diagnostic criteria for AD exclude a priori the pathogenic participation of vascular factors (to which cholesterol is traditionally associated), and require modifications. Randomized controlled trials suggest that the dementia stage of AD may already be too late for significant benef

Inhibition of DNA synthesis by aspirin in Swiss 3T3 fibroblasts.

In Swiss 3T3 fibroblasts, aspirin inhibited proliferation induced by the complete mitogenic factors platelet-derived growth factor (PDGF) and bombesin. Aspirin decreased the maximum mitogenic effect of bombesin without modifying the concentration necessary to obtain half maximal DNA synthesis stimulation. In contrast, aspirin only decreased mitogenesis at subsaturating PDGF concentrations. The effect of aspirin was found to be concentration dependent. The half-maximal effect occurred at approximately 150 microM. The maximal inhibition was obtained when aspirin was added during the first hour a

Air Pollution's Effects on the Human Respiratory System.

The World Health Organization defines air pollution as "any chemical, physical or biological agent that modifies the natural characteristics of the atmosphere." The most common pollutants include particulate matter, carbon monoxide, ozone, nitrogen oxide, and sulfur dioxide. The two types of air pollution, indoor and ambient, both contribute to a host of cardiac and respiratory illnesses. Exposure to excess levels of air pollution is significantly associated with a variety of acute and chronic respiratory illnesses, such as chronic obstructive pulmonary disease, asthma, respiratory allergies,

A retrospective study of the effect of long-term topical application of retinaldehyde (0.05%) on the development of actinic keratosis.

BACKGROUND: The role of topical retinoids on photocarcinogenesis is still unclear. Retinaldehyde is a natural metabolite of vitamin A used as a cosmetic product. Its effect on actinic keratoses has not been studied to date. OBJECTIVE: To study the incidence of actinic keratoses during long-term application of retinaldehyde in order to evaluate a possible chemoprophylactic effect. METHODS: We conducted a retrospective study on 61 patients who had applied retinaldehyde on photoexposed body areas for a period ranging from 6 to 142 months. We counted the total number of actinic keratoses and cut
