Knowledge and practice of family planning among pregnant tribal women in Southern India: an observational study.

BACKGROUND: There are over 700 Scheduled Tribes (ST) living in 30 Indian states. As with other indigenous groups across the world, Indian ST have some of the poorest infant and child health outcomes of any communities in India. A child born to an ST family is 19% more likely to die in the first month of life and has a 45 percent risk of dying in their first year compared with other Indian populations. Research suggests that early conception, high fertility, and low use of family planning methods are large contributors to these disparities. METHODS: A cross sectional survey in Kannada was cond

Oral anticoagulant safety in family practice: prognostic accuracy of Bleeding Risk Scores (from the CACAO study).

BACKGROUND: To assess bleeding risk of patients treated by oral anticoagulants, several scores have been constructed to assist physicians in the evaluation of the benefit risk. Most of these scores lack a strong enough level of evidence for use in family practice. OBJECTIVE: To assess the predictive prognostic accuracy of 13 scores designed to assess the risk of major or clinically relevant non-major (CRNM) bleeding events in a French ambulatory cohort receiving Vitamin-K antagonists (VKA) or direct oral anticoagulants (DOACs) in a family practice setting. METHODS: CACAO (Comparison of Accid

COVID-19 vaccine short-term adverse events in the real-life family practice in Krakow, Poland.

BACKGROUND: In manufacturers' trials, vaccination against COVID-19 proved to be safe and effective. The officially reported frequency of vaccine adverse events (VAEs) in Poland is lower than that declared by the manufacturers. The anti-vaccination activists questioned the trustworthiness of official data. OBJECTIVES: The aim was to explore the real-life prevalence of VAEs in general practice settings and the factors that may influence it. METHODS: In this pragmatic, mixed prospective and retrospective study, patients vaccinated against COVID-19 between May and October 2021 in three GP practi

BMC family practice integrated GP care for patients with persistent physical symptoms: feasibility cluster randomised trial.

BACKGROUND: Patients continue to suffer from medically unexplained symptoms otherwise referred to as persistent physical symptoms (PPS). General practitioners (GPs) play a key role in the management of PPS and require further training. Patients are often frustrated with the care they receive. This study aims to assess the acceptability of an 'integrated GP care' approach which consists of offering self-help materials to patients with PPS and offering their GPs training on how to utilise cognitive behavioural skills within their consultations, as well as assessing the feasibility of conducting

Utility of FRAX (fracture risk assessment tool) in primary care and family practice setting in India.

The prevalence of osteoporosis in postmenopausal women is about 35 to 50% and the mortality associated with hip fractures is about 20%. Despite these figures, osteoporosis continues to be under diagnosed in the primary care setting. One of the reasons for this is attributed to the poor availability of DXA (Dual Energy X-ray Absorptiometry) scanners in rural areas. The fracture risk assessment tool (FRAX) is an online web-based tool that takes into account multiple factors that help in predicting the 10-year risk of developing hip or major osteoporotic fractures. The tool was developed and vali

Nurses' practice environment and work-family conflict in relation to burn out: a multilevel modelling approach.

OBJECTIVES: To investigate associations between nurse work practice environment measured at department level and individual level work-family conflict on burnout, measured as emotional exhaustion, depersonalization and personal accomplishment among Swedish RNs. METHODS: A multilevel model was fit with the individual RN at the 1st, and the hospital department at the 2nd level using cross-sectional RN survey data from the Swedish part of RN4CAST, an EU 7th framework project. The data analysed here is based on a national sample of 8,620 RNs from 369 departments in 53 hospitals. RESULTS: General

Clinical and economic outcomes in patients switched to simvastatin in a community-based family medicine practice.

BACKGROUND: The introduction of a generic formulation of simvastatin has created the potential to provide significant low-density lipoprotein cholesterol (LDL-C) reduction in a highly cost-effective manner. METHODS: This retrospective cohort analysis utilised electronic medical record data from a United States, community-based, independent physician family medicine practice. Patients switched from other statins or statin combinations to simvastatin by the family medicine physicians during routine patient care from January 2002 to October 2008 were identified. Equivalent statin dosing, lipid p

Cardiovascular disease and musculoskeletal disorder labels in family practice acted as markers of physical health severity.

OBJECTIVE: Family practitioner diagnostic labels applied in consultation provide a signpost for treatment and management. Yet, it is unknown whether each label reflects the health of the respective patient group. STUDY DESIGN AND SETTING: Consultation records of 7,799 patients aged 50 years and older from six family practices were linked to a cross-sectional baseline health survey. Associations between six mutually exclusive cardiovascular disease and nine mutually exclusive musculoskeletal disorder categories, and physical health severity as measured by the Short Form-12 questionnaire were e

Validity of the medical outcomes study depression screener in family practice training centers and community settings.

BACKGROUND AND OBJECTIVES: Screening inventories that identify primary care patients suffering from depression should be brief, correlate with depression diagnoses, and be evaluated in populations with whom the inventories will be used. METHODS: A total of 566 patients from a medical school training center and 457 patients from six community family practice offices completed the Medical Outcomes Study (MOS) depression screener and a questionnaire reporting demographic information and health habits. All subjects scoring above the recommended screener cut-off, and a random sample of those scori

Review of clinical use of sertraline by family practice physicians in a small Air Force hospital.

The clinical use of sertraline for 1 year in a family practice clinic in a small Air Force hospital was reviewed. Retrospective chart review showed that 85% of patients receiving five or more prescriptions for sertraline had a diagnosis of depression; the remainder were treated for chronic pain or dysthymia. The patients in the review needed dose increases (above the recommended starting dose of 50 mg qd) in nearly 50% of the cases. Clinicians using sertraline for the treatment of affective illness in the primary care setting should be aware of the likelihood for dose increases to achieve maxi

Surge capacity and practice management challenges of Canadian family physicians during COVID-19: a qualitative study.

BACKGROUND: Planning for surge capacity, that is, the ability of a health service to expand beyond normal capacity and meet an increased demand for clinical care, is an essential component of public health emergency preparedness. During the COVID-19 pandemic, family physicians (FPs) were called upon to provide surge capacity in settings such as hospital units and emergency departments while also maintaining their primary care responsibilities. Most research reports on projection models, hospital settings, or the use of virtual care, with limited focus on the firsthand experiences of FPs in thi
