Despite an improvement in our understanding of the aetiopathogenesis of diabetic foot problems in the last 2 decades, the 21st Century epidemic of type 2 diabetes will ensure that the incidence of foot problems will continue to increase in the diabetic population. In the aetiopathogenesis it is important to understand that a number of factors working together usually result in foot ulceration: the commonest trio is neuropathy, deformity and trauma. In Western countries, the incidence of neuroischaemic ulcers is now increasing making early detection of those at risk even more important. In the

Macrodactyly is a rare congenital anomaly defined by an enlargement of all the structures of fingers or toes. Two forms of macrodactyly have been described. One form is static with an enlargement present at birth and growing proportionately to the other digits. The other is progressive with an enlargement beginning in early childhood and growing faster than that of the normal digits. Macrodactyly is responsible of a functional handicap and an aesthetic prejudice. Treatment may be indicated for this reason. Treatment must be individualised and depends on the proportionate size of the enlarged f

The foot is particularly exposed to injury and infection in the tropical areas. This article provides a review of the main diseases affecting the foot in the tropics including leprosy, ainhum, ulceration due to Mycobacterium ulcerans, mycetoma, chromomycosis, Kaposi's sarcoma, elephantiasis, podoconiasis, dracunculosis, tungiasis, syphilis and endemic treponematosis, larva migrans, scytalidiosis, and envenomation. Prevention is essential.

Epithelioid hemangioma (angiolymphoid hyperplasia with eosinophilia, EH/ALHE) is a rare benign angioproliferative lesion which typically occurs in the region of the head and neck. In the literature, occurence on the extremity is only rarely described. A case of multiple occurence of EH/ALHE in the skin of the toes and metatarsal bone with osteolysis is reported. Occurence on the extremity, superficial and deep affection and some "atypical" microscopic features may cause diagnostic dilemma. The key diagnostic features of EH/ALHE are vascular channels lined with epithelioid endothelial cells, su

Destruction in diabetic feet is secondary to neuropathy (peripheral and autonomic nervous system) in association with microangiopathy. The loss of sensation to pain and the static trouble lead to increase the pressure in some areas and predispose to pedal skin ulceration, the precursor of osteomyelitis. Plain radiography should be the first step in the evaluation for diagnosis and follow-up. The initial patterns are nonspecific but very rapid evolution associating osteolysis, osteosclerosis and fragmentation lead to the Charcot foot. When osteomyelitis is suspected, scintigraphy with labelled

Podiatric physicians are frequently the first clinicians with the opportunity to diagnose a rheumatologic disease. Awareness of the multisystem nature of the more common rheumatologic conditions will assist podiatrists in making the appropriate diagnosis. The specific joints affected, the temporal pattern of joint involvement, and the distribution of affected joints give clues to the diagnosis. Knowledge of the current treatment for rheumatic diseases as well as early referral for evaluation by a medical physician is essential for the appropriate care of patients with systemic arthritis.

It is imperative that the physician participating in the care and treatment of foot and ankle trauma appreciate the epidemiology, the unique characteristics, and principles of treatment of gunshot injuries involving the foot and ankle. The current author will provide the reader with knowledge of how the bullet velocity, its mass, and its area are important in determining the energy transmitted to the tissues struck by the bullet. These forces imparted by the projectile interact with the specific characteristics of the tissues to determine the nature and extent of the trauma sustained by the fo

A few selectec metric and numeric malformations of the foot (Fibular deficiency, vertical talus, coalition, syndactylism, macrodactylism, metatarsal malformation, cleft foot, overriding fifth toe) are discussed to demonstrate, that concepts for their treatment should be developed as early as possible in order to allow for bringing those structures that are developed in optimum use. Besides function cosmetic aspects of the foot also deserve being respected.

OBJECTIVE: Our aim was to establish the differences between polydactyly of the hand, the foot and those affecting both the hands and feet.
PATIENTS AND METHODS: One hundred twenty-five cases of hand polydactyly (HP), 105 of foot polydactyly (FP) and 25 cases of combined hand and foot polydactyly (HFP) were reviewed. We differentiated between preaxial, postaxial, axial and peculiar polydactyly and a group constituted by all other non-preaxial location (OTHERL) was also formed. In all patients the following parameters were analyzed: sex, laterality, antecedence of malformation in the family (FA

An extremely rare case of primary extragenital donovanosis affecting the dorsa of right foot is reported. Clinical and histopathological features of the disease are described and the rarity, absence of genital lesions, and consequent difficulty in diagnosis are discussed.

Following complex foot injuries (incidence up to 52%) in the multiply-injured patient the ultimate goal remains the same as for all significant foot injuries: the restoration of a painless, stable and plantigrade foot to avoid corrective procedures with moderate results. In the case of a complex trauma of the foot (5 point-score)--e.g. a crush injury--primary amputation in the multiply-injured patient (PTS 3-4) is indicated. Limb salvage (PTS 1-2) depends on the intraoperative aspect during the second look (within 24-48 hours after injury): the debridement has to be radical, the selection of a

The incidence and severity of fractures and dislocations vary depending on their location and on the direction of the forces responsible for the injury. The radiologist's role in evaluation of the injured foot is to recognize the types of injuries produced by particular forces. Because there is considerable overlap of fractures and dislocations of the foot and ankle, evaluation of one area to the neglect of the other can lead to significant oversights and failure to recognize additional injury patterns. For example, fractures of the talus, calcaneus, base of the fifth metatarsal, and to a less

This article includes a description of common problems of greatest interest to nondermatologist physicians who treat the foot. These include plantar warts, corns and callouses, toenail problems, and some common skin disorders, such as tinea pedis, pitted keratolysis, hyperkeratosis and fissures, and contact dermatitis. Certain neoplasms, including squamous cell carcinoma, verrucous carcinoma, and melanoma also are discussed.

Overall physical fitness has become increasingly popular in the 1990s, especially in the geriatric community. More runners, bikers, swimmers, and aerobic advocates over the age of 55 are requiring medical attention for foot complaints, as the feet are a necessary tool for almost any athletic activity. This paper discusses the most common foot disorders in active geriatric patients and outlines a variety of conservative and surgical approaches to management. The goal is early return to pain-free activity.

A 56-year-old woman presented with a chronic infection of her right first toe. The woman had a 15-year history of diabetes mellitus and had been insulin dependent for the past five years. Her toe had been injured one month earlier when hit by a frozen chicken that fell out of the freezer. The accident caused a bruise and a small cut. Serous to purulent drainage then developed. When she presented, the toe was reddened and draining. Physical examination showed a nonobese woman with no fever or other evidence of systemic infection. The wound showed no evidence of necrotizing fasciitis. Peripheral

Treatment of diabetic foot pathology requires an accurate assessment of the etiologic factors involved. Dysvascular deterioration may be amenable to vascular reconstruction. When the vascular status cannot be improved, however, amputation is preferable to continued conservative care. Neuropathic ulceration is the product of decreased sensation, deformity, and increased pressure at the site of the ulcer. Pressure relief by conservative or surgical means is required for healing. Neuroarthropathy is increasing in frequency and results primarily from autonomic denervation. Protection from weight b
