Effects of different medial arch support heights on rearfoot kinematics.

BACKGROUND: Foot orthoses are usually assumed to be effective by optimizing mechanically dynamic rearfoot configuration. However, the effect from a foot orthosis on kinematics that has been demonstrated scientifically has only been marginal. The aim of this study was to examine the effect of different heights in medial arch-supported foot orthoses on rear foot motion during gait. METHODS: Nineteen asymptomatic runners (36±11years, 180±5cm, 79±10kg; 41±22km/week) participated in the study. Trials were recorded at 3.1 mph (5 km/h) on a treadmill. Athletes walked barefoot and with 4 different no

Orthotic Prescription for Pediatric Flexible Flat Feet using Convolutional Neural Networks.

Pediatric flexible flat foot (PFFF) is known to in-crease the foot structure's load, causing potential disability. Foot orthoses are one of the most common non-surgical methods to improve the medial longitudinal arch of the foot for improving PFFF. However, orthoses are not routinely prescribed due to their high cost, and discomfort caused by a restriction of foot movement. Furthermore, there are no quantitative standards or guidelines for an orthotic prescription, which makes the decision-making process of less experienced podiatrists chal-lenging. In this study, the authors investigated conv

Gait in children with arthrogryposis multiplex congenita.

PURPOSE: Lower limb contractures and muscle weakness are common in children with arthrogryposis multiplex congenita (AMC). To enhance or facilitate ambulation, orthoses may be used. The aim of this study was to describe gait pattern among individuals wearing their habitual orthotic devices. METHODS: Fifteen children with AMC, mean age 12.4 (4.3) years, with some lower limb involvement underwent 3-D gait analysis. Three groups were defined based on orthosis use; Group 1 used knee-ankle-foot orthoses with locked knee joints, Group 2 used ankle-foot orthoses or knee-ankle-foot orthoses with open

Mechanisms of neuroimmune crosstalk in vestibular disorders: Linking inflammation with neural circuit dysfunction (Review).

BACKGROUND: Foot orthoses (FOs) are prescribed as an important conservative treatment option in patients with foot problems related to rheumatoid arthritis. However, a broad variation in FOs is used, both in clinical practice and in research. To date, there is no overview on the outcomes of the treatment with different kinds of FOs in patients with rheumatoid arthritis and a specific foot problem. The objectives of the present study were to summarize the comparative effectiveness of FOs in the treatment of various foot problems in patients with rheumatoid arthritis, on the primary outcomes foo

[Hollow foot in adults].

Contrary to the general impression, the hollow foot is much more common than the flat foot. We distinguish three categories of hollow foot: varus hollow (clinical form I), valgus hollow (clinical form II), and pseudo-hollow (clinical form III). These three clinical forms have a common deviation dynamics, the consequence of shortening of the sural-Achilles-calcaneoplantar system. Metatarsalgia often precedes deformation of the front of the foot. Talalgia is linked to excess traction or to a conflict between the heel and the back of the shoe. Common therapy consists of plantar orthoses, eventual

Triceps surae activation is altered in male runners with Achilles tendinopathy.

UNLABELLED: Achilles tendinopathy is a common injury in running sports however the exact etiology of Achilles injury is still unclear. In recent years, altered neuromotor recruitment patterns of the triceps surae have been hypothesized to create differential intra-tendinous loads leading to pathology; however, this hypothesis has not been investigated. Further, the effect foot orthoses may have on neuromotor recruitment of the triceps surae in Achilles tendinopathy has not been investigated. METHODS: The electromyographic activity of the triceps surae was recorded during an over-ground runnin

[Application progress on functional insoles in the prevention and treatment of diabetic foot].

Diabetic foot (DF) is one of the most serious chronic complications of diabetes. The incidence rate among global diabetes patients is as high as 15% to 25%, and about 50% of patients will develop contralateral foot ulcers within 5 years after the first unilateral ulcer. As a non-invasive prevention and control solution, the application progress of functional insoles is mainly reflected in the following aspects:(1) Material innovation. The application of new composite materials and smart materials has significantly enhanced the pressure reduction effect and comfort. (2) Structural optimization.

A wearable gait analysis protocol to support the choice of the appropriate ankle-foot orthosis: A comparative assessment in children with Cerebral Palsy.

BACKGROUND: Cerebral Palsy is, nowadays, the most common cause of pediatric disabilities, particularly debilitating for daily living activities. While the adoption of ankle-foot orthoses is very well established as gait treatment, the choice of the most appropriate orthotic configuration is not strongly supported by scientific evidence. The aim of this study was to develop an instrumented assessment protocol based on wearable gait analysis to support clinicians in ankle-foot orthoses configuration selection. METHODS: Ten children with spastic diplegic Cerebral Palsy were assessed (7 males, ag

Mechanical effectiveness of lateral foot wedging in medial knee osteoarthritis after 1 year of wear.

The use of lateral foot wedging in the management of medial knee osteoarthritis is under scrutiny. Interestingly, there have been minimal efforts to evaluate biomechanical effectiveness with long-term use. Therefore, we aimed to evaluate dynamic knee loading (assessed using the knee adduction moment) and other secondary gait parameters in patients with medial knee osteoarthritis wearing lateral foot wedging at a baseline visit and after 1 year of wear. Three-dimensional gait data were captured in an intervention group of 19 patients with symptomatic medial knee osteoarthritis wearing their pre

Biomechanical evaluation of the efficacy of external stabilizers in the conservative treatment of acquired flatfoot deformity.

This study quantified and compared the efficacy of in-shoe orthoses and ankle braces in stabilizing the hindfoot and medial longitudinal arch in a cadaveric model of acquired flexible flatfoot deformity. This was addressed by combining measurement of hindfoot and arch kinematics with plantar pressure distribution, produced in response to axial loads simulating quiet standing. Experiments were conducted on six fresh-frozen cadaveric lower limbs. Three conditions were tested: intact-unbraced; flatfoot-unbraced; and flatfoot-braced. Flatfoot deformity was created by sectioning the main support st

Enhancing Functional Rehabilitation Through Orthotic Interventions for Foot and Ankle Conditions: A Narrative Review.

Non-surgical, conservative approaches to foot and ankle conditions are of important consideration. Orthotics play a significant role in treating these conditions, preventing progression, and alleviating pressure on affected areas, thereby promoting normal gait. This article aims to assess the utility and effectiveness of various orthotic treatments in different clinical scenarios. We reviewed 27 peer-reviewed articles using electronic databases, employing keywords such as "orthoses," "orthotic treatment," "arthritis," "neuropathy," and "foot and ankle trauma." Studies conducted in recent decad

The influence of ankle-foot orthosis stiffness on walking performance in individuals with lower-limb impairments.

BACKGROUND: Passive-dynamic ankle-foot orthoses utilize stiffness to improve gait performance through elastic energy storage and return. However, the influence of ankle-foot orthosis stiffness on gait performance has not been systematically investigated, largely due to the difficulty of manufacturing devices with precisely controlled stiffness levels. Additive manufacturing techniques such as selective laser sintering have been used to successfully manufacture ankle-foot orthoses with controlled stiffness levels. The purpose of this study was to use passive-dynamic ankle-foot orthoses manufact

Effectiveness of a 3D-printed silicone medial arch support on foot pain in individuals with pes planus: A randomized controlled trial.

BACKGROUND: Pes planus, a foot deformity that causes foot pain and functional limitations, is often treated with custom-made foot orthoses as a conservative approach to managing symptoms. In this study, silicone medial arch orthotics produced by 3D printing technology were evaluated for their ability to reduce pain and improve foot function, compared to conventional Total Contact Insoles (TCI). OBJECTIVES: To assess the effectiveness of 3D-printed orthoses made from soft and hard silicone by evaluating primary outcomes (pain and foot function) and secondary outcomes (plantar pressure, heel va
