Measurement of health status, functional status, and quality of life in children with juvenile idiopathic arthritis: clinical science for the pediatrician.

Several groups have undertaken research on health status, functional status, and quality of life in the pediatric rheumatic diseases, particularly juvenile idiopathic arthritis (JIA) and juvenile rheumatoid arthritis. This article highlights the principles involved in this type of measurement, discusses the measures that have been developed for JIA, and describes the outcomes determined from recent retrospective and prospective longitudinal outcome studies. These studies suggest that although there has been improvement in overall outcomes, significant numbers of individuals persist with active

B vitamins in breast milk: relative importance of maternal status and intake, and effects on infant status and function.

Infants should be exclusively breastfed for the first 6 mo of life. However, maternal deficiency of some micronutrients, conveniently classified as Group I micronutrients during lactation, can result in low concentrations in breast milk and subsequent infant deficiency preventable by improving maternal status. This article uses thiamin, riboflavin, vitamin B-6, vitamin B-12, and choline as examples and reviews the evidence for risk of inadequate intakes by infants in the first 6 mo of life. Folate, a Group II micronutrient, is included for comparison. Information is presented on forms and conc

A new pediatric trauma center quality benchmarking metric: Center-level variability in postinjury functional impairment using the Functional Status Scale (FSS).

BACKGROUND: Over 7 million children are injured annually in the United States, leading to 500,000 disability-adjusted life years. Functional outcomes after injury, however, are not routinely measured. Whether disability is modifiable by trauma center care remains unclear. The purpose of this study was to assess variability between trauma centers in functional impairment rates. METHODS: The "Assessment of Health-Related Quality of Life and Functional Outcomes after Pediatric Trauma" prospective observational dataset was analyzed. Functional status among injured children (≤14 y) was assessed at

Functional decline after severe COVID-19: duke activity status index for preoperative risk assessment.

INTRODUCTION: Survivors of severe COVID-19 (Coronavirus Disease 2019) frequently experience long-term functional impairment, a condition known as post-COVID syndrome. As these patients may eventually undergo surgical procedures, evaluating their functional capacity becomes essential for safe anesthetic management. The Duke Activity Status Index (DASI) is a validated, easy-to-administer tool that correlates with cardiopulmonary fitness and surgical risk. OBJECTIVE: To evaluate changes in functional capacity using DASI scores in patients who survived severe COVID-19, comparing pre- and post-inf

Association between electrographic seizures and better neurological functional outcomes in non-convulsive status epilepticus.

OBJECTIVE: In non-convulsive status epilepticus (NCSE), predicting patient outcomes is crucial in deciding treatment intensity. However, electroencephalograms (EEGs) of NCSE show various patterns, and their relationship with functional outcomes remains unclear. We aimed to examine the relationship between NCSE EEG patterns and functional outcomes. METHODS: We retrospectively collected data from 105 NCSE patients without cardiopulmonary arrest at two centers and compared functional outcomes in NCSE patients with electrographic seizures (ESz) and those with rhythmic/periodic patterns (RPPs) at

Distinct functional connectivity phenotypes in preadolescent children with binge eating disorder by BMI status.

OBJECTIVE: The neurobiological mechanisms underpinning binge eating disorder (BED) in children remain largely unclear, as the alterations that have been identified to date may be attributable to BED, obesity, or compound effects. This study aimed to delineate functional connectivity (FC) patterns in inhibitory control and reward networks in preadolescent children with and without BED from the Adolescent Brain Cognitive Development (ABCD) Study according to BMI. METHODS: Resting-state FC was examined in the inhibitory control network by using seeds in the dorsolateral prefrontal cortex, the an

A systematic review and meta-analysis of functional vitamin B12 status among adult vegans.

The dietary intake of vitamin B12 among unsupplemented vegans is notably lower compared to both vegetarians and omnivores. Prolonged low intakes of vitamin B12, such as seen in those adhering to a vegan diet, lead to physiological deficiency of vitamin B12 and an elevated risk of B12-related morbidity. However, while serum B12 serves as a conventional biomarker for assessing B12 status, its utility is limited given its sensitivity and specificity in ascribing physiological deficiency of B12 and the functional vitamin B12 status of those adhering to vegan diets is unclear. We conducted a system

Premorbid functional status in patients requiring veno-arterial extracorporeal membrane oxygenation after cardiac surgery.

Assessment of a patient's functional status prior to undergoing cardiac surgery may be a useful marker for predicting outcomes when postoperative veno-arterial extracorporeal membrane oxygenation (V-A ECMO) is required. In this short communication, we present retrospective data on 83 patients at a single center who required V-A ECMO after cardiac surgery. Our results did not show a statistically significant association between premorbid functional status and mortality, though age was predictive of mortality. Future studies should explore other markers of functional status and relationships wit
