Combination of Forced Diuresis with Additional Late Imaging in 68Ga-PSMA-11 PET/CT: Effects on Lesion Visibility and Radiotracer Uptake.

Renal excretion of some prostate-specific membrane antigen (PSMA) ligands and consequently increased bladder activity can obscure locally relapsing prostate cancer lesions in PSMA PET/CT. Furthermore, additional late imaging in PSMA PET/CT provides a useful method to clarify uncertain findings. The aim of this retrospective study was to investigate a modified imaging protocol combining late additional imaging with hydration and forced diuresis in individuals undergoing additional late scanning for uncertain lesions or low prostate-specific antigen. Methods: We compared an older protocol with a

Impact of Metastasis-directed Therapy Guided by Different PET/CT Radiotracers on Distant and Local Disease Control in Oligorecurrent Hormone-sensitive Prostate Cancer: A Secondary Analysis of the PRECISE-MDT Study.

Prospective trials suggest that metastasis-directed therapy (MDT) is an effective treatment for patients with oligometastatic prostate cancer (PCa). Gallium 68 (68Ga) prostate-specific membrane antigen (PSMA)-11 PET/CT-guided MDT seems to improve the oncologic outcome in these patients compared with fluorine 18 (18F)-fluorocholine and 18F-PSMA-1007 PET/CT-guided MDT, but the effects in terms of local or distant disease control remain unclear. Thus, the present subanalysis of the PRECISE-MDT study analyzed patients with hormone-sensitive PCa who underwent MDT guided by PET/CT for nodal or bone

Prostate-Specific Membrane Antigen-Avid Meningioma Detected in 68Ga-Prostate-Specific Membrane Antigen PET/CT.

A 64-year-old man with recently diagnosed prostate adenocarcinoma with a total prostate-specific antigen level of 3.32 ng/mL and Gleason score 4 (3 + 4) was referred for Ga-prostate-specific membrane antigen PET/CT scan for pretreatment staging. Prostate-specific membrane antigen-avid lesion was noted in the right lobe of the prostate gland, corresponding to the known primary lesion. Note was made of another prostate-specific membrane antigen-avid lesion in the right parietal lobe of brain, likely meningioma. MRI scan performed later confirmed the brain lesion to be meningioma.

Regularized reconstruction of digital time-of-flight 68Ga-PSMA-11 PET/CT for the detection of recurrent disease in prostate cancer patients.

Accurate localization of recurrent prostate cancer (PCa) is critical, especially if curative therapy is intended. With the aim to optimize target-to-background uptake ratio in 68Ga-PSMA-11 PET, we investigated the image quality and quantitative measures of regularized reconstruction by block-sequential regularized expectation maximization (BSREM). Methods: The study encompassed retrospective reconstruction and analysis of 20 digital time-of-flight (TOF) PET/CT examinations acquired 60 min post injection of 2 MBq/kg of 68Ga-PSMA-11 in PCa patients with biochemical relapse after primary treatmen

[PSMA-radioguided surgery in localized recurrent prostate cancer : Current and future aspects].

Recently, PSMA-radioguided surgery (PSMA-RGS) was introduced for targeted resection of localized prostate cancer recurrence. Prerequisite for preoperative patient selection and localization of tumor recurrence is a positive 68Ga-HBED-CC PSMA positron emission tomography (PET) scan with preferably only singular soft tissue or lymph node recurrence. After injection of In-PSMA I&T or Tc-PSMA-I&S single photon emission computer tomography (SPECT)/computer tomography (CT) examination is performed in every patient to verify radiotracer uptake in tumor lesions. In a preliminary study, 111In-PSMA I&T

A modified technique for efficient radiolabeling of 68Ga-citrate from a SnO2-based 68Ge/68Ga generator for better infection imaging.

Our aim was to develop a practical method to prepare 68Ga-citrate using a SnO2-based 68Ge/68Ga generator and evaluate its use in infection imaging. 68Ga-citrate synthesis was performed in a straight forward, quantitative, one-step-aseptic procedure; an amended labeling method was applied using ACD-A buffered citrate as a precursor. Study participants were imaged on a Siemens Biograph 40 PET/CT scanner 60 min post intravenous injection. Our results showed: 90%-95% 68Ga-yield was obtained and subsequently used at 324-527 MBq to perform three to four parallel 68Ga-citrate syntheses. 68Ga-citrate

Can 99m Tc-PSMA SPECT/CT Presenting Mirror Image of 68 Ga-PSMA PET/CT Be Used for Response Assessment to 177 Lu-PSMA in Metastatic Castration-Resistant Prostate Cancer?

A 69-year-old man, with a known case of metastatic castration-resistant prostate cancer (mCRPC) underwent 68 Ga-PSMA PET/CT and 99m Tc-PSMA for evaluation of disease burden and response assessment to 177 Lu-PSMA therapy. 99m Tc-PSMA provided a mirror image of 68 Ga-PSMA. This suggests the potential of 99m Tc-PSMA SPECT/CT for disease evaluation and response assessment to PSMA-based radioligand therapies in advanced-stage mCRPC patients, especially when 68Ga-PSMA PET/CT is not available.

Additive Value of [68Ga]Ga-RM26 PET/CT to [68Ga]Ga-PSMA-617 PET/CT in Detecting Pelvic Lymph Node Metastasis in Prostate Cancer: A Prospective, Single-Center, Phase II Study.

Lymph node staging in prostate cancer is crucial for treatment and prognosis, yet [68Ga]Ga-PSMA-617 PET/CT has limited sensitivity in detecting pelvic lymph node metastasis (PLNM). [68Ga]Ga-RM26 PET/CT, targeting the gastrin-releasing peptide receptor, complements [68Ga]Ga-PSMA-617 PET/CT in assessing primary tumor extension and aggressiveness. However, its role in detecting PLNM and complementing [68Ga]Ga-PSMA-617 PET/CT remains underexplored. Methods: This prospective study enrolled newly diagnosed yet untreated prostate cancer patients who underwent [68Ga]Ga-RM26 PET/CT and [68Ga]Ga-PSMA-61

Early Injection of Furosemide Increases Detection Rate of Local Recurrence in Prostate Cancer Patients with Biochemical Recurrence Referred for 68Ga-PSMA-11 PET/CT.

The aim of this study was twofold. First, we aimed to assess the impact of forced diuresis with early furosemide injection on the detection rate of local recurrence in prostate cancer patients with biochemical recurrence referred for 68Ga-labeled Glu-NH-CO-NH-Lys(Ahx)-HBED-CC (68Ga-PSMA-11) PET/CT. Second, we determined whether intravenous administration of furosemide shortly after tracer injection increases renal washout of 68Ga-PSMA-11 before it binds to the PSMA receptor with possible influence on biodistribution and intensity of tracer uptake in organs with physiologic tracer accumulation.
