AIM: To evaluate the cost-effectiveness of first-line treatments, such as erlotinib, gefitinib, afatinib, dacomitinib, and osimertinib, for patients diagnosed with stage IIIB/IV NSCLC harboring EGFR mutations.
MATERIALS & METHODS: A partitioned survival model was developed to estimate quality-adjusted life-year (QALY) and incremental cost-effectiveness ratio (ICER) from the perspective of the Spanish National Health System. Two Bayesian NMAs were performed independently, by using the polynomial fraction method to fit Kaplan-Meier curves for overall survival and progression-free survival. Dete

PURPOSE: Brain metastases (BrMs) frequently manifest in patients with epidermal growth factor receptor (EGFR)-mutant non-small cell lung cancer (NSCLC), and the optimal treatment approach for these individuals remains controversial. This study aimed to evaluate the cost-effectiveness of adding pemetrexed-platinum chemotherapy to first-line gefitinib in EGFR-mutant NSCLC patients with BrMs, considering the perspective of the Chinese healthcare system.
PATIENTS AND METHODS: Constructing a Markov model, we simulated disease progression over a 10-year horizon in 4-week intervals. The model includ

The malignant behavior of non-small cell lung cancer (NSCLC) is caused by different driver mutations, which may include alterations in the epidermal growth factor receptor (EGFR) signaling pathway. Activating mutations in exons 19 or 21 of EGFR in NSCLC are associated with increased sensitivity to EGFR tyrosine kinase inhibitors (TKIs) such as gefitinib and erlotinib. However, approximately 10% of NSCLC patients show primary resistance to TKIs, and the resistance mechanism is poorly understood. We report the case of a 72-year-old nonsmoking Caucasian woman who underwent pulmonary segmentectomy

The crystallinity of drug-loaded lipid nanoparticles is believed to affect drug release rates; however, effective control over lipid crystallinity has not been achieved by current lipid nanoparticle preparation methods. The present study investigates control over the crystallinity of drug-loaded nanoparticle aerosol lipid matrices (NALM) through differences in evaporation rates of precursor solution drops and the subsequent control over drug release rates from these matrices. Gefitinib-loaded NALM were synthesized in an aerosol reactor using precursor solutions of gefitinib and stearic acid at

The return of blood flow to ischemic heart after myocardial infarction causes ischemia-reperfusion injury. There is a clinical need for novel therapeutic targets to treat myocardial ischemia-reperfusion injury. Here we screened for targets for the treatment of ischemia-reperfusion injury using a combination of shRNA and drug library analyses in HL-1 mouse cardiomyocytes subjected to hypoxia and reoxygenation. The shRNA library included lentiviral constructs targeting 4625 genes and the drug library 689 chemical compounds approved by the Food and Drug Administration (FDA). Data were analyzed us

The EGFR-TKIs (epidermal growth factor receptor-tyrosine kinases inhibitors) offer significant benefits to lung cancer patients with sensitive EGFR mutations; however, the development of acquired resistance poses a significant challenge and leads to poor prognosis. Thus, exploring novel therapeutic strategies to overcome EGFR-TKI resistance is urgently needed. This study introduces an innovative approach utilizing folic acid-modified milk exosomes loaded with c-kit siRNA (FA-mExo-siRNA-c-kit) to target EGFR-TKI resistance in lung cancer. Initially, gefitinib-resistant lung cancer cells exhibit

BACKGROUND: Tyrosine kinase inhibitors (TKIs) that act against the epithelial growth factor receptor (EGFR) were once widely used in chemotherapy for many human cancers. However, acquired chemoresistance occurred in almost all patients, limiting the clinical application of EGFR-TKI. Thus far, no effective methods existing can resolve this problem. Designing a therapeutic treatment with a specific multi-target profile has been regarded as a possible strategy to overcome acquired EGFR-TKI resistance.

BACKGROUND: Considering cell cycle dependent cytotoxicity, intercalation of chemotherapy and epidermal growth factor receptor (EGFR) tyrosine kinase inhibitor (TKI) may be a treatment option in non-small cell lung cancer (NSCLC). This randomized phase 2 study compared the efficacy of paclitaxel and carboplatin (PC) intercalated with gefitinib (G) versus PC alone in a selected, chemotherapy-naïve population of advanced NSCLC patients with a history of smoking or wild-type EGFR.
METHODS: Eligible patients were chemotherapy-naïve advanced NSCLC patients with Eastern Cooperative Oncology Group pe

Lung cancer is the leading cause of cancer-related death and has the second-highest incidence worldwide. For patients with advanced EGFR-mutated non-small cell lung cancer, EGFR tyrosine kinase inhibitors (EGFR-TKIs) are the preferred treatment option; however, acquired resistance to TKIs is inevitable. Gefitinib and osimertinib, the first-generation and third-generation EGFR-TKI, have shown promising results in patients with EGFR-mutated lung cancer in clinical treatment. Here, we identified that pyruvate dehydrogenase kinase 1 (PDK1) was up-regulated in gefitinib- and osimertinib-resistant c

A 61-year-old never-smoking woman with stage IV lung adenocarcinoma with initially unknown epidermal growth factor receptor (EGFR) status, lung metastasis, pleural dissemination, and malignant pleural effusion in 2007 received 10 prior anti-cancer regimens including gefitinib as second- and ninth-line treatments, with minimal efficacy with gefitinib. EGFR mutation analysis performed in pleural effusion specimens during ninth-line gefitinib was negative. In 2015, she developed progressive disease with peritoneal dissemination. Interstitial lung disease (ILD) is a toxicity of epidermal growth factor receptor tyrosine kinase inhibitors (EGFR-TKIs). We characterized agent-specific fatality and timing patterns in the Japanese Adverse Drug Event Report database (JADER). We analyzed March 2026 JADER reports accepted from 2004 Q4 through 2025 Q2. Eligible reports involved single-agent osimertinib, gefitinib, erlotinib, or afatinib as the suspected drug for non-small cell lung cancer-related indications and were coded with the core ILD term. Fatal outcome, time to onset (TTO), and operationally defined pre-existing lung dis

A series of quinazoline-based PAK4-selective inhibitors were designed via structure-based modification of leads Czh-226 and compound 12i using a heterocyclic tail strategy. Kinase and cell viability assays identified two close analogues, 11j and 11k, differing only by the position of one nitrogen atom. Both showed low nanomolar PAK4 inhibition, high selectivity over PAK1, and low-micromolar antiproliferative activity against three TNBC cell lines, with 11j consistently more potent. Molecular docking and dynamics simulations indicated that 11j formed more stable interactions with the PAK4 catal

A series of 5-benzylidene-2-hydrazinomethine-thiazolidin-4-one derivatives 5a-y was designed and synthesized based on advanced EGFR TKIs. In vitro EGFR inhibition screening identified twelve compounds with activity comparable to or exceeding that of gefitinib and osimertinib. Thirteen compounds were selected from the NCI for single-dose assay leading to five-dose evaluation of four promising candidates. Compound 5c emerged as the most promising candidate with an IC50 value of 0.090 μM outperforming osimertinib (0.540 μM) and closely matching gefitinib (0.076 μM). In the NCI-60 panel, 5c showed

BACKGROUND: Afatinib, Dacomitinib, Osimertinib, Aumolertinib, Furmonertinib, Gefitinib, Erlotinib, and Icotinib have all been shown to work and be safe for people with epidermal growth factor receptor (EGFR)-positive non-small cell lung cancer (NSCLC) in recent years, but differences in efficacy, safety, and lack of comparative trials cause clinical confusion in treatment selection. This study analyzes their efficacy and safety via network meta-analysis to inform clinical decisions.

BACKGROUND: Epidermal growth factor receptor (EGFR) mutation positive (M+) non-small cell lung cancer (NSCLC) is an important subtype of lung cancer comprising 10% to 15% of non-squamous tumours. This subtype is more common in women than men, is less associated with smoking, but occurs at a younger age than sporadic tumours.
OBJECTIVES: To assess the clinical effectiveness of single-agent or combination EGFR therapies used in the first-line treatment of people with locally advanced or metastatic EGFR M+ NSCLC compared with other cytotoxic chemotherapy (CTX) agents used alone or in combination

OBJECTIVES: Lung cancers harboring epidermal growth factor receptor (EGFR) mutations inevitably develop resistance to EGFR tyrosine-kinase inhibitors (EGFR-TKIs). Therefore, we sought to establish clinically relevant lung-cancer mouse models to achieve deep remission of cancers.
MATERIALS AND METHODS: We previously established two transgenic lung-cancer mouse models harboring human EGFR exon 21 L858R substitution (hLR) and mouse Egfr exon 19 deletion (mDEL) in the C57BL/6 J background. Lung tumors from these two transgenic mouse strains were transplanted subcutaneously into BALB/c-nunu mice o

The epidermal growth factor receptor (EGFR) signaling pathway is widely activated in non-small cell lung cancer (NSCLC). However, only a subset of patients with NSCLC is sensitive to EGFR tyrosine kinase inhibitors (TKIs), particularly those with activating EGFR mutations. The mammalian target of rapamycin (mTOR) is another key intracellular kinase that plays an important role in the onset and progression of many types of human cancers and has been proven to be linked with primary resistance to EGFR inhibitors. We performed this study to investigate the combined inhibitory effect of the mTOR i
