Novel Heterozygous Variant in RP1L1 Gene With Retinitis Pigmentosa Phenotype: A Case Report.

This report presents a novel heterozygous mutation in RP1L1 resulting in an asymptomatic retinitis pigmentosa (RP) phenotype. A 37-year-old woman with no visual complaints and 20/20 best-corrected visual acuity was incidentally found to have bilateral retinal pigment mottling and diffuse speckled hypoautofluorescence in both eyes. Ultra-high-resolution optical coherence tomography (OCT) showed intact outer retinal structures. Visual field testing only showed nonspecific defects, in the context of multiple fixation losses and poor reliability. Genetic testing identified a novel mutation in the

Case Report: A Novel Homozygous Variant in the SLX4 Gene Causes Fanconi Anemia.

Background: Fanconi anemia (FA) is a rare genetic disorder that affects multiple systems in the body and is the most prevalent congenital syndrome, leading to bone marrow failure. Twenty-two genes have been identified as contributors to the disease. Significant advancements have been made in the past 2 decades in understanding the genetic and pathophysiological processes involved. Whole exome sequencing (WES) is employed to diagnose rare Mendelian disorders when standard tests fail to provide a definitive pathological diagnosis. However, WES has the potential to reveal pathogenic variants that

Extrarenal Clinical Features are Reported for Most Genes Implicated in Genetic Kidney Disease.

INTRODUCTION: Genetic kidney disease is often suspected based on a family history of the disease or the presence of extrarenal features. This study examined how often a positive family history or syndromic features are found. METHODS: A total of 255 genes from the Genomics England "green" lists for congenital anomalies of the kidney and urinary tract (CAKUT) (n = 57), ciliopathies and cystic kidney diseases (n = 90), hematuria (n = 5), renal proteinuria (n = 55), and renal tubulopathies (n = 48) were examined for mode of inheritance and, in the Online Mendelian Inheritance in Man (OMIM), for

Heterozygous CYP27A1 gene mutation presenting with Achilles tendon xanthoma: a case report.

BACKGROUND: Cerebrotendinous xanthomatosis is a rare autosomal recessive lipid storage disorder involving bile acid biosynthesis. Reduced mitochondrial cytochrome P450 enzyme activity leads to abnormal lipid accumulation in various tissues, especially tendons, lenses, and the central and peripheral nervous systems. This condition manifests with systemic symptoms such as neurological disorders, atherosclerosis, tendon xanthomas, and cataracts. Cerebrotendinous xanthomatosis typically presents in individuals with homozygous or compound heterozygous mutations in the CYP27A1 gene because of its au

Missense mutation (Ser654Leu) in the ITGA8 gene associated with renal hypodysplasia: A case report.

Renal hypodysplasia is a congenital kidney anomaly that occurs when part of the kidney does not fully develop in the womb. Numerous genes have been identified that when mutated, result in renal dysplasia. This has encouraged the authors to search for additional genes and variants potentially linked to renal anomalies. Using next-generation sequencing combined with Sanger sequencing, a missense variant in the ITGA8 gene (NM_003638.2:c.1961C>T; p.Ser654Leu) associated with renal hypodysplasia was identified. Detailed studies on this variant revealed that Ser654 is conserved across species, and t

Identification of Compound Heterozygous Variants in OBSCN Gene Associated With Rhabdomyolysis: A Case Report.

BACKGROUND: The obscurin protein encoded by the OBSCN gene is an important structural protein in the regulation of myocyte sarcoplasmic nodule stability and sarcoplasmic reticulum function and is particularly closely associated with calcium ion (Ca2+) signaling. With increasing genomic studies, pathogenic variants in the OBSCN gene have been shown to be associated with a variety of inherited diseases, such as cardiomyopathy. However, case reports of its variants causing rhabdomyolysis are more limited. METHODS: We performed whole exome sequencing on a patient with exercise-induced rhabdomyoly

Identification of nonsense variants in the ATM gene mimicking SCID phenotype: a brief report.

Severe combined immunodeficiency (SCID) represents a life-threatening inborn error of immunity, necessitating rapid diagnosis and intervention to prevent fatal outcomes. While SCID is characterized by profound T-cell lymphopenia, it may overlap with other conditions like ataxia-telangiectasia (AT), which also presents with T-cell deficiencies. This study examines two cases of suspected SCID in infants, later identified as AT due to pathogenic variants in the ATM gene. Despite initial negative results from SCID-targeted gene panels, further genetic testing revealed nonsense mutations (p.Y2036X

A mutation in transthyretin gene in a Mexican patient leading to hereditary amyloidosis: a case report.

BACKGROUND: Transthyretin is a protein produced by the liver, and when normal, it carries out the role of transporting thyroid hormone and vitamin A in the body. Mutations in the gene that codes for this protein can cause it to misfold. A misfolded protein can not carry out its functions and can also build up in different organs leading to a group of diseases known as amyloidosis. Depending upon the site in which this protein accumulates, a large variety of symptoms can be seen. If the protein deposits in the heart, it can lead to heart failure and associated symptoms and is known as cardiac a

Human Leukocyte Antigen-G Gene Polymorphism in Peninsular Malaysia: A Preliminary Report.

Introduction: Expression of the nonclassical human leukocyte antigen (HLA)-G gene is upregulated in placenta during pregnancy. In other cells, HLA-G is upregulated during parasitic infections and allergic reactions. Polymorphism at the HLA-G gene locus has been reported for many populations, but so far not for any ethnic groups in Malaysia. In this survey, we screened for genetic variation in HLA-G genes from representative Malay, Chinese, and Indian individuals living in Peninsular Malaysia. Materials and Methods: Blood samples were obtained with informed consent, and ethnicity classes were a

Oligomeganephronia with PAX2 gene deletion diagnosed at the third renal biopsy: a case report.

Oligomeganephronia is a congenital anomaly of the kidney and urinary tract. It is often categorized as one of the hypoplastic kidney conditions. The pathological diagnosis of oligomeganephronia is challenged by the absence of clear diagnostic criteria, which often leads to subjective interpretations by pathologists. This report presents the case of a 7-year-old girl who was diagnosed with oligomeganephronia through a third renal biopsy, which was confirmed by gene analysis revealing PAX2 deletion. Two previous renal biopsies, with the naked eye through a microscope, failed to identify glomerul

Dasatinib for chronic myelomonocytic leukemia with ZMIZ1-ABL1 fusion gene: a case report.

The fusion gene ZMIZ1-ABL1 is rare, with only one known case reported in lymphatic system malignancies, and none reported in a myeloid tumor. Here, we report the case of a patient with chronic myelomonocytic leukemia with the ZMIZ1-ABL1 fusion gene. Elevated leukocytes and splenomegaly were the main manifestations. Remission was achieved with the second-generation tyrosine kinase inhibitor (TKI) dasatinib, and the response has been sustained for 10 months. The treatment results in this case suggest that dasatinib is effective in treating ZMIZ1-ABL1 fusion gene-positive disease.
