De Novo exposomic geospatial assembly of chronic disease regions with machine learning & network analysis.

BACKGROUND: Determining spatial relationships between diseases and the exposome is limited by available methodologies. aPEER (algorithm for Projection of Exposome and Epidemiological Relationships) uses machine learning (ML) and network analysis to find spatial relationships between diseases and the exposome in the United States. METHODS: Using aPEER we examined the relationship between 12 chronic diseases and 186 pollutants. PCA, K-means clustering, and map projection produced clusters of counties derived from pollutants, and the Jaccard correlation between these clusters with chronic diseas

Tracking Cardiac Rehabilitation Participation and Completion Among Medicare Beneficiaries to Inform the Efforts of a National Initiative.

BACKGROUND: Despite cardiac rehabilitation (CR) being shown to improve health outcomes among patients with heart disease, its use has been suboptimal. In response, the Million Hearts Cardiac Rehabilitation Collaborative developed a road map to improve CR use, including increasing participation rates to ≥70% by 2022. This observational study provides current estimates to measure progress and identifies the populations and regions most at risk for CR service underutilization. METHODS AND RESULTS: We identified Medicare fee-for-service beneficiaries who were CR eligible in 2016, and assessed CR

Gut Biogeography Accentuates Sex-Related Differences in the Murine Microbiome.

Recent studies have highlighted the influence of factors such as sex and sex-linked hormones on microbiome composition, raising concerns about the generalizability of findings. Here, we explore whether gut geography, specifically the upper and lower gastrointestinal tract (GI), contributes to sex-linked microbiome differences in mice. We collected microbial samples throughout the length of the GI from male and female C57B6/J mice at 6- and 8-weeks old, and conducted 16S rRNA sequencing. Our findings revealed significant sex-related differences, with Clostridium_sensu_stricto_1 more abundant in

Prevalence, burden and manifestation of atrial fibrillation: a large-scale analysis of cardiac patch monitors.

BACKGROUND: Atrial fibrillation (AF) is frequently paroxysmal and asymptomatic, making its detection and burden quantification challenging. The distribution of AF burden across clinically monitored populations remains poorly characterised. We sought to describe the distribution of AF burden and its variation by monitoring indication, symptomatology, arrhythmia characteristics and geography. METHODS: This study analysed cardiac patch monitor reports collected between May 2017 and July 2023. Analyses were stratified by monitoring indication, AF burden grouping and geography. AF burden subgroups

Association between HIV PrEP indications and use in a national sexual network study of US men who have sex with men.

INTRODUCTION: HIV pre-exposure prophylaxis (PrEP) is effective in preventing HIV transmission. United States Public Health Service (USPHS) clinical practice guidelines define biobehavioral indications for initiation. To assess guideline implementation, it is critical to quantify PrEP nonusers who are indicated and PrEP users who are not indicated. We sought to estimate current PrEP use among US men who have sex with men (MSM), characterize whether their PrEP use aligned with their current indications for PrEP, and assess whether the association between PrEP indications and PrEP use differed by

Dietary Effects, Age, and Urban-Rural Dynamics in Shaping Gut Microbiota of Elderly Vietnamese: A Cross-Sectional Study.

Aging is associated with alterations in gut microbiota, yet the combined effects of geography and diet remain underexplored in elderly populations. This study investigated the gut microbiota of 227 healthy Vietnamese individuals aged ≥60 years, stratified by select urban and rural residence in both Hanoi and Thanh Hoa provinces, and across three age groups (60-69, 70-79, ≥80 years). Dietary patterns were collected and recorded for each participant. 16S rRNA gene sequencing revealed significant differences in microbial diversity and composition associated with geographical location (urban, rura

A machine learning approach for modeling the occurrence of the major intermediate hosts for schistosomiasis in East Africa.

Schistosomiasis, a prevalent water-borne disease second only to malaria, significantly impacts impoverished rural communities, primarily in Sub-Saharan Africa where over 90% of the severely affected population resides. The disease, majorly caused by Schistosoma mansoni and S. haematobium parasites, relies on freshwater snails, specifically Biomphalaria and Bulinus species, as crucial intermediate host (IH) snails. Targeted snail control is advisable, however, there is still limited knowledge about the community structure of the two genera especially in East Africa. Utilizing a machine learning

Geographic variation of parathyroidectomy in patients receiving hemodialysis: a retrospective cohort analysis.

BACKGROUND: Secondary hyperparathyroidism (SHPT) is associated with adverse outcomes in patients receiving maintenance dialysis. Parathyroidectomy is a treatment for SHPT; whether parathyroidectomy utilization varies geographically in the US is unknown. METHODS: A retrospective cohort analysis was undertaken to identify all patients aged 18 years or older who were receiving in-center hemodialysis between 2007 and 2009, were covered by Medicare Parts A and B, and had been receiving hemodialysis for at least 1 year. Parathyroidectomy was identified from inpatient claims using relevant Internati

Sleeping sickness in West Africa (1906-2006): changes in spatial repartition and lessons from the past.

OBJECTIVE: To review the geography and history of sleeping sickness (Human African trypanosomiasis; HAT) over the past 100 years in West Africa, to identify priority areas for sleeping sickness surveillance and areas where HAT no longer seems active. METHOD: History and geography of HAT were summarized based on a review of old reports and recent publications and on recent results obtained from medical surveys conducted in West Africa up to 2006. RESULTS/CONCLUSIONS: Active HAT foci seem to have moved from the North to the South. Endemic HAT presently appears to be limited to areas where annu

Capturing geographical inequalities in children's experience of dental health support using synthetic estimation technique and understanding their relationship with geographical inequalities in child dental health in England.

This study aimed to estimate the proportion of families with five-to-fifteen-year-olds experiencing inadequate dental health support as a consequence of their parents' socioeconomic conditions and deprivation in their neighbourhood across small geographies in England using synthetic estimation technique and explore its association with area-level proportion of children experiencing dental caries. Secondary analyses were performed using Children's Dental Health Survey (CDHS) 2013, National Dental Epidemiological Programme (NDEP) 2018-2019, and Census 2011. Multilevel logistic regression using C

"There Would Be More Black Spaces": Care/giving Cartographies during COVID-19.

Black geographies, Black feminist anthropology, and related fields have provided substantial evidence attesting to the effects of racially violent spatial practices such as dispossession, racial segregation, mass incarceration, and redlining for the health outcomes and life chances of Black communities and other racialized groups, and conversely, the political and healing potential of placemaking projects. We foreground theory from Black geographies and Black feminist work on care to examine care/giving cartographies at the height of the COVID-19 pandemic. We present findings from semi-structu

Modeling peer effect modification by network strength: The diffusion of implantable cardioverter defibrillators in the US hospital network.

We develop methodology that allows peer effects (also referred to as social influence and contagion) to be modified by the structural importance of the focal actor's position in the network. The methodology is first developed for a single peer effect and then extended to simultaneously model multiple peer-effects and their modifications by the structural importance of the focal actor. This work is motivated by the diffusion of implantable cardioverter defibrillators (ICDs) in patients with congestive heart failure across a cardiovascular disease patient-sharing network of United States hospita

Global, regional, national, and selected subnational levels of stillbirths, neonatal, infant, and under-5 mortality, 1980-2015: a systematic analysis for the Global Burden of Disease Study 2015.

BACKGROUND: Established in 2000, Millennium Development Goal 4 (MDG4) catalysed extraordinary political, financial, and social commitments to reduce under-5 mortality by two-thirds between 1990 and 2015. At the country level, the pace of progress in improving child survival has varied markedly, highlighting a crucial need to further examine potential drivers of accelerated or slowed decreases in child mortality. The Global Burden of Disease 2015 Study (GBD 2015) provides an analytical framework to comprehensively assess these trends for under-5 mortality, age-specific and cause-specific mortal

Tick-borne encephalitis virus-neutralizing antibodies in different immunoglobulin preparations.

Patients with primary immunodeficiency (PIDs) depend on the presence of a variety of antibody specificities in intravenous immunoglobulin (IVIG). Using the tick-borne encephalitis virus (TBEV), geographic variability in IVIG antibody content was shown. Care should therefore be exercised when treating PIDs in a given geography, as only locally sourced plasma contains the antibody specificities against the circulating pathogens in the given locality.

Cardiology across continents, case series session #15: complicated post-op course in an infant with Tetralogy of Fallot.

Clinical experience during fellowship training and post-graduation depends on training length, resources, and time at specific institutions, contributing to knowledge deficits. Institutional collaboration overcomes such barriers wherein case discussions share knowledge and management strategy, enhancing patient outcomes. Our discussion of the management of Tetralogy of Fallot highlights key insights between programmes differing in geography and resources, hosted by Heart University.

Towards standardized gut microbiota diagnostics: normobiosis beyond geographical borders.

Defining clinically meaningful reference states of the human gut microbiota remains a major barrier to the clinical translation of microbiome testing, largely due to variability across populations. We aimed at evaluating whether dysbiosis can be identified in a standardized, geography-independent manner, using a composite, system-level microbiome diagnostic framework. We performed a retrospective observational analysis of 831 adult stool samples collected from healthy individuals and patients with inflammatory bowel disease, irritable bowel syndrome, or other chronic inflammatory conditions ac

Ecology and environment predict spatially stratified risk of H5 highly pathogenic avian influenza clade 2.3.4.4b in wild birds across Europe.

Highly pathogenic avian influenza (HPAI) represents a threat to animal and human health, with the ongoing H5N1 outbreak within the H5 2.3.4.4b clade being one of the largest on record. However, it remains unclear what factors have contributed to its intercontinental spread. We use Bayesian additive regression trees, a machine learning method designed for probabilistic modelling of complex nonlinear phenomena, to construct species distribution models (SDMs) for HPAI clade 2.3.4.4b presence. We identify factors driving geospatial patterns of infection and project risk distributions across Europe

Lateral epicondylitis treatment: international survey of surgeons' preferences and literature review.

BACKGROUND: Lateral epicondylitis (LE) is a common cause of elbow pain. Despite a relatively high prevalence and morbidity, there is still no single effective ('gold standard') treatment for LE. We hypothesised that a surgeon's experience, country of origin and area of expertise would influence choices concerning patient management. The purpose of this survey was to describe the current trends and common practices in treating LE worldwide. MATERIAL AND METHODS: A total of 291 orthopaedic surgeons of 12 subspecialties from 57 countries were surveyed on their choice of LE treatment modalities.

Using geographical information systems and cartograms as a health service quality improvement tool.

INTRODUCTION: Disease prevalence can be spatially analysed to provide support for service implementation and health care planning, these analyses often display geographic variation. A key challenge is to communicate these results to decision makers, with variable levels of Geographic Information Systems (GIS) knowledge, in a way that represents the data and allows for comprehension. The present research describes the combination of established GIS methods and software tools to produce a novel technique of visualising disease admissions and to help prevent misinterpretation of data and less opt
