BACKGROUND: Bisphosphonates are becoming recognized increasingly as having a significant impact on dental therapies. This case report describes adverse clinical sequelae and successful treatment following periodontal surgery in a dental patient receiving bisphosphonate treatment.
CASE DESCRIPTION: A 78-year-old woman experienced a nonhealing interproximal wound subsequent to a minor periodontal procedure performed to facilitate restoration of an adjacent tooth. Her medical history revealed that she had been taking an oral bisphosphonate every day for the previous five years for treatment of o

Gingivitis is a common inflammatory condition of the gingival tissues, influenced by microbial plaque and various systemic factors, including hormonal changes. Oral contraceptives containing estrogen and progestin are known to exacerbate gingival inflammation. We report the case of a 20-year-old Japanese woman who presented with gingival swelling and bleeding despite self-care, with a history of orthodontic treatment and oral contraceptive use. Clinical examination revealed probing pocket depths of ≥ 4 mm and bleeding on probing in more than half of the sites, a periodontal inflamed surface ar

Objectives: This study investigated patients' perceptions of laser-assisted esthetic crown lengthening (ECL) for treating "gummy smiles" using either an open-flap (OF) or flapless (FL) technique. Materials and Methods: This study included 36 healthy patients with altered passive eruption who underwent ECL surgery and were randomly divided into two equal groups: OF and FL. Gingivectomy and ostectomy were performed using an Er, Cr:YSGG laser in both groups. Patients' perceptions of postsurgical morbidity were assessed using a 5-point self-administered Likert scale. The participants provided rati

BACKGROUND: Gingival enlargement is a clinical condition that has been widely studied and is directly associated with specific local or systemic conditions. It is very rare that gingival enlargement presents without a clear underlying etiology.
METHODS: A 24-year-old African American female presented with gingival enlargement that manifested postpartum and was most likely related to changes in estrogen/progesterone. The gingival enlargement was treated with non-surgical and surgical periodontal therapies.
RESULTS: There was no evidence of recurrence of the gingival enlargement at 9 years pos

"Gingival enlargement" is the term now used to describe medication-related gingival overgrowth or gingival hyperplasia (AAP, 2004), a condition commonly induced by three main classes of drugs: anticonvulsants, antihypertensive calcium antagonists and the immunosuppressant cyclosporin. It is important that the health practitioner is aware of the potential aetiologic agents and characteristic features in order to be able to accurately diagnose and successfully manage patients who present with a condition such as outlined in the following case presentation.

The potential of periodontal ligament-derived tissues to regenerate periodontal attachment after cryosurgical trauma to the PDL in dogs was evaluated. The buccal alveolar plate of each canine tooth was exposed by a semi-lunar excision. A 3 mm thick cryoprobe, cooled to -81 degrees C, was placed on the bone 5 mm apical to the crest for 10 s. This induced cellular devitalization in the bone directly in contact with the probe and the PDL under it. The freezing-thawing cycle was repeated 3 times. Control sites were sham-operated at room temperature. Histologic sections from the center of the lesio

Two surgical techniques for exposure of impacted labially positioned maxillary canines were compared as to periodontal conditions at the end of the orthodontic treatment period. In the radical exposure group the alveolar mucosa and bone covering the crown were removed. In the other group a pedicle flap including keratinized gingiva was used for exposure. After the removal of bone the flap was apically positioned and sutured. The non-treated contralateral canines served as controls. The result showed that radical exposure in the region of alveolar mucosa resulted in significantly reduced width

OBJECTIVE: This article describes an interdisciplinary treatment that helped a patient with displaced upper anterior teeth. A gingivectomy, root canal therapies, digital smile design, digital wax-up, and guided tooth preparations were applied.
CLINICAL CONSIDERATIONS: A patient with pathologically migrated teeth asked for treatment without orthodontic involvement due to a primary failed orthodontic treatment history. A smile photo was taken and superimposed with the dentition in a CAD software to accomplish a digital smile design. The jaw movements were recorded with two different methods, a

BACKGROUND: Hereditary gingival fibromatosis (HGF) is an uncommon condition characterized by an accumulation of extracellular matrix resulting in a fibrotic enlargement of the gingiva. The goal of this article is to describe one kindred affected with HGF and discuss the diagnosis, treatment, and control of the disease. The pattern of inheritance, histopathologic characteristics, and proliferative potential of epithelial and mesenchymal cells of HGF are also emphasized.
METHODS: To characterize the pattern of inheritance and the clinical appearance of gingival overgrowth, 117 family members we

Organ transplant patients are frequently medicated with triple immunosuppressive therapy that includes both cyclosporin and the corticosteroid, prednisolone. Many of these patients experience gingival overgrowth that necessitates surgical intervention. Chronic dosing with corticosteroids can lead to suppression of the hypothalamic-pituitary axis, and subsequent adrenocortical suppression. To circumvent possible suppression, supplementary steroids are administered to such patients prior to so-called "stressful events". We have examined the need for supplementary steroids in 20 organ transplant

BACKGROUND: Restoration of structurally compromised endodontically treated teeth with extensive coronal destruction and subgingival margins remains a significant restorative challenge. Contemporary biomimetic restorative approaches emphasize preservation of the remaining tooth structure through adhesive reinforcement strategies that improve biomechanical performance while minimizing additional radicular dentin removal.
CASE PRESENTATION: This case report describes the biomimetic rehabilitation of a previously root canal-treated maxillary canine with severe coronal destruction and subgingival

BACKGROUND: Although psoriasis and chronic periodontitis (CP) may share an underlying immune dysregulation as part of their pathologies, to date only one small-scale cross-sectional pilot study has investigated the potential association between CP and psoriasis.
OBJECTIVES: This study aimed to investigate the subsequent risk for psoriasis following a diagnosis of CP by utilizing a cohort study design and population-based dataset in Taiwan.
METHODS: In total, 115 365 patients with CP were included in the study cohort and 115 365 patients without CP were included in the comparison cohort. We i

Sturge-Weber syndrome (SWS) is a very rare condition characterized by abnormal vascular formations that encompass several manifestations: cutaneous, neurologic, ocular, and oral. Neurologic conditions are the most important factor, especially epilepsy, which frequently leads patients to make use of anticonvulsants. These drugs are capable of inducing abnormal tissue growth in the oral cavity that can be situated over areas with vascular malformation, requiring special attention by the clinician. This report describes 1 case of SWS and performs a literature review of treatments for this conditi

Excessive gingival display (EGD) is an esthetic concern affecting a substantial portion of the population. Identification, diagnosis, and classification of all factors resulting in EGD are imperative for its appropriate management. While many authors have described these factors individually, the authors of the current study propose a simple classification, which includes major etiologies of EGD. Where EGD is associated with maxillary lip hypermobility, a proposal of a subclass 1-3 is offered. A "decision-making tree" to help guide clinicians in managing EGD is included. A detailed description

This case report describes a Class I crowded malocclusion with an ankylosed maxillary central incisor that was in infraocclusion and labially displaced. The patient had wide maxillary teeth, and the option of extracting the maxillary central incisors followed by space closure, with lateral incisors substituting for the central incisors, was chosen.

UNLABELLED: Dentists often encounter patients with missing or malformed teeth. The maxillary lateral incisor is the second most common congenitally absent tooth. There are three treatment options that exist for replacing missing lateral incisors. They include canine substitution, a tooth-supported restoration, or a single-tooth implant. Selecting the appropriate option depends on the malocclusion, specific space requirements, tooth-size relationship, and size and shape of the canine. The ideal treatment is the most conservative option that satisfies individual esthetic and functional requireme
