Pedal ectrodactyly: a case report with a new surgical management.

A case report is presented regarding a patient with type IV bilateral ectrodactyly treated with a double surgical approach: in forefoot to correct the malformation and in rearfoot to prevent secondary deformity of the subtalar joint. The forefoot was enlarged and in particular the second and third rays were absent. There was also a metatarsus primus varus with interphalangeal hallux abductus. The second cuneiform bone was removed with a wedge resection of the midfoot. The reduction in transverse diameter of the forefoot was obtained by cerclage of the first and fourth metatarsal bones. For hal

Treatment of first metatarsal longitudinal epiphyseal bracket by excision before closure.

The longitudinal metatarsal epiphyseal bracket is a rare ossification abnormality of tubular bones in which an abnormal epiphysis brackets the diaphysis of a phalanx, metacarpal, or metatarsal. This leads to progressive shortening and angular deformity of the involved bone. When it affects the first metatarsal, the longitudinal metatarsal epiphyseal bracket leads to a short, trapezoidal metatarsal. We present the case of a 6-year-old patient with an ossified longitudinal epiphyseal bracket along the medial side of the first metatarsal. The abnormality was surgically treated by bracket excision

Roger A. Mann Award. Juvenile hallux valgus: etiology and treatment.

In an 11-year retrospective study of 45 patients (60 feet) with juvenile hallux valgus, a multiprocedural approach was used to surgically correct the deformity. A Chevron osteotomy or McBride procedure was used for mild deformities, a distal soft tissue procedure with proximal first metatarsal osteotomy was used for moderate and severe deformities with MTP subluxation, and a double osteotomy (extra-articular correction) was used for moderate and severe deformities with an increased distal metatarsal articular angle (DMAA). The average hallux valgus correction was 17.2 degrees and the average c

Our experience with double metatarsal osteotomy in the treatment of hallux valgus.

Adolescent hallux valgus (HV) is a progressive deformity of adolescent age consisting of metatarsus primus varus and hallux valgus. It has a high recurrence rate after conventional surgical correction. Ten feet in nine patients (two males, seven females) were treated surgically with the Peterson Newman bunion procedure, with a minimum follow-up of one year. During the final follow-up all these patients had no complaints of pain, joint stiffness or limping. Even though the patients had some mild loss of range of movements at the MTPjoints 4-6 degrees compared to preoperative value, it did not c

Radiographic and Clinical Outcomes of Hallux Valgus and Metatarsus Adductus Treated With a Modified Lapidus Procedure.

BACKGROUND: Previous studies have found an increased rate of deformity recurrence in hallux valgus (HV) patients with concomitant metatarsus adductus (MA) undergoing metatarsal osteotomies. The purpose of this paper was to determine if there were radiographic or clinical outcome differences between HV patients with and without MA undergoing a modified Lapidus procedure. METHODS: One hundred forty-seven feet that underwent a modified Lapidus procedure for HV were divided into 2 groups based on their preoperative modified Sgarlato's angle: (1) the MA group had an angle ≥20 degrees and (2) the H

Lateralising DMMO (MIS) for simultaneous correction of a pes adductus during surgical treatment of a hallux valgus.

The surgical correction of a hallux valgus deformity presenting with a pes adductus has long proven to be a difficult undertaking. The medial shift of the metatarsal bones limits the scope for surgical correction and leads to inherently high reoccurrence rates. Current invasive treatments often give rise to profound soft tissue trauma and prolonged swelling, while requiring strict relief from weight-bearing in the affected foot. In this paper, it is aimed to introduce an easy and useful modification of the Distal Metatarsal Minimal-invasive Osteotomy (DMMO) to perform the effective, simultaneo

Indications for supramalleolar osteotomy in patients with ankle osteoarthritis and varus deformity.

BACKGROUND: Osteotomies are reported to be effective for the treatment of most cases of primary and traumatic moderate osteoarthritis of the ankle joint. Because of unsatisfactory results following supramalleolar tibial osteotomy in several of our patients, we investigated the cause of the unsatisfactory results and the indications for this surgical procedure. METHODS: Supramalleolar tibial osteotomy combined with fibular osteotomy was performed in sixteen ankles (sixteen patients) to treat moderate medial ankle osteoarthritis. The median duration of follow-up was 2.3 years (range, one to 6.5

Examination of the varus ankle, foot, and tibia.

A detailed clinical examination is an essential component in the assessment of the cavus foot. A complex interaction of pathologic conditions can only be assessed completely with physical examination. Imaging such as computed tomography or magnetic resonance imaging (MRI) may confound the physician, such as in anterior talofibular ligament tears on MRI while the ankle is stable or arthritic joints that are asymptomatic but abnormal on imaging. At the end of the day, the physical examination supersedes all other investigations. After investigations have been performed, the patient needs to be r

Inverted Z-scarf osteotomy for hallux valgus deformity correction: intermediate-term results in 55 patients.

The Z-scarf osteotomy is used for hallux valgus deformity correction by foot and ankle surgeons worldwide. Inverting the Z-scarf osteotomy configuration strengthens the construct in both sawbone and cadaver models, but clinical results of this configuration have not been reported in the literature. This retrospective study evaluates the subjective and intermediate-term postoperative radiographic results of 73 inverted Z-scarf osteotomy procedures for hallux valgus correction in 55 patients from January 1994 to December 2003. The simple bunion: anatomy at the metatarsophalangeal joint of the great toe.

The pathomechanics for the development of the hallux valgus deformity takes place at the first metatarsophalangeal joint--the sesamoid complex. The sesamoid complex consists of seven muscles, eight ligaments, and two sesamoid bones. When the first metatarsal escapes the complex and drifts medially, the sesamoids remain twisted in situ, several of the ligaments "fail," and others contract. The authors propose reduction of the metatarsus primus varus by first metatarsal osteotomy and appropriate ligament releases and plications to restore alignment. A detailed understanding of the pathomechanics

[Metatarsal osteotomies for hallux valgus].

In the surgical treatment of hallux valgus, proximal, diaphyseal, and distal osteotomies of the first metatarsal bone are commonly used. In an overview article different procedures are described as well as our own stage-adapted concept. Deformities with congruent articulation of the metatarsophalangeal joint are successfully treated with a distal chevron osteotomy. In an incongruent joint a distal soft tissue procedure is required for reduction of the joint. The metatarsus varus displacement is corrected with a proximal osteotomy of the first metatarsal bone. The indications and details of the

[Our results of the Lapidus procedure in patients with hallux valgus deformity].

PURPOSE OF THE STUDY: The most frequent deformity of the big toe and forefoot associated with a collapse of the transverse arch of the foot is a valgus deformity. For correction of a hallux valgus, several procedures are described in the literature. A valgus deformity often develops due to a varus deviation of the first metatarsal bone when the intermetatarsal angle between the first and second metatarsals is greater than 10 degrees. When the intermetatarsal angle is larger then 10 degrees or the first ray is hypermobile, a Lapidus procedure is one of the options. The objective of this study w

Hallux valgus in males--part 1: demographics, etiology, and comparative radiology.

BACKGROUND: The high frequency of hallux valgus deformities in females is well known and has been widely reported in the literature. This finding tends to obscure the importance and the characteristic details of hallux valgus deformities in males. The severity of the deformity, its onset at an earlier age, and its inheritability seem to be more frequent in males, but there are no reports in the literature to substantiate these concepts. The purpose of this study was to analyze these questions in regard to males with hallux valgus. METHODS: The records and plain radiographs of 31 males (53 fee

[Diagnosis and treatment of disorders of the foot in children].

During the skeletal development of the axis and foot of the child physiological specialties can occur. Those are thick fatty sole, increased valgus in the ankle joint, tibial varus as a toddler with first varus, later on valgus in the knee, increased laxity of the ligaments, increased anteversion of the femoral neck with "kneeing in" and "toeing in", and with knees first in flexion, later on hyperextension. The knowledge of those specialties is important to be able to diagnose and treat deformities of the foot during childhood. Included are club foot, flexible flatfoot, vertical talus, pes add

Modified lapidus arthrodesis with crossed screw fixation: early weightbearing in 136 patients.

Modified Lapidus arthrodesis is a versatile and powerful procedure for correcting the hallux valgus deformity typically associated with significant metatarsus primus varus or increased first ray mobility. Traditionally, patients have remained non-weightbearing until the arthrodesis has consolidated. More recently, numerous studies have evaluated the outcomes of early postoperative weightbearing using a variety of fixation constructs. The present retrospective cohort study evaluated 136 consecutive patients who had undergone modified Lapidus arthrodesis for hallux valgus deformity with conventi

Return to activity after sesamoidectomy in athletically active individuals.

Sesamoidectomy of the first metatarsophalangeal joint in athletically active patients may be indicated in cases of chronic sesamoiditis resistant to nonsurgical care or symptomatic displaced fractures or nonunion. Painful scar, hallux deviation, and delayed return to activity are all potential complications. These need to be considered especially when performing surgery in the athletically active individual. Twenty-six sesamoidectomies in 24 patients (21 females and 3 males) were reviewed for type of sesamoidectomy, incision location, time to return to activity, and complications. Mean age was
