The role of culture in appraisals, emotions and helplessness in response to threats.

In honour cultures, such as Turkey, reputation management is emphasised, whereas in dignity cultures such as northern US, self-respect and personal achievements are central. Turkey is also a collectivistic culture, where relationship harmony is as important as reputation management. When Turkish people's reputation is threatened, they may experience an internal conflict between these two motives and display helplessness. The purpose of the present study was to examine how people from Turkey (an honour culture; n = 52) and northern US (a dignity culture; n = 48) would perceive and respond to re

What are the factors in risk prediction models for rehospitalisation for adults with chronic heart failure?

BACKGROUND: Risk prediction models can assist in identifying individuals at risk of adverse events and also the judicious allocation of scare resources. Our objective was to describe risk prediction models for the rehospitalisation of individuals with chronic heart failure (CHF) and identify the elements contributing to these models. METHODS: The electronic data bases MEDLINE, PsychINFO, Ovid Evidence-Based Medicine Reviews and Scopus (1950-2010), were searched for studies that describe models to predict all-cause hospital readmission for individuals with CHF. Search terms included: patient r

Supersizing the penis following penile prosthesis implantation.

INTRODUCTION: Following implantation of a penile prosthesis, some couples are dissatisfied with penile length, girth, shaft, or glans engorgement. This may be delusional because of the procedure per se or preexisting risk factors such as neglected priapism, Peyronie's disease, radical prostatectomy, or overhanging suprapubic fat. AIM: In this work, we try to enhance penile size in patients dissatisfied with its dimensions following implantation of a penile prosthesis, using various augmentation techniques. METHODS: Eighteen patients who have had penile prostheses implanted were enrolled in t

Outcome measures in rheumatologic clinical trials and systemic sclerosis.

OMERACT (Outcome Measures in Rheumatologic Clinical Trials) is a loose organization of rheumatologists, epidemiologists and statisticians whose aim is to improve measurements in the rheumatic diseases. In this context, some SSc measures of response have been found to be valid: the modified Rodnan skin score, the Raynaud's condition score, the forced vital capacity as part of pulmonary function tests, right heart catheterization haemodynamics, serum creatinine, blood pressure and complete blood counts in scleroderma renal crisis and serum creatine phosphokinase as a measure of muscle disease in

Does mini-incision donor nephrectomy improve quality of life in living kidney donors?

BACKGROUND: Living kidney donation helps to avoid or reduce the time period of dialysis and on waiting lists in patients requiring a new organ. Mini-incision donor nephrectomy (MIDN) shows to result in better clinical outcome in comparison with traditional open donor nephrectomy (ODN). This study was performed to evaluate the impact of different surgical procedures on the quality of life (QoL) in patients that underwent donor nephrectomy. METHODS: The aim of the study was to detect differences in QoL assessed with the Short Form-36 Version 2 (SF-36v2) questionnaire between MIDN (n = 34) and O

On ecological fallacy, assessment errors stemming from misguided variable selection, and the effect of aggregation on the outcome of epidemiological study.

In social and environmental sciences, ecological fallacy is an incorrect assumption about an individual based on aggregate data for a group. In the present study, the validity of this assumption was tested using both individual estimates of exposure to air pollution and aggregate data for 1,492 schoolchildren living in the in vicinity of a major coal-fired power station in the Hadera region of Israel. In 1996 and 1999, the children underwent subsequent pulmonary function tests (PFT), and their parents completed a detailed questionnaire on their health status and housing conditions. The associa

Cluster analysis classification of SF-36 profiles for patients with spinal pain.

STUDY DESIGN: A k-means cluster analysis of patients with spinal and radicular pain based on the SF-36 Health Survey scales. OBJECTIVE: The aim was to determine whether spine patients fall into clusters according to self-reported health status as measured by the SF-36 and to determine if clustering is similar across four common diagnostic categories: herniated disc, spinal stenosis, spondylosis, and chronic pain syndrome. SUMMARY OF BACKGROUND DATA: Cognitive-behavioral classifications of chronic pain patients have previously identified three patient groups described as Dysfunctional, Interp

Environmental health and the family doctors.

Medicine, that is human medicine, has a long tradition of the use of biomarkers in assessing multiple departures from the health status: it is assumed that in ecosystem health assessment this experience could be helpfully shared with all those--from many scientific disciplines--involved in this research, crucial for the future of life on earth. The growing importance of ecology is evident in all aspects of modern life. Social and economically the ecological analysis cannot be ignored in future by politicians or medical doctors alike. Environmental Health is thus taking over many responsibiliti

Rates and Predictors of Patient Underreporting of Hospitalizations During Follow-Up After Acute Myocardial Infarction: An Assessment From the TRIUMPH Study.

BACKGROUND: Many clinical investigations depend on participant self-report as a principal method of identifying health care events. If self-report is used as the trigger to collect and adjudicate medical records, any event that is not reported by the patient will be missed by the investigators, reducing the power of the study and misrepresenting the risk of its participants. We sought to determine the rates and predictors of underreporting hospitalization events during the follow-up period of a prospective study of patients hospitalized with an acute myocardial infarction. METHODS AND RESULTS

Health in household context: living arrangements and health in late middle age.

People living in some arrangements show better health than persons in other living arrangements. Recent prospective studies document higher mortality among persons living in particular types of households. We extend this research by examining the influence of household structure on health using longitudinal data. We theorize that individuals experience role-based household relations as sets of resources and demands. In certain household structures, individuals are more likely to perceive that the demands made on them outweigh the resources available to them. This perceived imbalance poses a ri

Implications of Social and Material Deprivation for Changes in Health of Older People.

Objective: We use the panel structure of the Survey of Health, Ageing and Retirement in Europe (SHARE) data for 14 countries to examine the implications of material and social deprivation for health deterioration in old age and mortality. Method: To minimize the potential endogeneity bias, we examine the relationship between deprivation and changes in health rather than levels of health. We include a substantial set of fixed "initial conditions," and extend the controls with health measures, as observed at the initial period. Results: The results of the probit regression suggest a strong and s

Undermet needs for assistance in personal activities of daily living among community-dwelling oldest old in China from 2005 to 2008.

Based on the 2005 and 2008 Chinese Longitudinal Healthy Longevity Survey, this study examined the prevalence of undermet needs for assistance in personal activities of daily living (ADL) and its associated risk factors among the oldest old aged 80+. Multilevel multinomial logistic modeling was used to analyze the risk factors and changes of undermet needs over time. The results show that the prevalence of slightly undermet needs decreased in urban China from 2005 to 2008. However, the prevalence of undermet needs remained high; 50% or more for both rural and urban residents. Compared to 2005,
