The Yin and the Yang of Hemostasis in End-Stage Liver Disease.

Patients with end-stage liver disease (ESLD) undergoing liver transplantation (LT) are prone to thromboses both while on the waiting list and in the perioperative period. This hypercoagulability is associated with significant endothelial dysfunction (ED) due to nitric oxide dysregulation. ED and increased thrombin generation are the main factors responsible for this hypercoagulability. Sepsis alone can significantly alter a patient's coagulation profile. In combination with ESLD, however, sepsis or septic shock are responsible for very complex changes. This makes both the assessment and manage

Hemostasis in the Pregnant Woman, the Placenta, the Fetus, and the Newborn Infant.

The hemostasis system is composed of procoagulant, anticoagulant, and fibrinolytic proteins that interact with endothelial and blood cells and with each other in a complex system of checks and balances to maintain blood flow while preventing both hemorrhage and thrombosis. Pregnancy is a unique physiological state in which biological alterations predispose both mother and fetus to both bleeding and clotting. The placenta is a vascular interface for maternal and fetal blood exchange which predisposes the mother to hemorrhage. Maternal hemostasis presents a compensatory hypercoagulability includ

Effects of Primary Mast Cell Disease on Hemostasis and Erythropoiesis.

Mast cell disease is an epigenetically and genetically determined disease entity with very diverse clinical manifestations in potentially every system and tissue due to inap pro priate release of variable subsets of mast cell mediators together with accumulation of either morphologically normal or altered mast cells. Easy bruising, excessive bleeding, and aberrancies of erythropoiesis can frequently be observed in patients with mast cell disease. A thorough history, including a family history, will guide the appropriate work-up, and laboratory evaluations may provide clues to diagnosis. In rec

High-Risk Pregnancies and Their Impact on Neonatal Primary Hemostasis.

Primary hemostasis, similar to other systems in the adjusting and transitioning neonate, undergoes developmental adaptations in the first days of life. Although platelets of neonates do not differ quantitatively compared with those of adults, they functionally present with major differences, thus supporting the theory of a "hypofunctional" phenotype that is counterbalanced by high hematocrit and more potent von Willebrand factor multimers. No clinical effect of bleeding tendency has hence been established so far for healthy term neonates. However, discrepancies in functionality have been noted

Thromboelastography as a Surrogate Marker of Perisurgical Hemostasis in Gaucher Disease.

Thromboelastography (TEG) has long been available for routine monitoring of perisurgical and postpartum hemostasis, especially at point of care. The purpose of this study is to retrospectively compare TEG parameters to concomitant standard clotting test results in an unselected cohort of patients with Gaucher disease to ascertain whether TEG values are specific and sensitive enough to substitute for classic coagulation tests for decision making. This remains a cogent concern because of high incidence of thrombocytopenia in patients with Gaucher disease. Thromboelastography values were compared

[Effect of melatonin on the thrombocyte hemostasis and its circadian organization].

Melatonin produced by pineal gland and some peripheral cells effect both spontaneous and induced aggregation of platelets. As a natural inhibitor of aggregation, melatonin limits hemostasis in night hours. A decrease in the plasma level of melatonin in early morning hours is probably responsible for cardiovascular disorders and may be recommended for use in pharmacotherapy.

Effects of in vitro adult platelet transfusions on neonatal hemostasis.

BACKGROUND: Thrombocytopenia is frequent among neonates, and 20-25% of affected infants are treated with platelet transfusions. These are frequently given for mild thrombocytopenia (platelets: 50-100 × 10(9) L(-1)), largely because of the known hyporeactivity of neonatal platelets. In tests of primary hemostasis, however, neonates have shorter bleeding and closure times (CTs) than adults. This has been attributed to their higher hematocrits, higher von Willebrand factor (VWF) concentrations, and predominance of longer VWF polymers. OBJECTIVE: To determine whether the 'transfusion' of adult (r

[Endoscopic methods of hemostasis in treatment of Mallory-Weiss syndrome].

Results of treatment of Mallory-Weiss syndrome using endoscopic methods of hemostasis in 549 patients were analyzed. The patients were divided into two groups: in 2000-2004 years without endoscopic methods, in 2004-2008 years using the endoscopic methods in the diagnostics and treatment. Considerably decreased operative activity and death rate was established in cases of treatment with endoscopic methods.

Role of the extrinsic pathway of blood coagulation in hemostasis and thrombosis.

Hemostasis requires both platelets and the coagulation system. At sites of vessel injury, bleeding is minimized by the formation of a hemostatic plug consisting of platelets and fibrin. The traditional view of the regulation of blood coagulation is that the initiation phase is triggered by the extrinsic pathway, whereas amplification requires the intrinsic pathway. The extrinsic pathway consists of the transmembrane receptor tissue factor (TF) and plasma factor VII/VIIa (FVII/FVIIa), and the intrinsic pathway consists of plasma FXI, FIX, and FVIII. Under physiological conditions, TF is constit

[The hemostasis system state in patients with ulcerous gastrointestinal bleedings].

In patients with gastrointestinal bleedings there occurs activation of the hemostasis system directed to arrest of bleeding. Short-term hypercoagulation directed to arrest of the bleeding gives place to hypocoagulative changes. Their degree and duration depend on the severity and rate of blood loss. The excessive intravascular activation of blood is accompanied by the formation of fibrin deposits not only in the area of the bleeding source but in other regions of the blood channel. There appears disseminated intravascular coagulation of blood deteriorating the reparative processes in the ulcer

Effects of sympathetic activation by adrenergic infusions on hemostasis in vivo.

Overactivity of the sympathetic nervous system (SNS) has been related to increased cardiovascular morbidity. Historical reports suggest hastening of blood coagulation following intravenous administration of epinephrine. Given the important role of the hemostatic system in atherosclerosis and thrombosis, it is surprising that short-term adrenergic effects on blood coagulation, fibrinolysis and platelet activity have not been scrutinized closely. To elucidate such effects in vivo, this paper reviews human studies in which alpha- and beta-sympathomimetic agents had been infused. The literature su

[Exploration of hemostasis disorders in children (excluding the neonatal period)].

There are three different aspects in the etiological diagnosis of hemostasis disorders: (1) primary hemostasis evaluation; (2) coagulation evaluation; and, (3) thrombosis evaluation. In every case, a look at the medical history is an essential step before proceeding to biological investigations. Bleeding time is the first step of the primary hemostasis evaluation; prolonged drug use (particularly salicylates) and thrombocytopenia must be first considered; only then will the Von Willebrand factor and platelet functions be studied. Coagulation evaluation first requires the study of the overall c

Disorders of hemostasis in childhood: risk factors for venous thromboembolism.

In both children and neonates with venous thrombosis, one or more predisposing risk factors can generally be identified. Underlying prothrombotic medical conditions are common in these patients. In addition, a "trigger factor" such as a catheter, surgery or trauma is usually present. However genetic and acquired coagulation abnormalities are also identified in children with venous thrombosis who are appropriately studied. A careful family history and assays for the LA, AT-III, protein C, protein S and the factor V Leiden mutation should be part of the evaluation of infants and children with ve

A biological model for the regulation of peri-implantational hemostasis and menstruation.

OBJECTIVE: To delineate the physiologic mechanisms whereby the human endometrium maintains hemostasis during endovascular trophoblast invasion but permits menstrual hemorrhage. METHODS: Experimental results are presented that are relevant to developing a comprehensive biological model for studying peri-implantational hemostasis and menstruation. RESULTS: A marked increase in the expression of tissue factor (TF) and type-1 plasminogen activator inhibitor (PAI-1) and an inhibition of tissue-type and urokinase-type plasminogen activators (tPA and uPA, respectively), matrix metalloproteinases (M

Hematology and hemostasis in the horse: normal functions and common abnormalities.

In diseased animals, laboratory evaluations of erythrocytes, leukocytes, and hemostasis provide important information that contributes to either narrowing the list of potential diagnoses or to determining a specific diagnosis. To adequately interpret the results of these evaluations, normal erythrocyte and leukocyte kinetics and normal hemostatic function must be understood. In addition, knowledge of common diseases resulting in abnormalities of these laboratory tests and of typical changes in these tests caused by these diseases is vital. This article has reviewed normal erythrocyte and leuko
