[Mental suffering requires a clinical stance of encounter. A reflective essay].

BACKGROUND: Mental suffering lies at the heart of psychiatric practice but is often approached as a disorder, defect, or symptom – thereby eclipsing the lived experience itself. AIM: To explore how mental suffering can be re-understood as a human experience in context, and what this implies for clinical attitude. METHOD: Reflective essay based on literature, philosophical analysis, and clinical experience. RESULTS: Mental suffering does not appear as a brain defect but as a disruption of human coherence -embodied, affective, relational, and temporal. It emerges when the natural contin

Shame over tooth loss: between psychological suffering and structural inequality in Chilean oral health.

The article analyzes how tooth loss among older adults is not merely a clinical issue but a social and emotional phenomenon marked by shame, stigma, and structural inequality in oral health in Chile. From a qualitative, narrative approach with a descriptive scope and a hermeneutic (interpretive) orientation, 12 interviews were conducted in the commune of Cerro Navia, in the Metropolitan Region of Chile, between August and September 2024. The narratives reveal that tooth loss affects self-esteem, identity, and social participation. Shame manifests in behaviors such as covering the mouth, avoidi

Navigating a fragile capability: meanings of encountering patients in a suicidal process from the perspective of ambulance clinicians.

PURPOSE: To illuminate the meanings of encountering patients in a suicidal process from the perspective of ambulance clinicians. METHODS: Narrative interviews were conducted with eighteen ambulance clinicians in Sweden. Data were analyzed using a phenomenological hermeneutical approach inspired by the philosophy of Ricœur. RESULTS: The main theme, "Navigating a fragile capability," reveals a profound tension within professional identity. Traditionally anchored in technical agency, this identity is challenged by a movement toward vulnerability and shared humanity. Capability in these encounte

[End-of-life practices and advance directives: a bioethical analysis based on discussions in the Brazilian Federal Legislative Branch].

The Brazilian legislative debate on end-of-life practices involves ethical-axiological tensions related to euthanasia, assisted suicide, palliative care, and advance directives, in which divergent conceptions of these practices and of the value of life confront one another, influencing the normative formulation of the dying process. This study analyzes how such tensions are constructed and justified in the discourses of the Federal Legislative Branch, examining the normative meanings and the hierarchy of values attributed to the palliative care framework and to advance directives, as well as t

Grounding psychosis research: why observable signs should anchor biological investigations.

Biological psychiatry faces a significant epistemic challenge in identifying valid objects for mechanistic research. Both diagnostic constructs and individual symptoms are abstract symbols defined circularly within a closed hermeneutic system, creating a symbol grounding problem that hinders the discovery of biophysical substrates (biomarkers). I argue that progress requires an epistemological separation between the ungrounded symptoms such as delusions and hallucinations, which are co-constructed through personal and clinical interpretation from grounded signs that are directly observable fea

A Quadripartite Compass for Holistic Nursing: Philosophy of Science, Islamic Teleology, and Culturally Responsive Care.

PurposeTo articulate the scholarly status, philosophical coherence, and disciplinary contribution of the Quadripartite Compass for Holistic Nursing.ApproachA comparative hermeneutic analysis places holistic nursing and caring science in dialogue with selected Western philosophical traditions and Islamic teleology.ArgumentThe Compass is a philosophical framework that operates at a metatheoretical level and functions in use as a reflective heuristic. Its four coordinates - ontology, axiology, epistemology, and methodology - are interdependent rather than additive: assumptions about personhood sh

The Experience of Physical Activity in Adolescents With Cerebral Palsy.

For adolescents with cerebral palsy (CP), participating in physical activity (PA) can be difficult due to functional limitations that not only affect an adolescent's ability or willingness to participate in PA but also create particular social concerns. Research in the area of PA and adolescents with CP is limited. This research study utilized hermeneutic phenomenology to gain a more comprehensive understanding of the lived experiences of 14 adolescents with CP who participated in PA. The interpretations of each participant offered common understandings and themes to be identified and warrante

Pain relief, spiritual needs, and family support: three central areas in intercultural palliative care.

OBJECTIVE: The purpose of this study was to draw out and coalesce the topic-specific information found in research literature regarding the provision of culturally sensitive palliative practice. METHOD: This was a literature study and Gadamerian hermeneutic text analysis. RESULTS: It is more difficult to assess the level of pain in ethnic minority patients, and healthcare providers may become frustrated and interpret pain symptoms as fabrication. These patients are more likely to receive inadequate pain medication. Physical symptom management has become the priority in palliative care, but p

Offending the other: deconstructing narratives of deviance and pathology.

Narratives of homophobia and the repathologization of gay men now emerge and coalesce in often unconsidered ways. Within this paper, we present a deconstructive analysis of a recent paper published within the British Journal of Social Psychology (Crossley, 2004) and highlight how a narrative of repathology emerges through the selective appropriation of particular textual sources. By employing pathological constructions of sexual conduct between men; focusing upon singular homogenized constructions of gay men and simplistic constructions of health promotion and through a process of reconstructi

Hindrances to achieve professional confidence: The nurse's participation in ethical decision-making.

BACKGROUND: Research suggests that nurses generally do not participate in ethical decision-making in accordance with ethical guidelines for nurses. In addition to completing their training, nurses need to reflect on and use ethically grounded arguments and defined ethical values such as patient's dignity in their clinical work. OBJECTIVES: The purpose of this article is to gain a deeper understanding of how nurses deal with ethical decision-making in daily practice. The chosen research question is "How do nurses participate in ethical decision-making for the patient?" DESIGN AND METHOD: We u

The hour of poetry: A toolkit for residents in psychiatry.

Contemporary psychiatry suffers from an epistemological impoverishment that weakens its capacity to encounter the intricacy of human suffering. Because psychopathological conditions are intrinsically ambiguous, contradictory, and affectively charged, psychiatric training cannot rely exclusively on rational-conceptual language and rigid operational categories. This article advances the claim that residents in psychiatry must also be trained in psychiatric poetics, that is, a paradoxical and poetic language and mode of thinking. Utilizing a hermeneutic approach, we analyze the structural role of

'Doctors do not know about Cystic Fibrosis': Examining structural vulnerability in the management of rare diseases in India.

Individuals living with rare diseases have conventionally been understood as being particularly vulnerable, which often promotes a negative and stigmatising interpretation of vulnerability. In this article, we embrace the framework of structural (health) vulnerability to gain a deeper understanding of the circumstances and factors contributing to adverse outcomes in the specific context of a Global South country, India, and a particular rare disease, Cystic Fibrosis (CF). By drawing on published materials and preliminary data from an evolving ethnographic research project, we contend that it i

Steeping in how "shut up and color" mindset manifests: Semantic analysis of open-ended responses for barriers to support seeking among veterans.

Despite the long use of surveys in veterans' needs assessments, a disconnect persists between the veterans' needs and the available services. This study employs a hermeneutical framework to semantically analyze open-ended responses and investigate how veterans' perceptions of service barriers are formed. We analyzed 3,325 open-ended responses specifying their perceived barriers in Texas Veterans Needs Survey data from 2023 (n = 16,733). Our analysis revealed that the hurdles stem from the combination of (1) obedience to authority, (2) covert mental health challenges, (3) hopelessness, and (4)

Perinatal meaning-making and meaning-focused coping in the COVID-19 pandemic.

INTRODUCTION: The COVID-19 pandemic caused unprecedented levels of stress amongst pregnant women and new mothers. The current qualitative study explored the ways in which perinatal women made meaning of their experiences during the COVID-19 pandemic. METHODS: Data came from a parent study in which 54 perinatal (pregnant and postpartum) women in the United States completed semi-structured interviews from October 2021 to January 2022 describing their experiences during the COVID-19 pandemic. The data was interpreted using a hermeneutic, phenomenological approach to delve deeply into the concept

Body Image Perceptions of Women Veterans With Military Sexual Trauma.

The researchers were invited to a transitional home for homeless women veterans to help veterans with body image issues. Convenience sampling was used to recruit 12 veterans who perceived they had a physical difference due to military service. Data were obtained in focus groups where the veterans were invited to share stories. Ricoeur's hermeneutic phenomenology guided the study. The research team learned early in the data collection stage that 11 of the 12 participants suffered military sexual trauma (MST). Three structures emerged in the data: (a) to speak up or not to speak, (b) from milita

Healthcare middle managers` experiences developing leadership capacity and capability in a public funded learning network.

BACKGROUND: Healthcare middle managers (HMMs) have, as the leaders closest to clinical practice, a crucial position in healthcare today. There is broad knowledge about the demands on HMMs' capacity, their situation in general, and the challenges this presents for the improvement of healthcare quality. There is less knowledge about how to facilitate HMMs` capacity and capability with regard to their leadership and how to handle this in a complex context. The purpose of this study was to identify and discuss the facilitation of HMMs' development of capacity and capability for leadership. METHOD
