Treatment of Developmental Dysplasia of Hip With Tubingen Hip Flexion Splint.

BACKGROUND: Developmental dysplasia of the hip is an anomaly of the hip joint. In patients with early diagnosis, within 3 to 6 months of life, the treatment is essentially conservative and involves the use of dynamic harness. The indication for the use of the Tubingen hip flexion splint is a dysplastic hip. The aim of this study is to report the experience of the Orthopaedic Clinic of the "University of Catania" regarding conservative treatment with the Tubingen harness of dysplastic hips diagnosed in children within 3 months of life. METHODS: From January 1997 to July 2012, 5137 infants (10,

Biomechanics of the Hip Capsule and Capsule Management Strategies in Hip Arthroscopy.

Recent advances in our understanding of the function of the hip capsule have clarified its importance to normal hip function and kinematics. The iliofemoral ligament is the primary stabilizing structure for controlling anterior translation and external rotation of the hip, and is violated by the arthroscopic interportal capsulotomy. Microinstability of the hip occurring after surgical trauma remains a poorly defined clinical entity. In certain at-risk populations, capsular repair should be considered as part of an arthroscopic hip procedure to achieve optimal outcomes and avoid iatrogenic inst

Complications following conversion of a hip hemiarthroplasty to a total hip arthroplasty.

PURPOSE: Conversion of hip hemiarthroplasty to total hip arthroplasty (CTHA) is a complication-prone procedure with high dislocation rates and early component loosening. The purpose of this study was to evaluate the complications of CTHA performed using contemporary implants. METHODS: Forty-six patients who had CTHA were retrospectively matched to a control group of 46 patients who had a first-time THA revision. The mean follow-up was 47 (range 6-149) and 23 (range 6-139) months for CTHA and control groups, respectively. Radiographs taken at the last follow-up visit were evaluated for signs o

Reconstruction of the Acetabulum in Developmental Dysplasia of the Hip in total hip replacement.

Developmental dysplasia of the hip (DDH) or congenital hip dysplasia (CDH) is the most prevalent developmental childhood hip disorder. It includes a wide spectrum of hip abnormalities ranging from dysplasia to subluxation and complete dislocation of the hip joint. The natural history of neglected DDH in adults is highly variable. The mean age of onset of symptoms is 34.5 years for dysplastic DDH, 32.5 years for low dislocation, 31.2 years for high dislocation with a false acetabulum, and 46.4 years for high dislocation without a false acetabulum. Thorough understanding of the bony and soft tis

Functional response to total hip arthroplasty in patients with hip dysplasia.

BACKGROUND: While recent studies have shown patients with developmental dysplasia of the hip (DDH) undergoing total hip arthroplasty (THA) to achieve comparable post-operative outcomes to patients with primary osteoarthritis (OA), it is unclear whether DDH patients display better or worse preoperative function than the general THA population. We aimed to compare the preoperative function and functional response to THA of DDH patients with OA patients. METHODS: Through a retrospective review of prospectively collected regional joint registry data, we compared the preoperative, 1-year post-oper

The outcome of cementless total hip arthroplasty in haemophilic hip arthropathy.

Total hip arthroplasty (THA) in haemophilic arthropathy is reported to be less successful than in non-haemophilic indications. Although preliminary results are encouraging, the survival and functional outcome of cementless THA in haemophilia are not known. The aim of this study was to analyse mid-term results of cementless THA in haemophilia. Twenty-seven consecutive cementless THAs with 23 patients performed between June 1995 and June 2003 were reviewed. Mean age at time of operation was 36 years and mean follow-up period was 92 months (range, 60-156). Radiographic assessment was done for fix

Outcome of hip resurfacing arthroplasty in patients with developmental hip dysplasia.

UNLABELLED: Patients with osteoarthritis secondary to developmental dysplasia of the hip (DDH) typically are young and active, which might affect functional ratings or failure rates after resurfacing arthroplasty. We therefore evaluated 24 patients (32 hips; mean age, 44.2 years) after hip resurfacing performed for osteoarthritis secondary to DDH. We used the Harris hip score (HHS), the University of California, Los Angeles (UCLA) activity scale, and a sports and activity questionnaire. A radiographic analysis also was performed. We followed patients a minimum of 28 months (mean, 43 months; ra

[Midterm results of total hip replacement in osteonecrosis of the hip joint].

OBJECTIVES: We evaluated the midterm results of total hip arthroplasty (THA) for femoral neck osteonecrosis. METHODS: The study included 59 patients (23 females, 36 males; mean age 45.6 years; range 24 to 66 years) who underwent THA in 72 hips. Osteonecrosis was secondary to fracture in the femoral neck in 11 hips (15.3%), was associated with steroid use in 23 hips (31.9%), and was idiopathic in 38 hips (52.8%). Functional evaluations were made with the hip scoring system of D'Aubigne and Postel. Bone-implant relations were assessed radiographically using the Gruen's seven zones in the femur

Ilizarov hip reconstruction for the late sequelae of infantile hip infection.

BACKGROUND: The late sequelae of infantile hip infection include absence of the femoral head and neck, proximal migration of the femur, lower-extremity length discrepancy, abnormal gait, and pain. The Ilizarov hip reconstruction includes an acute valgus and extension osteotomy at the proximal part of the femur combined with gradual distraction for realignment and lengthening at a second, more distal, femoral osteotomy. The purpose of this study was to determine whether this technique can successfully treat the sequelae of infantile hip infection. METHODS: We performed a retrospective review o

Use of the talon hip compression screw in intertrochanteric fractures of the hip.

A retrospective analysis of a compression hip screw with four reversibly deployable talons was done. Fifty-four patients had sufficient radiographs to be included in this analysis. One-year mortality was 17% and increased to 41% by 2 years. No lag screws cut out, and postoperative slide was reduced compared with that in many published series. Three patients had revision of a failed alternate-type hip pin with the Talon hip compression screw. Previous studies showed the talons provide the definitive difference in allowing enhanced compression at the time of surgery, preventing cut-out by enhanc

Factors related to occurrence of hip fracture during a fall on the hip.

Recently, it has been shown that hip fractures can be effectively prevented by use of hip protectors. To determine who would gain most benefit from use of hip protectors, we conducted a study with the aim to clarify factors that contribute to the occurrence of fracture in individuals who fall on their hip. Hip fracture patients were compared with individuals who had fallen on their hip without sustaining a fracture. The study group consisted of 123 consecutive hip fracture patients aged 70 years or over (mean age 82 years, female 82 years and male 80 years). The control group comprised 132 ind

[Changes of biomechanical parameters in dysplasia of the hip by total hip replacement].

AIM: Dysplastic hips or dislocated hips lead to loss of muscle strength claudication, and finally to coxarthrosis. This study analytically compares the hip joint forces in normal, dysplastic hips and hips after implantation of a hip prosthesis for several positions. METHOD: The results of 173 total hip replacements in 153 cases of either severe congenital dysplasia or dislocation were analyzed. A 2 dimensional mathematical model of the hip was developed to evaluate the effects of surgically achievable mechanical alterations such as acetabular placement, femoral shaft-prosthetic neck angle, an

[Results of hip surgical reconstruction in congenital dislocation of the hip in children].

The paper presents a series of 51 hips (5 boys, 33 girls) that underwent reconstructive surgery because of congenital hip dislocation. Surgical treatment involved a one-stage procedure, consisting of correction of the proximal femur position and acetabuloplasty. Acetabulum reconstruction was achieved by using one of three methods: Pemberton osteotomy, Salter osteotomy and a "shelf-type" acetabuloplasty. Excellent and good results were found in 84% of the cases under 4 years of age, irrespective of the acetabuloplasty implemented.

Sonographic hip screening and early management of developmental dysplasia of the hip.

The reliability of Graf's technique in diagnosing developmental dysplasia of the hip (DDH) is investigated in this report. In a prospective study, 6,548 neonates were examined clinically and sonographically; 470 children were reexamined at least once. Sonographic alpha angles and radiographic acetabular index (AI) angles were followed up and compared. Results were as follows: 84.6% of the hips were mature; 14.3% were physiologically immature; 1.1% were dysplastic. Of the sonographically dysplastic hips, 63% were clinically normal. Neonatal sonographic hip status was affected by family history,

Cementless total hip replacement in patients with developmental dysplasia of the hip.

This study was conducted to evaluate the clinical and radiographic results of 22 total hip replacements done in 17 consecutive patients for coxarthrosis due to developmental dysplasia or dislocation of the hip. All operations were done using an anterior approach without trochanteric osteotomy. Standard cementless prostheses were used in all cases. There was no custom-designed prosthesis used. The acetabular cup was placed in an anatomic position in the true acetabulum in every case. Bulk autograft was necessary to reconstruct the deficient acetabular roof in only 2 hips. The average follow-up
