Caring for the dying: how well prepared are general practitioners? A questionnaire study in Wales.

CONTEXT: General practitioners (GPs) and generalist hospital doctors provide the majority of palliative and terminal care in the UK. Studies have revealed problems with symptom control and communication in these settings and inadequate training for clinical students and junior doctors. OBJECTIVES: To investigate the training of GPs in Wales in palliative medicine throughout their careers, with a focus on the Welsh Valleys, an area of social deprivation and high levels of chronic ill health. To compare these data with those previously obtained from a survey of GPs in East Anglia. To develop re

How Does Physician Clinical Experience Affect Quality of Care, as Measured by Length of Stay?

INTRODUCTION: Length of stay (LOS) is a widely utilized metric of hospital quality and operational efficiency. Prolonged LOS is associated with increased healthcare costs, greater risk of hospital-acquired complications and infections, and reduced bed turnover, negatively affecting patient outcomes and institutional performance. Hospitalists and internal medicine physicians play an integral role in determining a patient's readiness for discharge and facilitating timely transitions of care. Examining factors that influence LOS, including physician characteristics such as clinical experience or

Association between point-of-care ultrasonography use and requests for formal imaging studies in hospital-based internal medicine: A retrospective cohort study.

BACKGROUND: Point-of-care ultrasonography (POCUS) has expanded rapidly among hospitalists. However, its impact on the use of additional formal investigations and healthcare costs remains unclear. This study aimed to describe the evolution of POCUS use within a university hospital general internal medicine department and assess its association with requests for formal radiological and cardiological investigations. METHODS: We conducted a retrospective observational study including all POCUS exams archived in the hospital's picture archiving system performed by hospital-based internists at the

Advance Care Planning, Race, and Age.

There are well-documented racial disparities in the provision of end-of-life care and advance care planning (ACP). It is unclear whether these observed disparities are explained by other variables. This analysis sought to determine whether any apparent racial disparities among general medicine inpatients in the provision of ACP services persisted after adjustment for potential confounders. The authors conducted a secondary analysis of electronic health records data collected as baseline data for a quality improvement program. The analysis included all patients who were discharged by hospitalis

Designing a clinical decision support tool to promote timely palliative care in heart failure.

BACKGROUND: Palliative care improves quality of life and reduces avoidable healthcare utilization for people with heart failure, yet referrals remain inconsistent and delayed. Clinical decision support offers a promising strategy to facilitate timely palliative care, but no tool currently exists to support palliative care decision-making specifically for this population. OBJECTIVES: To identify clinician needs, contextual factors, and design requirements to inform the development of a clinical decision support tool to promote timely palliative care for hospitalized patients with heart failure

Telepsychiatry Expansion of Inpatient Psychiatry Services During the COVID-19 Pandemic.

During the early COVID-19 pandemic, most psychiatric facilities did not admit SARS-CoV-2-positive youth, resulting in prolonged emergency department (ED) boarding and delayed psychiatric care. In response, our hospital enacted a small, single-site, innovative pilot enabling psychiatric admission of SARS-CoV-2-positive patients with lower behavioral acuity to an inpatient medical unit for comprehensive telepsychiatry programming. Patients transferred to the Telepsychiatry Model from our EDs or hospital medicine service after medical clearance. Psychiatrists from our adjacent traditional inpatie

Quality Improvement Through Nurse-initiated Kaiser Sepsis Calculator at a County Hospital.

BACKGROUND AND OBJECTIVES: Suspected early onset sepsis drives most antibiotic use in the newborn nursery. The Kaiser Sepsis Calculator (KSC) is a validated tool that safely decreases laboratory evaluation and antibiotic administration in infants aged ≥34 weeks. Our quality improvement aim was a nurse-initiated, KSC-based program to decrease blood cultures (BCx) and complete blood counts (CBC) by 10% from March 2021 to October 2021 among chorioamnionitis-exposed infants born ≥35 weeks' gestation. A secondary aim was to decrease antibiotic administration by 10%. METHODS: The KSC was implemente

Bullous Pemphigoid with Atypical Skin Lesions and Acute Interstitial Nephritis: A Case Report and Focused Literature Review.

BACKGROUND The hallmark of bullous pemphigoid (BP) is widespread tense blisters arising on normal or erythematous skin, often with marked pruritus, the diagnosis of which is confirmed by direct immunofluorescence (DIF). BP is an autoimmune process that can be induced, though rarely, by medications. Drug-induced BP often has atypical clinical presentation, which requires a good understanding of other dermatological conditions with similar presentations, in particular, bullous subtype of erythema multiforme. End organ involvement warrants differentiating it from one of the severe cutaneous adver

Care of children with medical complexity in the hospital setting.

Children with medical complexity are a subset of patients with special health care needs whose "health and quality of life depend on integrating health care between a primary care medical home, tertiary care services, and other important loci of care such as transitional care facilities, rehabilitation units, the home, the school, and other community based settings," according to Cohen et al. These children are characterized by (1) substantial health care service needs, (2) one or more severe chronic clinical condition(s), (3) severe functional limitations, and (4) high projected use of health

Geographic cohorting by clinical care team: a narrative review.

BACKGROUND AND OBJECTIVE: Geographic cohorting refers to localization of inpatients to designated hospital areas (typically a unit or a set of beds) based on specified criteria. One such criterion that has been commonly discussed and studied since the early days of the hospitalist movement in the US is a patient's assigned clinical care team. Because implementing cohorting of this type requires substantial operational investment, it is important to understand the benefits and the tradeoffs associated with bringing patients into closer spatial proximity with their full team of providers and all

Defining the Director of Education in hospital medicine: A cross-sectional descriptive study.

BACKGROUND: Hospital medicine (HM) groups must balance an expanding clinical enterprise with their educational mission. In response, many have established the role of Director of Education (DOE) to lead and support educational efforts. The structure, expectations, and impact of this role remain poorly defined. OBJECTIVE: To characterize the current landscape of the DOE role, including prevalence, funding, responsibilities, and evaluation strategies. METHODS: We conducted a cross-sectional survey of hospitalists serving as DOEs or HM leaders familiar with the role at a sample of highly ranked

What constitutes high risk for venous thromboembolism? Determining an appropriate threshold to initiate prophylaxis.

BACKGROUND: Guidelines recommend pharmacological venous thromboembolism (VTE) prophylaxis only for high-risk patients, but the probability of VTE that is considered "high risk" is not specified. OBJECTIVES: We compared cost-effectiveness analysis (CEA) with alternative approaches to define an appropriate probability threshold for initiating prophylaxis in medical inpatients. METHODS: Our analytic sample included 47 889 adults admitted to 10 Cleveland Clinic hospitals from October 2017 to January 2020, as well as 214 hospitalists and internal medicine residents who were surveyed from June to

Management of opioid use disorder and associated conditions among hospitalized adults: A Consensus Statement from the Society of Hospital Medicine.

Hospital-based clinicians frequently care for patients with opioid withdrawal or opioid use disorder (OUD) and are well-positioned to identify and initiate treatment for these patients. With rising numbers of hospitalizations related to opioid use and opioid-related overdose, the Society of Hospital Medicine convened a working group to develop a Consensus Statement on the management of OUD and associated conditions among hospitalized adults. The guidance statement is intended for clinicians practicing medicine in the inpatient setting (e.g., hospitalists, primary care physicians, family physic
