[Ultrasonographic measurement of the effect of rotatory cuff repair with coracoacromial ligament transfer on humeral head migration].

OBJECTIVES: In this study we evaluated whether ligament transfer caused humeral head migration in patients whose massive rotator cuff ruptures were repaired with total or partial coracoacromial ligament (CAL) transfer. The necessity of harvesting the ligament totally or partially in massive rotator cuff repairs was investigated with respect to the effect of the excision and transfer of the CAL on humeral head migration. PATIENTS AND METHODS: Forty patients (12 males, 28 females; mean age 54.3 years; range 39 to 66 years) operated on with free CAL transfer for massive rotator cuff rupture betw

Humeral head retroversion in competitive baseball players and its relationship to glenohumeral rotation range of motion.

DESIGN: Prospective case series. OBJECTIVE: To determine if an osseous component in the form of increased humeral head retroversion may contribute to observed differences in the amount of rotation between the throwing and nonthrowing shoulders of competitive baseball players. BACKGROUND: Differences in side-to-side shoulder rotation range of motion (ROM) are seen in throwing athletes. In the past, these differences have been attributed to adaptive changes to the surrounding soft tissue structures, including attenuation of the anteroinferior capsule and ligaments, and tightening of the poster

Four-part fracture after intrathoracic displacement of the humeral head: a case report and review of the literature.

Displaced four-part fracture dislocation represents the highest risk of osteonecrosis of the humeral head due to a disruption of the blood supply. In a few cases, that dislocation can be intrathoracic which worsens prognosis with life-threatening injuries such as hemothorax or pneumothorax. This paper presents the case of a 77-year-old woman after having fallen down the stairs with an intrathoracic dislocation four-part fracture leaving the head in the thorax. We analyze mechanisms and review the literature in order to highlight this lesion and provide a take home message for the treatment.

The Latarjet Procedure May Induce Pathokinematics with Posterior Humeral Head Subluxation: An Experimental Dynamic Radiostereometric Study.

BACKGROUND: The effectiveness of the Latarjet procedure in stabilizing the glenohumeral joint (GHJ) in the abducted and externally rotated position is well documented. However, evidence of its ability to restore the GHJ kinematics in other positions and without anterior-directed load is sparse. The purpose of this study was to evaluate the GHJ kinematics, throughout external rotation, following the Latarjet procedure in shoulders with 15% anterior glenoid bone loss. METHODS: Eight human donor arms were examined using dynamic radiostereometry during GHJ external rotation with anterior-directed

Age-dependent development and microarchitecture of the osteochondral unit of the humeral head in harbour porpoises (Phocoena phocoena).

The body morphology of harbour porpoises (Phocoena phocoena), like that of other fully aquatic mammals, differs greatly from terrestrial species due to adaptations to aquatic life. Thus, the developmental processes of the osteochondral unit (OCU) and its postnatal adaptation in harbour porpoises remain poorly understood. Here, we compared the humeral head microarchitecture in neonate, juvenile and adult harbour porpoises to better understand the relationship between environmental loading and the organisation of the OCU within diarthrodial joints. In neonates, the superficial layer of hyaline c

Multimodal characterization of articular cartilage degeneration in the humeral head using Raman spectroscopy, biomechanics, and imaging.

OBJECTIVE: This study evaluated articular cartilage degeneration in the humeral head using a multimodal approach integrating Raman spectroscopy, mechanical testing, biochemical analysis, and micro-computed tomography (µCT). The goal was to identify sensitive biomarkers of early matrix-level degeneration that precede overt radiographic changes in glenohumeral osteoarthritis (OA). METHODS: Eleven humeral heads were collected from cadaveric shoulders and stratified by the presence of a focal chondral defect and rotator cuff (RC) tear status, yielding four specimens without visual cartilage degen

Humeral Head Articular Cartilage Damage by a Retained Arthroscopic Suture Anchor Inserter in a College Athlete.

Arthroscopic repair of the glenoid labrum has been shown to be a safe and effective method to address recurrent shoulder instability. However, suture anchor devices must be used appropriately and with caution to avoid postoperative complications such as prominence, migration, and loosening, which can result in intraarticular pathology postoperatively. We report a case of significant glenohumeral articular cartilage damage that occurred as a result of a retained broken suture anchor inserter after arthroscopic Bankart repair. This case demonstrates an uncommon but serious complication that can

Superior humeral head osteophytes are associated with rotator cuff insufficiency in glenohumeral osteoarthritis: a retrospective analysis.

PURPOSE: The primary goal of this study was to investigate whether superior humeral head osteophyte (SHO) size is associated with rotator cuff insufficiency, including rotator cuff tear (RCT), supraspinatus tendon thickness, and fatty infiltration of the rotator cuff muscles. METHODS: Patients  ≥ 18 years who were diagnosed with glenohumeral osteoarthritis were retrospectively reviewed. SHO size was determined by radiograph. MRI measured SHO and RCT presence, type, and size; supraspinatus tendon thickness; and fatty infiltration of rotator cuff musculature. RESULTS: A total of 461 patients w

Surgical treatment of humeral head avascular necrosis in patients with sickle cell disease: a systematic review.

BACKGROUND: Sickle cell disease is the leading etiology for atraumatic humeral head avascular necrosis worldwide. Treatment of this condition is not standardized, with only few studies evaluating clinical outcomes after surgical interventions. The aim of this study was to review the available evidence on the results of surgical intervention for humeral head avascular necrosis in the sickle cell disease population. METHODS: A systematic electronic search was conducted using PubMed (MEDLINE), EMBASE, and Cochrane Library databases. Relevant studies that reported the outcomes of surgical interve

The humeral head: A review of the blood supply and possible link to osteonecrosis following rotator cuff repair.

Trauma, corticosteroid therapy and metabolic diseases are well established aetiologies of humeral head osteonecrosis; however, there is increasing evidence that arthroscopic rotator cuff surgery may be another possible cause. One of the reasons is that there may be inadvertent damage to the arterial blood supply to the humeral head during surgical intervention. The blood supply to the humeral head displays large amounts of variation with regard to origin, course and distribution. Therefore, to shed light on the pathogenesis, the blood supply of the humeral head is reviewed together with a summ

Bilateral Osteonecrosis of the Femoral and Humeral Heads after Short Term Corticosteroid Therapy. A Case Study.

Steroid induced osteonecrosis is a devastating injury that usually requires rebuild of the joint, with the femoral and the humeral head being most commonly affected. Steroid therapy is the most common reported cause of atraumatic osteonecrosis. The Collaborative Osteonecrosis Group Study confirmed that steroids are the primary cause of multi-focal osteonecrosis in 91% of the cases. Osteonecrosis is considered multifocal when three or more joints are involved. We report a rare case of bilateral steroid-induced osteonecrosis of the proximal femora and humeri nine months after a short course of i

Effect of electrical stimulation to long head of biceps in reducing gleno humeral subluxation after stroke.

BACKGROUND: Glenohumeral subluxation is the most frequent complication in post stroke hemiplegia and its reduction has been considered an important goal. Though it has been demonstrated that cyclical electrical stimulation of posterior deltoid and supraspinatus muscles can reduce subluxation, the role of biceps has not been given due consideration. OBJECTIVE: The purpose of this study was to determine whether electrical stimulation to the long head of biceps could more effectively reduce gleno humeral subluxation. METHODS: 24 patients were selected and consecutively assigned to group 1 (elec

Shoulder instability in the setting of bipolar (glenoid and humeral head) bone loss: the glenoid track concept.

BACKGROUND: An assortment of variables has been used in predicting anterior shoulder instability resulting from pathologic engagement of Hill-Sachs lesions on the glenoid. The glenoid track is a unique biomechanical model that relates both Hill-Sachs and bony Bankart lesions to predict shoulder engagement. We examined the glenoid track concept to determine if it provides a model that unifies glenoid rim and humeral head bone loss in predicting engagement. QUESTIONS/PURPOSES: In this review we addressed two questions: (1) How are humeral head and glenoid rim bony defects and their interactions

Proximal humeral fractures treated with hemiarthroplasty: does tenodesis of the long head of the biceps improve results?

INTRODUCTION: Pathology of the long head of the biceps (LHB) may be the cause of anterior shoulder pain after hemiarthroplasty for treatment of fractures of the proximal humerus. The currently available literature lacks adequate randomised trials examining whether tenodesis of the LHB improves results. The purpose of this study was to evaluate the effects of tenodesis of the LHB on the clinical outcome following hemiarthroplasty for fractures of the proximal humerus. METHODS: This prospective randomised study included 37 patients treated with hemiarthroplasty for four-part fractures, fracture

Reconstruction of the articular surface of the humeral head after locked posterior shoulder dislocation: a case report.

Posterior dislocations of the glenohumeral joint are extremely rare (2-4% of all shoulder dislocations) and often associated with bone or ligamentary injuries. Though the reverse Hill-Sachs lesion is a common injury associated with posterior shoulder dislocation, there have been only few articles describing specific treatments for this type of humeral head defect. This article describes the successful operative treatment of an acute locked posterior shoulder dislocation by reconstructing the articular surface of the humeral head with the use of autologous bone graft taken from the iliac crest.

Static posterior subluxation of the humeral head: an unrecognized entity responsible for glenohumeral osteoarthritis in the young adult.

Thirteen men with a mean age of 40 years had glenohumeral arthritis and symptoms of stiffness, pain, and locking of the joint at presentation. Provocative test results for anterior or posterior instability were negative. Computed tomography detected posterior humeral head subluxation relative to the glenoid and posterior cartilage wear. All patients demonstrated this posterior subluxation in the absence of posterior glenoid erosion. The mean glenoid retroversion was increased (14.5 degrees ). Increased glenoid retroversion was observed in the contralateral shoulder in 4 cases, and marked stati
