Ileostomy After Intestinal Transplantation: The First in Depth Report on Techniques, Complications, and Outcomes.

BACKGROUND: Temporary ileostomy during intestinal transplantation (ITx) is the standard technique for allograft monitoring. A detailed analysis of the ITx ileostomy has never been reported. METHODS: A retrospective review of a single-center ITx database was performed. The analysis was divided into ileostomy formation and takedown episodes. RESULTS: One hundred thirty-five grafts underwent ileostomy formation, and 79 underwent ileostomy takedown. Median age at ITx was 7.7 years and weight was 23 kg. Allograft types were intestine (22%), liver/intestine (55%), multivisceral (16%), and modified

Is Conversion of a Failed IPAA to a Continent Ileostomy a Risk Factor for Long-term Failure?

BACKGROUND: A continent ileostomy may be offered to patients in hopes of avoiding permanent ileostomy. Data on the outcomes of continent ileostomy patients with a history of a failed IPAA are limited. OBJECTIVE: This study aimed to assess whether a history of previous failed IPAA had an effect on continent ileostomy survival and the long-term outcomes. DESIGN: This was a retrospective cohort study. SETTINGS: This investigation took place in a high-volume, specialized colorectal surgery department. PATIENTS: Patients who underwent continent ileostomy construction after IPAA failure between

Variations of the renal function parameters in rectal cancer patients with a defunctioning loop ileostomy.

PURPOSE: The objective of this study is to investigate the impact of the temporary loop ileostomy on renal function and also to assess the factors associated with the change in renal function observed between the index surgery (the moment of the radical surgical procedure) and the closure of the ileostomy (the moment of the secondary surgical act of suppression of the ileostomy). METHODS: A total of 69 rectal cancer patients from a single referral surgical unit who had a loop ileostomy during low anterior resection of the rectum were included in this study. Serum creatinine levels were evalua

[Therapy of refractory proctocolitis and Crohn's disease. Incisionless laparoscopic proctocolectomy with a Brooke ileostomy].

BACKGROUND: Laparoscopic colorectal surgery is nowadays also widely used in surgery of inflammatory bowel disease. With the correct indications laparoscopic surgery is an attractive cosmetic alternative for the predominantly juvenile patients. Refractory fistulizing Crohn's proctocolitis is a very severe disease with a maximal limitation on the quality of life. Proctocolectomy with a Brooke ileostomy represents a very effective option for these patients. The laparoscopic technique can at least spare the patients a salvage laparotomy incision. We report about our preliminary experiences PATIEN

Controlled intraoperative water testing of left-sided colorectal anastomoses: are ileostomies avoidable?

Anastomotic leakage is a major problem in colorectal surgery, and previous studies have suggested that intraoperative identification of leaks allows repair at the time of surgery. This study examined whether testing allowed a defunctioning ileostomy to be safely omitted. A series of 102 consecutive patients underwent left-sided colorectal resection, 52 males and 50 females, mean age 65.7 years (range 16-89 years). After completion of the anastomosis, its integrity was tested by running saline into the rectum, using a manometer, to a maximum distending pressure of 30 cmH2O. Any leaks were repai

Squamous cell carcinoma occurring in a skin-grafted ileostomy stoma. Report of a case.

A squamous cell carcinoma arose in a skin-grafted ileostomy stoma 26 years after proctocolectomy for Crohn's colitis. The patient presented with peristomal ulceration and pain of several months' duration. Attention is drawn to this late complication of ileostomy which requires wide local excision and relocation of the stoma.

Definitive ileostomy in total colonic aganglionosis: Results and considerations from a single-center experience.

BACKGROUND: Total Colonic and Small Bowel Aganglionosis (TCSA) is a severe form of Hirschsprung Disease (<10 % of cases). Surgical gold standard is Pull-Through (P-T) procedure. After P-T, in selected cases, an unplanned ileostomy could be required to cope with poor bowel control. Aim of this study is to investigate unplanned ileostomies in TCSA. METHODS: A retrospective observational cohort study was performed. Charts of all TCSA patients admitted between 2017 and 2025 were reviewed. Patients requiring an unplanned ileostomy were identified. Data regarding clinical features, surgical managem

Ileostomy Adenocarcinoma After Recurrent Rectal and Colon Cancer: A Case Report and Literature Review.

Numerous complications of stoma have been documented in the literature, yet only a few involve malignancy. However, missing or delayed diagnosis can lead to disease progression, metastasis, and death. We present a case of a 71-year-old patient who had a history of recurrent colorectal cancer with multiple operations, including an end ileostomy, who suffered from an ileostomy adenocarcinoma, presenting with abnormal growth on the mucocutaneous junction of the stoma, in the absence of high-risk features, including familial adenomatous polyposis or inflammatory bowel disease. Diagnosis was confir

Ileostomy for Refractory Immune Checkpoint Inhibitor-Associated Colitis: A Case Report and Literature Review.

Immune checkpoint inhibitors (ICIs) are widely used to treat malignant tumors and have transformed cancer treatment; however, immune-related adverse events (irAEs) can occur due to excessive immune responses. Among these, immune checkpoint inhibitor-related colitis (irAE colitis) presents with symptoms resembling ulcerative colitis, for which high-dose corticosteroids and anti-TNF-alpha antibodies are recommended. Surgical treatment has not been well established for cases refractory to medical therapy. We report a case of a 64-year-old male patient with PD-L1-positive recurrent lung adenocarci

Incidence and Risk Factors for Incisional Hernia Following Ileostomy Takedown: A Retrospective Cohort Study.

Background: Incisional hernias are a frequent complication following ileostomy closure, with rates reaching 24%. Protective ileostomies are commonly performed in colorectal surgery, but their closure presents a significant risk for abdominal wall defects. Identifying risk factors for incisional hernias at the ileostomy site is crucial for improving patient outcomes. Methods: This retrospective study analyzed data from 95 patients who underwent loop ileostomy closure at two Romanian hospitals between 2018 and 2023. Patient demographics, surgical details, and follow-up data were reviewed. Incisi

Outcome of ileostomy performed among emergency cases: A prospective cross-sectional study in India.

Ileostomy is a life-saving procedure often performed in emergency settings for complicated cases such as ileal perforation, particularly due to enteric fever and intestinal tuberculosis. We conducted a prospective study between October 2018 and September 2020, including 57 patients who underwent temporary ileostomy for faecal diversion in emergency laparotomies. The majority of patients were male, with common presenting symptoms being abdominal pain, fever and vomiting. Postoperative complications were frequent, the most common being skin excoriation and ileostomy diarrhoea, while the overall

Right-Sided Ileostomy on Stoma Outlet Obstruction in Ulcerative Colitis: A Randomized Controlled Trial.

BACKGROUND: Stoma outlet obstruction is a common complication after total proctocolectomy with IPAA for ulcerative colitis, occurring in 20% to 40% of cases. In contrast, the incidence of stoma outlet obstruction after low anterior resection is less than 10%. Because stomas are typically created in the right lower abdomen, we considered that small-bowel torsion and tension might be more pronounced after IPAA. Therefore, we hypothesized that creating a stoma in the left lower abdomen during IPAA could reduce the incidence of stoma outlet obstruction. OBJECTIVE: To evaluate whether, compared wi

Efferent limb stimulation prior to loop ileostomy closure: a systematic review and meta-analysis.

BACKGROUND: Postoperative ileus (POI) remains a common phenomenon following loop ileostomy closure. Our aim was to determine whether preoperative physiological stimulation (PPS) of the efferent limb reduced POI incidence. METHODS: A PRISMA-compliant meta-analysis searching PubMed, EMBASE and CENTRAL databases was performed. The last search was carried out on 30 January 2023. All randomized studies comparing PPS versus no stimulation were included. The primary endpoint was POI incidence. Secondary endpoints included the time to first passage of flatus/stool, time to resume oral diet, need for

Comparative outcomes between palliative ileostomy and colostomy in patients with malignant large bowel obstruction.

OBJECTIVES: Palliative stoma creation should be considered in patients at high risk of colonic metallic stent failure. However, it is unclear whether ileostomy or colostomy is superior. This study compared short-term outcomes between palliative ileostomy and colostomy. METHODS: We identified 82 patients with malignant large bowel obstruction, caused by various advanced cancers, between January 2005 and December 2016. We compared short-term outcomes between the ileostomy group (n = 33) and the colostomy group (n = 49). RESULTS: For all 82 patients, clinical success was achieved. Three patient

Postoperative management after loop ileostomy closure: are we keeping patients in hospital too long?

INTRODUCTION: Recent studies have shown that closure of loop ileostomy can be performed in the day-case setting, reducing the length and cost of hospitalisation. By analysing our patients who have undergone reversal, we aimed to determine the length of hospital stay and potential factors behind stays beyond 24 h. PATIENTS AND METHODS: A database of patients undergoing closure of loop ileostomy at one colorectal unit was examined. The times taken to discharge, morbidity and re-admission rates were recorded. RESULTS: Eighty patients underwent reversal of ileostomy between January 2001 and Janu

Role of water-soluble enema before takedown of diverting ileostomy for low pelvic anastomosis.

The integrity of a low pelvic anastomosis is often studied radiographically before takedown of a diverting ileostomy. The aim of this study is to determine the impact of routine water-soluble enema studies (WSE) in our patient population with low pelvic anastomosis. We retrospectively reviewed the operative database for a county teaching hospital from 1998 to 2008. All patients with low pelvic anastomosis (ultralow colorectal, coloanal, and ileoanal pouch anastomosis) with diverting ileostomy who underwent subsequent takedown were identified. Fifty patients met inclusion criteria. Thirty-eight

Megacolon Associated with Multiple Endocrine Neoplasia Type 2B: A Case Successfully Managed with Ileostomy.

INTRODUCTION: Multiple endocrine neoplasia type 2B (MEN2B) is a rare genetic disorder characterized by medullary thyroid carcinoma, pheochromocytoma, mucosal neuromas, and Marfanoid body habitus. Megacolon is a common non-endocrine manifestation, with approximately 75% of patients experiencing gastrointestinal symptoms, such as constipation or diarrhea, often as early clinical signs. Although these symptoms may be important diagnostic signs, there are currently no distinct therapeutic guidelines. Here, we report a case of MEN2B-associated megacolon that was managed surgically, resulting in a f
