Spigelian hernia (SH) is a rare abdominal wall defect that frequently presents a diagnostic challenge because of its non-specific symptoms and deep anatomical location. We report the case of a 64-year-old woman with adequately controlled hypertension who presented with localized abdominal wall pain and a long-standing lateral abdominal bulge. A 30-year-old African American male presented with pain and swelling of the right foot one month after receiving a tattoo on this foot in prison. In the United States, approximately 180,700 women are incarcerated across jails and prisons, comprising a demographic with an aging population and a higher growth rate compared to men. In the May 2020 edition of this publication, I wrote a guest commentary entitled "In support of children of incarcerated parents." This article examined the factors associated with thoughts of ending life in a sample of incarcerated men. Data were obtained from the Cancer Risk in Incarcerated Men Study, a pilot study designed to examine cancer health disparities and cancer health education in a racially/ethnically diverse sample of male smokers in three state prisons in the northeast region of the United States from 2015 to 2017. The authors report a rare case of herniation and incarceration of the gallbladder at the site of abdominal drainage tube deployment in a patient undergoing treatment for aortic valve disease. An incarcerated postoperative ventral hernia was confirmed by contrast-enhanced CT of the abdomen. In this Review, we assess how mass incarceration, a monumental American policy experiment, has affected families over the past five decades. We reach four conclusions. First, family member incarceration is now common for American families. Second, individuals who will eventually have a family member incarcerated are worse off than those who never will, even before the incarceration takes place. INTRODUCTION: Inguinal hernia may contain vermiform appendix in very rare cases. This particular condition is known as Amyand's hernia and it has an incidence of 1%. Diagnosis is very difficult preoperatively, so it is usually an incidental finding.

Spigelian hernias are rare hernias, occurring through a defect in the Spigelian aponeurosis. Like other hernias, they may contain abdominal contents but are more likely to be incarcerated due to the small size of the fascial defect. Spigelian hernias are uncommon lateral ventral wall hernias with a significant rate of incarceration; these hernias often produce nonspecific clinical signs and symptoms as well as elusive imaging findings. Although there are reported cases of incarcerated appendices within Spigelian hernias, this case specifically illustrates the diagnostic difficulty these hernias present to both surgeons and radiologists. Additionally, we discuss important anatomy, demographics and risk factors, clinical symptoms, imaging pitfalls and recommendations for repair based on a review of literature.

INTRODUCTION: A femoral hernia is a rare, acquired condition, which has been reported in less than 5% of all abdominal wall hernias, with a female to male ratio of 4:1.
PRESENTATION OF CASE: We report a case in a female patient who had a previous open inguinal herniorrhaphy three years previously. She presented with right sided groin pain of one month duration. Ultrasound gave a differential diagnosis of a recurrent inguinal hernia or a femoral hernia. A transabdominal preperitoneal repair was performed and the patient made an uneventful recovery.
