Vaginal carriage rate of group B Streptococcus in pregnant women and its transmission to neonates.

BACKGROUND: Maternal vaginal colonisation with Group B Streptococcus (GBS, Streptococcus agalactiae) at the time of delivery can cause vertical transmission to the neonate. GBS is the leading cause of sepsis, meningitis and pneumonia in the infants. Asymptomatic colonisation of the vagina with GBS varies with the geographical location. METHODS: This was a cross-sectional study conducted in 2009 at Benazir Bhutto Hospital, Rawalpindi, Pakistan. Lower vaginal swabs were obtained from 200 pregnant women at the time of admission in the Gynaecology and Obstetrics Department for term, normal vagina

Role of glial cells in the functional expression of LL-37/rat cathelin-related antimicrobial peptide in meningitis.

Antimicrobial peptides are intrinsic to the innate immune system in many organ systems, but little is known about their expression in the central nervous system. We examined cerebrospinal fluid (CSF) and serum from patients with active bacterial meningitis to assess antimicrobial peptides and possible bactericidal properties of the CSF. We found antimicrobial peptides (human cathelicidin LL-37) in the CSF of patients with bacterial meningitis but not in control CSF. We next characterized the expression, secretion, and bactericidal properties of rat cathelin-related antimicrobial peptide, the h

Interventions to prevent hypothermia at birth in preterm and/or low birthweight infants.

BACKGROUND: Keeping vulnerable preterm infants warm is problematic even when recommended routine thermal care guidelines are followed in the delivery suite. OBJECTIVES: To assess efficacy and safety of interventions designed for prevention of hypothermia in preterm and/or low birthweight infants applied within 10 minutes after birth in the delivery suite compared with routine thermal care. SEARCH STRATEGY: We used the standard search strategy of the Cochrane Neonatal Review Group (CNRG). The review was updated in October 2009. SELECTION CRITERIA: Trials using randomised or quasi-randomised

Post-resuscitation management of asphyxiated neonates.

Inspite of major advances in monitoring technology and knowledge of fetal and perinatal medicine, perinatal asphyxia is one of the significant causes of mortality and long term morbidity. Data from National Neonatal Perinatal Database suggests that perinatal asphyxia contributes to almost 20% of neonatal deaths in India. "Failure to initiate or sustain respiration after birth" has been defined as criteria for the diagnosis of asphyxia by WHO. Perinatal asphyxia results in hypoxic injury to various organs including kidneys, lungs and liver but the most serious effects are seen on the central ne

Neonatal macrophages express elevated levels of interleukin-27 that oppose immune responses.

Microbial infections are a major cause of infant mortality worldwide because of impaired immune defences in this population. The nature of this work was to further understand the mechanistic limitations of the neonatal and infant immune response. Interleukin-27 (IL-27) is a heterodimeric cytokine of the IL-12 family that is produced primarily by antigen-presenting cells and is immunosuppressive toward a variety of immune cell types. We show that IL-27 gene expression is elevated in cord blood-derived macrophages relative to macrophages originating from healthy adults. We also evaluated the dur

Normal infant by a gestational carrier for a phenylketonuria mother: alternative therapy.

The consequences of pregnancies in untreated phenylketonuria (PKU) mothers are a high incidence of spontaneous abortion, intrauterine growth retardation with microcephaly, congenital malformations, and abnormal intellectual development. PKU fathers, on the other hand, produce normal children. Obviously children of PKU women and men are at least heterozygous, proving that the abnormalities produced by the PKU mothers are not genetic but "intrauterinely environmental." Exposure to the mother's metabolic abnormalities affects the fetus during the entire pregnancy. A PKU mother can produce a healt

Inter-rater and intra-rater reliability of quantitative ultrasound in preterm infants: an observational cohort study.

Extremely premature infants are at high risk of metabolic bone disease of prematurity. Current diagnostic tools require radiation exposure, additional risks to patients, and likely provide late diagnosis. Serum markers of minerals are the current standard of care, but there are no universal diagnostic definitions. QUS is a potential diagnostic method that is non-invasive, provides immediate results, can be performed in the incubator, and has no radiation exposure. QUS has been studied in preterm infants, but few studies have evaluated the intra-rater and inter-rater reliability of QUS in prete

Reversing the trend: The critical need for in-utero transfer to optimise preterm outcomes in tropical LMICs.

There is an inherent structural paradox faced by low- and middle-income countries (LMICs) when it comes to the field of maternal and child health. Whereas there has been success in providing basic obstetric care in the form of healthcare services in peripheral locations, modern neonatal and preterm birth care are concentrated in tertiary urban settings. Thus, high-risk pregnancies are delivered in peripheral clinics, but the fragile preterm neonates are brought to higher centres only after efforts to stabilise their clinical condition fail and severe physiological deterioration occurs. These '

Detection of respiratory syncytial virus (RSV) at birth in a newborn with respiratory distress.

Respiratory syncytial virus (RSV) is the most common respiratory pathogen in infants and young children. From the nasopharyngeal or conjunctival mucosa of infected individuals, RSV spreads to the lower respiratory tract causing acute bronchiolitis and pneumonia after an incubation period of 4-6 days. In addition to its well-documented tropism for the airway epithelium, it has been shown previously that RSV can also spread hematogenously and efficiently infect extrapulmonary tissues of human hosts. Furthermore, it has been shown in animal models that RSV can spread transplacentally from the res

The feasibility of using a portable xenon delivery device to permit earlier xenon ventilation with therapeutic cooling of neonates during ambulance retrieval.

BACKGROUND: Therapeutic hypothermia is the standard of care after perinatal asphyxia. Preclinical studies show 50% xenon improves outcome, if started early. METHODS: During a 32-patient study randomized between hypothermia only and hypothermia with xenon, 5 neonates were given xenon during retrieval using a closed-circuit incubator-mounted system. RESULTS: Without xenon availability during retrieval, 50% of eligible infants exceeded the 5-hour treatment window. With the transportable system, 100% were recruited. Xenon delivery lasted 55 to 120 minutes, using 174 mL/h (117.5-193.2) (median [i

Apparent failure of brain heart infusion broth to cultivate meningococci from clinically infected patients.

Brain Heart Infusion broth is a widely used medium for the isolation of pathogenic bacteria from body fluids. Cerebro spinal fluid and blood inoculated into this medium from clinically meningitic infants infected with Neisseria meningitidis appeared not to support the growth of this organism. In laboratory experiments the broth did not support the growth of Neisseria meningitidis unless adequately vented and with addition of blood. Thorough mixing was necessary to disperse microcolonies that formed in the base blood layer of blood cultures. When this was done they were isolated using standard

[A case of congenital tuberculosis].

A case of lethal acute congenital tuberculosis is reported in a 3-week-old infant who presented a crural adenopathy. An acute miliary pulmonary tuberculosis with extensive cutaneous reaction to tuberculin then appeared. Mycobacterium tuberculosis was found in the node and in urines. Specific treatment was ineffective and the child died within 5 weeks because of extensive lung lesions. A patent pulmonary tuberculosis was discovered in a grandmother that could not be incriminated to contamination because the incubation period was too short and the germ was of different type. A foetopathy could h

Individualized Reconstitution of Human Milk Microbiota: A Feasible Approach in Real-World Settings.

Mother's own milk (MOM) is the most complete nutritional resource for newborns. In cases where mothers are unable to produce sufficient milk or cannot breastfeed, the preferred alternative is pasteurized donor human milk (PDM), which is routinely provided by human milk banks. PDM offers a superior range of nutritional and immunological elements compared to any commercially available formula. However, to ensure biosafety, PDM undergoes pasteurization, a process that inactivates commensal microbiota and reduces certain bioactive compounds. This study presents a protocol designed to restore the m

Unjustified use of amber necklaces for teething symptoms alleviation: Succinic acid release underperforms compared with natural skin bacteria production.

BACKGROUND: There is limited evidence of succinic acid release from amber necklace that justifies its biological plausibility. AIM: This study aimed to evaluate the release of succinic acid from Baltic amber beads in the presence of Staphylococcus epidermidis. DESIGN: The Baltic amber beads from the necklace were stratified according to their weight (average 0.05 g ± 0.067). Subsequently, the beads (n = 8) were submerged in 0.9% buffered saline (Control) or brain-heart infusion culture medium in the presence of a commercial strain of S. epidermidis, a resident skin bacterium incubated at 37°

The newborn infant's thermal environment in the delivery room when skin-to-skin care has to be interrupted.

OBJECTIVE: Newborns are prone to hypothermia immediately following birth. Hypothermia is associated with increased morbidity and mortality rates. We sought to assess the thermal environment and metabolic costs associated with exposure to various situations in the delivery room when skin-to-skin care (SSC) has to be curtailed. METHODS: Environmental variables (air temperature: Ta; relative humidity: RH; radiative temperature: Tr; and air convection velocity) were recorded during sequences reproducing SSC, in the maternity unit's various rooms ("passive environments") and in incubators ("active

Heat loss prevention for preterm infants in the delivery room.

OBJECTIVE: Preterm infants are prone to hypothermia immediately following birth. Among other factors, excessive evaporative heat loss and the relatively cool ambient temperature of the delivery room may be important contributors. Most infants <29 weeks gestation had temperatures <36.4 degrees C on admission to our neonatal unit (NICU). Therefore we conducted a randomized, controlled trial to evaluate the effect of placing these infants in polyurethane bags in the delivery room to prevent heat loss and reduce the occurrence of hypothermia on admission to the NICU. METHODS: After parental conse

[Breast-feeding and the mother's sentiment towards child rearing. Analysis of database on health checkups].

OBJECTIVES: This study aimed to identify the breast-feeding rate and associated factors and to get better understanding on the mother's sentiment towards child rearing due to differences in feeding methods. This information can be used as primary sources providing effective support through community-based health care for mothers and infants. METHODS: The study was carried out in Osaka city from April 2005 to December 2009. Data from the health-checkup records on infants aged 3-5 months were used after excluding any personal information. The data used included feeding methods (exclusive breast

The impact of cobedding on sleep patterns in preterm twins.

Interest in cobedding multiple-gestation infants has grown as focus has increased on the developmental approach to the care of the neonate. Little data, however, exist on the infants' response to cobedding. It is important to evaluate the safety, efficacy, and physiologic impact of this practice. Cobedding was offered to parents of twin infants < 37 weeks gestation, without arterial lines or ventilator requirements, by the health care team according to standard practice in our nursery. After parental informed consent, infants were placed on an event-recording cardiorespiratory monitor for 12 h
