Respiratory muscle dysfunction is associated with prolonged and difficult weaning from mechanical ventilation. This dysfunction in ventilator-dependent patients is multifactorial: there is evidence that inspiratory muscle weakness is partially explained by disuse atrophy secondary to ventilation, and positive end-expiratory pressure can further reduce muscle strength by negatively shifting the length-tension curve of the diaphragm. Polyneuropathy is also likely to contribute to apparent muscle weakness in critically ill patients, and nutritional and pharmaceutical effects may further compound

Horseshoe lung is an extremely rare congenital malformation in which the right and left lungs are fused due to stenosis of the lung parenchyma. In anesthetic management, it is important to avoid hypoxemia and hypercapnia caused by a decline in lung capacity and functional residual capacity. A 3-year-old boy with horseshoe lung and left lung hypoplasia was scheduled to undergo cheiloplasty. Regarding respiratory management, to prevent hypoxemia and hypercapnia, we avoided intraoperative peripheral airway obstruction with positive end-expiratory pressure, set a long inspiratory phase time for su

The purpose of this study was to evaluate the electromyographic activity of the accessory inspiratory muscles and the diaphragmatic amplitude (DA) in nasal and mouth-breathing adults. The study evaluated 38 mouth-breathing (MB group) and 38 nasal-breathing (NB group) adults, from 18 to 30years old and both sexes. Surface electromyography (sEMG) was used to evaluate the amplitude and symmetry (POC%) of the sternocleidomastoid (SCM) and upper trapezius (UT) muscles at rest, during nasal slow inspiration at Lung Total Capacity (LTC) and, during rapid and abrupt inspiration: Sniff, Peak Nasal Insp

INTRODUCTION: Sarcopenia and chronic obstructive pulmonary disease (COPD) are risk factors for postoperative pulmonary complications (PPCs). Preoperative inspiratory muscle weakness is also a risk factor for PPCs. Sarcopenia and COPD are often associated with inspiratory muscle weakness. Respiratory sarcopenia has been defined as the coexistence of whole-body sarcopenia and respiratory muscle weakness. We report our experience with preoperative pulmonary rehabilitation, including inspiratory muscle training (IMT), in a patient with lung cancer and comorbid respiratory sarcopenia and COPD.

BACKGROUND: Neuromuscular diseases are characterized by the compromise of respiratory muscles, thoracic ventilation, muscle strength and coughing capacity. Patients have low quality of life and increased morbidity and mortality mostly due to respiratory impairment.
OBJECTIVE: To assess the benefits of adding inspiratory muscle training to neuromuscular patients' treatment and their compliance to the approach.
METHODS: We conducted a single-center prospective study with neuromuscular patients with decreased maximal inspiratory pressure. We developed an inspiratory muscle training protocol wit

Among 315 infants treated for respiratory distress syndrome (RDS) over a 2 year period, 32 prematures were studied retrospectively with the diagnosis of pulmonary interstitial emphysema (PIE). Eighteen died. In this group, birth weight below 1600 g, need for oxygen above 0.6 on the 1st day and appearance of bilateral pulmonary interstitial emphysema within the first 48 h of life were significant risk factors, with a mortality rate of 94%. In order to recognize one or more early criteria predictive of fatal PIE, we compared ventilation parameters on day 1 between neonates with fatal PIE and tho

BACKGROUND: Cardiac rehabilitation (CR) improves quality of life, functional capacity, and cardiovascular outcomes in patients with coronary heart disease (CHD). However, post-COVID-19 sequelae may affect baseline physical condition and potentially influence CR outcomes.
OBJECTIVES: To compare baseline inspiratory muscle strength, hand grip force, cardiorespiratory function and quality of life in CHD patients with and without prior COVID-19, and to evaluate their response to a CR programme.
METHODS: A prospective observational study was conducted with 77 CHD patients enrolled in a supervised

Diaphragmatic dysfunction is a hallmark of chronic obstructive pulmonary disease (COPD), especially in emphysema, where hyperinflation alters diaphragm geometry and impairs inspiratory mechanics. However, quantitative three-dimensional (3D) assessments of diaphragmatic dome shape and motion across COPD phenotypes are limited. This study applied 3D geometric morphometrics (3DGM) to characterize diaphragmatic kinematics and curvature in COPD patients compared to controls. We prospectively enrolled 126 adults: 40 COPD patients (17 bronchitis-predominant, 23 emphysema-predominant) and 86 asymptoma

BACKGROUND: Decreased respiratory muscle strength and muscle mass is key in diagnosing respiratory sarcopenia. However, the role of reduced diaphragm activity, expressed as the maximal level of diaphragmatic excursion (DEmax), in diagnosing respiratory sarcopenia in patients with chronic obstructive pulmonary disease (COPD) remains unclear. This study aimed to characterize patients with COPD and low DEmax and maximal inspiratory pressure (MIP), a measure of inspiratory muscle strength, and assess the role of DEmax in respiratory sarcopenia.
METHODS: Patients with COPD underwent spirometry, ex

Evidence from large multicentre exercise intervention trials in heart failure patients, investigating both moderate continuous aerobic training and high intensity interval training, indicates that the 'crème de la crème' exercise programme for this population remains to be found. The 'aerobic/resistance/inspiratory (ARIS) muscle training hypothesis in heart failure' is introduced, suggesting that combined ARIS muscle training may result in maximal exercise pathophysiological and functional benefits in heart failure patients. The hypothesis is based on the decoding of the 'skeletal muscle hypot

INTRODUCTION: Vocal cord dysfunction (VCD) is a rare disease characterized by a paradoxical closure of the vocal cords, usually in inspiration, that causes dyspnea and stridor. The spirometry pattern that is more often described is a plateau in the inspiratory curve, but it can be also found in the expiratory loop The aim of this study was to evaluate the most common spirometry characteristics of patients with VCD and, secondarily, to describe the clinical and demographic characteristics and the treatment of patients with a definitive diagnosis of this disease.

The principles of measuring inspiratory muscle tension-time index (TTim) and the ratio of the works of inspiration over the maximal works of inspiration (Wi/Wi(max)) were investigated and their formulae were deduced, i.e. TTim = (Pi x Ti)/(MIP x Ttot) and Wi/Wi(max) = (Pi x VT)/(MIP x IC). The importance of the inspiratory pressure and the maximal inspiratory pressure (MIP) measured at function residual capacity (FRC) level was emphasized. Both TTim and Wi/Wi(max) were measured in 35 normal subjects and 89 patients with chronic obstructive pulmonary disease (COPD). The results showed that norm

BACKGROUND: Respiratory muscle dysfunction is a recognized complication of multiple sclerosis (MS) and may contribute to functional decline. Sarcopenia related to neurological impairment may further impair respiratory muscle performance in MS. This study aimed to investigate the association between sarcopenia and respiratory function, respiratory muscle strength, and respiratory muscle endurance in patients with MS.
METHODS: This study was designed as a cross-sectional observational study. In this study, 52 MS patients (26 sarcopenic, 26 non-sarcopenic) were evaluated. Sarcopenia was assessed

BACKGROUND: Ultrasound allows observation of the thickness of the diaphragm in the zone of apposition in vivo during relaxation and maximum inspiratory efforts.
METHODS: Changes of diaphragm thickness were studied by B mode (two dimensional) ultrasound in 13 healthy men aged 29-54 years in the seated position. A high resolution 7.5 MHz ultrasound transducer was held perpendicular to the chest wall in the line of a right intercostal space between the anteroaxillary and mid-axillary lines to observe the diaphragm in the zone of apposition 0.5-2 cm below the costophrenic angle. Muscle efficiency increases with fiber length and decreases with load. Diaphragm efficiency (Eff(di)) in healthy humans, measured as power output (Wdi) relative to the root mean square of diaphragm electromyogram (RMS(di)), increases with hyperpnea due to phasic activity of abdominal muscles acting to increase diaphragm length at end expiration (L(di ee)) and decrease inspiratory load. In chronic obstructive pulmonary disease (COPD), hyperpnea may decrease Eff(di) if L(di ee) decreases and load increases due to airflow obstruction and dynamic hyperinflation. To examine this hypothesis, we meas

We compared the effects of the most commonly used respiratory muscle (RM) training regimes: RM endurance training (RMET; normocapnic hyperpnoea) and inspiratory resistive training (IMT), on RM performance. Twenty-six healthy men were randomized into 3 groups performing 4 weeks of RMET, IMT or sham-training. Lung function, RM strength and endurance were tested before and after training. RM fatigue during intermittent hyperpnoea was assessed by twitch oesophageal (P(oes,tw)) and gastric pressures with cervical and thoracic magnetic stimulation. The site and severity of upper airway obstruction accurately determined by analysis of the flow-volume curve obtained from a dyspneic patient with bullous pemphigoid. The limitation of maximum inspiratory flow to 0.5 L/s and of maximum expiratory flow to 0.7 L/s over most of the vital capacity suggested that the lumen of the extrathoracic trachea was narrowed to a diameter of 3 mm. The marked improvement in flow with the patient breathing a helium-oxygen mixture further confirmed that flow was limited in a large central airway. The predictions made from analysis of the flow-volume curve were c
