Arteries of the insula.

OBJECT: The insula is located at the base of the sylvian fissure and is a potential site for pathological processes such as tumors and vascular malformations. Knowledge of insular anatomy and vascularization is essential to perform accurate microsurgical procedures in this region. METHODS: Arterial vascularization of the insula was studied in 20 human cadaver brains (40 hemispheres). The cerebral arteries were perfused with red latex to enhance their visibility, and they were dissected with the aid of an operating microscope. Arteries supplying the insula numbered an average of 96 (range 77-1

Altered resting-state functional connectivity of the insula in individuals with clinical high-risk and patients with first-episode schizophrenia.

OBJECTIVES: Abnormalities in insular functional connectivity have been implicated in many clinical features of schizophrenia. The aim of this study was to determine to what degree such abnormalities occur in individuals with clinical high risk for psychosis (CHR), and whether which is associated with symptom severity. METHODS: Resting-state fMRI data were collected from 47 healthy controls, 24 CHR individuals and 19 patients with first-episode schizophrenia. Using the posterior, dorsal and ventral insular subregions as separate seeds, we examined resting-state functional connectivity differen

Error-related brain activation during a Go/NoGo response inhibition task.

Inhibitory control and performance monitoring are critical executive functions of the human brain. Lesion and imaging studies have shown that the inferior frontal cortex plays an important role in inhibition of inappropriate response. In contrast, specific brain areas involved in error processing and their relation to those implicated in inhibitory control processes are unknown. In this study, we used a random effects model to investigate error-related brain activity associated with failure to inhibit response during a Go/NoGo task. Error-related brain activation was observed in the rostral as

Structural Connections of Functionally Defined Human Insular Subdivisions.

The organization of the human insular cortex has traditionally been considered as an anterior-posterior dichotomy, where anterior and posterior subdivisions have unique structural and functional connections. However, recent functional neuroimaging research proposes a tripartite organization where insular subdivisions have both unique and overlapping functional profiles. Studies examining unique profiles show that the dorsal anterior insula (dAI) has connections with frontal areas supporting higher-level cognitive processes, the ventral anterior insula (vAI) has connections with limbic areas su

Negative motor areas: functional connectivity using stereoelectroencephalography.

Negative motor areas are cortical regions that, when stimulated, determine an inability to pursue voluntary movement or sustain voluntary muscle contraction without producing any other symptoms or signs. This is the first study that addresses the localization and functional connectivity of the negative motor network using the stereoelectroencephalographic method in patients explored for focal drug-resistant epilepsy. Responses to high frequency intracranial stimulations in 225 consecutive patients from two epilepsy centers (115/110) were reviewed. Forty-one stimulations (performed in 23 patien

Time-dependent effects of acute ethanol administration on regional cerebral blood flow in the rat.

The present study investigated the role of the postinjection interval in determining the functional consequences of acute ethanol administration in the CNS. Regional cerebral blood flow (RCBF) was determined by the [14C]iodoantipyrine method in 33 brain structures of ethanol-naive Sprague-Dawley rats. In the first experiment, changes in RCBF were assessed 5 and 15 min after a 0.8 g/kg (i.p.) dose of ethanol or water. Five minutes after treatment, rates of RCBF were increased in the motor cortex, agranular insular cortex, and the olfactory tubercle compared to water controls. No significant dif

[Functional brain mapping of pain perception].

In this review, we summarize the contribution of functional imaging to the question of nociception in humans. In the beginning of the 90's, brain areas supposed to be involved in physiological pain processes were almost exclusively the primary somatosensory area (SI), thalamus, and anterior cingulate cortex. In spite of these a priori hypotheses, the first imaging studies revealed that the main brain areas and those providing the most consistent activations in pain conditions were the insular and the SII cortices, bilaterally. This has been confirmed with other techniques such as intracerebral

A brain-to-bone marrow circuit drives neutrophil mobilization and exacerbates ischaemic brain injury.

Ischaemic stroke triggers a detrimental inflammatory response, characterised by neutrophil mobilisation from the bone marrow to the injured brain. The neural circuits driving this response are poorly understood. Here we identify a brain-to-bone marrow axis that governs this process. We used transient middle cerebral artery occlusion, magnetic resonance imaging, neurological behavioural assessments and infarct volume analysis to establish and evaluate ischaemic brain injury. In addition, viral tracing, optogenetic and chemogenetic manipulation, chemical and surgical interruption of sympathetic

Sexually dimorphic gray matter volume reduction in patients with panic disorder.

While clinical features of panic disorder show significant sexual dimorphism, previous structural MRI studies have not sufficiently controlled for sex when looking at regional brain abnormalities in panic disorder. Using optimized voxel-based morphometry (VBM), regional gray matter volume was compared between 24 patients (male/female: 9/15) with panic disorder and 24 healthy subjects matched for age and sex. Significant gray matter volume reductions were found in the bilateral dorsomedial and right ventromedial prefrontal cortices, right amygdala, anterior cingulate cortex, bilateral insular c

Multiple Posterior Insula Projections to the Brainstem Descending Pain Modulatory System.

The insular cortex is an important hub for sensory and emotional integration. It is one of the areas consistently found activated during pain. While the insular's connections to the limbic system might play a role in the aversive and emotional component of pain, its connections to the descending pain system might be involved in pain intensity coding. Here, we used anterograde tracing with viral expression of mCherry fluorescent protein, to examine the connectivity of insular axons to different brainstem nuclei involved in the descending modulation of pain in detail. We found extensive connecti

Cardiac dysrhythmias associated with central nervous system dysfunction.

Intracranial pathology is frequently associated with cardiac dysrhythmias, which are sometimes lethal. Stroke, subarachnoid hemorrhage, seizures and head trauma with or without increased intracranial pressure are observed to be accompanied by myocardial damage and by ECG abnormalities, including T-wave changes, shortened P-R interval, prolonged Q-T interval, premature ventricular contractions, ventricular ectopy, sinus bradycardia, ventricular and supraventricular tachycardias. Derangements of autonomic function have been shown to be responsible for these disturbances of rate, rhythm and condu

The tauopathy associated with mutation +3 in intron 10 of Tau: characterization of the MSTD family.

Multiple system tauopathy with presenile dementia (MSTD) is an inherited disease caused by a (g) to (a) transition at position +3 in intron 10 of Tau. It belongs to the spectrum of frontotemporal dementia and parkinsonism linked to chromosome 17 with mutations in Tau (FTDP-17T). Here we present the longitudinal clinical, neuropsychological, neuroimaging, neuropathological, biochemical and genetic characterization of the MSTD family. Presenting signs were consistent with the behavioural variant of frontotemporal dementia in 17 of 21 patients. Two individuals presented with an atypical form of p

Uncovering the functional anatomy of the human insula during speech.

The contribution of insular cortex to speech production remains unclear and controversial given diverse findings from functional neuroimaging and lesional data. To create a precise spatiotemporal map of insular activity, we performed a series of experiments: single-word articulations of varying complexity, non-speech orofacial movements and speech listening, in a cohort of 27 patients implanted with penetrating intracranial electrodes. The posterior insula was robustly active bilaterally, but after the onset of articulation, during listening to speech and during production of non-speech mouth

Examining the association between posttraumatic stress disorder and disruptions in cortical networks identified using data-driven methods.

Posttraumatic stress disorder (PTSD) is associated with lower cortical thickness (CT) in prefrontal, cingulate, and insular cortices in diverse trauma-affected samples. However, some studies have failed to detect differences between PTSD patients and healthy controls or reported that PTSD is associated with greater CT. Using data-driven dimensionality reduction, we sought to conduct a well-powered study to identify vulnerable networks without regard to neuroanatomic boundaries. Moreover, this approach enabled us to avoid the excessive burden of multiple comparison correction that plagues verte

The meltdown pathway: A multidisciplinary account of autistic meltdowns.

Autistic meltdowns are fits of intense frustration and often physical violence elicited by sensory and cognitive stressors. Despite the high prevalence of meltdowns among autistic individuals, the neural mechanisms that underlie this response are not yet well understood. This has thus far hampered progress toward a dedicated therapeutic intervention-beyond traditional medications-that limits their frequency and severity. Here, we aim to initiate an interdisciplinary dialogue on the etiology of sensory meltdowns. In doing so, we frame meltdowns as a consequence of underlying chronic hypervigila

["Ear rage": misophonia : Review and current state of knowledge].

Misophonia is a devastating disorder. It is known as an affective sound-processing disorder characterized by the experience of strong negative emotions (e.g., anger, distress) in response to human sounds such as eating/swallowing/breathing. Other sounds produced by humans but not directly by human bodies can also be misophonic triggers (e.g. pen clicking) or environmental sounds (animal sounds/sounds of machines). The type of aversive triggers is individual. The reaction to trigger sounds can depend on many factors, such as assessment of the sound, personal experience, social context or psycho

Altered brain function in persistent postural perceptual dizziness: A study on resting state functional connectivity.

This study used resting state functional magnetic resonance imaging (rsfMRI) to investigate whole brain networks in patients with persistent postural perceptual dizziness (PPPD). We compared rsfMRI data from 38 patients with PPPD and 38 healthy controls using whole brain and region of interest analyses. We examined correlations among connectivity and clinical variables and tested the ability of a machine learning algorithm to classify subjects using rsfMRI results. Patients with PPPD showed: (a) increased connectivity of subcallosal cortex with left superior lateral occipital cortex and left m

Brain mechanisms discriminating enactive mental simulations of running and plogging.

Enactive cognition emphasizes co-constructive roles of humans and their environment in shaping cognitive processes. It is specifically engaged in the mental simulation of behaviors, enhancing the connection between perception and action. Here we investigated the core network of brain regions involved in enactive cognition as applied to mental simulations of physical exercise. We used a neuroimaging paradigm in which participants (N = 103) were required to project themselves running or plogging (running while picking-up litter) along an image-guided naturalistic trail. Using both univariate and

The effect of scatter correction on 123I-IMP brain perfusion SPET with the triple energy window method in normal subjects using SPM analysis.

Scatter correction (SC) using the triple energy window method (TEW) has recently been applied for brain perfusion single-photon emission tomography (SPET). The aim of this study was to investigate the effect of scatter correction using TEW on N-isopropyl-p-[123I]iodoamphetamine (123I-IMP) SPET in normal subjects. The study population consisted of 15 right-handed normal subjects. SPET data were acquired from 20 min to 40 min after the injection of 167 MBq of IMP, using a triple-head gamma camera. Images were reconstructed with and without SC. 3D T1-weighted magnetic resonance (MR) images were a

Insular subdivisions functional connectivity dysfunction within major depressive disorder.

BACKGROUND: Major depressive disorder (MDD) is a mental disorder characterized by cognitive and affective deficits. Previous studies suggested that insula is a crucial node of the salience network for initiating network switching, and dysfunctional connection to this region may be related to the mechanism of MDD. In this study, we systematically investigated and quantified the altered functional connectivity (FC) of the specific insular subdivisions and its relationship to psychopathology of MDD. METHODS: Resting-state FC of insular subdivisions, including bilateral ventral/dorsal anterior in
