State-of-the-art colorectal disease: conservative surgical management of intestinal failure in adults.

PURPOSE: Intestinal failure (IF) is defined by the reduction of gut function under the minimum necessary for adequate absorption of macronutrients and/or electrolytes, requiring home parenteral nutrition (HPN) to maintain health and/or growth. Among the different causes of IF, short bowel syndrome (SBS) is one of the main causes of IF in adults. Management of IF and SBS is complex and requires a multidisciplinary approach. The aim of this study was to review the place and the modalities of conservative surgical procedures performed for IF in adults. RESULTS-CONCLUSION: HPN has drastically mod

Oral intake and plasma citrulline predict quality of life in patients with intestinal failure.

OBJECTIVE: Chronic intestinal failure (cIF) is a rare medical condition usually treated by long-term parenteral nutrition (PN). Owing to disease-associated symptoms and treatment-specific complications, patients with cIF commonly present with reduced quality of life (QoL) compared with healthy controls. The aim of this study was to identify factors associated with QoL in patients with cIF. METHODS: Ninety adult patients with cIF receiving PN were included in an observational study between 2014 and 2017. QoL based on the novel Short Bowel Syndrome-Quality of Life (SBS-QoL) scale and the Short-

A Comparison of Smoflipid® and Intralipid® in the Early Management of Infants with Intestinal Failure.

PURPOSE: Cholestasis is problematic for infants with intestinal failure (IF). The soy-based lipid Intralipid® (IL) has been implicated. An alternative, Smoflipid® (SMOF), is increasingly used. However, its role in cholestasis prevention is unclear. This study compares the incidence and degree of cholestasis between infants with IF receiving SMOF or IL. METHODS: Infants with IF receiving SMOF or IL during the first 8 weeks of parenteral nutrition (PN) support between 2014 and 2017 were reviewed. Clinical characteristics, cholestasis incidence (conjugated bilirubin (Cbili) >2 mg/dL for >2 weeks

Copper Deficiency in Children With Intestinal Failure: Risk Factors and Influence on Hematological Cytopenias.

BACKGROUND: We investigated to what extent serum copper influences hemoglobin (Hb), neutrophil, and platelet counts and the factors associated with serum copper in children with intestinal failure (IF) who have their micronutrient status monitored according to a standard protocol. METHODS: Children with IF admitted to a pediatric intestinal rehabilitation program and receiving home parenteral nutrition (PN) were followed up prospectively. Patients received vitamins and multi-trace elements (TEs) as part of the PN. Copper, iron, zinc, vitamin A, and cobalamin serum levels were routinely monito

Screening Endoscopy Contributes to Relevant Modifications of Therapeutic Regimen in Children With Intestinal Failure.

OBJECTIVE: The role of endoscopy in the diagnostic workup of children with intestinal failure (IF) is not well defined. It is unclear whether endoscopies should be performed as a screening procedure or only upon manifestation of symptoms. The aim of this study is to evaluate, whether performing screening endoscopy contributes to clinical management in children with IF. METHODS: Comparative retrospective case series study in children with IF (mean age 70.4 months ± 58.4 months) presenting for diagnostic workup in a single intestinal rehabilitation center. Endoscopies were performed either as a

A comparison of Broviac® and peripherally inserted central catheters in children with intestinal failure.

PURPOSE: Central venous catheters (CVCs) are a source of morbidity for children with intestinal failure (IF). Complications include infection, breakage, occlusion, and venous thrombosis. Broviacs® have traditionally been preferred, but peripherally inserted central catheters (PICCs) are gaining popularity. This study compares complications between Broviacs® and PICCs in children with IF. METHODS: After IRB approval, children with IF receiving parenteral nutrition (2012-2016) were reviewed. Complications were compared between Broviacs® and PICCs using the generalized estimation equation popula

Safety and Dosing Study of Glucagon-Like Peptide 2 in Children With Intestinal Failure.

BACKGROUND AND AIMS: A glucagon-like peptide 2 (GLP-2) analogue is approved for adults with intestinal failure, but no studies of GLP-2 have included children. This study examined the pharmacokinetics, safety, and nutritional effects of GLP-2 in children with intestinal failure. METHODS: Native human GLP-2(1-33) was synthesized following good manufacturing practices. In an open-label trial, with parental consent, 7 parenteral nutrition-dependent pediatric patients were treated with subcutaneous GLP-2 (20 µg/kg/d) for 3 days (phase 1) and, if tolerated, continued for 42 days (phase 2). [Therapeutic alternatives in case of failure of first-line treatment of intestinal helminthiasis in adults].

Development of antiparasite medications over the last 15 years has greatly reduced the number of treatment failures for intestinal helminthiasis. Benzimidazole derivatives, ivermectine, praziquantel and triclabendazole are easy to use, well tolerated and generally curative. First-line treatment are currently so reliable that failure should lead first to investigation of possible "false failure" causes such as misdiagnosis, poor identification of the parasite, inadequate or incorrect treatment, and repeat contamination, before concluding that genuine parasite resistance is involved and that alt

Role of prostanoids in respiratory failure during circulatory shock of intestinal origin in dogs.

The role of prostanoids in respiratory failure during circulatory shock of intestinal origin was investigated in anesthetized, non-ventilated dogs by measuring PGF2 alpha thromboxane B2 (TXB2) and 6-keto prostaglandin F1 alpha (PGF1 alpha) in arterial and mixed venous (right ventricle) blood samples during superior mesenteric artery occlusion-induced (SMAO) shock. Release of the SMAO caused a dramatic decrease in mean arterial blood pressure (MABP), arterial and mixed venous pO2, hyperventilation and a more than 2 fold increase in levels of prostanoid studied within 5 min. At the same time, ar

Vitamin D and infection risk in patients with short bowel syndrome and intestinal failure.

PURPOSE: Patients with short bowel syndrome (SBS) and intestinal failure (IF) are highly susceptible to micronutrient deficiencies and recurrent infections. Vitamin D plays an important role in immune modulation and has been linked to infection outcomes in other high-risk populations, yet its relevance in SBS/IF remains unclear. This study evaluated the effectiveness of vitamin D supplementation and the association between serum 25-hydroxyvitamin D [25(OH)D] levels and infection risk in SBS/IF patients. METHOD: In this single-center retrospective cohort study, 23 SBS/IF patients (8 pediatric,

Stomal Hyperflow and Intestinal Failure: A Case Report on the Use of a Chyme Reinstillation Device.

Stomal hyperflow, a frequent complication of ileostomy, is associated with high morbidity and mortality, particularly when linked to intestinal failure. This situation, especially when conventional treatments fail, often results in a poor prognosis for the patient. In this case report, we present the history of a patient suffering from stomal hyperflow with intestinal failure, in the context of short small bowel syndrome following multifocal venous mesenteric ischaemia. In a critical condition at the time of admission to our hospital, he was able to benefit from The Insides® System, a chyme re

A case of yogurt central line-associated bloodstream infection in a child with intestinal failure.

Central line-associated bloodstream infections (CLABSI) are a significant cause of morbidity in children with intestinal failure (IF). Probiotic therapy is discouraged in patients with IF because of the increased risk of bacteremia with the probiotic organism. We report an unusual, previously undescribed, clinical presentation of a Lactobacillus-species CLABSI linked to yogurt consumption in a toddler with IF secondary to Megacystis-microcolon-intestinal hypoperistalsis syndrome. Lactobacillus is abundant in fermented foods like yogurt and among the commensal constituents of the oral cavity mi

A comparison of catheter lock solutions and associated complications in pediatric intestinal failure patients.

BACKGROUND: Children with intestinal failure (IF) receiving parenteral nutrition (PN) are at risk for central venous catheter (CVC) associated complications including infection, occlusion, and breakage. A variety of catheter locks have been used to preserve patency and prevent infection when catheters are not in use. Taurolidine (T) and Ethylenediaminetetraacetic acid (EDTA) locks have antimicrobial and anticoagulant properties; however, superiority of one is unclear. MATERIAL AND METHODS: A multicenter, retrospective review of children with IF on PN and receiving care from an Intestinal Reha

Effects of Beraprost on Intestinal Microcirculation and Barrier Function in a Mouse Model of Renal Failure.

OBJECTIVE: Endothelial dysfunction plays an important role in the pathogenesis of chronic kidney disease. Prostacyclin (PGI2), an endothelial cell-produced endogenous prostaglandin, plays a crucial role in maintaining endothelial function. However, its effects on intestinal microcirculation and barrier function are not fully understood. We hypothesized that PGI2 improves intestinal microcirculation and barrier function via endothelial protective effects. METHODS: ICR and ICGN (a spontaneous nephrotic model) mice were used in this study. Intestinal microcirculation was visualized in vivo to in
