Intubated COVID-19 predictive (ICOP) score for early mortality after intubation in patients with COVID-19.

Patients with coronavirus disease 2019 (COVID-19) can have increased risk of mortality shortly after intubation. The aim of this study is to develop a model using predictors of early mortality after intubation from COVID-19. A retrospective study of 1945 intubated patients with COVID-19 admitted to 12 Northwell hospitals in the greater New York City area was performed. Logistic regression model using backward selection was applied. This study evaluated predictors of 14-day mortality after intubation for COVID-19 patients. The predictors of mortality within 14 days after intubation included old

Background A new piece of equipment, the Blockbuster (BB) Intubating Laryngeal Mask Airway (ILMA; Tuoren Medical India, Gurugram, India), was created in 2012. Dr. Chandy created the Fastrach (F) ILMA in 1997, which is another supraglottic airway equipment. The primary purpose of this study was to compare the success rate of intubation with BB-ILMA and F-ILMA. Methods In the chosen age category of >20 to <70 years, undergoing general anesthesia for intubation with ILMAs, 55 patients were in the BB-ILMA (group B), and 55 patients were in the F-ILMA (group F). These ILMAs were put in after the in

The skill of tracheal intubation with rigid scopes - a randomised controlled trial comparing learning curves in 740 intubations.

BACKGROUND: Rigid scopes are successfully used for management of difficult airways, but learning curves have not been established. METHODS: This randomised controlled trial was performed at the University Hospital Bern in Switzerland to establish learning curves for the rigid scopes Bonfils and SensaScope and to assess their performance. Fifteen consultant anaesthetists and 15 anaesthesia registrars performed a total of 740 intubations (10 to 20 intubations with each device per physician) in adult patients without predictors of a difficult airway under general anaesthesia. Submental intubation with intubating laryngeal mask airway as conduit: An airway option for oral submucous fibrosis release.

The main anesthetic concern with oral submucous fibrosis is progressive restriction of mouth opening due to fibrosis producing difficult airway. Securing airway by nasotracheal intubation and tracheostomy are associated with potential complications. Flexible fiberoscope is not available in all the institutes. Submental intubation using intubating laryngeal mask airway is an acceptable alternative technique in such situations. It also provides an unobstructed surgical field.

[Successful tracheal intubation through the air-Q intubating laryngeal airway in a patient with severe pharyngeal edema].

A 68-year-old man was diagnosed with severe pharyngeal edema after neck lymph node dissection for cancer of the external ear canal. He was scheduled for an emergency tracheotomy, but preoperative fiberoptic laryngoscopy revealed airway and glottic obstruction due to severe pharyngeal edema. As difficult mask ventilation and tracheal intubation were anticipated, intubation under spontaneous ventilation was performed to avoid a "can't ventilate, can't intubate" situation. The first attempt to intubate the patient using the Pentax-AWS Airwayscope with a thin Intlock resulted in failure due to hin

Impact of the intubation model on the efficacy of rocuronium during rapid sequence intubation: systematic review of randomized trials.

BACKGROUND: Propofol-rocuronium is thought to be superior to thiopental-rocuronium for rapid sequence intubation (RSI). The role of the intubation model per se has never been investigated. METHODS: Randomized comparisons of rocuronium with succinylcholine for true RSI (administration of muscle relaxant immediately after the hypnotic, intubation within 60 s) or modified RSI (delay between administration of the hypnotic and the muscle relaxant, intubation within 60 s) were sought. Good or excellent intubation conditions were expressed as relative risks (RR) with 95% confidence intervals (CI).

Use of the intubating laryngeal mask airway to facilitate awake orotracheal intubation in patients with cervical spine disorders.

Airway management in patients with unstable cervical spines remains a challenge for anesthesia providers. Because neurologic evaluations may be required following tracheal intubation and positioning for the surgical procedure, an awake intubation technique is desirable in this patient population. In this report, we describe the use of an intubating laryngeal mask airway (ILMA) to facilitate awake tracheal intubation in two patients with cervical spine disorders. After topical local analgesia, the ILMA was inserted easily, and a tracheal tube was passed through the glottic opening without compl

Improving the safety of emergency tracheal intubation.

PURPOSE OF REVIEW: Emergency tracheal intubation in critically ill patients remains associated with high rates of hypoxemia, hemodynamic instability, and cardiac arrest. Over the past decade, several large randomized trials and multicenter cohorts have evaluated strategies to improve the safety of this high-risk procedure. This review summarizes recent evidence to guide clinicians toward safer, physiology-focused airway management in emergency and critical care settings. RECENT FINDINGS: Recent studies support the use of noninvasive ventilation for preoxygenation to reduce severe hypoxemia, r

Persistent Odynophagia 27 Days After Emergent Intubation.

CASE PRESENTATION: We describe a case of persistent odynophagia due to a retained foreign body 27 days after emergent intubation. DISCUSSION: Dentures constitute a potential esophageal foreign body and warrant special consideration during airway management. Odynophagia, dysphagia, and changes in phonation should prompt consideration of retained esophageal foreign bodies, especially in the post-intubation setting.

Nasotracheal intubation in pediatrics: a narrative review.

Nasotracheal intubation (NTI) plays an important role in pediatric airway management, offering advantages in specific situations, such as oral and maxillofacial surgery and situations requiring stable tube positioning. However, compared to adults, NTI in children presents unique challenges owing to anatomical differences and limited space. This limited space, in combination with a large tongue and short mandible, along with large tonsils and adenoids, can complicate intubation. Owing to the short tracheal length in pediatric patients, it is crucial to place the tube at the correct depth to pre

Delayed Pneumothorax Following Bougie-Assisted Nasal Intubation.

A 45-year-old male with tongue cancer and obstructive sleep apnea presented for glossectomy and right neck dissection. He underwent a difficult nasal intubation assisted by a bougie and Glidescope. After an otherwise uneventful procedure, the patient was extubated and taken to recovery. Several hours later, he developed increased respirations and decreased oxygen (O2) saturation with decreased air movement on the right side of his chest. A chest X-ray confirmed a right pneumothorax. A chest tube was placed with immediate improvement of O2 saturation and breathing. Pneumothorax was presumably d

This case involved a 67-year-old female who was admitted for general anesthesia for a mitral clip procedure. Following anesthesia induction, the patient underwent an uneventful orotracheal intubation. Shortly afterwards she developed an ongoing respiratory failure, accompanied by neck and chest subcutaneous emphysema. Upon workup, she was found to have a 6 cm long tracheal laceration on the posterior side. Emergency repair surgery was performed using an extracorporeal membrane oxygenator (ECMO). The patient passed away 11 days later from sepsis. The aim of this case report is to increase aware
