Parental education and the WHO neonatal G-6-PD screening program: a quarter century later.

Neonatal screening for glucose-6-phosphate dehydrogenase (G-6-PD) deficiency in any population with a male frequency >3-5%, combined with parental education regarding the dietary, environmental and sepsis-related triggers for hemolysis was recommended by the WHO (World Health Organization) Working Group in 1989. As the aim of identifying G-6-PD deficiency in the newborn period is to avert or detect extreme hyperbilirubinemia developing at home, before the development of kernicterus, the parental role in identifying evolving icterus was considered integral to any screening program. Now, a quart

Minocycline blocks acute bilirubin-induced neurological dysfunction in jaundiced Gunn rats.

BACKGROUND: Extreme hyperbilirubinemia is treated with double volume exchange transfusion, which may take hours to commence. A neuroprotective agent that could be administered immediately might be clinically useful. Minocycline, an anti-inflammatory and anti-apoptotic semisynthetic tetracycline, prevents hyperbilirubinemia-induced cerebellar hypoplasia in Gunn rats. Acute brainstem auditory evoked potential (BAEP) abnormalities occur after giving sulfadimethoxine to 16-day-old jaundiced Gunn rats to displace bilirubin into tissue including brain. OBJECTIVE: To assess whether minocycline is ne

Rhesus haemolytic disease of the newborn: Postnatal management, associated morbidity and long-term outcome.

Rhesus haemolytic disease of the newborn can lead to complications such as hyperbilirubinaemia, kernicterus and anaemia. Postnatal management consists mainly of intensive phototherapy, exchange transfusion and blood transfusion. During the last decades, significant progress in prenatal care strategies for patients with Rhesus haemolytic disease has occurred. New prenatal management options have led to a remarkable reduction in perinatal mortality. As a result of the increase in perinatal survival, attention is now shifting towards short-term and long-term morbidity. This review focuses on the

Cerebellar hypoplasia in the hyperbilirubinemic Gunn rat: morphological aspects.

Gunn rats, a mutant strain of rats, suffer from autosomal recessive hyperbilirubinemia. The homozygotes (j/j) develop jaundice soon after birth and often exhibit kernicterus and cerebellar hypoplasia that are due to bilirubin. Therefore, j/j Gunn rats have been used as an animal model of bilirubin encephalopathy, as well as of neonatal hyperbilirubinemia. In this review, we discuss morphological aspects of the cerebellar hypoplasia that is due to bilirubin and describe the relationship between plasma bilirubin levels and cerebellar hypoplasia, as well as the pathogenesis of cerebellar hypoplas

Four clinical chemistry analyses for pediatric patients: glycosylated hemoglobin, free bilirubin, sweat electrolytes, neonatal thyroxine.

Laboratory methods evaluating pediatric patients for diabetes mellitus, cystic fibrosis, kernicterus and congenital hypothyroidism lack standardization of analyte and/or methodology. While these are the more common metabolic disorders of children the problems of the methods may result in improper diagnosis and treatment. In addition to standardization problems, glycosylated hemoglobin, free bilirubin and blood spot thyroxine have problems in analyte stability, critical testing steps and/or result interpretation. Sweat electrolytes and blood spot thyroxine require very regimented specimen colle

The jaundiced newborn. Understanding and managing transitional hyperbilirubinemia.

Neonatal jaundice is one of the most common conditions diagnosed by the pediatrician. This normally benign transitional phenomenon is a dynamic balance between the production and elimination of bilirubin. These processes can be exacerbated by a number of pathophysiologic conditions, which cause either an increase in bilirubin production rates, such as hemolysis, or a decrease in bilirubin elimination rates, such as bilirubin conjugation defects. The most dangerous circumstance for an infant is the combination of increased bilirubin production with impaired elimination. These infants are at con

Clinical features of bilirubin encephalopathy.

Clinical features of bilirubin encephalopathy vary depending on the age of the infant and the degree of hyperbilirubinemia. In term infants with hyperbilirubinemia, three distinct clinical phases are apparent in the first weeks of life, and long-term consequences include extrapyramidal disturbances (particularly athetosis), hearing loss, gaze abnormalities (particularly limitation of upward gaze), and, in a minority, intellectual deficits. In term infants with moderate hyperbilirubinemia, minor delay in motor development during the first year has been demonstrated, but with longer follow-up th

Autopsy findings associated with neonatal hyperbilirubinemia.

Hyperbilirubinemia is associated with bilirubin deposition in tissue in the newborn including the liver, kidney, heart, adrenal, lung, and brain. High levels of serum bilirubin will stain the skin and sclera and be called "jaundice" clinically, and similarly high levels of bilirubin will stain tissues internally, preferentially in areas of tissue damage. Whether the bilirubin causes the damage or marks the damage because of other insults or both remains controversial.

Neurophysiological abnormalities in adolescents with type I Crigler-Najjar syndrome.

We report neurophysiological abnormalities in two adolescents with type I Crigler-Najjar syndrome, an autosomal recessive disorder characterized by severe unconjugated hyperbilirubinemia. Electroencephalograms (EEGs) demonstrated frequent generalized single and polyspikes, and background slowing. Normal pattern reversal evoked responses (PRVERs) and normal central brain-stem auditory evoked responses (BAERs) were recorded in both patients. These findings differ from the EEG triphasic wave pattern seen in the EEG in hepatic encephalopathy and the central BAER abnormalities seen in many infants

Impact of bilirubin-induced neurologic dysfunction on neurodevelopmental outcomes.

Bilirubin-induced neurologic dysfunction (BIND) is the constellation of neurologic sequelae following milder degrees of neonatal hyperbilirubinemia than are associated with kernicterus. Clinically, BIND may manifest after the neonatal period as developmental delay, cognitive impairment, disordered executive function, and behavioral and psychiatric disorders. However, there is controversy regarding the relative contribution of neonatal hyperbilirubinemia versus other risk factors to the development of later neurodevelopmental disorders in children with BIND. In this review, we focus on the empi

[Physiologic hyperbilirubinemia in low birth weight newborn infants: relation to gestational age, neonatal weight and intra-uterine growth].

Neonatal hyperibilirubinaemia is a real problem for its possible repercussion on the psychomotor development, mainly in low birth weight infants. The Authors studied the physiologic course of bilirubinaemia in 513 low birth weight newborns and then related it to gestational age, birth weight and intrauterine growth. Results obtained show that neonatal hyperbilirubinaemia is strictly depending on gestational age, while both the birth weight and the intrauterine growth have no significant influence on its course. Certainly the very low birth weight infants run the higher risk of Kernicterus and

Clinical relevance of the plasma reserve albumin binding capacity for bilirubin (RABC) and "free" bilirubin concentration.

An analysis of 55 plasma samples from 46 jaundiced newborn infants showed that endogenous "free" bilirubin levels bear no significant correlation to reserve albumin binding capacities for bilirubin as determined by both the Sephadex G-25 gel filtration and the enzymatic peroxidation technique (r = 0.14, p less than 0.05). This contrasts with the significant correlation between the "free" bilirubin concentrations and the bilirubin/albumin molar ratios in the same plasma samples (r = 0.75, p less than 0.001). The results of this study suggest that if "free" bilirubin is the "driving force" in th

The mortality of exchange transfusions.

A prospective study of the effect and mortality of exchange transfusion was carried out in the Kandang Kerbau Hospital, Singapore. Altogether 140 exchange transfusions were performed on 122 infants. The exchanges were done for hyperbilirubinaemia due to "idiopathic" jaundice, ABO haemolytic disease, and glucose-6-phosphate dehydrogenase deficiency. Eight infants deteriorated during the exchange, in three of whom the procedure had to be terminated prematurely. Two deaths occurred two days after the exchange--the procedure being partly responsible in one case; necrotizing enterocolitis was prese

Clinical evaluation of Sephadex gel filtration in estimation of bilirubin binding in serum in neonatal jaundice.

Clinical experience with a Sephadex gel filtration test-kit for the evaluation of bilirubin binding affinity of serum in neonatal jaundice is reported. In serial serum specimens from 166 jaundiced neonates the results of the test in most cases were in accord with the independent clinical decision to perform exchange transfusion. In all 9 cases with clinical kernicterus the Sephadex test was strongly positive (very low bilirubin binding affinity of serum). The test was positive at lower bilirubin levels and bilirubin/albumin molar ratios in preterm as compared with term neonates, especially in

Hematological morbidity and management in neonates with hemolytic disease due to red cell alloimmunization.

Treatment of severe anemia with intrauterine red cell transfusions in fetuses with red cell alloimmunization has led to a dramatic increase in perinatal survival. Due to this increased survival focus is nowadays shifting towards improving postnatal treatment options. Phototherapy, exchange transfusions and intravenous immunoglobulin are used to treat hyperbilirubinemia and prevent kernicterus. Postnatal treatment of anemia consists of top-up transfusions, supplements to support erythropoiesis such as folic acid and iron, and occasionally erythropoietin treatment. In addition to anemia, other h
