Popliteal artery injury following fracture or dislocation at the knee. Diagnosis and management.

Fracture dislocations or dislocations of the knee can produce an arterial injury that may be difficult to evaluate clinically. Aggressive diagnosis and treatment can produce satisfactory results in both vascular and orthopedic situations if performed early after the injury. Five cases of popliteal arterial injury were seen from April 1974 to August 1976. Three cases were following posterior knee dislocation, one was a femoral fracture displacement, and one was a rotary tibiofibular displacement. Four of the patients had complete popliteal artery transection and one had intramural hematoma and

Indications for temporizing knee-spanning external fixation in the treatment of knee dislocations: A multi-center retrospective case series.

BACKGROUND: The indications for temporizing knee-spanning external fixation (KSEF) in the setting of knee dislocation (KD) are poorly defined, leading to significant uncertainty and inconsistency in clinical practice. This study aimed to analyze and describe the documented indications for temporizing KSEF in a series of patients with KDs. METHODS: A retrospective, multi-center review was conducted at two level I trauma centers from January 2001 to May 2024, identifying patients with documented KD treated with KSEF. Data were extracted from operative records, imaging, and clinical notes, and r

Outcomes of Knee Dislocations and Subluxations in Polytrauma Patients at a Major Trauma Center.

Background This is a retrospective service evaluation of outcomes of polytrauma patients sustaining knee dislocations and subluxations within a major trauma center (MTC). Polytrauma patients with knee dislocations are complex to manage and often sustain multiple life-threatening injuries. Although treatments have progressed, no consensus remains on management timing and strategy. Methods Information on knee dislocations and subluxations sustained in polytrauma patients was extracted using the Trauma Audit and Research Network database. This included demographic information on age, gender, and

Importance of Early Diagnosis and Care in Knee Dislocations Associated with Vascular Injuries.

BACKGROUND: Arterial injury secondary to acute knee dislocation (KD) is a rare but devastative complication. The aim of this study is to evaluate functional sequelae and factors of poor prognosis. METHODS: A retrospective monocentric series of consecutive KD with acute ischemia by popliteal artery injury was analyzed between 2005 and 2017. The main outcome was the amputation rate. RESULTS: Sixteen dislocations were included. Nine (56%) were due to public road accidents, 5 (31%) were due to falls from height, and 2 (13%) were due to sports injuries. Dislocation had occurred in the posterior l

Initial Assessment and Implications for Surgery: The Missed Diagnosis of Irreducible Knee Dislocation.

Irreducible knee dislocation is a rare but devastating orthopedic emergency. Limited discussion about its characteristics has been undertaken due to its low incidence. The purpose of this study was to present a series of irreducible dislocated knees and cumulatively reviewed all existing publications in this filed. A retrospective case series study was undertaken in patients with irreducible knee dislocation. Patients' data were carefully collected and presented. Historical cases of irreducible knee dislocation in published papers were reviewed, and their diagnosis, treatment, and prognosis we

Congenital dislocation of the knee at birth - Part I: Clinical signs and classification.

INTRODUCTION: Congenital dislocation of the knee (CDK) is rare, and clinical semiology at birth is not always suitably analyzed. Existing classifications fail to guide treatment. The aim of the present study was to develop a CDK classification for the neonatal period. HYPOTHESIS: A classification based on neonatal severity of clinical signs is easy to implement on simple criteria. MATERIAL AND METHODS: Fifty-one CDKs (40 patients) seen neonatally were included. Three types could be distinguished in terms of reduction and stability: type I, easily reducible CDK, with reduction snap when the f

Ultra-low velocity knee dislocations: patient characteristics, complications, and outcomes.

BACKGROUND: Knee dislocations resulting in multiligament knee injuries (MLIs) are usually associated with high-energy mechanisms such as motor vehicle accidents or sports injuries; however, obese patients are at risk of MLIs from simple falls. Termed "ultra-low velocity" (ULV) dislocations, these injuries occur in obese patients during activities of daily living and may be associated with higher associated risks and complications. HYPOTHESIS: Ultra-low velocity knee dislocations occur more commonly in obese female patients, are associated with higher risks of neurovascular injuries, and have

Current concepts in knee dislocations: PCL, ACL, and medial sided injuries.

Dislocations resulting in multiligament knee injuries are challenging to treat and diagnose. With proper diagnosis and anatomic reconstruction techniques, patients can have successful outcomes. This article describes the senior author's (J.P.S.'s) preferred reconstruction techniques, timing for surgery, and rehabilitation techniques for injuries involving the anterior cruciate ligament (ACL), posterior cruciate ligament (PCL), and posteromedial corner (PMC). We prefer to address these injuries in a staged fashion. The PCL, PMC, and any additional meniscal pathology are addressed in the index p

Can a Traumatic Pipkin Fracture - Dislocation and Ipsilateral Knee Dislocation Occur Simultaneously? A Rare Case Report with Mid-Term Follow-Up.

INTRODUCTION: Simultaneous ipsilateral hip and knee dislocations are extremely rare, especially when associated with fractures, and present significant challenges in diagnosis and management. These injuries, often resulting from high-velocity trauma, require prompt intervention to tackle the complications and achieve a good outcome. CASE REPORT: We present the case of a 28-year-old male who sustained an ipsilateral hip fracture dislocation and knee dislocation following a high-velocity road traffic accident. Initial clinical examination revealed significant deformities in both joints, and ima

Bilateral Knee Dislocation in an Elderly Patient: From Vascular Emergency to Functional Recovery.

Traumatic knee dislocations are rare but serious injuries associated with limb-threatening vascular compromise and a frequently poor functional prognosis. We report the case of a 70-year-old man with known dementia who sustained a high-energy road traffic accident resulting in bilateral knee dislocation. On admission, he was hemodynamically stable with preserved and symmetrical distal peripheral pulses. Initial radiographs confirmed bilateral dislocation, and computed tomography angiography of the lower limbs showed no arterial injury, particularly of the popliteal arteries. After pre-anesthet

Subacute High-Energy Traumatic Knee Dislocation in an Ambulatory Patient: A Case Report.

CASE: A 62-year-old man presented with a posterolateral knee dislocation sustained in a motorcycle collision initially managed nonsurgically. His treatment was complicated by recurrent dislocation from noncompliance with bracing and weight-bearing restrictions and by comorbidities including peripheral artery disease, type 2 diabetes, and heavy smoking and drinking. He was treated with a multiplanar knee-spanning external fixator and patellar tendon repair without further ligamentous reconstruction. This treatment provided a stable, functional knee with minimal pain. CONCLUSIONS: In this compl

Spinout/Dislocation in Mobile-Bearing Total Knee Arthroplasty: A Report of 26 Cases.

BACKGROUND: Excellent medium-term to long-term results for function and survivorship have been shown with mobile-bearing (MB) total knee arthroplasty (TKA). One of the key arguments against its use is the risk of "spinout" or dislocation of the MB. The aim of this study is to discuss the etiology, prevention, incidence, management, and outcome of spinout. METHODS: Between October 1993 and February 2016, 8373 consecutive primary MB TKAs were performed irrespective of preoperative deformity. Before 2001, soft-tissue knee balancing was achieved by release of collateral ligaments and all spinouts
