Knee joint biomechanics in open-kinetic-chain flexion exercises.

BACKGROUND: Different rehabilitation exercises such as open-kinetic-chain flexion and extension exercises are currently employed in non-operative and post-operative managements of joint disorders. The challenge is to strengthen the muscles and to restore the near-normal function of the joint while protecting its components (e.g., the reconstructed ligament) from excessive stresses. METHODS: Using a validated 3D nonlinear finite element model, the detailed biomechanics of the entire joint in open-kinetic-chain flexion exercises are investigated at 0 degrees, 30 degrees, 60 degrees and 90 degre

Osteoarthritic femoral articular cartilage of knee joint in man.

Osteoarthritis is the most common of the various articular disorders affecting man. The present study was conducted to observe the microstructure of osteoarthritic human femoral articular cartilage by light microscopy. Fifteen osteoarthritic cartilage specimens obtained from patients with primary osteoarthritis (52-80 years) undergoing total knee replacement were processed for paraffin sections. 5 m thick sections were observed under light microscope. The articular surface appeared to be very irregular and fibrillated in all the specimens. The cartilage did not show the normal zonation. Variou

Knee joint laxity in a native Canadian Indian population.

BACKGROUND: Clinical observation of increased laxity has been noted in native Canadians. Comparative studies support the possible relationship between joint hypermobility and the development of osteoarthritis or other joint ailments. If joint laxity predisposes to osteoarthritis, there may be far-reaching consequences to the general Native population. METHODS: A cohort of 52 Native Canadians (NC) and 52 non-Native Canadians (NNC) were evaluated for knee laxity. All patients had no prior history of knee injury or complaints of symptoms related to knee pathology at the time of the examination.

Parametric representation and screening of knee joint vibroarthrographic signals.

We have been investigating analysis of knee joint vibration or vibroarthrographic (VAG) signals as a potential tool for noninvasive diagnosis and monitoring of cartilage pathology. In this paper, we present a comprehensive comparative study of different parametric representations of VAG signals. Dominant poles and cepstral coefficients were derived from autoregressive models of adaptively segmented VAG signals. Signal features and a few clinical features were used as feature vectors in pattern classification experiments based on logistic regression analysis and the leave-one-out method. The re

Arthroscopic versus open debridement of penetrating knee joint injuries.

Arthroscopic debridement of penetrating knee joint injuries has become a common treatment method. A comparative study was undertaken to compare this method with open joint debridement. Fourteen penetrating knee joint injuries (fourteen patients) were treated by arthroscopic examination and debridement and were compared to sixteen penetrating knee joint injuries (fifteen patients) treated by open debridement. There were no resultant infections or operative complications in either group. Of note, the arthroscopic debridement group had a shorter postoperative hospital stay [mean of 1.6 days compa

Relationship between lower limb dynamics and knee joint pain.

To test the hypothesis that appropriate and timely neuromuscular control of limb motions plays an important role in the preservation of joint health, we kinematically and kinetically examined the behavior of the legs of young adult subjects at heel strike during natural walking. We compared a group of 18 volunteers, who, we presumed, were preosteoarthrotic because of mild, intermittent, activity-related knee joint pain, with 14 age-matched asymptomatic normal subjects. The two groups of subjects exhibited similar gait patterns with equivalent cadences, walking speeds, terminal stance phase kne

[The surgical treatment of knee joint extensor muscle contractures].

It is reported about the results after operative mobilization of knee joint in 19 patients with contractures of extension muscles of knee joints. After operative arthrolysis and/or Payr-Plasty the range of motion has increased on average from 23 degrees to 92 degrees (a relative gain of 52%). Patients with traumatic knee contracture of extension muscle achieved an improvement in the range of movement on average by 76 degrees, whereas those with contracture due to infection improved on average by only 43 degrees. The etiology, the duration, and the preoperative loss of movement are important fo

[Vascular and capsule-band reconstruction following knee joint dislocation].

This is a report about bilateral dislocations of the knee joints in one patient with arterial and venous stop because of endothelial lesions and thrombosis of the popliteal vessels on both sides. After reconstruction of circulation by means of vena saphena, the disrupted knee ligaments were also sutured. Large ventral exposure of the knee joint is recommended for both reconstruction of vessels and ligaments.

Degenerative joint disease in the mouse knee; histological observations.

The knee joints from males of two strains (CBA/ORT and STR/ORT) were studied histologically. The incidence of degenerative joint disease was very high in the STR/ORT strain. Degeneration of the cartilage invariably occurred first at the interface of the cruciate ligament and articular cartilage of the tibia. Lesions were only seen on the medial tibial and later the medial femoral condyles. Blocks of fibrillated, uncalcified cartilage were gradually lost across the condyle, leaving the tidemark as a secondary articulating surface. Meanwhile the subchondral bone thickened and erosion continued t

Total joint replacement in extremities with below-knee amputations.

Patients with knee and hip joint diseases or fractures in a limb with a distal portion amputated were successfully treated with total joint arthroplasties. Two patients with below-knee amputations recieved total joint replacements. One underwent a total hip arthroplasty and the other had a total knee arthryplasty on the side of the amputation. Physical therapy was modified minimally for the two patients in comparison to other patients with total joint replacements. Prosthetic changes were required in both cases. With new prostheses, both patients were able to return to work.

Knee joint infections with Staphylococcus aureus and Micrococcus species.

In rabbit knees 600 Staphylococcus aureus organisms produced progressive infection in 50 per cent of the knees injected, whereas 5.3 times 10-7 Micrococcus species caused only transient colonization of the joints. Methicillin, cephacetrile, and clindamycin administered before injection of the staphylococci gave protection. Stainless-steel particles in the knee did not increase susceptibility to infection from injected micrococci but did make established micrococcal infections more persistent. Knees containing steel debris failed to become infected during prolonged staphylococcal bacteremia. In

Effects of gait retraining in knee joint position sense.

BACKGROUND: Joint position sense (JPS) is crucial for maintaining posture, protecting joints, and carrying out daily activities such as walking. Studies show that exercises to strengthen muscles and improve proprioception can positively impact JPS during passive and less complex activities. Evidence suggests that motor training can effectively enhance sensory function, including JPS, due to the extensive connections between the motor cortex and somatosensory areas. Gait retraining using real-time feedback has improved outcomes among patients with musculoskeletal disorders. The effect of gait r

Xanthoma combining osteonecrosis in knee joint: a case report.

Xanthoma typically occurs in the subcutaneous tissues, with rare cases of xanthoma in the joints. However, the case of knee joint osteonecrosis combined with xanthoma is even more uncommon. In this article, we described a 50-year-old female patient who suffered xanthoma in the knee joint on the basis of osteonecrosis of the knee joint. The primary clinical symptoms were knee joint pain and limited mobility. The patient initially received conventional treatment for osteonecrosis. However, there was no significant improvement. Later, we found a synovial xanthoma in the patient's knee. Finally, s

Triple dislocation around the knee joint: a case report.

BACKGROUND: Dislocation of the knee is a serious and potentially limb-threatening injury. There are three types of dislocation around the knee joint: patellofemoral, tibiofemoral, and tibiofibular. Tibiofemoral dislocation is the variant that is deemed the most serious, with a higher risk of compromise to the popliteal artery and common peroneal nerve. Although simultaneous dislocations of two types have been described, there has been no such description of all three types occurring simultaneously. CASE PRESENTATION: We present a case of a 40-year-old hairdresser who suffered a fall off her m

Mycobacterial periprosthetic joint infection after primary total knee arthroplasty.

PURPOSE: Mycobacterial periprosthetic joint infection (PJI) is very rare and is generally associated with an immunosuppressive environment. Few large-scale studies of this unusual PJI have been conducted. The current study was performed to assess the clinical features and outcomes following two-stage re-implantation for mycobacterial PJI after primary total knee arthroplasty (TKA). METHODS: We conducted a retrospective review of data collected from our database involving ten cases of two-stage re-implantation manifesting mycobacterial PJI. Patients were followed for at least five years or unt

Diagnosis of knee prosthetic joint infection; aspiration and biopsy.

BACKGROUND: Prosthetic joint infection (PJI) is a significant cause of morbidity and mortality following knee replacement surgery. The diagnosis can be challenging and is based on a combination of clinical suspicion, radiographic findings and also biochemical/ microbiological investigations. Our Aim was to review the role of aspiration and biopsy in the diagnosis of PJI in Total Knee Arthroplasty (TKA). METHOD/RESULTS: Aspirated synovial fluid should be analysed by direct culture, via blood culture bottles, EDTA bottles for cell count and 'point of care' testing such as leucocyte esterase or

Blue rubber bleb nevus syndrome with knee joint disorder.

Blue rubber bleb nevus syndrome (BRBNS) involves cutaneous vascular malformation characterized by multiple venous malformations. This commonly affects the skin and gastrointestinal tract. BRBNS is associated with anemia and occasionally involves orthopedic manifestations. A 6-year-old boy was referred to hospital for evaluation of anemia. He presented with a rubber-like soft-tissue mass in the left knee and the right side of the neck, recurrent pain, and fixed flexion contracture of the knee. Blood examination indicated consumption coagulopathy and anemia caused by not only iron-deficiency ane

Trochlear groove osteochondritis dissecans of the knee patellofemoral joint.

BACKGROUND: The trochlear groove is the rarest location for osteochondritis dissecans (OCD) of the knee, with only about 50 previously reported cases, most of which were treated before the advent of magnetic resonance imaging (MRI) and modern techniques of cartilage fixation or osteochondral transplantation. The purpose of this multicenter study was to assess the patient presentation and clinical, radiographic, and functional results of treatment for trochlear groove OCD lesions. METHODS: Hospital records from 5 institutions of the Research in Osteochondritis of the Knee (ROCK) study group we
