The relationship between bone mineral density and immobilization duration in hemiplegic limbs.

OBJECTIVE: Prolonged immobilization in stroke is known to result in hypercalciuria, hypercalcemia, accelerated bone resorption, and osteoporosis. Furthermore, bone mineral loss accelerated with increasing duration of hemiplegia. Although stroke is a common disease that causes sudden immobilization, relatively few investigations of bone metabolism in stroke have been reported. The aim of this study was to investigate the changes in bone mineral density of the forearms and legs related to duration of hemiplegia-induced immobilization after stroke. METHODS: Forty-one hemiplegic patients with str

A light and electron microscopic study of the limb long bones perichondral ossification in the quail embryo (Coturnix coturnix japonica).

The perichondral ossification of the limb long bones in the quail embryo is investigated, in this study, by means of light and electron microscopy. Longitudinal sections of the humerus, radius, ulna, femur, tibia and fibula stained with haematoxylin-eosin were examined by the light microscope. Ultrathin cross sections were selected for the electron microscope as well. Light microscopic analysis showed that the ossification began at the same time in the long bones of the wing and leg. At the embryonic day 6, all the cartilaginous rudiments consisted of three zones. The central zone composed of

The effect of hemiplegia on bone mass and soft tissue body composition.

The content of bone mineral (BMC), lean tissue, and fat tissue were measured by single and dual photon absorptiometry in both the paretic and the non-paretic limbs of 15 patients, hemiplegic due to cerebrovascular accident 23-38 weeks earlier. Compared with the non-paretic arm, the paretic arm had approximately 10% lower (P less than 0.01) BMC. This difference was largest at the measuring site with the highest ratio of trabecular to compact bone. The paretic leg had a 4% (P less than 0.001) lower BMC than the non-paretic leg. For both the arms and the legs, the lean content was lower (P less t

The Influence of High-Impact Exercise on Cortical and Trabecular Bone Mineral Content and 3D Distribution Across the Proximal Femur in Older Men: A Randomized Controlled Unilateral Intervention.

Regular exercisers have lower fracture risk, despite modest effects of exercise on bone mineral content (BMC). Exercise may produce localized cortical and trabecular bone changes that affect bone strength independently of BMC. We previously demonstrated that brief, daily unilateral hopping exercises increased femoral neck BMC in the exercise leg versus the control leg of older men. This study evaluated the effects of these exercises on cortical and trabecular bone and its 3D distribution across the proximal femur, using clinical CT. Fifty healthy men had pelvic CT scans before and after the ex

[Sacral stress fracture in a recreational long-distance runner].

The current boom in recreational sports leads to an increase in the number of long-distance runners. In addition to typical disease patterns (e.g. of the Achilles tendon) stress fractures are seen more and more frequently. These mostly occur in the lower leg and foot bones and are rarely seen in the pelvis (e.g. sacrum). Atypical and protracted complaints therefore require a detailed medical history of the patient and should if in any doubt lead to radiological imaging. Once correctly diagnosed, rest from sports and sometimes semi-weightbearing or non-weightbearing is indicated to prevent the

Computed tomography of coxofemoral injury in five mute swans (Cygnus olor).

Five mute swans (Cygnus olor) were presented with inability to stand or with abnormal positioning of a leg. Clinical examinations indicated the possibility of femoral fractures or coxofemoral luxations. The suspected diagnosis was proven by means of computed tomography (CT), while superimposition of gastrointestinal contents or other artefacts limited radiographic diagnosis in three birds. A typical CT sign for lesions of the coxofemoral joint apart from femoral displacement was haemorrhage within the pelvic bones (especially around the acetabulum), found in four of the five birds. Small femor

Hypochondroplastic dwarfism in the Irish setter.

Two test matings in the irish setter were performed, and genetic, clinical, morphometric, radiographic, bone histological and plasma and urine biochemical features of dwarfism were studied. All offspring were phenotypically normal at birth and weaning, but at the age of 2.5 to 4 months the longitudinal growth of the spine and leg bones was retarded in the dwarfs compared with the normal littermates. Most dwarfs performed well, even in the field. Radiographic and histological evaluations revealed a hypochondroplasia. A morphometric diagnostic method for Irish setter dwarfism was developed. A si

Indications for the application of Wagner's method of external fixation across the knee joint.

External fixation with Wagner's apparatus can, because of its mechanical properties, be employed for temporary immobilization of the knee joint. This method is especially suitable for patients for whom immobilization in plaster is problematic or disadvantageous. It can be applied to the following categories of patients: Patients with injuries to the bones or ligaments of the knee which are associated with wounds or injuries of the blood vessels or nerves; Patients with multiple injuries of the leg (e.g., fractures of the tibia and femur) and concomitant intra-articular knee injuries; Patients

Nanohydroxyapatite Based Ceramic Carrier Promotes Bone Formation in a Femoral Neck Canal Defect in Osteoporotic Rats.

Hip fractures are among the most common types of fracture risks in old age osteoporotic patients that often end up with immobile disabilities. Weak bones due to loss of mineral content along with an increase in the porosity of the femur neck canal in osteoporosis reduce the mechanical properties of the bone and predispose the patients to fractures. In this study, we have used calcium sulfate/nanohydroxyapatite based nanocement (NC) as carrier of recombinant human bone morphogenetic protein-2 (BMP-2), zoledronate (ZA), and bone marrow mesenchymal stromal cells (MSCs) derived exosomes (EXO) to e

Repair of severe composite tissue defects in the lower leg using two different cross-leg free composite tissue flaps.

This article reports 2 cross-leg free composite tissue flaps for repairing the severe composite tissue defects in lower leg without suitable adjacent recipient vasculature for microvascular anastomosis. The osseous myocutaneous flap of ilium and tensor fascia lata pedicled with ascending branch of lateral femoral circumflex vessels and the osseous muscle flap of scapula and latissimus dorsi pedicled with subscapular vessels were performed, respectively, to reconstruct the bone and soft-tissue defects in the lower leg of 2 patients. Both donor vessels were the posterior tibial artery and great

The role of sympathetic control in bone vasculature: insights from spinal cord injury.

Bone vasculature is richly innervated by an extensive network of sympathetic nerves. However, our understanding of bone blood flow regulation and its contribution to human bone health is limited. Here, we further our previous findings by characterizing bone vascular responses in the absence of sympathetic control-studying individuals with spinal cord injury (SCI), a population with known peripheral sympathetic disruption. We assessed tibial vascular responses to isometric handgrip exercise (IHE) in individuals with SCI (n = 12) and controls (n = 12). When sustained to fatigue, IHE increases pe

Toe Grip Strength is Another Indicator of Muscle Strength and Bone Mass in Older Men.

Toe grip strength (TGS) measures foot muscle strength and evaluates stability maintenance, as well as the risk of falls. Handgrip strength (HGS) is considered a simple and effective method for assessing overall strength, and it correlates with bone variables. The aim of this study was: first, to investigate the relationship between TGS and overall strength and bone variables; and second, to compare the relationships between TGS and HGS with overall strength and bone variables. This cross-sectional study included 59 Caucasian men aged 60 to 84 years. Participants underwent muscle strength testi

Development of the mineralisation of individual bones and bone regions in replacement gilts according to dietary calcium and phosphorus.

Skeleton bones, distinguished by trabecular and cortical bone tissue content, exhibit varied growth and composition, in response to modified dietary calcium and phosphorus levels. The study investigated how gilts adapt their individual bone and bone region mineralisation kinetics in response to changing intake of Ca and P. A total of 24 gilts were fed according to a two-phase (Depletion (D) 60-95 and Repletion (R) 95-140 kg BW, respectively). During the D phase, gilts were fed either 60% (D60) or 100% (D100) of the estimated P requirement. Subsequently, during the R phase, half of the gilts fr

Heterogeneous Spatial and Strength Adaptation of the Proximal Femur to Physical Activity: A Within-Subject Controlled Cross-Sectional Study.

Physical activity (PA) enhances proximal femur bone mass, as assessed using projectional imaging techniques. However, these techniques average data over large volumes, obscuring spatially heterogeneous adaptations. The current study used quantitative computed tomography, statistical parameter mapping, and subject-specific finite element (FE) modeling to explore spatial adaptation of the proximal femur to PA. In particular, we were interested in adaptation occurring at the superior femoral neck and improving strength under loading from a fall onto the greater trochanter. High/long jump athletes

The influence of genetic increases in shank width on body weight, walking ability, and reproduction of turkeys.

Body weight and breast width have been greatly increased in the modern turkey. However, the relative amounts of leg muscles and leg bones have declined. A similar decline also occurs with age. It was theorized that this is an inherent weakness that magnifies other causes of leg problems. In an attempt to increase the relative amount of leg bone, a subline (FL) was developed by mass selection for increased shank width at 16 weeks of age from a long-term growth-selected line of turkeys (F). Shank width of FL was increased greatly by selection. The realized heritability of shank width, based on t

My child's leg is in a plaster. Can they fly?

Air travel in a cast with a fracture is associated with an increased risk of leg swelling in a tight compartment. We have hypothesised that there should be an accessible, universal policy for travel recommendations with the cast supported by high-quality literature. We have checked the 20 biggest airlines looking for their guidance on travel recommendation /restrictions in the presence of a broken leg in a cast, particularly in children. We have also carried out a literature review in the last 30 years to ascertain whether there is the best evidence relating to flying safely with a child in a

Knee, lower leg, foot, and ankle disorders in children.

The literature of the past year increased our understanding of the etiology of some common lower leg disorders in children, and increased our knowledge about the results and complications of some old and new treatment methods. Histochemical and vascular studies and magnetic resonance imaging were used to study the etiology and treatment of congenital clubfoot. Results of early intervention protocols and the ilizarov method were reported. Undercorrection at primary surgery was identified as a major reason for reoperation. Excision of the coalition with interposition of fat or tendon produced go
