Reviewing the therapeutic management of leprosy in primary care: demand case series referred to a University Hospital in the Midwest region of Brazil.

BACKGROUND: Leprosy still represents a negleted public health problem in Brazil. Early and adequate treatment of leprosy, carried out in a primary health network is essential to reduce morbidity and sequelae. OBJECTIVE: To analyze the therapeutic management of leprosy patients referred from primary healthy services to a specialized service. METHODS: An analytical retrospective study using medical records and the Notifiable Diseases Information System. Patients diagnosed with leprosy, referred to a specialized outpatient clinic, between 2016 and 2017, in Goiás state, were included. The treatm

The curious constraints on clofazimine.

Clofazimine is a WHO-recommended therapy for multibacillary leprosy and an emerging option for drug-resistant tuberculosis and non-tuberculous mycobacterial infections. Yet in high-income countries like the USA, access remains restricted through expanded access pathways and other limitations, despite decades of safe use. This review examines clofazimine's regulatory trajectory in the USA, contrasts global access models, and highlights the consequences of relying on a single manufacturer and informal donation programmes. We argue that existing systems can be ill-suited to ensure equitable, sust

[Leprosy: control of household contacts in the municipality of Londrina-PR for a ten-year period].

The purpose of this descriptive study is to analyze variables related to leprosy patients' household contacts who received treatment in Londrina-PR-Brazil for a ten-year period. The data analysis was based on the health service's records and from a system of infectious disease. Out of 1055 leprosy's patients, it was recorded 3394 contacts with an average of 3,2. The most exposed individuals were those aged up to 40 (71,5%); son/daughter (40.6%) and husband/wife (17.8%). Of the1731 contacts (51.0%) examined, 183 showed some signs of the disease: there were 16 confirmed cases, 47 were excluded a

Reproductive health issues in Pakistan; do myths take precedence over medical evidence?

OBJECTIVES: In the Philippines, there has been a lack of information on the concordance between classifications of Hansen's disease or leprosy clinically, histopathologically, and with AFS results. The study ultimately aimed to determine the concordance between the clinical diagnosis, histopathological results, and AFS results of patients with leprosy seen at the Dr. Jose N. Rodriguez Memorial Hospital and Sanitarium (DJNRMHS). METHODS: This is a descriptive, retrospective, single-center study conducted at the DJNRMHS, a tertiary government hospital and one of the last remaining sanitaria in

Persistent serpentine supravenous hyperpigmented eruption in lepromatous leprosy after minocycline.

Persistent serpentine supravenous hyperpigmented eruption (PSSHE) is a peculiar patterned eruption characterised by hyperpigmented streaks following the superficial venous network on the skin. Unlike the superficial thrombophlebitis, it is characterised by underlying vessels that are patent. It has been described most commonly after injection of chemotherapeutic drugs. We describe a 40 year old man with lepromatous leprosy who developed PSSHE subsequent to starting modified multidrug therapy--multibacillary regimen in the form of minocycline and ofloxacin.

A Multiepitope Protein Based on Selected Synthetic Peptides Enables Effective Serological Screening of Exposure to Mycobacterium leprae.

Early and accurate identification of individuals exposed to leprosy is essential for controlling the transmission. Our study assessed the potential of a novel recombinant multiepitope protein, designed using five synthetic Mycobacterium leprae-specific peptides that were selected based on their prior performance in ELISA assays. The protein was expressed in a bacterial system and further evaluated in ELISA assays for IgG and IgM. The reactivity was assessed in 357 serum samples from leprosy patients, 234 household contacts (HHC), 26 tuberculosis patients, and 61 healthy controls from different

Coinfection of leprosy and tuberculosis.

Leprosy and tuberculosis (TB) are endemic to India, however, their coinfection is not frequently encountered in clinical practice. Here, we report a 32-year-old female patient who presented with a history of high-grade intermittent fever, cough and painless skin lesions since a month, along with bilateral claw hand (on examination). The haematological profile was suggestive of anaemia of chronic disease, chest radiograph showed consolidation, sputum smears were positive for Mycobacterium tuberculosis, and skin slit smear confirmed leprosy. The patient was prescribed WHO recommended multidrug t

Post-kala-azar dermal leishmaniasis mimicking leprosy relapse: a diagnostic dilemma.

OBJECTIVES: Post-kala-azar dermal leishmaniasis (PKDL) is well recognized in the Indian subcontinent and is not infrequently confused with leprosy. The present report describes findings in an unusual case of PKDL. METHODS: We report an adult male who presented with firm nodules on the face and extremities, along with hypopigmented macules on the neck with deformity and hypoesthesia over the right upper extremity. The patient had experienced similar lesions seven years previously and had used multibacillary multi-drug therapy for leprosy with complete resolution of the nodules. RESULTS: At th

Powerful bactericidal activities of clarithromycin and minocycline against Mycobacterium leprae in lepromatous leprosy.

Thirty-six patients with newly diagnosed lepromatous leprosy were allocated randomly to three groups and treated for 56 days with minocycline (100 mg daily), clarithromycin (500 mg daily), or clarithromycin (500 mg) plus minocycline (100 mg daily). All groups had rapid and remarkable clinical improvement and significant decline of the bacterial and morphologic indices in skin smears during treatment. More than 99% and > 99.9% of the viable Mycobacterium leprae had been killed by 28 and 56 days of treatment, respectively, as measured by inoculation of organisms recovered from skin samples, take

A Rare Combination of Pure Neuritic Leprosy with Morphea Leading to Diagnostic Confusion.

A circumscribed sclerotic plaque of morphea can sometimes be mistaken for tuberculoid leprosy and vice versa can also happen. However, the co-existence of a patch of morphea mimicking as Leprosy patch in an underlying case of neuriticleprosy, can be very misleading. We present a case with glove and stocking anaesthesia and peripheral nerve enlargement with a single large hypopigmented, non-anaesthetic macule on trunk, clinically diagnosed as Hansen's disease (Borderline Tuberculoid - BT). Slit skin smears proved to be negative for AFB and histopathology of the skin lesion was consistent with m

Challenges of the public health response to a rare case of non-autochthonous Mycobacterium leprae, Ireland, 2024.

This case report details the public health response to a multibacillary leprosy case in Ireland. The case presented with hypopigmented skin lesions and neurological symptoms. Challenges included delayed recognition in the clinical setting, contact tracing within a congregate setting and lack of specific Irish guidelines. Comprehensive contact tracing, chemoprophylaxis and follow-up care were implemented, guided by international protocols. This case underscores the need for tailored guidelines and stigma mitigation strategies for this neglected tropical disease in non-endemic regions.

Immunogold labeling method for Mycobacterium leprae-specific phenolic glycolipid in glutaraldehyde-osmium-fixed and Araldite-embedded leprosy lesions.

Phenolic glycolipid (PGL)-I, a Mycobacterium leprae-specific antigen currently used for serodiagnosis of preclinical leprosy, has thus far not been localized subcellularly in leprosy bacilli and their host cells. In this study, we developed an immunogold-labeling technique for qualitative identification of PGL-I sites in glutaraldehyde-osmium-fixed and Araldite-embedded M. leprae and host macrophages in human skin biopsies. Such "hard-fixed," plastic-embedded skin and nerve biopsies from patients with varying cell-mediated immunity to leprosy are amply available worldwide. Our method involves

Autoantibodies and vitamin D in leprosy patients in the Brazilian Amazon.

INTRODUCTION: Leprosy can vary in clinical forms, resulting in dermatoneurological disease with physical disabilities and leprosy reactions. This reaction involves complex immune system activity that can be influenced by vitamin D activity. The objective of this study was to evaluate the presence of autoantibodies in leprosy patients in Marabá, associating it with sociodemographic aspects and vitamin D levels. MATERIALS AND METHODS: Cross-sectional study with leprosy patients treated at a Family Health Unit. Surveys of autoantibodies, vitamin D, and sociodemographic aspects were conducted. RES

Clinical manifestations, diagnosis and classification of leprosy.

Mycobacterium leprae, the causative organism of leprosy is slow-growing and the reason is its long incubation period of 2-4 years. Males are predominantly affected and deformity is produced in less than 2% of people affected with the disease. The disease manifests in the skin as macules, papules, nodules, plaques or infiltration. Hypopigmented or erythematous skin patches with definite sensory deficit is one of the clinical cardinal signs by which one can make a definite diagnosis. Demonstration of bacilli in the slit skin smear is the bacteriological cardinal sign used to make definite diagno

BCG-induced immunity profiles in household contacts of leprosy patients differentiate between protection and disease.

Leprosy is an infectious disease caused by Mycobacterium leprae leading to irreversible disabilities along with social exclusion. Leprosy is a spectral disease for which the clinical outcome after M. leprae infection is determined by host factors. The spectrum spans from anti-inflammatory T helper-2 (Th2) immunity concomitant with large numbers of bacteria as well as antibodies against M. leprae antigens in multibacillary (MB) leprosy, to paucibacillary (PB) leprosy characterised by strong pro-inflammatory, Th1 as well as Th17 immunity. Despite decades of availability of adequate antibiotic tr

Leprosy with dystrophy in twenty nails: Detection of acid-fast bacilli in nail biopsy.

Leprosy is a chronic granulomatous infectious disease caused by Mycobacterium leprae (M. leprae) and/or M. lepromatosis that has a wide spectrum of clinical presentations and multi-organ involvement, primarily on the skin and peripheral nerves. Recent studies have addressed the possibility of nail involvement in leprosy, but to this day limited studies have provided histological evidence confirming the presence of acid-fast bacilli (AFB) in the nail apparatus. A case of lepromatous leprosy-related dystrophy in 20 nails in a 37-year-old male was reported. Physical examination revealed, on all 2

A man with erythematous nodules: what is the diagnosis? Lepromatous leprosy.

A 65-year-old man presented with an approximately 5-year history of cutaneous nodules of the arms and legs. In addition, physical examination revealed bilateral thickening of ulnar and tibial nerves, distal weakness with hallux extension and finger abduction, and distal hypoesthesia of the left hallux. Histopathological analysis of the skin biopsy specimen showed a dense inflammatory infiltrate in the hypodermis, characterized by vacuolated macrophages containing multiple organisms. The Fite stain was positive confirming the diagnosis of multibacillary leprosy.

Leprous neuropathy.

Leprous neuropathy, which is due to infection of nerve cells by Mycobacterium leprae, still affects millions of people in many developing countries. The clinical and pathological manifestations are determined by the natural resistance of the host to invasion of M. Leprae. Failure of early detection of leprosy often leads to severe disability in spite of eradication of mycobacterium at a later date. In the lepromatous type, bacilli are easily found in the skin and in nerve cells including Schwann cells, endothelial cells, and macrophages. In the tuberculoid type, a strong cell-mediated immune r

Ancient DNA confirmation of lepromatous leprosy in a skeleton with concurrent osteosarcoma, excavated from the leprosarium of St. Mary Magdalen in Winchester, Hants., UK.

To establish a biological profile and disease aetiologies for one of four burials recovered during a Time Team dig at the St. Mary Magdalen leprosarium, Winchester, UK in AD 2000. Osteological techniques were applied to estimate age at death, biological sex, stature and pathology. Visual assessment of the material was supplemented by radiographic examination. Evidence for leprosy DNA was sought using ancient DNA (aDNA) analysis. The remains are those of a male individual excavated from a west-east aligned grave. The skeleton shows signs of two pathologies. Remodelling of the rhino-maxillary ar

A prospective study to validate various clinical criteria used in classification of leprosy: a study from a tertiary care center in India.

BACKGROUND: Various clinical criteria are used to categorize leprosy patients into paucibacillary (PB) and multibacillary (MB), thus aiding in appropriate treatment. However, comprehensive studies validating these criteria are minimal. AIMS: To assess sensitivity and specificity of different clinical criteria individually and in combination for classifying leprosy into PB/MB spectrum. METHOD: A prospective study was conducted wherein 50 newly diagnosed, untreated leprosy cases were recruited and classified into PB and MB using the following clinical criteria: number of skin lesions (NSL), nu
