A network approach to addressing the needs of patients with incurable head and neck cancer and their families.

BACKGROUND: Patients with incurable head and neck cancer have considerable unmet needs and complex symptom burden, with evidence of substantial geographical and/or socioeconomic inequalities. Accurate information on healthcare needs, resource utilisation and service provision in the last year of life is lacking. This places limits on service delivery planning and the development and testing of interventions to better meet needs. Our partnership spans three regions, which nationally have some of the highest rates of incurable head and neck cancer. AIMS: The overall aims were to (1) establish a

Assessing the clinical outcome of Vim radiosurgery with voxel-based morphometry: visual areas are linked with tremor arrest!

INTRODUCTION: Radiosurgery (RS) is an alternative to open standard stereotactic procedures (deep-brain stimulation or radiofrequency thalamotomy) for drug-resistant essential tremor (ET), aiming at the same target (ventro-intermediate nucleus, Vim). We investigated the Vim RS outcome using voxel-based morphometry by evaluating the interaction between clinical response and time. METHODS: Thirty-eight patients with right-sided ET benefited from left unilateral Vim RS. Targeting was performed using 130 Gy and a single 4-mm collimator. Neurological and neuroimaging assessment was completed at bas

Aging-Related Modular Architectural Reorganization of the Metabolic Brain Network.

Background: Modules in brain network represent groups of brain regions that are collectively involved in one or more cognitive domains. Exploring aging-related reorganization of the brain modular architecture using metabolic brain network could further our understanding about aging-related neuromechanism and neurodegenerations. Materials and Methods: In this study, 432 subjects who performed 18F-fluorodeoxyglucose (FDG) positron emission tomography (PET) were enrolled and divided into young and old adult groups, as well as female and male groups. The modular architecture was detected, and the

Lesion mapping in acute stroke aphasia and its implications for recovery.

Patients with stroke offer a unique window into understanding human brain function. Mapping stroke lesions poses several challenges due to the complexity of the lesion anatomy and the mechanisms causing local and remote disruption on brain networks. In this prospective longitudinal study, we compare standard and advanced approaches to white matter lesion mapping applied to acute stroke patients with aphasia. Eighteen patients with acute left hemisphere stroke were recruited and scanned within two weeks from symptom onset. Aphasia assessment was performed at baseline and six-month follow-up. St

Implementation of Behavioural Supports Ontario (BSO): An Evaluation of Three Models of Care.

Behavioural Supports Ontario (BSO) was launched to enhance the healthcare services for Ontario's seniors, their caregivers and families living and coping with responsive behaviours associated with dementia and other neurological conditions. The implementation of the program varied across and within the local health integration networks (LHINs). By 2015, there were three BSO models operating within the long-term care (LTC) home sector: in-home BSO teams, a mobile team that serves multiple LTC homes within a sub-area of a LHIN and a LHIN-wide mobile team that provides services to all homes. Functional MRI Task Comparison for Language Mapping in Neurosurgical Patients.

BACKGROUND AND PURPOSE: Language task-based functional MRI (fMRI) is increasingly used for presurgical planning in patients with brain lesions. Different paradigms elicit activations of different components of the language network. The aim of this study is to optimize fMRI clinical usage by comparing the effectiveness of three language tasks for language lateralization and localization in a large group of patients with brain lesions. METHODS: We analyzed fMRI data from a sequential retrospective cohort of 51 patients with brain lesions who underwent presurgical fMRI language mapping. We compa

A Comprehensive Hybrid Approach for Indoor Scene Recognition Combining CNNs and Text-Based Features.

Indoor scene recognition is a computer vision task that identifies various indoor environments, such as offices, libraries, kitchens, and restaurants. This research area is particularly significant for applications in robotics, security, and assistance for individuals with disabilities, as it enables the categorization of spaces and the provision of contextual information. Convolutional Neural Networks (CNNs) are commonly employed in this field. While CNNs perform well in outdoor scene recognition by focusing on global features such as mountains and skies, they often struggle with indoor scene

The structural basis for interhemispheric functional connectivity: Evidence from individuals with agenesis of the corpus callosum.

Agenesis of the corpus callosum (AgCC) is a rare congenital malformation characterized by partial or complete absence of the corpus callosum (CC). The effects of AgCC on cerebral structural and functional networks are not clear. We aimed to utilize AgCC as a model to characterize the relationship between brain structure and function. Diffusion tensor imaging and resting-state fMRI data were collected from nine AgCC and ten healthy subjects. The interhemispheric functional connectivity (FC) was quantified using a voxel-mirrored-homotopic-connectivity (VMHC) method, and its correlation with the

Suprapontine Structures Modulate Brainstem and Spinal Networks.

Several spinal motor output and essential rhythmic behaviors are controlled by supraspinal structures, although their contribution to neuronal networks for respiration and locomotion at birth still requires better characterization. As preparations of isolated brainstem and spinal networks only focus on local circuitry, we introduced the in vitro central nervous system (CNS) from neonatal rodents to simultaneously record a stable respiratory rhythm from both cervical and lumbar ventral roots (VRs).Electrical pulses supplied to multiple sites of brainstem evoked distinct VR responses with stagge

Pesticides and their degradates in groundwater reflect past use and current management strategies, Long Island, New York, USA.

Long Island, New York, has a mix of urban/suburban to agricultural/horticultural land use and nearly 3 million residents that rely on a sole-source aquifer for drinking water. The analysis of shallow groundwater (<40 m below land surface) collected from 54 monitoring wells across Long Island detected 53 pesticides or pesticide degradates. Maximum concentrations for individual pesticides or pesticide degradates ranged from 3 to 368,000 ng/L. The highest concentrations and most frequent pesticide detections occurred in samples collected from the pesticide management (PM) network, set in an agric

Discoid lupus erythematosus at a site of previous injury.

A 71-year-old man with three patches of discoid lupus erythematosus (DLE) confined to the right preauricular region drew our attention because of the unusual linear arrangement of the lesions. Twenty-five years previously, the patient had suffered a trauma in the same area from falling off his motorcycle. We believe that, despite the great lapse in time, this injury may have facilitated the onset of DLE in the very same area, through long-term destabilization of the local neuroimmune network. The case fits the recently coined concept of the immunocompromised district, a cutaneous region with a

Visual place recognition with panoramic images using hybrid neural network models.

A mobile robot can localize itself in a mapped area by finding a recorded image of a visited place that is most similar to the current view, a technique known as Visual Place Recognition (VPR). We focus on VPR with panoramic images in indoor environments and on direct VPR methods in contrast to feature-based methods. In this context, a key challenge of VPR are appearance changes in the environment, e.g. due to variations in illumination, camera tilt, and rearrangement of objects. To improve the quality in these situations, we propose a novel combination of convolutional neural networks (CNNs)

Cortical Network Reorganization During Balance Control in Chronic Stroke: Premotor-Parietal Behavioral Uncoupling and Directional Prefrontal Compensation.

BACKGROUND: Balance control is essential for independence in activities of daily living, and its impairment is a leading contributor to falls in stroke survivors. Stroke-induced brain reorganization alters cortical activation patterns; however, localized changes do not reveal the underlying network dynamics critical for the exchange of task-related information. How stroke affects balance-related cortical network dynamics remains unclear. METHODS: We studied cortical functional connectivity during a challenging balance task in fifteen chronic stroke survivors and fifteen age- and gender-matche

Functional structure of local connections and differentiation of cerebral cortex areas in the neonate.

Neuroimaging research on functional connectivity can provide valuable information on the developmental differentiation of the infant cerebral cortex into its functional areas. We examined healthy neonates to comprehensively map brain functional connectivity using a combination of local measures that uniquely capture the rich spatial structure of cerebral cortex functional connections. Optimal functional MRI scans were obtained in 61 neonates. Local functional connectivity maps were based on Iso-Distance Average Correlation (IDAC) measures. Single distance maps and maps combining three distinct

Alterations in resting-state neural networks in subacute non-fluent aphasia after stroke: a fNIRS study.

BACKGROUND: Post-stroke aphasia is a prevalent and often disabling language impairment. Despite its high incidence, the neuropathological mechanisms underlying post-stroke aphasia and the neural substrates of functional recovery remain poorly understood. Understanding these mechanisms is essential for developing targeted rehabilitation strategies. METHODS: In this study, we used fNIRS to collect resting-state data from three participant groups. Pearson correlation was applied to compute whole-brain functional connectivity, followed by graph theory analysis to compare global and regional netwo

Malaria surveillance reveals parasite relatedness, signatures of selection, and correlates of transmission across Senegal.

Parasite genetic surveillance has the potential to play an important role in malaria control. We describe here an analysis of data from the first year of an ongoing, nationwide program of genetic surveillance of Plasmodium falciparum parasites in Senegal, intended to provide actionable information for malaria control efforts. Looking for a good proxy for local malaria incidence, we found that the best predictor was the proportion of polygenomic infections (those with multiple genetically distinct parasites), although that relationship broke down in very low incidence settings (r = 0.77 overall

DFRSeisNet: Fluctuation-Prior-Regularized Background Noise Attenuation for Seismic Signal Denoising.

Seismic exploration has progressively expanded into urban fringe regions and areas with intensive human activities, where anthropogenic interference has increased markedly, aggravating the background noise problem and substantially affecting the accuracy of subsequent seismic data processing. Conventional denoising methods struggle to cope with such complex noise patterns and typically rely on manually designed parameter settings, which limits their ability to suppress complex background noise in real seismic data. With the rapid development of deep learning, various neural-network-based metho

The posterior insular-opercular cortex: An access to the brain networks of thermosensory and nociceptive processes?

In spite of systematic investigations, the existence of a specific cortex that could encode for the intensities of somatosensory stimuli, including within nociceptive ranges, is still a matter of debate. The present consensus is that pain is expressed in a distributed network made of thalamus, SII, insula, ACC, and, less consistently, SI. Here we argument that there must be an entrance to this network. The common denominator to every functional imaging study is that the subjects can distinguish between noxious and non-noxious stimuli, or between two different intensities of noxious stimuli. Th

PURPOSE: We aimed to investigate alterations in the topological organization of functional brain networks in acute pain. METHODS: A total of 29 capsaicin group (CAP) and 19 sham controls (Sham) underwent a 10-min resting-state functional near-infrared spectroscopy scan. The CAP group applied capsaicin cream (0.1%) to the lower back, whereas the Sham group applied a hand cream without capsaicin ingredients to the same area. All subjects were healthy individuals prior to the experiment and did not report any pain or other medical history. The pain in the CAP was only caused by the topical appli
