Subtle Manifestation of Lower Cavernous Sinus Syndrome as the Initial and Sole Presentation of Pterygopalatine Squamous Cell Carcinoma Invasion from the Skull Base: A Case Report.

We present a case highlighting the critical role of early recognition of partial cavernous sinus syndrome (CSS), as diagnostic delay can lead to irreversible tumor progression. A 50-year-old woman presented with subtle binocular diplopia and facial tingling sensation for 1 month. Neurological examination revealed isolated right abducens nerve (VI) palsy and paresthesia at territories of right ophthalmic (V1) and maxillary (V2) nerves. Partial CSS was suspected. Brain magnetic resonance image (MRI) revealed a contrast-enhancing mass involving the pterygopalatine fossa, sphenoid, and maxillary s

Effect of rapid maxillary expansion on intracranial pressure.

OBJECTIVE: The aim of this study was to evaluate the effects of rapid maxillary expansion on the optic nerve sheath diameter and to examine its possible effects on intracranial pressure. DESIGN: 20 patients with bilateral crossbite were selected. Hyrax Expander was applied and activated twice daily until the overcorrection was achieved. The optic nerve sheath diameter (ONSD) was measured via ultrasonography before the first activation (T0), then repeated after 1 (T1) and 10 min (T2). At the end of the expansion, ONSD was measured (T3) again, then the screw was activated for the last time, and

[Submandibulectomy in the treatment of pathology of the submandibular gland: our experience 1970-1995].

Submandibular gland excision is proposed in the treatment of neoplastic and non neoplastic diseases; this surgical procedure can be performed by transoral or transcervical approach. The aim of the study is to demonstrate that cervical approach must be preferred because it is safer and allows a wider exposition of the surgical field. From 1970 to June 1995, 54 patients (47 with chronic sialadenitis, 7 with benign tumors and 7 with malignant tumors) were submitted to excision of the submaxillary gland. Of the 54 resections performed, 2 were completed with "functional" cervical lymphadenectomy an

Early hemi-mandibular lengthening by distraction osteogenesis contributes to compensatory maxillary growth.

Mandibular distraction osteogenesis at an early age is the standard hemifacial microsomia treatment. Nevertheless, the recurrence rate remains high and the definition of early age is controversial. We explored the optimal timing for mandibular distraction, when the surrounding skeleton, such as maxilla, can grow compensatory, to reduce recurrence. Hemifacial microsomia patients were prospectively divided into Groups A (1-3 years old) and B (4-6 years old), according to maxillary and mandibular growth curves. Computed tomography scans were obtained before distractor implantation and after remov

Tic douloureux.

Tic douloureux (trigerminal neuralgia) usually has its onset in the sixth or seventh decade and is characterized by severe, excruciating facial pain which is almost always unilateral. The mandibular division and the maxillary division of the fifth cranial nerve are most commonly affected. Carbamazepine is the drug of choice. Currently popular surgical procedures include decompression of the fifth cranial nerve and percutaneous thermal lesioning of the gasserian ganglion.

Orbital restoration surgery in the zygomaticotemporal and zygomaticofacial nerves and important anatomic landmarks.

A variety of etiologies may result in functional and aesthetic deficiencies requiring orbital reconstruction. Fractures of the zygomaticomaxillary complex in the acute stage are frequently accompanied by sensory disturbances of the zygomatic nerve (ZN). The purpose of the current study was to describe the anatomic and topographic landmarks of the ZN in 18 adult human cadavers regarding the localization and dimensions in the orbit. The zygomaticotemporal (ZTN) and zygomaticofacial nerves (ZFN) along the lateral wall of the orbit passed through the zygomaticotemporal and zygomaticofacial foramen

The removal of an implant beneath the optic canal by modified endoscopic-assisted sinus surgery.

Displacement of dental implants into the maxillary sinus is one of the rare complications during the implant surgery. However, it is always possible that clinicians confront with this unexpected result and patients should be informed ahead of maxillary implant surgery. For clinicians, it is important to be aware of how to deal with the complication, because the migrated implant must be removed as early as possible. There have been several classical ways to approach the maxillary sinus, such as the Caldwell-Luc procedure and endoscopic sinus surgery and these methods still have been chosen by m

Neurostimulation at pterygopalatine fossa for cluster headaches and cerebrovascular disorders.

There are numerous neural structures (parasympathetic, sympathetic, and trigeminal sensory) that are compacted in a small well defined area of the pterygopalatine fossa (PPF). These targets can be readily accessed via minimally invasive neuromodulation techniques making the methods more desirable than neurosurgical deep brain or hypothalamic intervention. Recent research has shed light over the important role of the sphenopalatine ganglion (SPG), which is located within the PPF, in cerebrovascular autonomic physiology as well as in the pathophysiology of different headache disorders (cluster h

Endoscopic Transnasal Transmaxillary Approach to Orbital Apex through the Meningo-Orbital Band: A Cadaveric Feasibility Study.

BACKGROUND: Extradural transcranial release of the meningo-orbital band occupying the lateral part of the superior orbital fissure is used to approach the orbital apex and middle cranial fossa. The authors tested the feasibility of the release of the meningo-orbital band via an endonasal transmaxillary approach. MATERIALS AND METHODS: Five injected cadaveric heads were assessed for dimensions of superior orbital fissure by computerized tomography. An endonasal transsphenoid transpterygoid approach was done to the superior orbital fissure and annulus of Zinn medially, down to the maxillary ner

[A case report of sphenoid sinus Schwannoma].

Schwannomas are neurogenic tumors that arise from the Schwann cells of the nerve sheath, also called shwann cell tumor. More common in patients with 30 to 40 years old. This tumor always occurs in the septum, maxillary sinus, ethmoid sinus, etc. The Schwannoma is rare in sphenoid sinus with only sporadic cases at home and abroad. Our institute received and cured a sphenoid sinus nerve sheath tumor patient, the preoperative misdiagnosis is the sphenoid sinus cyst and the postoperative pathological diagnosis is sphenoid sinus nerve sheath tumor. Therefore, we introduce a case of the sphenoid sin

Trigeminal herpes zoster and Ramsay Hunt syndrome with a lesion in the spinal trigeminal nucleus and tract.

We report the case of a 77-year-old immuno-competent man who developed herpes zoster in the maxillary and mandibular branches of the trigeminal nerve. Within 3 weeks, he developed ipsilateral peripheral facial palsy, hearing loss, vesicles over the external auditory canal, and pain in the face and ear. A T(2)-weighted MRI of the brain revealed a hyper-intense lesion at the right medulla corresponding to the spinal trigeminal nucleus and tract. Gadolinium enhancement was seen over the right facial nerve. These lesions suggest a possibility of transaxonal spread of the varicella zoster virus bet

Prolongation of action of local anaesthetic by a possible complexation: clinical trials.

Long lasting pain relief for patients of malignancy involving head, neck, face and patients of trigeminal neuralgia was sought by the use of lignocaine with addition of complexing agents. Five patients with severe intractable pain of malignancy involving the trigeminal (V cranial) nerve mainly the maxillary and mandibular divisions, two patients of trigeminal neuralgia with trigger zone in maxillary division were treated. Remarkable prolongation of action of pain relief up to 6 weeks could be achieved. The recurred pain was also of less severity especially radiation was less. An explanation of

Trigeminal sensory neuropathy with abnormal taste following acute sinusitis.

We report a case of isolated trigeminal sensory neuropathy associated with impairment of taste sensation following acute sinusitis. Sensory disturbance was distributed mainly in the ophthalmic division, and partly in the maxillary and mandibular divisions. No other cranial nerves were involved. An otological procedure resulted in complete recovery. The unique combination of trigeminal neuropathy and abnormal taste seemed to be caused by the infectious process involving the gasserian ganglion of the trigeminal nerve.

Temporary Blindness after Inferior Alveolar Nerve Block.

Inferior Alveolar Nerve Block (IANB) anaesthesia is one of the common procedures in dental clinic. This procedure is safe, but complications may still occur. Ocular complications such as diplopia, loss of vision, or ophthalmoplegia are extremely rare. This case report explains an event where due to individual anatomic variation of the sympathetic vasoconstrictor nerve and maxillary and middle meningeal arteries, intravascular administration of anaesthetic agent caused unusual ocular signs and symptoms such as temporary blindness.

Endoscopic Endonasal Resection of Temporal Encephalocele and Repair of Cerebrospinal Fluid Fistula at the Lateral Sphenoid Sinus.

We demonstrate endoscopic endonasal transsphenoidal resection of a temporal encephalocele (TEn) and repair of a cerebrospinal fistula at the lateral sphenoid sinus. A 48-year-old woman presented with sudden onset of massive serous nasal discharge with severe headache. Head magnetic resonance imaging disclosed medial temporal lobe herniated though a bony defect into the sphenoid sinus. We created a three-dimensional computer model that clearly disclosed TEn protruding between the maxillary and Vidian nerves. We discussed the surgical strategy1-4 with this three-dimensional model and determined

Osteopathic manipulative treatment for facial numbness and pain after whiplash injury.

Whiplash injury is often caused by rear-end motor vehicle collisions. Symptoms such as neck pain and stiffness or arm pain or numbness are common with whiplash injury. The author reports a case of right facial numbness and right cheek pain after a whiplash injury. Osteopathic manipulative treatment techniques applied at the level of the cervical spine, suboccipital region, and cranial region alleviated the patient's facial symptoms by treating the right-sided strain of the trigeminal nerve. The strain on the trigeminal nerve likely occurred at the upper cervical spine, at the nerve's cauda, an
