Inside-Out Approach to Meniscus Repair: Still the Gold Standard?

PURPOSE OF REVIEW: The purpose of this review is to provide an up-to-date summary on the current literature and trends regarding use of the inside-out approach to meniscus repair. Additionally, the paper describes the authors preferred techniques for inside-out meniscus repair utilizing posteromedial and posterolateral exposures. RECENT FINDINGS: There has been a substantial increase in recent publications regarding meniscus repair. However, comparisons regarding the optimal repair technique have not been conclusive. Despite the recent increase in use of all-inside devices, multiple investiga

A systematic review about long-term results after meniscus repair.

PURPOSE: Aim of this systematic review was to analyze long-term results after meniscus refixation. METHODS: A systematic literature search was carried out in various databases on studies on long-term results after meniscus refixation with a minimum follow-up of 7 years. Primary outcome criterion was the failure rate. Secondary outcome criteria were radiological signs of osteoarthritis (OA) and clinical scores. RESULTS: A total of 12 retrospective case series (level 4 evidence) were identified that reported about failure rates of more than 7 years follow-up. There was no statistical differenc

Rebar Repair of Radial Meniscus Tears: A Reinforced Suture Technique.

The purpose of this paper is to describe the rebar repair as a technique for repair of radial meniscus tears and compare the rebar technique with current techniques used for meniscus repairs. This technique consists of 4 sutures placed with the inside-out technique. First, the vertical mattress reinforcement sutures are placed anteriorly and posteriorly to the tear. Then, 2 parallel horizontal sutures are placed directly in juxtaposition to the vertical sutures, ensuring the needles pass on the side of the reinforcing stitch away from the tear. This technique is less technically challenging th

Transtibial Pullout Repair Reduces Posterior Extrusion of the Medial Meniscus.

Medial meniscus posterior root tear causes rapid knee cartilage degradation by inducing posteromedial displacement of the medial meniscus. We evaluated medial meniscus posterior extrusion before and after pullout repair for medial meniscus posterior root tear using magnetic resonance images. Twenty-eight patients with symptomatic medial meniscus posterior root tear were included. The inclusion criteria were: acute (< 3 months) or chronic (≥3 months) medial meniscus posterior root tear after painful popping events. The exclusion criteria were: other meniscus and anterior cruciate ligament injur

Compromised autophagy precedes meniscus degeneration and cartilage damage in mice.

OBJECTIVE: Autophagy is a cellular homeostasis mechanism that facilitates normal cell function and survival. Objectives of this study were to determine associations between autophagic responses with meniscus injury, joint aging, and osteoarthritis (OA), and to establish the temporal relationship with structural changes in menisci and cartilage. METHODS: Constitutive activation of autophagy during aging was measured in GFP-LC3 transgenic reporter mice between 6 and 30 months. Meniscus injury was created by surgically destabilizing the medial meniscus (DMM) to induce posttraumatic OA in C57BL/6

How Should We Secure Our Transplanted Meniscus? A Meta-analysis.

BACKGROUND: Meniscal allograft transplant (MAT) is a salvage procedure for young patients with symptomatic meniscal deficiency. Although many studies report good outcomes, MAT remains controversial, and an optimal fixation technique for the meniscus roots is debated. PURPOSE: To assess the overall outcome of MAT and compare the results of different meniscal root fixation techniques. STUDY DESIGN: Meta-analysis. METHODS: Following specific guidelines, we systematically evaluated all current literature for studies reporting on the outcomes of patients who underwent MAT. Using random effect mo

Centralization of extruded medial meniscus delays cartilage degeneration in rats.

BACKGROUND: Meniscus extrusion often observed in knee osteoarthritis has a strong correlation with the progression of cartilage degeneration and symptom in the patients. We recently reported a novel procedure "arthroscopic centralization" in which the capsule was sutured to the edge of the tibial plateau to reduce meniscus extrusion in the human knee. However, there is no animal model to study the efficacy of this procedure. The purposes of this study were [1] to establish a model of centralization for the extruded medial meniscus in a rat model; and [2] to investigate the chondroprotective ef

Arthroscopic Removal of Shotgun Pellet From Within the Medial Meniscus.

Arthroscopic techniques are effective for the removal of intra-articular bullet and metal fragments after gunshot wounds to the shoulder, hip, knee, and sacroiliac joints. Surgical removal of bullets retained within the synovial joints is indicated; lead is dissolved by synovial fluid over time, leading to proliferative synovitis, lead arthropathy, elevated serum lead levels, and lead toxicity. We present an arthroscopic technique for removal of a shotgun pellet retained within the medial meniscus. In this technique, diagnostic knee arthroscopy is initially performed, which allows for localiza

Surgical treatment of temporomandibular joint ankyloses: meniscus conservation and relocation.

Ankylosis of the temporomandibular joint is a serious complication, mainly after trauma and local or systemic infection. In rare cases, ankylosis is associated with systemic disease such as ankylosing spondylitis, rheumatoid arthritis, and psoriasis. According to the functional restriction and the provoked disturbances of facial growth in the youth, an early and effective therapy is desirable. There is a wide variety of surgical approaches to temporomandibular joint ankylosis, ranging from chondro-osseous grafts to prothesis. In the article the authors present the clinical case of a 60-year-ol

Irreducible open posterolateral knee dislocation due to medial meniscus interposition.

Posterolateral knee dislocations are generally irreducible due to the interposition of the medial capsule and retinaculum. These injuries have a 'dimple sign' which shows the invagination of the tissues in the medial joint line. We present an unusual case of an open posterolateral traumatic knee dislocation (KD-4 [ACL/PCL/MCL/LCL-PLC torn] open knee dislocation) without a 'dimple sign'. Closed reduction attempts were unsuccessful. In surgery, it was found that the medial meniscus was detached from the meniscocapsular junction and entrapped in the joint. The medial meniscus was extracted from t

Medial discoid meniscus completely coalesced with the anterior cruciate ligament.

Medial discoid meniscus is an uncommon abnormality. To date, only a few reports have described the associated anomaly with discoid meniscus. However, there are no reports on medial discoid meniscus accompanied by an anomaly of the meniscus itself. The authors found that a medial discoid meniscus completely coalesced with the anterior cruciate ligament. This anomaly strongly supports the congenital development theory of discoid meniscus. Both magnetic resonance imaging and the arthroscopic findings are presented. The patient was treated successfully by saucerization of the discoid meniscus up t

How Successful are Meniscus Repair All-Inside Implants during Deployment?

The purpose of this study was to determine the percentage of implants successfully deployed during arthroscopic all-inside meniscal repair. A data query of meniscus repair (Current Procedural Terminology [CPT] codes: 29882 and 29883) procedures was performed at a single institution. The query was limited to include procedures performed between June 1, 2020, and June 1, 2023. Multiple different manufacturer implants were used by five sports medicine fellowship-trained orthopaedic surgeons. The number of implants successfully used and the number of implants wasted due to intraoperative failure d

Three-dimensional-printed shape-memory composite scaffold for meniscus repair.

ObjectiveThe meniscus has poor intrinsic healing capacity, particularly in avascular regions. Meniscal injury is strongly associated with progressive knee dysfunction, chronic pain, and accelerated osteoarthritis development. Current treatments, such as allografts and partial meniscectomy, are limited by donor scarcity and secondary joint degeneration. Therefore, this study aimed to develop a regenerative meniscal scaffold with integrated biomechanical and biological functionality.MethodsA biomimetic composite scaffold was fabricated via low-temperature three-dimensional printing using poly(D,

Isolated Posterior Horn Medial Meniscus Bony Avulsion: A Case Report.

BACKGROUND: The menisci provide mechanical stability and shock absorption in the knee joint, distributing stress across the tibial plateau. Root tears to the menisci are functionally equivalent to a total meniscectomy, frequently leading to rapidly progressive degenerative osteoarthritis within the knee joint. The majority of medial meniscal bony avulsions have been reported in younger, skeletally immature patients. Cases of bony avulsions in older patients have sparsely documented. CARE PRESENTATION: In this case report, we highlight the case of a 45-year-old male with a history of osteoporo
