The impact of a community mental health initiative on outcomes for offenders with a serious mental disorder.

BACKGROUND: The Community Mental Health Initiative (CMHI) is mandated to assist offenders with serious mental disorders in their transition from institutions to the community, but this incorporates different styles of service. An important unanswered question is whether these are equivalent. AIMS/HYPOTHESES: Our aim was to compare outcomes for different intervention styles within the CMHI, a programme for serious offenders in prison who also have at least one major mental disorder. Our specific research questions were as follows: do outcomes differ according to whether offenders with mental h

Involuntary detention and treatment of the mentally ill: China's 2012 Mental Health Law.

The long-awaited Mental Health Law of China was passed on 26 October 2012 and took effect on 1 May 2013. Being the first national legislation on mental health, it establishes a basic legal framework to regulate mental health practice and recognizes the fundamental rights of persons with mental disorders. This article focuses on the system of involuntary detention and treatment of the mentally ill under the new law, which is expected to prevent the so-called "Being misidentified as mentally disordered" cases in China. A systematic examination of the new system demonstrates that the Mental Healt

Mental disorders diagnosed in childhood and at-risk mental state in a help-seeking population.

AIM: Disorders usually first diagnosed in infancy, childhood or adolescence (DUFD-ICA) may have preceded the onset of psychosis by several years and share some co-morbidity with psychotic disorders, but only a few studies have investigated this aspect. We looked for past or current DUFD-ICA in a sample of first adult psychiatric service users assessed for the presence of an at-risk mental state with the Structured Interview for Psychosis-risk Syndromes (SIPS). METHODS: We interviewed with the SIPS 159 first-time help seekers (age range 13-30 years) at adult psychiatric services who volunteere

Continuing challenges for the mental health consumer workforce: a role for mental health nurses?

The aim of this paper is to discuss issues impacting on consumer workforce participation and challenges that continue to arise for these workers, other service providers, and the mental health system. The literature identifies the following issues as problematic: role confusion and role strain; lack of support, training, and supervision structures; job titles that do not reflect actual work; poor and inconsistent pay; overwork; limited professional development; insufficient organizational adaptation to expedite consumer participation; staff discrimination and stigma; dual relationships; and th

Differences in attributions of mental illness and social distance for portrayals of four mental disorders.

For individuals with mental illness, others' perceptions of mental illness often limit integration into communities. Perceptions of mental illness manifest as social stigma in the form of social distance and may depend on individuals' attributions of the origins of mental illness. 180 university students completed a survey on attribution of mental illness and social distance across several disorders (psychiatric and physical). Participants indicated greater social distance for severe mental illness (i.e., schizophrenia) than less severe mental illness and physical illness. More desire for soci

"We mentally ill smoke a lot": identity, smoking, and mental illness in America.

Most of the history of the tobacco industry over the last few decades has focused on the conflicts between tobacco industry leaders who promoted smoking and tobacco control advocates who warned of the health consequences. Yet a view of this conflict from the perspective of smokers who are also mentally ill raises questions about how to frame public health policy for these individuals. Mentally ill consumers wrote to the tobacco industry between the 1970s and 1990s and expressed their commitment to smoking and to cigarette companies, despite their awareness of the health risks. This paper explo

Mental health and inequity: a human rights approach to inequality, discrimination, and mental disability.

Mental disability and mental health care have been neglected in the discourse around health, human rights, and equality. This is perplexing as mental disabilities are pervasive, affecting approximately 8% of the world population. Furthermore, the experience of persons with mental disability is one characterized by multiple interlinked levels of inequality and discrimination within society. Efforts directed toward achieving formal equality should not stand alone without similar efforts to achieve substantive equality for persons with mental disabilities. Structural factors such as poverty, ineq

Mental illness and/or mental health? Investigating axioms of the complete state model of health.

A continuous assessment and a categorical diagnosis of the presence (i.e., flourishing) and the absence (i.e., languishing) of mental health were proposed and applied to the Midlife in the United States study data, a nationally representative sample of adults between the ages of 25 and 74 years (N = 3,032). Confirmatory factor analyses supported the hypothesis that measures of mental health (i.e., emotional, psychological, and social well-being) and mental illness (i.e., major depressive episode, generalized anxiety, panic disorder, and alcohol dependence) constitute separate correlated unipol

The history of community mental health treatment and rehabilitation for persons with severe mental illness.

The authors review the evolution of the treatments for persons with severe mental illnesses over the past 40 years in three areas: pharmacological and other somatic treatments, psychosomatic treatments, and rehabilitation. Current treatments are based on a much stronger evidence base, are more patient-centered, and are more likely to target autonomy and recovery.

The waxing and waning of mental disorders: evaluating the stability of syndromes of mental disorders in the population.

This article examines the stability of symptoms, syndromes, and diagnoses of specific anxiety and depressive disorders, as well as diagnostic shifts from one syndrome to another over time. Using retrospective and longitudinal prospective data from the baseline and first follow-up investigation (19.7 months later) of the Early Developmental Stages of Psychopathology Study (EDSP), we focus on establishing stability measures for early stages of mental disorders in a community sample of adolescents aged 14 to 17 years at baseline. The results are as follows: (1) Although only about 30% developed a

Preventable fraction of mental retardation: analysis based on individuals with severe mental retardation.

Reduction in the prevalence of mental retardation has been identified as a national goal since 1971. President Nixon proposed the year 2000 as a benchmark, by which time the prevalence of severe mental retardation should be reduced by as much as 50%. An analysis to determine the fraction of severe mental retardation that could be prevented with strategies currently available indicates that the goal will not be met. Knowledge of the causes of severe mental retardation is fundamental to developing prevention strategies. Increased diagnostic capability-clinical and laboratory-must be dramatically

Psychopathic disordered, mentally ill, and mentally handicapped sex offenders: a comparative study.

Although there currently exists a large amount of research on the characteristics and treatment of psychopathic disordered (PD) sex offenders, little if any empirical studies have addressed the mentally ill (MI) and mentally handicapped (MH) offender populations. A total of 106 PD, MI, and MH sex offender records from Rampton (Special) Hospital were reviewed for the study. Offender categories were compared by age of first documented sex offence, IQ at the time of admission, sex offence type, frequency of each offence type, history of violence during sex offences, age and gender of sex offence

The Mental Health of Mothers Currently and Formerly Incarcerated in Jails and Prisons: An Integrative Review on Mental Health, Mental Health Treatment, and Traumatic Experiences.

UNLABELLED: Most incarcerated women in the United States are mothers who report high rates of mental health issues and traumatic experiences, yet their needs are often overlooked because they comprise a smaller proportion of the incarcerated population compared with men. OBJECTIVE: This integrative review aimed to synthesize the literature on the mental health, mental health treatment, and traumatic experiences of currently and formerly incarcerated mothers. METHODS: We searched PsychINFO, CINAHL, and Criminal Justice Abstracts for all research articles that were written in English; included

Association of functional disability with mental health services use and perceived unmet needs for mental health care among adults with serious mental illness.

BACKGROUND: Approximately 13.1 million U.S. adults experienced serious mental illness (SMI) in 2019. Persons with disability (PWD) have higher risks of SMI. Ensuring adequate access to mental health (MH) services for PWD is imperative to ameliorate this burden. METHODS: Using the 2015-2019 National Survey on Drug Use and Health, we obtained study variables for U.S. adults with SMI in the past year and used multivariable logistic regression models to examine the association of disability with MH services and perceived unmet needs. RESULTS: The sample comprised 12,532 respondents, representing

Addressing sexuality and intimate relations in community mental health services for people with serious mental illness: A qualitative study of mental health practitioners' experiences.

OBJECTIVE: The literature on the experience of mental health providers (MHPs) working with people with serious mental illness (SMI) in community-based mental health programs related to sex, sexuality, and intimacy is scarce. The purpose of the present study was to explore the situations, thoughts, feelings, dilemmas, and challenges experienced by MHPs around these issues. METHOD: Semistructured interviews were conducted with 13 MHPs working with people with SMI in six different community mental health programs in Israel. RESULTS: Four main themes emerged: (a) lack of professional training an

Brief cognitive screening of the elderly: a comparison of the Mini-Mental State Examination (MMSE), Abbreviated Mental Test (AMT) and Mental Status Questionnaire (MSQ).

One hundred and fifty unselected elderly community subjects were assessed by Mini-Mental State Examination (MMSE), Abbreviated Mental Test (AMT) and Mental Status Questionnaire (MSQ). The effects on cognitive test scores of potential confounding (non-cognitive) variables were evaluated. Sensitivities and specificities were: MMSE 80% and 98%; AMT 77% and 90%; and MSQ 70% and 89%. The MMSE identified significantly fewer false positives than the AMT and MSQ. The major effect of intelligence on cognitive test scores has previously been underestimated. Age, social class, sensitivity of hearing and

The lived experiences of familial mental illness stigma among persons living with mental illnesses.

Persons with mental illnesses may experience stigma from their immediate family members in addition to other forms of stigma. The researchers used semi-structured interviews to investigate the experiences of familial mental illness stigma among 15 people diagnosed with mental illnesses in a mid-sized city in Canada. Five themes that spoke to participants' experiences of familial mental illness stigma and ways to reduce it were identified by the investigators. The themes included: diagnosis as a 'double-edged sword,' potential familial isolation, familial stigma as societal stigma localized, st
