Client-Related Factors Associated with a "Less than Good" Experience of Midwifery Care during Childbirth in the Netherlands.

BACKGROUND: A "less than good" experience during childbirth can affect a mother's early interaction with her child and may significantly influence a woman's emotional well-being. In this study, we focus on clients who experienced midwifery care provided during childbirth as "less than good" care. The aim of this study was to understand the relationship between client-related factors and the experience of midwifery care during childbirth to improve this care. METHODS: This study was part of the "DELIVER study" where mothers report on the care they received. We used generalized estimation equat

Midwifery dilemma: to fast or feed the labouring woman. Part I: The case for restricting oral intake in labour.

This paper presents a literature review which pursues both sides of the labouring woman's oral intake debate--to fast or to feed the labouring woman. Part 1 provides an exploration of the phenomenon of gastric content aspiration associated with general anaesthesia and its historical and physiological underpinnings. Part 2 provides an examination of the literature concerning the benefits of fasting labouring women.

Midwifery Continuity of Care Supports Shared Decision-Making in Pregnancy With Mild Uterine Prolapse: A Qualitative Case Report.

Mild uterine prolapse during pregnancy can complicate counseling regarding mode of birth and challenge women's confidence in pelvic floor safety. This case report describes the care of a multiparous woman with first-degree uterine prolapse and persistent urinary hesitancy following a postpartum urinary retention episode. She was followed within a midwifery continuity of care model and a coordinated multidisciplinary pathway, including obstetric, urogynecologic, and pelvic floor physiotherapy support. Although the woman initially requested an elective cesarean, iterative counseling within a tru

Shared decision-making ability among women with postpartum urinary incontinence: A mixed-methods study to inform midwifery practice.

PROBLEM: Pelvic floor physical therapy (PFPT) is the first-line treatment for postpartum urinary incontinence (PPUI), but barriers such as limited access and low engagement in decision-making reduce adherence. BACKGROUND: Shared decision-making empowers women to participate in care and improve adherence. Variations in shared decision-making ability may limit engagement. AIM: To measure shared decision-making ability regarding PFPT and identify its influencing factors among women with PPUI through a quantitative phase, and derive midwifery support strategies based on qualitative insights. Caseload midwifery care versus standard maternity care for women of any risk: M@NGO, a randomised controlled trial.

BACKGROUND: Women at low risk of pregnancy complications benefit from continuity of midwifery care, but no trial evidence exists for women with identified risk factors. We aimed to assess the clinical and cost outcomes of caseload midwifery care for women irrespective of risk factors. METHODS: In this unblinded, randomised, controlled, parallel-group trial, pregnant women at two metropolitan teaching hospitals in Australia were randomly assigned to either caseload midwifery care or standard maternity care by a telephone-based computer randomisation service. Women aged 18 years and older were

Techniques to reduce perineal pain during spontaneous vaginal delivery and perineal suturing: a UK survey of midwifery practice.

OBJECTIVE: To investigate use of pharmacological and non-pharmacological methods of perineal analgesia used by midwives during the second stage of labour and perineal repair in the UK. DESIGN: Postal survey. METHODS: Self-complete questionnaires were sent to Heads of Midwifery in all 219 maternity units in the UK. Information was requested on the number and type of deliveries undertaken in the previous year and on the midwifery procedures used to provide pain relief immediately before delivery and for perineal repair. Details were also sought on local anaesthetics given before episiotomy or

Experiences of midwifery students and graduates in conflict-affected settings in Yobe State, Nigeria: Evidence from qualitative data.

Students and graduates in conflict-affected settings face challenges that impact training and practice. This qualitative study, nested in a broader mixed methods study, explored factors influencing midwifery students' and new graduates' experiences in conflict affected contexts in Yobe State Nigeria. Four focus group discussions (FGDs) were conducted in October 2023 with students and graduates from basic and community midwifery programs. The groups consisted of 7-8 female participants, aged 19-26 years. Discussions were audio-recorded, transcribed and analysed thematically. Participants report

Innovative Teaching Approaches to Integrating Substance Use Disorder Training into Midwifery and Women's Health Nurse Practitioner Education Programs.

Substance use disorder (SUD) is the most common cause of pregnancy-associated deaths for women in Illinois. In this article, we describe how to provide multimodality education and teaching opportunities in SUD and medication for opioid use disorder, incorporate community outreach to offer new experiential clinical learning opportunities, and enhance preceptor relationships to bolster educational partnerships as an innovative approach to incorporating SUD education into midwifery programs. Our teaching efforts identify the most vulnerable communities and create an innovative, midwifery-led solu

The midwifery initiated oral health-dental service protocol: an intervention to improve oral health outcomes for pregnant women.

BACKGROUND: Evidence is emerging that women's poor oral health and health practices during pregnancy are associated with poor oral health in their children and potentially an increased risk of pre-term or low-birth weight infants. METHODS/DESIGN: The Midwifery Initiated Oral Health-Dental Service (MIOH-DS) trial is a three arm multicentre randomised controlled trial which will recruit women from three metropolitan hospitals aimed at improving women's oral health and service access and indirectly reducing perinatal morbidity. All three arms of the trial will deliver oral health promotion mater

Role of nursing and midwifery in mainstreaming genomics in Australia: mixed-methods study exploring scope of practice and strategies for implementation.

BACKGROUND: The integration of genomics into Australian health services offers significant benefits for the diagnosis and delivery of targeted treatments, but its success relies on a workforce equipped to deliver genomic-informed care. Nurses and midwives, Australia's largest healthcare workforce, have the potential to play a key role in the integration of genomic care into mainstream services by enabling access to the diverse and geographically spread Australian population. To achieve this, it is imperative to clearly define their roles in genomic practice and identify specific educational an

Evaluating a midwifery student employment model in Aotearoa New Zealand: A national cross-sectional survey of students and midwives.

PROBLEM: The Maternity Care Assistant (MCA) role was introduced during the COVID-19 pandemic to provide midwifery workforce support in Aotearoa-New Zealand, but its broader impact remains unclear. BACKGROUND: The MCA role offers casual paid employment to Bachelor of Midwifery students. While designed to support the workforce, its impact has not been formally evaluated, particularly for Māori and Pacific students. AIM: To evaluate the MCA role and its impact on the transition to practice. METHODS: A national cross-sectional survey was distributed through midwifery networks using Qualtrics®.

The Role of Passive Descent and Epidural Analgesia in Outcomes Associated With Prolonged Pushing Among Nulliparous Individuals in Midwifery Care.

INTRODUCTION: Efforts to reduce primary cesarean birth may include supporting longer second stages of labor. Although midwifery-led care is associated with lower cesarean use, little has been published on associated outcomes of prolonged second stage (≥3 hours of pushing) for nulliparous individuals in US hospital-based midwifery care. Epidural analgesia and the role of passive descent in midwifery-led care are also underexplored in relation to the second stage. In this study, we report the incidence of prolonged second stage stratified by epidural analgesia and/or passive descent. Secondary a

Knowledge of female genital schistosomiasis and urinary schistosomiasis among final-year midwifery students in the Volta Region of Ghana.

BACKGROUND: Female genital schistosomiasis (FGS) is a gynaecological complication of urinary schistosomiasis (US) with an estimated burden of 20-120 million cases in endemic areas. A neglected sexual and reproductive health disease in sub-Saharan Africa, FGS increases susceptibility to sexually transmitted infections including cervical cancer and infertility among other morbidities. However, there appears to be limited FGS knowledge among practicing and pre-service health providers with implications for control. We assessed FGS awareness among final-year midwifery students who would soon be de

Effectiveness of midwifery-led care on pregnancy outcomes in low- and middle-income countries: a systematic review and meta-analysis.

BACKGROUND: Midwifery-led care is an evidence-based practice in which a qualified midwife provides comprehensive care for low-risk pregnant women and new-borns throughout pregnancy, birth, and the postnatal period. Evidence indicates that midwifery-led care has positive impacts on various outcomes, which include preventing preterm births, reducing the need for interventions, and improving clinical outcomes. This is, however, mainly based on studies from high-income countries. Therefore, this systematic review and meta-analysis aimed to assess the effectiveness of midwifery-led care on pregnanc

Development and use of a scale to assess gender differences in appraisal of mistreatment during childbirth among Ethiopian midwifery students.

Mistreatment during childbirth occurs across the globe and endangers the well-being of pregnant women and their newborns. A gender-sensitive approach to mistreatment during childbirth seems relevant in Ethiopia, given previous research among Ethiopian midwives and patients suggesting that male midwives provide more respectful maternity care, which is possibly mediated by self-esteem and stress. This study aimed a) to develop a tool that assesses mistreatment appraisal from a provider's perspective and b) to assess gender differences in mistreatment appraisal among Ethiopian final-year midwifer

Development and implementation of a clinical needs assessment to support nursing and midwifery students with a disability in clinical practice: part 1.

Equality and disability legislation, coupled with increasing numbers of students with a disability, and inadequate supports in clinical practice, acted as catalysts to explore how best to support undergraduate nursing and midwifery students on clinical placements. Historically, higher education institutions provide reasonable accommodations for theoretical rather than clinical modules for practice placements. This paper describes the development and implementation of a Clinical Needs Assessment designed to identify the necessary supports or reasonable accommodations for nursing and midwifery s

Women's experiences of trauma-informed care in midwifery and maternity services and implications for maternal wellbeing: A qualitative systematic review.

INTRODUCTION: Trauma-informed care has been increasingly advocated within maternity services as an approach to promote safety, autonomy, and respectful care. However, qualitative evidence exploring how participants experienced trauma-informed care across maternity and midwifery services has not been systematically synthesized. This review therefore aimed to synthesize qualitative evidence on women's experiences of trauma-informed care across maternity services, including midwifery care, during pregnancy, childbirth, and the postpartum period. METHODS: A systematic review was conducted in acco
