Correlates of posttraumatic stress disorder symptoms in Marines back from war.

The effect of combat and operational stress on the mental health of military personnel is a major concern. The objective of this study was to identify factors associated with possible posttraumatic stress disorder (PTSD). A questionnaire was completed by 1,569 Marines who deployed in support of conflicts in Iraq and Afghanistan (2002-2007). Using the PTSD Checklist with a cutoff score of 44, 17.1% of the sample screened positive for possible PTSD. Of 9 demographic and psychosocial factors examined in relation to PTSD, 4 were significant in a multivariate analysis: deployment-related stressors,

Exercise-associated Excessive Dynamic Airway Collapse in Military Personnel.

RATIONALE: Evaluation of military personnel for exertional dyspnea can present a diagnostic challenge, given multiple unique factors that include wide variation in military deployment. Initial consideration is given to common disorders such as asthma, exercise-induced bronchospasm, and inducible laryngeal obstruction. Excessive dynamic airway collapse has not been reported previously as a cause of dyspnea in these individuals. OBJECTIVES: To describe the clinical and imaging characteristics of military personnel with exertional dyspnea who were found to have excessive dynamic collapse of larg

Dermatologic Implications of Sleep Deprivation in the US Military.

Short sleep duration is common among US adults and is even more common among people working in protective services and the military. Military service predisposes members to disordered sleep due to the rigors of deployments and field training. In this article, we explore possible mechanisms by which sleep deprivation may affect the skin. We also review the potential impacts of sleep deprivation on specific topics in dermatology, including atopic dermatitis (AD), psoriasis, alopecia areata, physical attractiveness, wound healing, and skin cancer.

A Case Report Utilizing Ultrasound for the Identification of Traumatic Pulmonary Contusion.

Pulmonary contusions are a common injury in both military and civilian trauma patients. In austere and resource-limited settings common to deployment, military physicians may be limited on their ability diagnose or differentiate this entity from other traumatic injuries. We describe the use of ultrasound for the identification of pulmonary contusion in a patient with a gunshot wound while performing an extended Focused Assessment with Sonography (eFAST). The utility of ultrasound in polytraumatic patients stretches far beyond the initial FAST exam and can drastically inform clinical decision m

Exploring School Victimization and Weapon Carrying Among Military-Connected Lesbian, Gay, Bisexual, and Transgender Youth in California Schools.

Military-connected youth often experience daily stressors that affect their academic success and social and emotional development. Stressors such as multiple deployments and frequent school transitions may weaken the social ties that military-connected youth have with school communities, placing them at risk of social alienation and victimization. Within this youth population, military-connected lesbian, gay, bisexual, and transgender (LGBT) youth may be especially at risk of school victimization. However, to the authors' knowledge, no empirical studies have been conducted on the school experi

How Should Ongoing Surgical Care Needs be Met During Global Health Engagements?

The Department of Defense has a long history of Global Health Engagement, beginning with identifying and developing vaccines for yellow fever, influenzae, and malaria. Contemporary examples of global health engagement include the worldwide deployment of military assets to aid in natural disasters, and deployment of ship and land based medical teams for short-term medical and surgical engagements. In 2017, the Department of Defense Instruction on Global Health Engagement was published to help guide these efforts. Surgery is an indispensable part of global health and presents unique ethical chal

Association Between Predeployment Optimism and Onset of Postdeployment Pain in US Army Soldiers.

IMPORTANCE: Pain after deployment is a major health care concern. While risk factors have been previously studied, few studies have explored protective factors. OBJECTIVE: To examine the prospective association between predeployment optimism and the onset of new pain after deployment in US Army soldiers. DESIGN, SETTING, AND PARTICIPANTS: This prospective longitudinal cohort study examined US Army soldiers (active duty, Reserve, and National Guard) who deployed to Afghanistan or Iraq between February 12, 2010, and August 29, 2014, and completed the necessary psychological and health assessme

Role of Deployment History on the Association Between Epilepsy and Traumatic Brain Injury in Post-9/11 Era US Veterans.

BACKGROUND AND OBJECTIVES: Traumatic brain injury (TBI) is a well-established epilepsy risk factor and is common among service members. Deployment-related TBI, where combat/blast may be more common, may have different outcomes than nondeployment-related TBI. This work examined associations of all TBI exposures (not just combat), and epilepsy, while adjusting for comorbidities associated with epilepsy, among veterans by deployment status. METHODS: The cohort included post-9/11 veterans with ≥2 years of care in both Veterans Health Administration and Defense Health Agency systems. We identified

Review: management of adjustment disorder in the deployed setting.

Adjustment disorder is a maladaptive response to a stressor that causes functional impairment. The contribution of deployment stress on the diagnosis and treatment of adjustment disorder has not been previously explored. Despite the high cost and commonality of adjustment disorder as a behavioral health condition diagnosed during deployment, there are few clinical trials and no standardized recommendations found in this review related to treatment of military service members in the deployed setting. This manuscript reviews the best practices for adjustment disorder management and applies these

Risk factors for decline in cognitive performance following deployment-related mild traumatic brain injury: A preliminary report.

Thorough identification of risk factors for delayed decline in cognitive performance following combat-related mild traumatic brain injury (mTBI) is important for guiding comprehensive post-deployment rehabilitation. In a sample of veterans who reported at least one deployment-related mTBI, preliminary results indicate that factors including a history of loss of consciousness over 1 min, current obesity and hypertension, and Black race were more prevalent in those with decreased scores on a measure of memory function. These factors should be considered by clinicians and researchers working with

Occupational Risk of Low-Level Blast Exposure and Health Outcomes: A Longitudinal Study of Postdeployment Army Enlisted Active Duty Soldiers.

OBJECTIVE: Evaluate the relationship between risk of occupational low-level blast (LLB) exposure and health outcomes (ie, substance use disorder, long-term opioid receipt, headaches, and back pain). We integrated 2 established military occupation-based LLB exposure risk classification strategies (Belding and colleagues and Carr and colleagues) to use military service occupation as a proxy for LLB exposure risk. SETTING: Records were drawn from the Substance Use and Psychological Injury Combat Study and included Military Health System inpatient and outpatient care data, pharmacy fill records,

MGUS prevalence among active-duty military personnel.

Monoclonal gammopathy of undetermined significance (MGUS) is a precursor condition for multiple myeloma, and although environmental exposures have been implicated in its pathogenesis, it is unknown whether exposure to open-air burn pits used during U.S. military operations in Iraq affects prevalence of MGUS in active-duty service members (ADSMs). ADSMs represent a distinct young and physically fit population, with higher proportions of male and Black individuals compared to the general U.S. population. This retrospective cohort study evaluated MGUS prevalence among ADSMs. Participants included

Predeployment gender differences in stressors and mental health among U.S. National Guard troops poised for Operation Iraqi Freedom deployment.

Increased exposure of women soldiers to combat in current conflicts heightens interest in the question of whether risk and resilience factors differ for female and male military personnel prior to deployment. The authors examined this question in a panel of 522 National Guard soldiers (462 men and 60 women) poised for deployment to Iraq. Soldiers completed a battery of self-report measures, including the PTSD Checklist, Beck Depression Inventory-II, and scales from the Deployment Risk and Resilience Inventory. Modest differences were observed between women and men on predeployment risk factors

Immunization to protect the US Armed Forces: heritage, current practice, and prospects.

Americans serving with the US Armed Forces need protection from the dangerous infections that they can contract during training, based on occupation, during overseas deployment, or because of underlying health status. For over 230 years, the military health-care system has immunized troops to protect them personally and to help them accomplish their missions. Military researchers have invented, developed, and improved vaccines and immunization delivery methods against more than 20 diseases. This article consolidates content from several previous historical reviews, adds additional sources, and

Screening for alcohol misuse and alcohol-related behaviors among combat veterans.

OBJECTIVE: The U.S. military conducts health screenings of service members three to six months after they return from combat deployments. This population health program includes a modified version of the Two-Item Conjoint Screen (TICS), which is widely used in primary care to screen for alcohol misuse. Rates of referral in the military for alcohol treatment are very low, and the utility of these screening questions in predicting serious alcohol-related behaviors is unknown. METHODS: Anonymous surveys were collected from 6,527 U.S. Army soldiers who were screened three to four months after ret

The timeliness of the US military response to the 2014 Ebola disaster: a critical review.

In the face of an unprecedented epidemic of Ebola Virus Disease, in September 2014, the US military began sending thousands of personnel to Liberia and supporting areas in Senegal in its largest deployment to the African continent in over two decades. In this review, media reports, published photographs and official statements are evaluated and summarized to identify and describe key time points in the US military response. Specific events include the initial establishment of the Monrovia Medical Unit and the buildup of forces for the expanded mission, which involved enhancement of laboratory

Anti-Dengue IgG Seroprevalence and Exposure-Related Risk in Italian Military Personnel Deployed on Overseas Missions: A Cross-Sectional Study.

Dengue virus infection remains a significant public health challenge in endemic regions, with growing evidence of autochthonous transmission in Europe. Assessing serological exposure in high-risk populations such as military personnel deployed to endemic areas is essential to quantify exposure risk and support operational decision-making, particularly regarding pre-deployment counselling and risks associated with secondary infection. We conducted a cross-sectional study involving 1355 members of the Italian Armed Forces, measuring anti-dengue IgG antibodies by ELISA and collecting data on depl

Role of Particulate Matter from Afghanistan and Iraq in Deployment-Related Lung Disease.

Approximately 3 million United States military personnel and contractors were deployed to Southwest Asia and Afghanistan over the past two decades. After returning to the United States, many developed persistent respiratory symptoms, including those due to asthma, rhinosinusitis, bronchiolitis, and others, which we collectively refer to as deployment-related lung diseases (DRLD). The mechanisms of different DRLD have not been well defined. Limited studies from us and others suggest that multiple factors and biological signaling pathways contribute to the onset of DRLD. These include, but are n

Smallpox vaccine complications: the dermatologist's role in diagnosis and management.

In 2002, the United States implemented a new program for smallpox vaccinations among military personnel using a live vaccinia virus product. Approximately 2.4 million US military service members and health care workers have since been inoculated, with considerable numbers experiencing adverse reactions. Military dermatologists are at the forefront of describing and treating these reactions, from relatively benign generalized vaccinia (GV) and erythema multiforme (EM) to more severe progressive vaccinia (PV) and eczema vaccinatum (EV). A wide range of providers, including civilian dermatologist
