Intraoperative neuromonitoring (IONM) has become an essential tool in modern spine surgery to protect neural structures and minimize the risk of neurological complications. Various IONM techniques, such as motor evoked potentials, electromyography, transcranial motor stimulation, and somatosensory evoked potentials, have been developed to allow surgeons to monitor spinal cord and nerve function in real time and make critical decisions that improve surgical outcomes and reduce neurological deficits. IONM has been effective in improving surgical precision and patient safety in low- and middle-in

The educational pathway for expertise in intraoperative neurophysiological monitoring (IONM) is complex and lengthy, requiring a solid foundation in neuroscience, neurophysiology, and neuroanatomy. It also demands direct familiarity with a broad range of neurosurgical scenarios, including supratentorial, infratentorial, and spinal procedures, gained through exposure to at least ten distinct surgical approaches. Intraoperative neurophysiology must be tailored to each patient's preoperative assessments. It relies on a variety of methods to collect, analyze, and report neurophysiological signals

Continuous Intraoperative Neurophysiologic Monitoring (cIONM) is a widely used technology to improve surgical outcomes and prevent cranial nerve injury during skull base surgery. Monitoring of free-running electromyogram (EMG) plays an important role in cIONM, which can be used to identify different discharge patterns, alert the surgeon to potential nerve damage promptly, etc. In this dataset, we collected clinical multichannel EMG signals from 11 independent patients' data using a Neuromaster G1 MEE-2000 system (Nihon Kohden, Inc., Tokyo, Japan). Through innovative classification methods, the

BACKGROUND: Neurophysiological monitoring in surgery for hemifacial spasm has been used since the 1990s, when Moller et al. demonstrated the effectiveness of intraoperative assessment of lateral spread response (LSR) regarding postoperative outcomes. Currently, there are conflicting data on effectiveness and feasibility of this technique. Widespread hemifacial spasm determines the relevance of neurophysiological monitoring in surgical treatment of these patients.
OBJECTIVE: To evaluate the effectiveness of various methods of intraoperative neurophysiological monitoring in surgical treatment o

Transnasal endoscopic skull base surgery has been increasing in volume in recent years and its indications are constantly expanding. The potential occurrence of intraoperative and postoperative neurovascular complications deserves special attention from neurosurgeons. Multimodal intraoperative neurophysiological monitoring technology allows neurosurgeons to monitor cerebral perfusion and the functional status of the associated cranial nerves in real time, thereby enabling surgeons to make prompt adjustments in surgical procedures and strategies and reduce the risks of postoperative neurologica

The most significant complication of parotid gland tumor surgery is facial weakness. This study compares the occurrence of transient facial palsy in patients with parotid gland tumors who underwent surgery without monitoring to those who underwent surgery with monitoring. The study's aim was to investigate facial nerve function in patients undergoing parotidectomy as well as the effect of intraoperative facial nerve monitoring and the effect of certain risk factors on the surgery and onset of postoperative facial palsy. This prospective study included 100 patients who underwent parotidectomy.

BACKGROUND: It is unclear whether parathyroidectomy guided by intraoperative parathormone (PTH) monitoring is predictive of operative success in patients with normohormonal hyperparathyroidism (nhHPT), a variant of primary hyperparathyroidism (pHPT) in which patients develop clinical manifestations similar to those of pHPT. This study examined intraoperative PTH monitoring in patients undergoing parathyroidectomy for nhHPT.
METHODS: We performed a retrospective review of prospectively collected data from adult (age > 18 yr) patients who underwent parathyroidectomy for pHPT at 1 of 2 North Ame

BACKGROUND: Surgery is the only curative treatment option for primary hyperparathyroidism (PHPT). The intraoperative parathormone (IOPTH) monitoring is recommended to confirm that all pathological glands have been removed. This study aimed to evaluate the effect of IOPTH monitoring on the surgical success of parathyroidectomy performed for PHPT.
METHODS: The demographic, biochemical, operative and pathological data of patients who underwent parathyroidectomy for PHPT in a single institute over a three-year period were retrospectively analyzed.
RESULTS: The total number of patients included i

Vagal schwannomas are rare, benign tumors. Intermittent intraoperative neuromonitoring via selective stimulation of splayed motor fibers running on the schwannoma surface to elicit a compound muscle action potential has been previously reported as a method of preserving vagal motor fibers. In this case report, vagal sensory fibers are mapped and continuously monitored intraoperatively during high vagus schwannoma resection using the laryngeal adductor reflex (LAR). Mapping of nerve fibers on the schwannoma surface enabled identification of sensory fibers. Continuous LAR monitoring during schwa

Recurrent laryngeal nerve (RLN) palsy can result in motility disorders of the vocal cord muscles and/or fatal respiratory complications. Procedures aimed at checking for intraoperative RLN injuries have not been established. Intraoperative nerve monitoring( IONM) informs electromyographic activity of the vocal cord muscles to a surgeon by visual and aural signals. In this chapter, we introduce methods for using IONM during esophageal cancer surgery. Since we introduced IONM, several cases in which surgeons have had difficulty visually identifying the RLN have been reported. In those cases, the

Introduction Surgery is the first-line management option for patients with primary hyperparathyroidism (pHPT). Minimally invasive parathyroidectomy (MIP) is now preferable but few centres offer this service, mainly because of lack of intraoperative parathyroid hormone (IOPTH) testing. The aim of this study was to identify whether the measurement of IOPTH in patients having minimally invasive parathyroidectomy for pHPT alters their management. Methods A retrospective review was carried out of 78 consecutive patients who underwent parathyroid surgery by a single surgeon with a special interest i

BACKGROUND: Glossopharyngeal neuralgia (GN) is a rare functional disorder representing around 1% of cases of trigeminal neuralgia. Lancinating throat and ear pain while swallowing are the typical manifestations, and are initially treated using anticonvulsants such as carbamazepine. Medically refractory GN is treated surgically. Microvascular decompression (MVD) is reportedly effective against GN, superseding rhizotomy and tractotomy.
METHODS: We encountered three patients with medically refractory GN who underwent MVD using intraoperative neurophysiological monitoring (IONM). The offending ve

OBJECTIVE: Minimal invasive parathyroidectomy (MIP) is a common surgical technique for the treatment of primary hyperparathyroidism (PHPT) and is usually done in conjunction with positive imaging techniques. We aimed to assess the results of this technique, performed without the use of intraoperative tests, in cases with PHPT caused by a single parathyroid adenoma.
MATERIAL AND METHODS: The data for patients who were diagnosed with PHPT were assessed retrospectively. Only those who had undergone a parathyroid adenoma localization study with ultrasonography (US) and parathyroid scintigraphy (P

PURPOSE: Multimodal intraoperative neurophysiologic monitoring (IOM) provides assessment of spinal cord pathways during neurosurgery. Despite widespread use, few data exist regarding sensitivity and specificity of IOM in predicting neurologic outcome during decompression and instrumentation for epidural spine tumors.
METHODS: Retrospective analysis evaluated consecutive spine procedures involving IOM modalities (somatosensory evoked potentials [SSEP], motor evoked potentials [MEP], and electromyography [(EMG]) from 2007 to 2009. Demographic and surgical information, intraoperative neurophysio
