Pulmonary complications can be the result of direct chest trauma or can occur from indirect trauma outside of the thorax. Understanding the mechanism of pulmonary function in determining intrapulmonary shunt and physiologic deadspace can assist the clinician in assessing the severity and monitoring the progression of pulmonary injury in patients. This article reviews assessment parameters, physiology, and treatment of direct and indirect pulmonary trauma.

Saliva is a complex physiologic fluid that contains an abundance of biological analytes, or biomarkers. Recent research has shown that these biomarkers may be able to convey the physiologic health of a person. Work has been done linking derangements in these salivary biomarkers to a wide variety of pathologic disorders ranging from oncologic diseases to atopic conditions. The specific area of interest for this review paper is esophageal disorders. Particularly because the diagnosis and management of esophageal disorders often includes invasive testing such as esophagogastroduodenoscopy, prolon

The authors performed 72-hour polysomnography, including a levodopa-placebo-controlled choice reaction time test with continuous monitoring of cardiovascular variables in two patients with Parkinson's disease complaining of excessive daytime sleepiness on levodopa monotherapy. The subacute levodopa challenge was accompanied by physiologic sleep episodes, worsened reaction times, and a reduction in mean blood pressure and heart rate over baseline values, linked temporally to the sleep episodes.

Doppler ultrasound applications in normal and complicated pregnancies are presented in this article. Anatomy and pathophysiology of fetal placental blood flow are described. Potential pitfalls in the evaluation of fetal placental blood flow are explored. Doppler flow velocity waveform analysis combined with other clinical and physiologic information is valuable in monitoring of the high-risk pregnancy. Duplex technology may also be useful in the first trimester.

Heat stroke victims lack thermoregulatory control. Treatment includes immediate cooling, circulatory support and monitoring for secondary complications. Neuroleptic malignant syndrome is a complication of neuroleptic drug therapy; skeletal muscle hypertonicity helps distinguish this entity from heat stroke. Malignant hyperthermia should be considered in any patient who is under physiologic or anesthetic stress and develops hyperthermia plus skeletal muscle rigidity, tachypnea, hypoxia, tachycardia and hyperkalemia.

Shock is a life-threatening condition marked by inadequate tissue perfusion and oxygen supply, leading to organ failure if not rapidly addressed. Clinical management of shock involves detecting and correcting altered macro hemodynamic parameters. However, these parameters may not accurately reflect microcirculatory alterations or abnormalities in oxygenation. A resuscitation strategy focused on peripheral perfusion, which can be non-invasively monitored, may allow for earlier shock detection and treatment, potentially reducing mortality. This literature review aims to study the available techn

PURPOSE: The present study aimed to investigate the correlation between transcutaneous carbon dioxide partial pressure (PtcCO2) and arterial carbon dioxide pressure (PaCO2) and the accuracy of PtcCO2 in predicting PaCO2 during laparoscopic surgery in pediatric patients.
METHODS: Children aged 2-8 years with American Society of Anesthesiologists (ASA) class I or II who underwent laparoscopic surgery under general anesthesia were selected. After anesthesia induction and tracheal intubation, PtcCO2 was monitored, and radial arterial catheterization was performed for continuous pressure measureme

The postvoid residual (PVR) is an important measure of bladder function, but obtaining PVR is burdensome because bladder volume must be measured at the time of voiding. The PVR measurement problem has led to experimental tricks in animal studies (infusing the bladder at supraphysiological rates and limiting animal observation windows) to keep the number of observed voids statistically robust while reducing the time an experimenter must be present. Our solution to the PVR measurement problem is a system called Automatic Monitoring for Efficient, Awake, Sensitive, Urine Residual Estimation (AMEA

Background and Clinical Significance: Decompressive hemicraniectomy (DHC) is a life-saving intervention for malignant middle cerebral artery (MCA) infarction, but postoperative secondary injury mechanisms and functional outcome remain difficult to evaluate using intracranial pressure (ICP) alone. The pressure reactivity index (PRx), calculated as the moving correlation coefficient between ICP and mean arterial pressure (MAP), provides a measure of cerebral autoregulation. The utility of PRx monitoring in ischemic stroke, especially following DHC, remains uncertain. Case Presentation: We descri

This review highlights the key role of imaging modalities in diagnosing and managing myositis. The authors underscore MRI's superiority in identifying muscle edema and fat infiltration, marking it as essential for evaluating disease activity and damage. They also suggest ultrasound's emerging significance for diagnosis and monitoring of the disease, valued for its ease of use, and real-time capabilities. Furthermore, PET scans' unique physiologic capabilities, especially useful for malignancy detection and assessing lung disease, are emphasized. Collectively, these imaging techniques offer a t

Refeeding syndrome (RS) in preterm infants is a scenario of fetal malnutrition, primarily resulting from placental insufficiency, followed by a postnatal physiologic adaptation and response to an imbalance of nutrients provided parenterally. Growth restriction and small gestational age status are common findings in infants at risk of developing RS. Adverse clinical outcomes associated with RS may be severe and life-threatening. The biochemical abnormalities that occur in RS may be mitigated through careful monitoring and adaptation of the clinical management of parenteral and enteral nutrition

Anesthesiologists faced with a patient with sepsis and concurrent cardiac dysfunction must be cognizant of the patient's cardiac status and cause of the cardiac problem to appropriately adapt physiologic and metabolic monitoring and anesthetic management. Anesthesia in such patients is challenging because the interaction of sepsis and cardiac dysfunction greatly complicates management. Intraoperative anesthesia management requires careful induction and maintenance of anesthesia; optimizing intravascular volume status; avoiding lung injury during mechanical ventilation; and close monitoring of

Real-time monitoring of on-duty firefighters has the potential to reduce the risk of cardiovascular events and injuries by enabling early detection of fatigue and occupational hazards. Recent work uses wearable sensors to track metrics like heart rate and body temperature. However, most existing systems lack integrated environmental sensing, making it difficult to discern whether elevated vitals stem from physical exertion, ambient heat, or a combination of the two. To address this, a wireless sensing system is proposed that synchronizes physiological and environmental measurements during acti

BACKGROUND: The consistency of cardiac output (CO) measured by noninvasive cardiac output monitoring (NICOM), pulse index continuous cardiac output (PiCCO), and ultrasound in the hemodynamic monitoring of critically ill patients was studied. Using the NICOM built-in passive leg raising (PLR) test, stroke volume index variation (∆SVI) was calculated and was used to predict volume responsiveness in patients with circulatory shock (excluding cardiogenic shock).
METHODS: Critically ill patients requiring hemodynamic monitoring were admitted during the study period. The CO of each included patient

Although rare, status epilepticus refractory to conventional therapy may require the initiation of pentobarbital anesthesia and intensive monitoring in the PACU. Barbiturate therapy mandates that the nurse be able to perform mechanical ventilation and advanced cardiopulmonary monitoring as well as be familiar with electroencephalographic monitoring. Careful attention to the potential side effects of barbiturate therapy and anticipation of end-organ complications can increase the likelihood of seizure termination and recovery of status epilepticus patients.

Appropriate monitoring during obstetric epidural analgesia consists of: 1. Indirect BP and pulse monitoring before epidural insertion, frequently after every dose, and intermittently thereafter. 2. The aspiration test before all injections. 3. Frequent clinical monitoring for signs of intravascular injection during administration of small intermittent doses (not more than 3-5 ml at a time). 4. Frequent clinical monitoring for sympathetic, sensory and motor signs indicating upward extension of the block. 5. Frequent monitoring of the fetal heart rate (FHR) and other signs of fetal welfare. Right ventricular (RV) injury (including RV dilatation/dysfunction/limitation/failure) and pulmonary vascular dysfunction are common in patients with acute respiratory distress syndrome (ARDS). Despite increasing recognition, RV injury is associated with increased mortality in patients with ARDS, and implementation of multimodal monitoring and timely RV-targeted interventions may therefore confer outcome benefit. The aim of this narrative review is to explore the clinical applications of diagnostic modalities for the RV and pulmonary circulation in invasively ventilated patients with ARDS, inc
