Opioid use disorder and homelessness in the Veterans Health Administration: The challenge of multimorbidity.

OBJECTIVE: The aim of this study is to estimate the prevalence and sociodemographic and clinical correlates of opioid use disorder (OUD), a major cause of morbidity and mortality in the United States, among homeless veterans nationally in the Veterans Health Administration (VHA). DESIGN: Administrative data on 256,404 veterans who were homeless and/or had OUD in fiscal year 2012 were analyzed to evaluate OUD as a risk factor for homelessness along with associated characteristics, comorbidities, and patterns of service use. Bivariate analyses and logistic regression were used to compare homele

Multimorbidity and polypharmacy in diabetic patients with NAFLD: Implications for disease severity and management.

An observational study describing the number and type of chronic conditions and medications taken by diabetic patients with NAFLD and identifying characteristics that may impact liver disease severity or clinical management.Adults with type 2 diabetes have a high prevalence of nonalcoholic fatty liver disease (NAFLD) and increased risk of developing advanced liver disease. Appropriate management should consider the characteristics of the diabetic NAFLD population, as comorbid conditions and medications may increase the complexity of treatment strategies.Diabetic patients with NAFLD at risk of

Interventions for improving outcomes in patients with multimorbidity in primary care and community settings.

BACKGROUND: Many people with chronic disease have more than one chronic condition, which is referred to as multimorbidity. The term comorbidity is also used but this is now taken to mean that there is a defined index condition with other linked conditions, for example diabetes and cardiovascular disease. It is also used when there are combinations of defined conditions that commonly co-exist, for example diabetes and depression. While this is not a new phenomenon, there is greater recognition of its impact and the importance of improving outcomes for individuals affected. Research in the area

Hypertension: a cross-sectional study of the role of multimorbidity in blood pressure control.

BACKGROUND: Hypertension is the most prevalent cardiovascular long-term condition in the UK and is associated with a high rate of multimorbidity (MM). Multimorbidity increases with age, ethnicity and social deprivation. Previous studies have yielded conflicting findings about the relationship between MM and blood pressure (BP) control. Our aim was to investigate the relationship between multimorbidity and systolic blood pressure (SBP) in patients with hypertension. METHODS: A cross-sectional analysis of anonymised primary care data was performed for a total of 299,180 adult patients of whom 3

Association of handgrip strength with chronic diseases and multimorbidity: a cross-sectional study.

The prevalence of chronic diseases has risen along with increased longevity. Co-occurrence of two or more chronic diseases in an individual (multimorbidity) is prevalent and poses a huge burden to individuals and the society. However, determinants of multimorbidity are largely unknown. Handgrip strength is a general indicator of muscle strength and linked with premature mortality. However, its role in multimorbidity has never been evaluated. To investigate the relationships between handgrip strength and multiple chronic diseases and multimorbidity, and to assess the usefulness of age and handg

Classification and Severity Assessment of Obesity in Clinical Risk Prediction of Multimorbidity Trajectories.

Obesity is a key risk factor for chronic disease and multimorbidity, yet variation in how obesity is defined may influence risk estimation in clinical practise. Waist-to-hip ratio (WHR) and waist-to-height ratio (WHtR) are better at capturing fat distribution than body mass index (BMI) and may have a role in obesity related multimorbidity prediction. To examine multimorbidity risk across obesity groups classified using BMI in combination with WHR/WHtR. A UK Biobank cohort free of comorbidity at baseline (n = 179 876) was followed for incident first long-term condition and multimorbidity (≥ 2 o

Diabetes Distress in Adults with Type 2 Diabetes and Multimorbidity: A Scoping Review.

BACKGROUND: Diabetes distress is a distinct psychological construct often conflated with depression in adults with Type 2 Diabetes Mellitus (T2DM) and multimorbidity. Current literature lacks a unified synthesis explaining how "therapeutic competition", where managing multiple conditions creates conflicting self-care demands, and cumulative regimen complexity specifically drive distress in this population. OBJECTIVE: This scoping review systematically maps the biopsychosocial determinants of diabetes distress in adults with T2DM and multimorbidity to inform integrated, patient-centered interv

Proteomics of multimorbidity progression across cardiometabolic diseases and cancer in a multinational cohort.

BACKGROUND: Multimorbidity, defined here as the co-occurrence of cardiovascular disease (CVD), type 2 diabetes (T2D), and/or cancer is a major public health challenge. However, its underlying biological mechanisms remain unclear, limiting progress toward identifying shared interventional targets. METHODS: We applied large-scale plasma proteomics (SomaScan 7k; 7,289 aptamers) in 13,270 European Prospective Investigation into Cancer and Nutrition (EPIC) participants to identify protein signatures of multimorbidity. We modelled multimorbidity progression as sequential disease transitions, i.e.,

Reframing TB Care: A Perspective on Multimorbidity-Centered Care for People with TB.

Tuberculosis (TB) rarely occurs in isolation; most people with TB experience multiple coexisting conditions, including HIV, diabetes, undernutrition, depression, and substance use disorders, which worsen disease severity and compromise treatment outcomes. Although the World Health Organization has issued disease-specific guidance for managing key comorbidities, TB care remains largely siloed and poorly equipped to address the growing burden of multimorbidity, particularly in African health systems. In this perspective article, we propose a phased framework for multimorbidity-centered TB care.

Risk factors for multimorbidity of cardiovascular diseases in the prospective Million Women Study.

OBJECTIVE: Cardiovascular diseases often occur together, but little is known about what increases the risk of developing cardiovascular multimorbidity (CVM), particularly in women. This study investigated the associations of cardiovascular risk factors with CVM incidence in UK women. METHODS: 1.3 million women aged 50-64 years were recruited into the Million Women Study in 1996-2001. Women reported information on demographic and cardiovascular risk factors (weight, height, smoking, alcohol consumption, physical activity and treatment for high blood pressure, diabetes and high blood cholestero

Trends in Asthma-Rhinitis Allergic Multimorbidity and Polysensitization in China: The CARRAD Study.

The trends in allergic comorbidities secondary to the environmental variations in China remain unclear. We aimed to determine the variation of allergic comorbidities and polysensitization among asthma and/or rhinitis patients in the past decade. We assessed two nationally representative cross-sectional datasets from 2008 to 2009 and 2018 to 2019, which enrolled 2322 and 2353 patients, respectively. Over the present 10-year study period, the prevalence of allergic symptoms and allergen sensitivity among patients with multiple sensitivities in the 2018-2019 cohort was significantly higher than t

Multimorbidity in the modulator era: challenges of managing an ageing cystic fibrosis population.

INTRODUCTION: Cystic Fibrosis (CF) is an autosomal recessive disorder caused by mutations in the Cystic Fibrosis Transmembrane Conductance Regulator (CFTR) gene, leading to defective chloride ion transport and multisystem disease. The introduction of CFTR modulators, particularly elexacaftor/tezacaftor/ivacaftor (ETI), has significantly improved life expectancy and quality of life for people with CF (pwCF). As survival increases, an aging CF population faces different new challenges. AREAS COVERED: This review aims to highlight emerging challenges and comorbidities in an aging CF population t

A systematic analysis of the contribution of genetics to multimorbidity and comparisons with primary care data.

BACKGROUND: Multimorbidity, the presence of two or more conditions in one person, is common but studies are often limited to observational data and single datasets. We address this gap by integrating large-scale primary-care and genetic data from multiple studies to interrogate multimorbidity patterns and producing digital resources to support future research. METHODS: We defined chronic, common, and heritable conditions in individuals aged ≥65 years, using two large primary-care databases [CPRD (UK) N = 2,425,014 and SIDIAP (Spain) N = 1,053,640], and estimated heritability using the same de

Cumulative Burden of Digital Health Technologies for Patients With Multimorbidity: A Systematic Review.

IMPORTANCE: Digital health technologies (DHTs) aiming to monitor, treat, and manage diseases can be prescribed for patients with multimorbidity; yet most DHTs are designed for individual conditions or problems, while approximately half of patients with chronic conditions have multiple chronic conditions. OBJECTIVES: To identify DHTs approved by the US Food and Drug Administration (FDA) or listed in the Organisation for the Review of Care and Health Apps (ORCHA) library and prescribable for a hypothetical patient with 5 conditions and to model the number of DHTs this patient should be prescrib

Prevalence of Multimorbidity Among School-Aged Children in the Yangzhou District of China.

Background: Health issues among school-age children have emerged as a global public health concern. These conditions often do not occur in isolation but tend to cluster, indicating a widespread issue of multimorbidity among this population. This study examined the prevalence and clustering of multimorbidity among school-aged school students in the Yangzhou district. Methods: A repeated cross-sectional analysis was conducted from 2019 to 2024, including 22,512 students aged 6-18 years. Common diseases, under national key monitoring, including myopia, dental caries, obesity, elevated blood press

Multidose Drug Dispensing in Community Healthcare Settings for Patients With Multimorbidity and Polypharmacy.

Multidose drug dispensing (MDD) is the dispensing of different drugs in dose bags containing one, some, or all units of medicine that a patient needs to take at specific times. The aim of this narrative review is to provide an overview of the literature describing the use of MDD systems in community healthcare settings in patients with multimorbidity and polypharmacy. A literature search identified 14 studies examining adherence, medication knowledge, quality of drug prescription (including inappropriate drug use, drug-drug interactions), medication incidents, and drug changes after MDD initia

Uncovering the heritable components of multimorbidities and disease trajectories using a nationwide cohort.

Quantifying the contribution of genetics and environmental effects on disease initiation and progression, as well as the shared genetics of different diseases, is vital for the understanding of the disease etiology of multimorbidities. In this study, we leverage nationwide Danish registries to provide a granular atlas of the genetic origin of disease phenotypes for a cohort of all Danes 1978-2018 with partially known pedigree (n = 6.3 million). We estimate the heritability and genetic correlation between thousands of disease phenotypes using a novel approach that can be scaled to nationwide da
