We examined tears from 12 MS patients and 20 normal controls. In MS tears, the total IgG concentration was increased. Electrophoresis revealed low molecular weight bands (25 to 34k) in tears from patients with demyelinating optic neuritis; similar, less intense bands were seen in some patients with a history of optic neuritis. Production of tears was impaired in almost one-half of MS patients, especially in clinical exacerbations. Analysis of tears may offer new insight into immune function in MS, particularly of the mucosal immune system.

Paroxysmal phenomena are rare but relatively typical, and occasionally initial, clinical symptoms of multiple sclerosis. They should be recognized since they can serve as clinical indicators of the disease and can be treated efficiently with carbamazepine. Such shortlived clinical symptoms and signs are probably caused by ephaptic transmission due to myelinoaxonal dissociation within the demyelinated plaque. The clinical picture, pathophysiology and treatment of these disorders are discussed.

3 patients with multiple sclerosis (MS) who developed severe widespread bullous pemphigoid (BP) are presented. MS preceded the presentation of BP by 13-23 years (mean 18 years). BP was confirmed histologically and immunopathologically. Upon successful therapy with steroids, no recurrence of BP was observed over a 3-5 year (mean 3.7 years) follow-up. The mean interval between first and last clinical interview in 170 patients with multiple sclerosis was 8.8 years. The main initial symptoms were sensory disturbances and paresis of the cranial nerves. An uncharacteristic symptomatology was found in 6%, psychiatric symptoms in 6%. In the last investigation psychiatric symptoms were found in 2%, uncharacteristic symptoms were seen in 4%. A normal neurological status was found in 9%. Apart from the clinical well defined typical MS patient, also benign forms of the diseases have been found.

The values of the individual audiological and vestibular examination methods in multiple sclerosis (MS) diagnosis are discussed. Our experience shows that the most accurate indications are provided by acoustic stapedius reflex, brainstem auditory evoked potentials (BAEPs) and vestibular investigation. The pattern electroretinogram obtained in patients affected by multiple sclerosis were compared with those in a normal population. The pattern electroretinogram amplitude in multiple sclerosis optic neuritis frequently appeared normal, but pattern electroretinogram amplitude abnormalities were found in patients suffering from recurrent multiple sclerosis optic neuritis. Pattern electroretinogram examination appears a useful method in the differential diagnosis between axonal and demyelinating optic nerve impairment.

A questionnaire study on sexual problems occurring with multiple sclerosis (MS) was carried out with 217 patients who had previously participated in the University of Washington Multiple Sclerosis Project. The pathogenesis of multiple sclerosis is still poorly understood. Clearly, exogenous factors and genetic susceptibility combine to induce the disease, but studies on twins have shown that the genetic part is of minor importance. The immunological abnormalities observed do not fit easily into a coherent system. Hearing loss in an uncommon symptom in multiple sclerosis (MS). In nine patients with MS, seven with unilateral hearing loss and two with bilateral impairment, accompanying symptoms and signs included facial numbness, hemifacial paresis or spasms, ipsilateral limb ataxia, nystagmus, vertigo, tinnitus, and spastic-ataxic gait. Multiple sclerosis is a demyelinating disease of the central nervous system, often producing abnormalities in sexual function and urinary control. Eighty-six patients with this disorder were referred to our neurourologic facilities for evaluation (45 women and 41 men). Symptomatic voiding dysfunction was present in 84 patients (97 per cent). The sera of 176 MS patients and of 150 healthy adult controls were assayed for antibodies against mumps, rubella, Sendai and herpes simplex viruses, a higher prevalence of measles c.f.a. having already been demonstrated in the MS patients.
