Pictorial essay. Myositis ossificans circumscripta.

Myositis ossificans circumscripta (MOC) is a localized, non-neoplastic, heterotopic ossification within the muscle. Despite advances in medical imaging, it remains difficult to distinguish it from other disorders. As a consequence, a biopsy is frequently performed. We illustrate some common findings and non-specific imaging features of MOC with respect to radiography, computed tomography, bone scintigraphy, ultrasonography, and magnetic resonance imaging. The differentiation among MOC and its mimics is also presented in order to help the radiologists to make an appropriate diagnostic decision.

Cryptococcus neoformans myositis without dissemination.

We report a case of isolated cryptococcal myositis involving the paraspinal muscle without evidence of disseminated disease in a patient with a large B-cell lymphoma diagnosed at the time of presentation. Biopsy of the muscle involved grew a pure culture of Cryptococcus neoformans and periodic acid-Schiff staining showed numerous budding yeast consistent with Cryptococcus spp. The patient responded to systemic antifungal therapy with complete resolution of his infection. We also present a review of 5 previously published cases of cryptococcal myositis.

[Progressive myositis ossificans. Stone man].

Myositis ossificans progressiva (MOP) is a rare hereditary connective tissue disease, genetically inherited as an autossomal dominant trait with complete penetrance but variable expression. The onset usually takes place during childhood and progressive involvement of the spinal cord and proximal extremities leads to immobilization and articular dysfunction. We present a case of a 29-year-old woman with the typical features of MOP and review the pathogenesis, clinical manifestations and treatment options of this rare disease.

Treatment of inclusion body myositis.

Sporadic inclusion body myositis (s-IBM) is considered the most common muscle disease in patients older than 50 years, with a male predominance. Features of s-IBM include insidious onset, slowly and relentlessly progressive muscle weakness, a characteristic distribution and atrophy of both the proximal and distal muscle groups, and resistance to immunosuppressive drugs. The most characteristic pathologic feature is vacuolar degeneration of muscle fibers accompanied by intrafiber congophilia and clusters ("tangles") of paired-helical filaments, containing phosphorylated tau. The response of s-I…

Inclusion body myositis--a review.

Inclusion body myositis (IBM), a sporadic inflammatory myopathy, is the most frequently occurring progressive myopathy in adults older than 55 years. It more commonly affects men and usually is clinically and pathologically distinguishable from dermatomyositis or polymyositis. Muscle biopsy specimens will display inflammation in virtually all cases of sporadic IBM, along with rimmed vacuoles accompanied by the accumulation of beta-amyloid and other substances similar to those found in the degenerating neurons of Alzheimer's disease. The similarities between IBM and other inflammatory myopathie…

Acute orbital myositis. Case report.

The case of 22-year old, white woman with bilateral orbital myositis following an acute upper respiratory tract infection is reported. The most important clinical findings were ocular pain, proptosis, restricted eye motility and swelling of the eyelids. The enlarged eye muscles were seen on orbital computerized tomography scan. The clinical findings of inflammatory orbital myositis and clinical response to corticotherapy are emphasized.

Focal myositis: a clinicopathological study.

Focal myositis is a rare, benign inflammatory condition that may clinically simulate a soft tissue sarcoma. It was first described in 1977 and around 30 cases have been reported to date. We report two further cases on which we have performed immunocytochemistry and electron microscopy. Histology of both lesions was identical, showing a destructive inflammatory myopathy with evidence of regeneration. Stains for micro-organisms were negative and no viral particles were seen on electron microscopy. The immunocytochemical profile of our two cases differed from that of polymyositis: with a panel of…

Idiopathic myositis: a rheumatological view.

Twenty five patients with idiopathic myositis attended this department for long term follow up from 1980 to 1989. Twelve patients had primary polymyositis (four men, eight women) and six had primary dermatomyositis (three men, three women); five women had an overlap syndrome. Two patients had a malignant condition associated with the myositis. The mean age at diagnosis was 40 years. All of the patients had proximal muscle weakness, 18/25 had a raised creatine kinase value (mean 2325 IU/l), 19/20 had an abnormal electromyogram, and 19/24 had positive muscle biopsy samples. Of the disease specif…

Inclusion body myositis and paramyxoviruses.

Inclusion body myositis (IBM) is a distinct type of muscle disease. The characteristic electron microscopic findings, intranuclear or intracytoplasmic inclusions composed of microtubular filaments, morphologically resemble paramyxovirus nucleocapsids. These findings and the reported immunoreactivity of the inclusions with mumps virus antibodies have suggested that inclusion body myositis is a chronic virus infection. We analyzed skeletal muscle specimens from three patients with characteristic light microscopic features and electron microscopically verified inclusions of IBM by immunocytochemi…

[Myositis ossificans following extensive burns].

A report on a case of myositis ossificans following 50-60% burns (second to third-degree) on the trunk and extremities with periarticular localization of the ossification at the hip and knee joints. In terms of its extent and localization the ossification is primarily similar to post-paraplegic ossification of soft tissues.

Primary suppurative myositis in children.

Primary suppurative myositis is rare in the United States when compared with the incidence of disease in the tropics. Clinically, it may mimic many of the more common diseases, such as hematoma, osteomyelitis, arthritis, or appendiceal abscess. It usually has a benign course, with complete recovery after appropriate treatment. Prolonged morbidity and an increased mortality may result from unfamiliarity with this entity. Six children had primary suppurative myositis; one died as a result of perforation of a psoas abscess into the dural sac causing staphylococcal meningitis.

[Lockjaw in progressive myositis ossificans].

Frequency, symptomatology, course and therapy of myositis ossificans progressiva are described by means of literature references and personal observation in a patient. The patient was born in 1935 and the disease manifested itself for the first time in 1956 in the right sternocleidomastoid muscle. Despite surgery total trismus could not be prevented and later-on muscles and connective tissue of other areas ossified, too.
