Baseline prevalence and correlates of HIV and HCV infection among people who inject drugs accessing a syringe services program; Miami, FL.

BACKGROUND: Blood-borne viral infections, such as HIV and hepatitis C (HCV), are common infections among people who inject drugs (PWID). This study aims to determine the prevalence of HIV and HCV infection among PWID accessing the first legal syringe services program (SSP) in the state of Florida, along with examining baseline correlates of HIV and HCV infection. METHODS: Baseline behavioral enrollment assessments of 837 participants accessing an SSP for the first time were analyzed. Patients self-reporting or testing HIV or HCV positive at the enrollment visit were included. Socio-demographi

HIV, Drug Injection, and Harm Reduction Trends in Eastern Europe and Central Asia: Implications for International and Domestic Policy.

PURPOSE OF REVIEW: Scaling up evidence-based HIV prevention strategies like opioid agonist therapies (OAT), syringe services programs (SSPs), and antiretroviral therapy (ART) to mitigate the harms of drug injection is crucial within Eastern Europe and Central Asia (EECA), the only region globally where HIV incidence and mortality are increasing. RECENT FINDINGS: Though the proportion of new HIV cases directly attributable to drug injection has recently declined, it remains a critical driver of HIV, especially to sexual partners. Concurrently, scale-up of OAT, SSPs, and ART has remained low, c

Knowledge of hepatitis C status moderates the relationship between history of drug treatment and sterile syringe use.

High-risk injection related behavior including use of non-sterile syringes is associated with negative health outcomes among people who inject drugs (PWID). Drug treatment programs have been reported to curb hepatitis C (HCV) transmission. This study aims to assess the role of drug treatment programs and knowledge of HCV status, and how they influence current injection-related risk. Data were collected in 2012 by the New Orleans arm of the CDC funded National HIV Behavioral Surveillance. Respondent driven sampling was used to recruit a sample of PWID. The analytic sample consisted of 473 parti

The furthest left behind: the urgent need to scale up harm reduction in prisons.

Purpose Raise awareness about the disproportionate impact of HIV and hepatitis C (HCV) on prisoners worldwide and the need for key harm reduction services such as needle and syringe programmes and opioid substitution therapy in prisons offer practical recommendations to assist policy makers in implementing or scaling up these services. The paper aims to discuss these issues. Design/methodology/approach This study is a desk review of existing data and evidence on HIV, HCV and harm reduction in prisons, analysis of political barriers and formulation of key policy recommendations. Findings Harm r

Can needle and syringe programmes and opiate substitution therapy achieve substantial reductions in hepatitis C virus prevalence? Model projections for different epidemic settings.

AIMS: To investigate the impact of scaling-up opiate substitution therapy (OST) and high coverage needle and syringe programmes (100%NSP-obtaining more sterile syringes than you inject) on HCV prevalence among injecting drug users (IDUs). DESIGN: Hepatitis C virus HCV transmission modelling using U.K. estimates for effect of OST and 100%NSP on individual risk of HCV infection. SETTING: Range of chronic HCV prevalent (20/40/60%) settings with no OST/100%NSP, and U.K. setting with 50% coverage of both OST and 100%NSP. PARTICIPANTS: Injecting drug users. MEASUREMENTS: Decrease in HCV prevalen

Screening of hepatitis C virus in dried plasma spots among people who inject drugs and attending syringe exchange programme in Delhi, India.

This study investigated the use of dried plasma spots (DPS) in a community-based non-treatment-seeking population of people who inject drugs (PWID) for HCV testing. Individuals attending targeted intervention (TI) services were approached. The sociodemographic and drug use history were recorded. Blood samples were collected, dried plasma spots were prepared, and HCV screening was done. The participants (n=188) were with mean (SD) age of 27.44 (6.21) yr. HCV prevalence was found to be 69.7%. A good correlation (r=0.90; P<0.05) was observed between plasma and DPS samples, with sensitivity 98.5%,

"It was good because they have a relationship with us": A qualitative study on low-threshold buprenorphine treatment at syringe services programs.

INTRODUCTION: Syringe service programs (SSPs) reach people who inject drugs with opioid use disorder (OUD) and are novel "low-threshold" venues to initiate buprenorphine treatment. The study investigated patients' experiences with SSP-initiated buprenorphine treatment, which could aid in improving buprenorphine treatment delivery at SSPs. METHODS: The study included 12 participants who completed qualitative exit interviews after a randomized controlled trial of buprenorphine treatment at SSPs. In the parent study, participants received buprenorphine treatment through an onsite model at an SSP

Naloxone distribution amidst shifting drug use patterns: Insights from a needs-based syringe services program.

OBJECTIVE: In the context of rising overdose deaths among people smoking opioids and stimulants nationally, we aimed to evaluate naloxone distribution within a needs-based syringe services program in Southern California. METHODS: Using service delivery data spanning January-June 2024, we examined the distribution of pipes for safer smoking, sterile syringes, and naloxone. Modified Poisson regression estimated prevalence ratios (PRs) of encounters involving pipes only, syringes only, pipes and syringes together, and neither pipes nor syringes, and whether these encounters included naloxone. For a renewed harm reduction model.

In the early 1990s, the spread of HIV among heroin injectors prompted a shift in drug policy internationally, including in France. This led to the emergence of a new policy known as Harm Reduction (HR) and related tools, including needle exchange programmes, opioid substitution therapy programmes to manage illicit opiate consumption, as well as reception facilities and support systems for the most precarious People Who Use Drugs (PWUDs). This new policy is based on the assertion that drugs have always been there and will always be a part of society, and that we have to live with them and not t

INTRODUCTION: A group of synthetic opioids, the nitazenes, is appearing in the illicit drug market. Monitoring these novel psychoactive substances is challenging due to constantly evolving analogues and the small amounts of these potent drugs that are consumed. This study aimed to investigate the presence of nitazenes in drug paraphernalia in South Australia using sensitive and selective analytical methods. METHODS: Injecting drug paraphernalia samples were collected from needle and syringe programme sites in South Australia that offer disposal services. Chemical analysis using liquid chromat

Changes in Australians' attitudes towards supervised injecting facilities.

INTRODUCTION: Supervised injecting facilities (SIF) have been shown to reduce negative outcomes experienced by people who inject drugs. They are often subject to intense public and media scrutiny. This article aimed to explore population attitudes to SIFs and how these changed over time in Australia. METHODS: Data were drawn from the National Drug Strategy Household Survey, a national sample collecting data on illicit drug use and attitudes towards drug policy among Australians (2001-2019). Ordinal logistic regression assessed sociodemographic characteristics associated with different attitud

Detection of drugs and hepatitis C virus in used syringes from a needle exchange in Gothenburg, Sweden.

People who inject drugs (PWID) are exposed to serious health risks such as lethal overdoses, addiction and infections. The patterns of drug use and the prevalence of hepatitis C virus (HCV) infection vary greatly between and even within countries. Data on drugs used for injection are important to inform PWID of risks and adapt healthcare. This study aimed to determine which substances are injected in Gothenburg, Sweden, and estimate the risk of HCV transmission. A total of 150 syringes handed in at the needle and syringe exchange program (NEP) in Gothenburg over a week in November 2021 were an

Needle and Syringe Programs and HIV-Related Risk Behaviors Among Men Who Inject Drugs: A Multilevel Analysis of Two Cities in Iran.

BACKGROUND: Many studies have found significant differences in HIV risk at the community and socioeconomic levels. However, few have considered variations in needle and syringe program (Jin et al., Oral Dis. 1;22(7):609-19) coverage and other community characteristics on HIV risk behaviors among people who inject drugs (PWIDs). Our objective was to study the relationship between individual factors and city-level characteristics (such as the city's coverage of harm reduction programs) on HIV risk behavior among PWID residing in two cities in Iran. METHODS: The study was conducted from March to

A qualitative exploration of harm reduction in practice by street-based peer outreach workers.

BACKGROUND: Despite the widespread use of the phrase "harm reduction" and the proliferation of programs based on its principles during the current opioid epidemic, what it means in practice is not universally agreed upon. Harm reduction strategies have expanded from syringe and needle exchange programs that emerged in the mid-1980s primarily in response to the HIV epidemic, to include medication for opioid use disorder, supervised consumption rooms, naloxone distribution, and drug checking technologies such as fentanyl test strips. Harm reduction can often be in tension with abstinence and rec

Interest in smoking cessation among injection drug users.

We describe the degree of nicotine addiction and readiness to quit smoking among people with a history of injection drug use, comparing those in a methadone maintenance treatment program (MMTP) with active illicit drug injectors in a needle exchange program (NEP). Interview data were collected from 452 persons in Providence, RI, from July 1997 to March 1998. Ninety-one percent (91%) of the population currently smoked cigarettes. Smokers were more likely to be female and from an NEP. Higher nicotine dependence by the Fagerstrom Test for Nicotine Dependence was found in Caucasians, those with a

Effectiveness of periodic incentives on clinic attendance and hepatitis C testing and treatment for people who inject drugs: A five-year retrospective program evaluation.

BACKGROUND: There is a significant global burden of chronic hepatitis C virus (HCV) infection, with many affected individuals, particularly people who inject drugs (PWID), not receiving testing and treatment. Financial incentives may increase HCV care uptake among PWID, but current evidence is limited. This study evaluated the effectiveness of routine financial incentives for clinic appointment attendance, HCV testing, and HCV treatment among PWID in Newcastle, Australia. METHODS: During twice-yearly 4-7-week periods from January 2016 to December 2020, people accessing a needle and syringe pr
